    Study on the preference for “Internet+ TCM Nursing Services” among colorectal cancer patients

    Objective To explore the preferences and needs of colorectal cancer patients for “Internet+ TCM nursing services”,aiming to provide a basis for the improvement of service quality and the formulation of relevant policies,so as to better meet the needs of patients. Methods A total of 189 patients with colorectal cancer who were admitted to a tertiary A Chinese Medicine hospital in Qingdao from September 2023 to July 2024 were selected as the research subjects. A general data questionnaire and Questionnaire on “Internet+ Traditional Chinese Medicine Nursing Service” Preference of Colorectal Cancer Patients were used with 6 attributes,including average out-of-pocket cost,nurse qualification,service waiting time,service mode,comprehensiveness of service content,and platform service evaluation,each attribute with 2 to 3 levels. The preference,payment intention of “Internet+ TCM nursing services” were analyzed by constructing a mixed Logit model,and the influencing factors of choice preference of different colorectal cancer patients was explored through subgroup analysis. Results Totally 189 questionnaires were sent out,and 156 valid questionnaires were collected,with an effective recovery rate of 82.54%. Among the respondents,112 patients(71.79%) actively accepted “Internet+ TCM nursing services”,and the attributes of “Internet+ TCM nursing services” such as payment fee,service evaluation,service content and nurse qualification had an impact on nursing service preference of colorectal cancer patients(P<0.05). Based on willingness to pay,the ranking of attributes was service evaluation,service content and nurse qualification. Factors such as education level and the presence of ostomy also affected the preference of“Internet+ TCM nursing services” in patients with colorectal cancer(P<0.05). Conclusion Patients with colorectal cancer prefer “Internet+ TCM nursing services”,which has an average service cost of 50 yuan per time,excellent platform service evaluation,comprehensive service content,specialized TCM nurses. The preferences and specific characteristics of patients with colorectal cancer can be considered to optimize the allocation of nursing resources and provide services in line with their preferences.

    Objective Exploring the effect of Bian-stone-based herbal heat therapy on herbal heat ironing in gastrointestinal reactions during the preconditioning period of hematopoietic stem cell transplantation patients. Methods Convenience sampling method was used to select 68 cases of hematopoietic stem cell transplantation patients who attended the hematology department of a tertiary-level Chinese medicine hospital in Hangzhou from October 2023 to April 2024 as the study subjects,and the SPSS 26.0 statistical software was used to generate a random number for grouping into an experimental and a control group,with 34 cases in each group. On the basis of intravenous antiemetic medication and routine,the experimental group implemented the Bian-stone-based herbal heat therapy on the basis of intravenous antiemetic medication and conventional nursing care. In the control group,intravenous antiemetic drugs and routine care were used,and the intervention duration of both groups was 14 d,of which 7 d was a course of treatment,with a total of 2 courses of treatment. The incidence of gastroin-testinal reactions,Gastrointestinal Symptom Rating Scale(GSRS) score,Pepsin Ⅰ(PG Ⅰ),Pepsin Ⅱ(PG Ⅱ),and the ratio of PG Ⅰ to PG Ⅱ(PGR) before and after the intervention were compared between the 2 groups. Results The final sample of 66 cases was collected in this study,and 1 patient was dislodged from each of the control group and the experimental group. The comparison of the incidence of gastrointestinal reactions between the 2 groups within 14 days showed that the incidences of nausea,vomiting,abdominal distension,and diarrhea were lower in the experimental group than those in the control group(P<0.05). GSRS scores on days 1,8,and 14 of intervention were compared,and there were effects between groups in both groups(F=5.338,P=0.017). The levels of PG Ⅰ,PG Ⅱ in the experimental group on day 8 of the intervention were lower than those in the control group.The levels of PGR was higher than that in the control group(all P<0.05).The safety of the 2 groups after treatment was evaluated,and the results showed that no serious adverse events occurred in the 2 groups. Conclusion Bian-stone-based herbal heat therapy can improve gastrointestinal reactions and reduce the incidence of gastrointestinal reactions during the preconditioning period of hematopoietic stem cell transplantation patients,which provides clinical guidance for the application of Bian-stone-based herbal heat therapy by nursing staff in the future.

