    Study on the development trajectory and influencing factors of frailty in patients undergoing cardiac surgery

    Objective To explore the potential categories of frailty development trajectories in patients undergoing cardiac surgery from pre-operation to 6 months post-operation,and analyze the influencing factors. Methods By a longitudinal study design,patients undergoing elective heart valve replacement surgery or coronary artery bypass grafting in a tertiary general hospital in Zunyi City from August 2022 to June 2023 were selected by the convenience sampling method. Tilburg frailty scale was used to investigate the frailty level of cardiac surgery patients 1 day before surgery(T0),1 month(T1),3 months(T2) and 6 months after surgery(T3). The growth mixture model was used to identify the trajectory categories,and the influencing factors of different frailty trajectories were analyzed by binary Logistic regression. Results 261 patients were enrolled at T0,with 22,9,and 3 patients lost at T1 to T3,and 227 patients were finally included in the analysis. There were 2 types of trajectories being identified as the low frailty decline group and the high frailty maintenance group. Age,use of sedative and analgesic drugs,pace,and depression were the factors influencing the frailty in cardiac surgery patients(P<0.05). Conclusion There are 2 kinds of frailty development trajectories in patients undergoing cardiac surgery. Medical staff should formulate precise interventions and nursing measures according to the factors influencing frailty,so as to improve frailty degree and quality of life of patients undergoing cardiac surgery.

    Longitudinal qualitative study of supportive care needs on heart transplant patients

    Objective To explore the supportive care needs of heart transplant patients at different stages. Methods Purposive sampling method was used to select 15 heart transplant patients who were hospitalized in a tertiary A hospital in Guangdong Province from July 2023 to February 2024 as research subjects. According to the “Timing It Right” theory,5 semi-structured interviews were conducted with patients. Interpretive phenomenology was used to sort out and analyze the data. Results A total of 5 themes and 18 subthemes were extracted,including urgent desire for transplant information and psychological support during diagnosis,strong physiological and emotional needs during hospitalization,significant demand for health education in the preparation period for discharge,expectation of family support during the adjustment period,increasing demand for social support during the adaptation period. Conclusion The supportive care needs of heart transplant patients at different stages are dynamic. Medical staff should adopt the concept of dynamic and continuous care,and provide personalized care in stages,plans and continuance,in order to improve the quality of life of heart transplant patients.

    Objective To explore the home rehabilitation experience of individuals with fully magnetically levitated left ventricular assist device,and to provide a basis for developing scientific nursing intervention plans and follow-up strategies for clinical practice. Methods Using phenomenological research methods,a semi-structured interview was conducted on 13 patients with fully maglev left ventricular assist device implantation in a tertiary A comprehensive hospital in Wuhan from September to October 2022. Colaizzi’s 7-step analysis method was used to analyze the data and extract themes. Results A total of 3 themes and 9 subthemes were extracted,including complex emotions in home rehabilitation(challenges for new characters,anxieties and concerns,regaining new life and full of confidence),difficulties in home self-management(lack of professional knowledge,insufficient exercise,difficulty in managing pump cable bag wounds,weak ability to recognize and respond to external controller alarms),and needs for home rehabilitation(need for continuity of care,desire for more social support). Conclusion Complex emotional experiences and many difficulties and needs exist in the home rehabilitation process of patients after implantation of full magnetic levitation left ventricular assist device. Healthcare professionals should actively pay attention to the postoperative psychological changes of patients,strengthen pre-discharge training to improve patients’ self-management ability,and actively build a multi-party Internet home rehabilitation management platform to improve the rehabilitation quality of patients.

    To summarize the nursing experience of a patient with Danon disease after heart transplantation. Nursing key points:closely monitor heart function,prevent right heart failure;phased analgesic and sedative measures were implemented to reduce the occurrence of related complications. Skeletal muscle function was evaluated and rehabilitation training was strengthened to promote the prognosis of patients. The patient was transferred to the general ward 5 days after operation,and was discharged 22 days after operation.