    Objective To construction and investigate the application effect of auricular acupressure technology in patients with cough with different syndrome types. Methods From December 2022 to March 2023,the Delphi method was used to consult experts to form a technical scheme for auricular pressure in patients with cough with different syndrome types in the early and middle stages. 122 patients with stage I and stage Ⅱ lung cancer cough from the oncology department of 3 tertiary hospitals of Traditional Chinese Medicine in Beijing between July 2023 to January 2024 were randomly divided into an experimental group(61 cases) and a control group(61 cases). The experimental group referred to the constructed auricular acupressure technique program to identify and take acupoints, and the control group used the original auricular acupressure program for 7 d of continuous intervention. The changes in the scores of the Visual Analog Scale(VAS) and the scores of The MD Anderson Symptom Inventory for lung cancer(MDASI-LC) were observed in the 2 groups before and after the intervention. Results The final scheme included 3 first-level items,12 second-level items,and 14 third-level items. The 2 rounds of expert authority coefficients were 0.88,indicating a high degree of authority,and the Kendall harmony coefficients of the 2 rounds of correspondence were 0.170 and 0.130(P<0.001),respectively,and the results tended to be consistent. A total of 122 patients completed the study. After intervention,the VAS score of cough severity in the experimental group was 3.0(2.0,3.0) points,which was lower than 3.0(3.0,4.0) points in the control group. The MDASI-LC score of the experimental group was 17.0(9.5,31.5) points,which was lower than 28.0(15.5,47.5) points in the control group,and the difference was statistically significant(P<0.05). Conclusion The constructed auricular acupressure technique program has certain scientificity,and it can effectively relieve cough symptoms of stage I~Ⅱ lung cancer patients,and the severity of symptom clusters has been improved,which provides the basis for TCM nursing intervention in cough symptoms of lung cancer patients.

    Objective To explore the effectiveness of auricular Gua Sha based on auricular vagus nerve stimulation in patients with chronic renal failure accompanied by nausea and vomiting. Methods Using a convenience sampling method,100 patients with chronic renal failure and nausea and vomiting admitted to the Nephrology Department of a tertiary hospital in Zhejiang Province between January 2022 and December 2023 were selected.They were randomly divided into a test group and a control group,with 50 patients in each group.The test group received the conventional treatment and care for chronic renal failure with nausea and vomiting,along with auricular Gua Sha,with a 3-day intervention.The control group received only conventional treatment and care.The Traditional Chinese Medicine(TCM) symptom scores for nausea and vomiting,total effective rate of the efficacy index,symptom improvement time,total number of bowel movements,and adverse events were compared between the 2 groups. Results A total of 100 patients completed the study.After intervention,the scores for nausea and vomiting symptoms were lower in both groups compared to the scores before intervention,with the experimental group having lower scores than the control group(P<0.001);the total effective rate of the experimental group was 98.00%,higher than 88.00% in the control group(P<0.001).Following intervention,the time for improvement of nausea and vomiting symptoms in the experimental group was(7.50±1.74) minutes,shorter than(13.38±2.31) minutes in the control group(P<0.001);the total number of bowel movements in the experimental group was(3.34±0.63) times,more than (2.46±0.58) times in the control group(P<0.001);no adverse events occurred during the study. Conclusion Auricular Gua Sha based on auricular vagus nerve stimulation helps reduce nausea and vomiting symptom scores,improves the total effective rate of the efficacy index,shortens the symptom improvement time,increases the frequency of bowel movements,and it is safe for patients with chronic renal failure.

    Objective To explore the application effects of a discharge planning service program based on King's Goal Attainment Theory in patients undergoing transcatheter heart valve surgery. Methods Using convenience sampling method,80 patients who underwent transcatheter heart valve surgery at a tertiary comprehensive hospital in Zhejiang Province from August 2023 to January 2024 were selected as the research subjects. Among them,40 patients from November 2023 to January 2024 were selected as an experimental group. A discharge preparation service plan for patients undergoing transcatheter heart valve surgery based on King's Goal Attainment Theory was adopted. Totally 40 patients who underwent transcatheter heart valve surgery from August to October 2023 were selected as a control group and underwent routine measures for discharge preparation.The differences in the scores of the discharge preparation scale,discharge guidance quality scale,self-management scale for patients after mechanical heart valve replacement surgery,and unplanned readmission rate were compared between 2 groups before and after intervention. Results During the application of the plan,a total of 2 cases were excluded,and 38 cases were included in the experimental group and 40 cases were included in the control group. After intervention,the experimental group had higher scores in discharge readiness,discharge guidance quality,and self-management ability than the control group,and the differences were statistically significant(P<0.001). The unplanned readmission rate within 30 days in the experimental group(2.63%) was lower than that in the control group (7.50%),and the difference was not statistically significant(P>0.05). Conclusion The discharge preparation service plan for patients undergoing transcatheter heart valve surgery based on King's Goal Attainment Theory can improve the discharge preparation,discharge guidance quality,and self-management ability of patients undergoing transcatheter heart valve surgery,and has a positive effect on improving their quality of life.