    The rehabilitation and nursing experience of a patient with recurrent coronary vasospasm and emergency scrotal hematoma in a short time after cardiac radiofrequency ablation was summarized. Key nursing points include:identification and emergency care of coronary vasospasm after cardiac radiofrequency ablation;enhanced psychological care and targeted guidance for smoking cessation management;implementation of scrotal hematoma nursing plan with integrated Chinese and western medicine;development of individualized follow-up and continuous rehabilitation nursing,help to back to society. Through careful and professional treatment and nursing after 27 d,the patient’s left ventricular ejection fraction(LVEF) recovered from 39% to 57%,and the patient was discharged smoothly. After discharge,the patient underwent continuous rehabilitation care,and the scrotal hematoma subsided significantly in 1 month after discharge,and the activity endurance increased.

    Objective A scoping review of studies on pulmonary prehabilitation in cardiac surgery patients was conducted to provide evidence support for the construction of a preoperative pulmonary rehabilitation program for cardiac surgery patients that suitable for China’s national conditions. Methods In accordance with the scope review’s research methodologies,databases including PubMed,Embase,Web of Science,CINAHL,CNKI and Wanfang were searched by the computer for relevant studies. The deadline for retrieval is from the establishment of databases to June,2024. The included literature was systematically analyzed. Results 26 articles were finally included. Among them,4 were quasi-experiment studies,while the other 22 were randomized controlled trials. Forms of the intervention included comprehensive breathing exercises,inspiratory muscle training,positive expiratory pressure,incentive spirometer training and balloon blowing training. The intervention initiation ranged from 10 weeks to 1 day preoperatively;the outcome measures included postoperative pulmonary complications,lung function metrics,the 6-Minute Walk Test,duration of mechanical ventilation,length of hospital stay,patient-reported outcomes and so on. Conclusion There remains a deficiency in standardized protocols for preoperative pulmonary rehabilitation training among patients undergoing cardiac surgery. High-quality studies should be conducted,and intervention strategies for pulmonary prehabilitation in cardiac surgery patients should be optimized and a unified evaluation standard system should be established.

    Construction of a regional collaborative cloud-based treatment model for patients with severe traffic injuries and evaluation of the timeliness of care

    Objective To construct a regional collaborative cloud-based treatment model treatment model for patients with severe road traffic injuries,and to preliminarily evaluate the differences in nursing timeliness indicators and outcomes. Methods The regional collaborative cloud-based treatment platform includes 4 ports,including public security traffic police,pre-hospital emergency center,regional trauma center triage,and regional trauma center resuscitation unit. This forms a standardized real-time interactive treatment process between regional medical services and police for patients with severe road traffic injuries. Using a concurrent control study design,241 patients with severe road traffic injuries admitted to the emergency department of a regional trauma center in Zhejiang Province from May 2022 to May 2024 were selected as the study subjects. Among them,120 patients treated with the regional real-time collaborative cloud-based treatment model were designated as an experimental group,while 121 patients treated with the original trauma care process were designated as a control group. The differences in timeliness indicators and outcomes between the 2 groups were compared. Results The study included 241 patients with severe trauma. After the application of the regional collaborative cloud-based treatment model,the time from the scene of the accident to the hospital,the proportion of information early warning,completion time of pre-examination and triage,waiting time of the trauma team,the time of the first CT,the length of multidisciplinary consultation,and the time for completing hospitalization procedures in the experimental group were shorter than those in the control group(P<0.05),the proportion of information early warning in the experimental group was 100%(120/120),and the proportion of information early warning in the control group was 52.1%(63/121). The difference between the two groups was statistically significant(P<0.001). The survival rate of the experimental group was 90.8%(109/120),and that of the control group was 86.0%(104/121). There was no significant difference between the two groups( χ2=1.399,P=0.237). Conclusion The regional collaborative cloud-based treatment model improves the timeliness and standardization of the treatment of patients with severe road traffic injury,which has certain reference significance and promotion value.