    Objective To explore the latent classes and influencing factors of blood glucose trajectories after transplantation in lung transplantation patients,and provide references for identifying high-risk population of post-transplant diabetes mellitus. Methods 122 lung transplantation patients who were hospitalized in a tertiary A general hospital in Hangzhou from January 2022 to March 2023 were selected conveniently as survey subjects. Socio-demographic and disease-related data were collected,and fasting plasma glucose at 1 week before surgery (T0),1 week after surgery(T1),1 month after surgery(T2),3 months after surgery(T3),6 months after surgery (T4),1 year after surgery(T5) were collected. Growth mixture model was used to identify categories of blood glucose trajectories after lung transplantation,and binary Logistic regression analysis was used to explore the influencing factors. Results A total of 109 lung transplantation patients were enrolled in the study,and 2 latent classes of blood glucose trajectories were identified:high risk(23.85%) and low risk(76.15%) of post-transplant diabetes mellitus. BMI,drinking history,afternoon blood glucose and tacrolimus trough concentration were the influencing factors of latent classes of blood glucose trajectories after lung transplantation(all P<0.05). Conclusion There are 2 latent classes of blood glucose trajectories after lung transplantation,namely high risk and low risk of post-transplant diabetes mellitus. Medical staff should pay attention to diabetes screening and assessment of lung transplantation patients who are overweight or obese,have a drinking history before transplantation,have high afternoon blood glucose in early stage of transplantation and have high tacrolimus trough concentration in stable stage of transplantation,so as to formulate a comprehensive and individualized blood glucose management program.

    Objective To explore the potential categories of diet management ability in elderly patients suffering from diabetes and analyze the influencing factors of different potential categories,for the reference of clinical nursing practice. Methods A convenient sampling method was used to select elderly patients suffering from diabetes with the risk of malnutrition treated in endocrinology departments in 2 tertiary hospitals in Zhenjiang City,Jiangsu Province from September 2023 to September 2024. General information questionnaire,Diabetes Diet Management Scale,Electronic Health Literacy Scale,Simple Disease Perception Scale and Perceived Social Support Scale were used for investigation. Results A total of 376 questionnaires were sent out,in which 349 were effectively collected with an effective questionnaire recovery rate of 92.8%. Totally 3 potential categories of diet management behavior were identified through the analysis of effective questionnaire,namely low self-management(24.9%),middle self-management(45.9%) and high self-management(29.2%). Disease duration of diabetes,glycosylated hemoglobin level,Albumin level,E-health literacy,perceived family support level and disease perception level were the risk factors of diet management ability in elderly patients suffering from diabetes with the risk of malnutrition(P<0.05). Conclusion There are significant category characteristics of diet management ability in elderly patients suffering from diabetes with the risk of malnutrition. Medical staff can formulate targeted clinical intervention measures to help patients improve their diet management ability according to different category characteristics.

    Development and reliability and validity test of the Post-Intensive Care Syndrome Scale for Children