    Objective To establish a full-chain follow-up system for tumor patients during chemotherapy and to evaluate its application effect. Methods A full-chain follow-up system was built according to the actual needs of cancer patients during chemotherapy,including 5 modules:patient health records,follow-up plan,risk identification and abnormal reminder,online consultation,interaction between doctors and patients and data visualization analysis and intelligent statistical management. Convenience sampling method was used to select 96 patients with tumor at chemotherapy stage who were treated in a tertiary A hospital in Deyang City,Sichuan Province from February to June 2023 as a test group,and the full chain follow-up system for tumor patients at chemotherapy stage was applied for follow-up;93 patients with tumor at chemotherapy stage were treated from February to June 2022 as a control group for routine follow-up care. Indicators of self-management ability,quality of life and satisfaction with follow-up services were compared between the 2 groups before and after the application of the system. Results 94 cases in the test group and 92 cases in the control group completed the study. After the application of the system,the test group scored(169.44±11.96) on the Cancer Patients Self-Management Scale,higher than(150.76±13.44) in the control group. The score on the Cancer Patients Quality of Life Core Scale was(65.50±7.90),higher than(59.81±7.27) in the control group. The score on the Questionnaire of Satisfaction with the Follow-Up Service was(52.00±3.30),higher than(48.89±3.54) in the control group,and the difference between the 2 groups was statistically significant(P<0.001). Conclusion The application of the full-chain follow-up system can meet the diversified and multi-level follow-up needs of tumor patients during chemotherapy,achieve accurate,professional and intelligent nursing follow-up services,effectively improve patients’ symptom self-management ability and quality of life,and improve patients’ satisfaction with nursing services.

    The effect of resistance training on alleviating lymphedema in breast cancer patients after surgery

    Objective To construct a resistance training program for breast cancer related lymphedema,and to examine its feasibility and effects. Methods The first draft of the resistance training program was developed based on the 2 intervenable factors of “muscle contraction” and “chest negative pressure” in lymphatic reflux power source and the best evidence of resistance training. The program was revised and its feasibility was analyzed through 2 rounds of expert demonstration and pilot study. The patients enrolled in 2022.1-2022.5 in a tertiary A hospital in Hefei were assigned to an experiment group(34 cases) and patients enrolled in 2022.9-2023.1 in a tertiary A hospital in Wuhu were assigned to an control group(34 cases) based on convenient sampling method. Both the experiment and control group were given comprehensive decongestive therapy with manual lymphatic drainage,wearing the pressure clothing,and skin care for 12 weeks. The patients in the experiment group were given additional resistance training for 12 weeks. Grip strength and extracellular water/intracellular water(ECW/ICW) of affected upper limb,forced expiratory volume in the first second(FEV1) were evaluated before and 2,4,8,12 weeks after intervention. Results The resistance training program included 4 parts,including resistance breathing training,resistance muscle training,warm-up training and relaxation training. The expert authority coefficient is 0.89;the Kendall coordination coefficient of the first round of expert consultations is 0.42;the coefficient of variation is 0.08~0.27. The Kendall coordination coefficient of the second round of expert consultations is 0.67,and the coefficient of variation is 0~0.19. The pilot study results showed that the completion degree of 5 patients was 62.50%~100%,and there are no adverse events occurred during intervention. A total of 31 patients in the experimental group and 30 patients in the control group completed the 12-week intervention. The results of repeated measurement analysis of variance showed that the increase of grip strength in affected upper limb and FEV1 value in the experiment group were significantly higher than that in the control group(P<0.05);the reduction of ECW/ICW value in the experiment group was significantly greater than that in the control group(P<0.05). Conclusion The resistance training program of breast cancer-related lymphedema is scientific and feasible. It can improve the “muscle contraction”and “chest negative pressure”of lymphatic reflux power source,and relieve lymphedema.

    Analysis of potential changes in symptom characteristics of breast cancer patients before and after chemotherapy and nursing countermeasures

    Objective To explore the time-varying pattern of potential symptoms in breast cancer patient before and after chemotherapy,and further analyze its influencing factors,aims to provide reference for clinical nursing practice. Methods 233 cases of breast cancer in a tertiary A hospital in Ningxia from June to October,2023 were selected as the research subjects. By the general information questionnaire and the Chinese version of Anderson Symptom Assessment Scale,the potential change of symptoms of breast cancer patients undergoing chemotherapy 1-3 days before the first chemotherapy(T1,233 cases) and 7-14 days after the third or fourth chemotherapy(T2,225 cases) was analyzed. Results 225 cases of breast cancer patients completed 2 surveys,and their symptom characteristics before and after chemotherapy can be divided into 2 categories:a high symptom troubled group and a low symptom troubled group. The high-symptom troubled group has strong stability,and the probability of maintaining the original group is 85.20%,while the low-symptom troubled group is more inclined to change to the high-symptom troubled group,and the transition probability is 26.60%. Logistic regression analysis showed that the mass was located on the left side,and the patients who obtained disease knowledge mainly through the Internet were more likely to change from the high symptom troubled group to the low symptom troubled group,while the patients who were employed,with unsatisfactory sleep at night,and never exercised were more likely to change from the low symptom troubled group to the high symptom troubled group. Conclusion The symptoms of breast cancer patients before and after chemotherapy are heterogeneous. Patients who are on the job,have unsatisfied sleep at night and never exercise are more likely to change from the low-symptom troubled group to the high-symptom troubled group,which suggests that medical staff should identify potential high-risk groups at an early stage,and make full use of the predictive function of symptom characteristics to carry out individualized management based on the dynamic monitoring of symptoms according to different types of population characteristics,so as to reduce the burden of symptoms and improve their quality of life.

    An analysis of the present situation and barriers to home care for elderly patients with postoperative osteoporotic fractures

    Objective A cross-sectional survey on the postoperative home care status and barriers was conducted among elderly patients with osteoporotic fractures nationwide,in order to provide a basis for promoting the improvement of standardized home care for elderly patients with osteoporotic fractures. Methods From October to November 2023,a survey on the current situation and barriers of home environment protection was conducted among elderly patients with osteoporotic fractures in the orthopedic wards of 594 hospitals across 31 provinces (autonomous regions/municipalities directly under the central government) using a convenience sampling method. Results A total of 14,349 questionnaires were distributed,and 12,496 valid questionnaires were collected,resulting in an effective response rate of 87.09%. Among the patients,5,502 cases(44.03%) had implemented home-based prevention and treatment of osteoporosis before the fracture. 2 095(16.77%) of the patients experienced a subsequent fracture,of which 65.11% of the patients who experienced a subsequent fracture received medication intervention after the initial fracture,while 19.86% of the patients who experienced a subsequent fracture did not comply with the treatment for osteoporosis after the initial fracture. Additionally,77.66%(n=1 627/2 095) of the patients received community medical services after the initial fracture. Barriers to care factors in the home environment after fracture from the patient’s perspective presented the complexity of the social-ecological system model in 6 dimensions at 2 levels:micro(basic personal situation,physiological factors,psychological factors,and behavioural factors),and meso(social support factors,and healthcare worker factors). Conclusion In the vast majority of elderly patients in China,before osteoporotic fracture,home-based measures to prevent osteoporosis have not been adequately implemented;after the initial osteoporotic fracture,the pathway of re-fracture prevention and management in the patient’s home environment is not yet complete and its popularity needs to be improved;the barriers to home care faced by elderly patients with osteoporotic fracture are complex. It is recommended to promote effective linkages among hospitals,community health centres and families to strengthen the closed-loop management of re-fracture prevention and management.