    Objective To develop an assessment scale for post-intensive care syndrome in pediatrics (PICS-p) and evaluate its reliability and validity,aiming to provide a scientifically sound and feasible tool for clinical assessment. Methods Based on the PICS-p conceptual framework,combined with literature review,semi-structured interviews,and Delphi expert consultation,a preliminary scale was developed. From June to December 2023,a survey was conducted among 330 pediatric patients who were discharged from a tertiary children's hospital in Hangzhou,followed by a two-week retest with 30 children to assess the reliability and validity of the scale. Results Finally 304 children completed the survey. The Post-Intensive Care Syndrome Scale for Children consists of 4 dimensions of physical dysfunction,cognitive dysfunction,psychological dysfunction and social dysfunction,with a total of 29 entries,with a cumulative variance contribution of 61.705%. The content validity index for individual items ranged from 0.800 to 1.000,and the content validity index for the scale as a whole was 0.98. The dimensions of the scale showed positive correlations with the Functional Status Scale(r=0.438-0.581,P<0.001). The overall Cronbach's α coefficient of the scale was 0.847;the split-half reliability was 0.868;the test-retest reliability was 0.832. Conclusion The scale demonstrates good reliability and validity,and it is suitable for assessing the severity of PICS-p in pediatric patients discharged from the PICU.

    A qualitative study on elderly inpatients' experiences with intelligent nursing services

    Objective To analyze and understand the experiences and feelings of elderly inpatients during the use of intelligent nursing services,providing a scientific basis for the development of intelligent nursing models in intelligent ward systems. Methods Adopting the phenomenological research method,purposive sampling was used to select 14 elderly inpatients from a tertiary hospital in Beijing between October and December 2024 for semi-structured interviews. Thematic analysis was applied for data analysis. Results The experiences of elderly inpatients with intelligent nursing services were mainly summarized into 3 themes,including the dual track of acceptance challenges and convenience perception of intelligent nursing services,the integration of aging-appropriate innovation and humanistic care in intelligent nursing services,and the health orientation and privacy security concerns of intelligent nursing services. Conclusion Elderly inpatients can generally recognize the utility of intelligent nursing services in enhancing convenience and their health,but they still face challenges,and have clear aging-appropriate needs,while expressing concerns about privacy security. Managers and healthcare professionals need to design multi-faceted aging-appropriate innovations or intervention strategies to expand the accessibility of intelligent nursing,ensuring that elderly patients can truly benefit from these.

    Objective To explore the experience of social isolation among caregivers of stroke patients,and to provide corresponding references for the development of targeted intervention strategies. Methods A descriptive qualitative study was used to select 14 caregivers of hospitalized stroke patients attending the Department of Neurology and Neurosurgery of a tertiary-level hospital in Kunming City from September to November 2023 through purposive sampling method,and the data were analyzed using traditional content analysis. Results A total of 3 themes and 10 sub-themes were extracted,including characterization of social alienation(accumulation of negative emotions,limited social contact,dissolution of self-identity),drivers of social alienation(limitations of disease perception,multiple role conflicts,lack of support resources,constraints of caregiving responsibilities),and coping needs for social alienation(craving for informational support,appeal for emotional support,and expectation of policy support). Conclusion Caregivers of stroke patients are socially alienated due to limited social contact,multiple role conflicts,and lack of support resources,etc. Healthcare professionals should conduct regular assessments,provide targeted education and training,and strengthen multidimensional support in order to help them reintegrate into society and improve the overall quality of life of stroke families.

    Influencing factors and path analysis of compassion fatigue symptoms in orthopedic nurses

    Objective This study aims to investigate the current status and determinants of compassion fatigue symptoms among orthopedic nurses,and to analyze the interrelationships among these determinants to provide references for targeted intervention development. Methods From May to June 2024,a convenience sampling method was employed to recruit orthopedic nurses from 20 tertiary hospitals across 14 provinces and cities in China. The survey instruments encompassed the general demographic information sheet,the Compassion Fatigue Scale,the Social Support Rating Scale,the Nurses' Psychological Capital Questionnaire,and the Professional Identity Questionnaire. The data were analyzed using univariate analysis,Spearman correlation analysis,and multiple linear regression analysis. A structured equation model was also developed. Results Totally 1 397 valid questionnaires were collected,and the effective response rate was 96.94%(1 397/1 441). The average score of compassion fatigue symptoms was 47.41±12.16. The multiple linear regression analysis indicated that professional identity,social support,psychological capital,length of service,and frequency of night shifts per week were independent determinants of compassion fatigue symptoms(P<0.05). The path analysis demonstrated a good fit with the specified indicators. Psychological capital exhibited a direct mediating effect on compassion fatigue symptoms(β=-0.15,P<0.05). Furthermore,professional identity(β=1.46,P<0.001;β=-0.38,P<0.001) and social support (β=0.14,P<0.001;β=-0.99,P<0.001) served as mediators in the relationship between psychological capital and compassion fatigue symptoms. Conclusion The compassion fatigue symptoms among orthopedic nurses are relatively severe. It is imperative for nursing managers to address associated factors of compassion fatigue symptoms among orthopedic nurses and implement effective preventive strategies to mitigate its level.