    Objective To explore the status and trajectory of urinary incontinence in patients with bladder cancer after orthotopic neobladder surgery,and analyze the characteristics of different trajectory categories,to provide references for the intervention and management of urinary incontinence in patients after orthotopic neobladder surgery. Methods A prospective study design was adopted,and the convenience sampling was used to select 142 eligible patients who underwent in orthotopic neobladder surgery in a tertiary hospital from January 2019 to August 2022. Demographic and clinical data of the patients were collected. The continence status of patients was evaluated by the International Consultation on Incontinence Questionnaire-Short Form(ICI-Q-SF) at 1 month after surgery,3 months after surgery,6 months after surgery and 1 year after surgery. Additionally,postoperative physical condition scores,pelvic floor muscle training,and timed urination information were collected. The Group-Based Trajectory Model was used to identify the development trajectory of urinary incontinence,and the logistic regression was conducted to identify influencing factors of urinary incontinence trajectories. Results 142 cases were included. There are 3 distinct daytime and nocturnal incontinence trajectory groups that were respectively identified:the significantly improved group(21.83%,29.58%),the slowly alleviated group(52.82%,48.59%),and the risk fluctuation group(25.35%,21.83%). Multivariate logistic regression analysis indicated that age,preservation of neurovascular bundles,and adherence to timed voiding were factors significantly influencing urinary incontinence trajectory categories(P<0.05). Conclusion The trajectory of urinary incontinence in patients undergoing orthotopic neobladder surgery showed an overall improvement trend within a year,with group differences. Clinical attention should be paid to the dynamic evaluation of postoperative urinary incontinence in older,unreserved nerves,and those who did not adhere to scheduled urination,and a phased matching postoperative rehabilitation plan should be developed to improve the urinary control level of patients.

    Objective To explore the potential profile of body image and its influencing factors among head and neck cancer patients. Methods Convenience sampling was employed to investigate 335 head and neck cancer patients in a tertiary hospital in Shandong Province from October 2023 to May 2024. Data was collected using the patient’s general information questionnaire,Body Image Scale,Medical Coping Modes Questionnaire,Perceived Social Support Scale,Comprehensive Score for Financial Toxicity based on the Patient-Reported Outcome Measures. Latent profile analysis and multivariate Logistic regression analysis were used to explore distinct profiles and influencing factor of body image in head and neck patients. Results A total of 335 valid questionnaires were collected in this study, and 95.4% of the valid questionnaires were included. Body image of patients with head and neck cancer could be divided into 3 latent profiles:body emotional stability(41.5%),body cognitive dissatisfaction(33.7%) and social behavior avoidance(24.8%). Age,location of cancer onset,stage of cancer,surgical treatment,medical coping style(facing coping,yielding coping),social support and economic toxicity were the factors influencing the latent profiles of body image in patients with head and neck cancer(P<0.05). Conclusion There is heterogeneity in body image of patients with head and neck cancer. Medical staff should implement targeted management strategies based on the influencing factors of different profiles to reduce the body image disorders in patients with head and neck cancer,and to promote patients to better return to society.

    Objective To investigate the desire and demand for the retraining of specialist nurses medical institutions in China,and to provide references for the further work of the re-certification of specialist nurses. Methods Convenient sampling method was used to select the head nurses in the nursing departments of 5 030 medical institutions in 30 provinces,autonomous regions and municipalities as the survey subjects in May 2023. The questionnaire was self-designed,and the survey was conducted by anonymous electronic questionnaire. Results 4 563 valid questionnaires were collected. Among the 4 563 medical institutions,the training of specialized nurses was carried out in various fields,among which 46.31% of the medical institutions have carried out the training of intensive care specialized nurses. 82.29% of the medical institutions considered it necessary to retrain specialized nurses;60.86% of the medical institutions suggested that the training cycle should be once every 5 years;68.05% of the medical institutions suggested that the training method should be online;47.97% of the medical institutions thought that the conditions could be applied without limitation. More than 80% of medical institutions believe that specialized knowledge and clinical practice ability should be the main training and assessment content. Conclusion Most medical institutions have the need for retraining of specialist nurses,and there are differentiated needs for the implementation of retraining. Retraining can be carried out on a pilot basis in the field of intensive care. The retraining and assessment of specialist nurses should mainly focus on specialist knowledge and clinical practice ability.