    Objective The core competence index system of nurses providing “internet + postpartum maternal and infant home care” service was constructed and preliminarily verified,so as to provide references for objective evaluation of their core competence. Methods Based on literature review and semi-structured interviews,the core competence index system of nurses in “internet + postpartum maternal and infant home care” service was initially constructed with the iceberg model as the theoretical framework. In April to May 2024,the indicators are organized and analyzed using the Delphi method to form a final indicator system. In December 2024,30 nurses providing “internet + postpartum maternal and infant home care” service in a tertiary A obstetrics and gynecology hospital in Hangzhou were selected as the survey subjects,and the index system was preliminarily verified by questionnaires. Results A total of 20 experts completed 2 rounds of correspondence. The authoritative coefficients of the 2 rounds of expert inquiry were 0.88 and 0.90,respectively;the Kendall coordination coefficients were 0.34 and 0.39,respectively(P<0.001);the variation coefficients were 0~0.17 and 0.05~0.18,respectively. The final index system includes 3 first-class indicators,12 second-class indicators and 68 third-class indicators. The comprehensive,specialized,necessary,applicable,and overall satisfaction scores of the indicator system are(4.27±0.77) points,(4.43±0.56) points,(4.43±0.72) points,(4.47±0.56) points,and(4.46±0.56) points,respectively. Conclusion The core competence index system of nurses in“internet + postpartum maternal and infant home care” service is comprehensive,specialized,scientific and reliable,and can provide references for the training,assessment and evaluation of “internet + postpartum maternal and infant home care” talents.

    Objective To construct and evaluate an evaluation index system for the training effect of cardiac surgery specialist nurses and provide a reference for the quality evaluation of cardiac surgery specialist nurse training. Methods Based on the Kirkpatrick Model,the preliminary draft of the evaluation index system for the training effect of cardiac surgery specialist nurses was formed through literature review and semi-structured interviews. The Delphi method was used to conduct the expert consultation with 24 experts nationwide in September 2024,and the weight of each index was determined by the analytic hierarchy process. In February 2025,60 cardiac surgery nurses from 3 tertiary A hospitals in Nanjing were selected as the survey subjects,and a satisfaction questionnaire on the evaluation index system for the training effect of cardiac surgery nurses was used for the survey. Results The effective recovery rates of the 2 rounds of expert questionnaires were 96% and 100%,respectively. The expert authority coefficient was 0.913,and the Kendall harmony coefficient was 0.127-0.212 and 0.152-0.176,respectively(both P<0.05). The final evaluation index system for the training effect of cardiac surgery specialist nurses included 4 primary indexes,14 secondary indexes and 59 tertiary indicators. The satisfaction questionnaire score of the evaluation index system for the training effect of cardiac surgery nurses was (18.75±2.21). Conclusion The evaluation index system for the training effect of cardiac surgery specialist nurses was scientific and reliable,and can guide significance for the subsequent training,assessment and evaluation of cardiac surgery specialist nurses.

    To summarize the nursing management experience of 9 patients with lung transplantation undergoing extracorporeal membrane oxygenation(ECMO) combined with continuous renal replacement therapy (CRRT) during perioperative period. The key points of nursing include forming a professional team to ensure the whole process management;comprehensive evaluation and pretreatment of adverse events during the operation to maintain the safe operation of the equipment;refined volume management based on the surgical process ensures intraoperative hemodynamic stability;optimization of intraoperative anticoagulation management to reduce the incidence of complications;multi-device safe transport management ensures the continuity of postoperative treatment. Through comprehensive and targeted nursing intervention,9 patients were successfully treated with ECMO combined with CRRT during the operation. The patients were transferred out of ICU 3~12 days after the operation,and were followed up for 3 months. The patient's condition is good.