    Objective The Hybrid Education Competence Instrument(HybridEduCom) was sinicized,and its reliability and validity were tested. Methods The translation,cultural adjustment and pre-investigation of HybridEduCom were followed by the Brislin translation model. The reliability and validity of the HybridEduCom was tested by surveying 553 educators in medical and nursing related fields by convenience sampling from March to April 2024. Results The Chinese version of HybridEduCom consisted of 46 items in 5 dimensions:planning and resourcing competence,technology competence,interaction competence,digital pedagogy competence,and ethical competence in the hybrid education. The confirmatory factor analysis resulted in the second-order five-factor model,with the chi-square and degree of freedom ratio being 1.274,approximate root mean square error being 0.022,goodness of fit index being 0.913,and standard fit index(NFI) being 0.934,and Tucker-Lewis and comparative fit index being 0.985. All main evaluation indexes were within the acceptable range of the judgment criteria. The item-content validity index was 0.833-1.000,and the scale-content validity index was 0.968. The Cronbach’s α for the whole scale was 0.982,while varied from 0.892 to 0.936 for each dimension. The test-retest reliability of the scale was 0.978. Conclusion The reliability and validity of the Chinese version of HybridEduCom were satisfactory,and it can serve as a valid assessment instrument for the implementation of hybrid education competence of Chinese higher medical and nursing educators.

    To summarize the nursing experience of a case of adolescent type 1 diabetes with anorexia nervosa. The key points of nursing:to strengthen blood glucose management and be vigilant against hypoglycemia and ketoacidosis;to establish a regular eating pattern to improve nutritional status;to implement personalized graded exercise programs to reduce compulsive exercise behaviors;to carry out phased family therapy to enhance treatment effectiveness;to implement family centered health education to enhance disease management capabilities. After careful treatment and care by a multidisciplinary team,the patient recovered well and was discharged smoothly after 89 days of hospitalization.

    To summarize the nursing experience of a child with maple diabetes complicated with abdominal compartment syndrome during liver transplantation. Nursing points:dynamic monitoring of intra-abdominal pressure to alleviate symptom of abdominal compartment syndrome;strengthen the nursing care during the open period of the abdominal cavity and prepare for the second phase suture surgery;implement abdominal lavage technique to avoid intestinal fluid accumulation;carry out personalized rehabilitation training to promote the development of the nervous system;provide nutritional support,strengthen skin observation and care;empower parents of sick children and reduce the burden of care. After 48 days of diagnosis,treatment,and care,the patient recovered and was discharged from the hospital. With the follow-up for 3 months,the patient’s condition is good.

    Objective To review the application of user profiles in the health management of elderly patients with chronic diseases,and to provide references for nursing staff to apply this technology to manage chronic diseases in the elderly. Methods According to the scoping review methodology,a systematic search was conducted across 9 databases including Wanfang Database,CNKI,VIP,Chinese Biomedical Abstracted Database,PubMed,Embase,Web of Science,CINAHL and Cochrane Library. The search time was from the establishment of the database to January 2024. The literature was screened according to inclusion and exclusion criteria,and the information in the literature was independently extracted and summarized by 2 researchers. Results A total of 10 pieces of the literature were included in the study. The results showed that the process of constructing user profiles mainly includes data collection,user feature extraction,and profiles generation and visualization. User profiles are widely used in the health management of chronic diseases in the elderly,commonly used in the management of diabetes,heart failure and other chronic diseases in the elderly,including the formulation of personalised intervention plans,the provision of personalised health education and information,and the adjustment of health management strategies. User profiles significantly enhanced the accuracy of health information delivery and health services,improved user experience,enhanced patients’ self-management ability and quality of life. Conclusion As an emerging health information integration tool,user profiles can merge and analyze multidimensional health data to provide personalized and precise health management for elderly patients with chronic diseases. This approach enhances remote monitoring,prevents and manages complications,improves treatment outcomes,and reduces medical costs,thereby demonstrating significant potential in the health management of elderly patients with chronic conditions.

    With the increasing aging of the population in China,the problem of cognitive decline with age is becoming more prominent,and how to move forward to maintain and improve cognitive function in older adults has become an important research topic. Active participation in social activities has a protective effect on cognitive function in older adults. This paper summarizes the concepts of social activities,relationships with cognitive function,and strategies for promoting interventions. The corresponding nursing inspirations were proposed,aiming to provide new perspectives for improving cognitive function in older adults.