    Summary of nursing experience was conducted in a case of emergency liver transplantation during the second trimester of pregnancy complicated by acute liver failure. The key nursing points are as follows:preoperative participation in multidisciplinary discussions and emergency plan desktop exercises;preparation of preoperative equipment and supplies. Intraoperative fetal protection at critical stages,prevention and management of hyperkalemia,correction of coagulation function,maintenance of circulatory stability,implementing anticipatory surgical coordination to curtail operative duration,and prevention of perioperative hypothermia under a proactive nursing model. Through multidisciplinary team collaboration and meticulous nursing care,the patient recovered well 8 days postoperatively and successfully delivered a female infant via cesarean section. The patient was transferred back to the general ward 8 days after the cesarean section and was discharged 47 days postoperatively. Follow-up for over 2 months showed good recovery for both the patient and the infant.

    To summarize the nursing experience of a patient with coronary heart disease who was left in the radial artery during the removal of the radial artery sheath after coronary angiography via the radial artery pathway. Nursing points:to start the emergency transfer process,to shorten the treatment transfer time;to assist to locate the position of the sheath to provide a basis for the selection of surgical incision;to conduct dynamic assessment of hemostatic effect,prevention of radial artery occlusion;to closely monitor pain and signs to prevent vasovagal reflex;to implement the whole psychological intervention,and to reduce the psychological burden of patients and their families. The ruptured sheath tube was successfully removed by emergency surgery of vascular surgery. A total of 6 days after the operation,the patient was transferred to cardiac surgery for coronary artery bypass grafting,and was discharged 23 days later. After 3 months of follow-up,the blood supply of the limbs was good,and the incision healed well.

    Objective To conduct a scoping review of research on the relationship between 24-hour movement behaviours and cognitive function in the elderly,so as to provide references for future research. Methods Totally 8 Chinese and English databases were systematically searched. The search period spanned from the establishment of databases until November 30,2024. According to the scoping review framework,2 researchers independently screened the studies,extracted the data of the included studies,and collaborated on the final analysis. Results A total of 12 papers were included,and the distribution of 24-hour movement behaviors in the elderly was as follows:3.0-5.4 h/d for light physical activity,0.6-1.5 h/d for moderate vigorous physical activity,8.9-12.3 h/d for sedentary behaviour and 6.7-8.4 h/d for sleep. Measures included objective,subjective and subject-objective measures. There are complex interactions among physical activity,sedentary behavior,sleep and cognitive function,which jointly affect cognitive function in the elderly. Conclusion Despite current differences in the measurement of 24-hour movement behaviours,the decreased physical activity,the increased sedentary time,and insufficient or excessive sleep are common problems among the elderly. By increasing physical activity,reducing sedentary behavior and ensuring adequate sleep,the overall cognitive function,executive function of the elderly can be improved. In the future,we should pay attention to the time allocation of 24-hour movement behaviours of the elderly,develop accurate and standardized measurement methods,and promote research in this field.

    ObjectiveTo systematically search the research literature related to the application of machine learning models in hospice care,with a view to providing references for clinical practice. Methods A systematic search of Wanfang database,CNKI,VIP database,China Biomedical Literature Database,PubMed,Embase,Scopus,Cochrane Library,Web of Science,and CINAHL was conducted in accordance with the methodology of the scoping review as a guideline,with the timeframe of searching from the establishment of the database to August 30,2024,and the included literature was screened,summarized,extracted,and analyzed. Results Totally 17 studies were included. Analysis revealed that supervised machine learning algorithms(including random forest,decision tree,and neural networks) predominated in palliative care applications. Data sources and collection methods varied widely,with models applied across diverse scenarios. Model functions include assessing hospice needs,predicting a patient's risk of death,assisting with symptom management,analyzing hospice communication content,and more. Conclusion Machine learning models in palliative care demonstrate considerable utility and broad applicability. Future research should enhance data quality,optimize model development workflows,and improve model performance.

    In recent years,the incidence of childhood cancer has shown a steady upward trend. Due to the unique nature of this disease,the issue of affiliate stigma among primary caregivers of children with cancer has gradually drawn attention. Affiliate stigma not only directly affects caregivers' mental health and quality of life,but also leads to reduced social support and lower self-efficacy,thereby impacting their engagement in the caregiving process and affecting the treatment adherence and prognosis of children with cancer indirectly. This article provides a review covering 5 main areas:the conceptual definition of affiliate stigma,measurement tools,influencing factors,intervention strategies,and insights and recommendations,to provide a theoretical basis and guidance for subsequent research and the development of interventions.
