    Guideline for assessment and maintenance of intrinsic capacity in older adults

    Objective This study aims to develop a guideline for assessing and maintaining intrinsic capacity in older adults,offer recommendations to professionals regarding these assessments,and encourage the implementation of evidence-based clinical practices across various settings,including communities,hospitals,nursing homes,and other geriatric care environments. Methods An evidence-based approach guided the collection of questions through a literature review. Preliminary recommendations were developed through a systematic search of domestic and international guideline networks,professional association websites,and comprehensive databases. Subsequently,the recommendations were revised,and the consensus was achieved through a round of expert consensus meetings and 3 rounds of expert correspondence,culminating in the formation of the guidelines. Results The developed guideline encompasses 2 aspects and 5 dimensions of assessment and maintenance,comprising a total of 28 questions and 39 recommendations. Specifically,6 questions and 9 recommendations were formulated for the cognitive dimension,5 questions and 7 recommendations for the locomotion dimension,6 questions and 7 recommendations for the vitality dimension,6 questions and 9 recommendations for the psychological dimension,and 5 questions and 7 recommendations for the sensory dimension. Among these,34 are classified as strong recommendations,while 5 are categorized as weak recommendations. Conclusion The guideline offers scientifically robust,acceptable,and comprehensible recommendations that equip the professionals with a foundation for decision-making aiming at preserving the intrinsic capacity of older adults.

    Application of a multicomponent exercise and cognitive stimulation program in elderly patients with mental disorders and sarcopenia

    Objective To explore the effect of a multicomponent exercise and cognitive stimulation program in elderly patients with mental disorders and sarcopenia,so as to provide references for reducing the risk of falls,preventing and improving sarcopenia,and enhancing cognitive function in patients. Methods The multi-component exercise and cognitive stimulation program was formulated through literature review and expert meeting. In this quasi-experimental study,76 elderly patients with mental disorders and sarcopenia who were hospitalized in a tertiary mental health center in Shanghai from January 2023 to February 2024 were selected as research subjects. They were randomly divided into an experimental group and a control group according to their hospitalization building number(38 cases in each group). The experimental group was treated with multicomponent exercise combined with cognitive stimulation program based on routine nursing,and the control group was treated with routine nursing. The risk of falls,skeletal muscle mass,muscle strength,physical function,cognitive function,and incidence of adverse events were compared between 2 groups after 12 weeks of intervention. Results A total of 75 patients with 37 in the control group and 38 in the experimental group completed the study. The TUG time,6M walking speed and the score of Short Physical Performance Bettery of the experimental group were all lower than those of the control group(P<0.05),and the scores of skeletal muscle mass,muscle strength,calf circumference,physical function and cognitive function of the experimental group were all higher than those of the control group(P<0.05). Neither group experienced any adverse events. Conclusion The application of this multicomponent exercise combined cognitive stimulation program developed for elderly patients with mental disorders and sarcopenia can effectively reduce the risk of falls,enhance the skeletal muscle mass and muscle strength and improve the cognitive function in elderly patients with mental disorders and sarcopenia.

    Construction and validation of a risk prediction model for oral frailty in the elderly community population

    Objective This study examines the factors influencing oral frailty in the elderly community,develops a risk prediction model,and validates its efficacy,so as to provide references for identifying and preventing oral weakness in the elderly. Methods 556 elderly individuals from 4 communities were selected by convenience sampling from June to August 2024 in Zigong City Sichuan Province. They were randomly divided into a training group(383 cases)and a validation group(165 cases). Data were collected by a general information questionnaire,Social Frailty Scale,Geriatric Depression Scale,and the Oral Frailty Index-8 screening tool. Logistic regression was used to determine the influencing factors,and R software was used to establish a nomogram model for predicting the risk of oral frailty. Bootstrap method and the validation group were used for internally validation of the model. Calibration curve was used to evaluate the prediction performance of the model. Results 548 valid questionnaires were collected. The final model variables included whether the age≥80 years,wearing removable dentures,reduced frequency of going out,brushing teeth less than twice a day,frequent dry mouth,increased difficulty in eating hard foods,and choking. The area under the receiver operating characteristic curve of the training group was 0.95(95%CI:0.93~0.97),and the best cutoff value was 0.687. The model achieved an accuracy of 87%,sensitivity of 91%,specificity of 85%,positive predictive value of 0.75,and negative predictive value of 0.95. The Hosmer-Lemeshow fitting test show that χ2=3.036,P=0.932,indicating a good model fit. Conclusion The oral frailty prediction model demonstrated a good discrimination,calibration,and clinical utility,which can provide a scientific basis for the prevention and early screening of oral frailty in the elderly.

    Objective To investigate the prevalence of comorbidity of oral frailty and sarcopenia in elderly inpatients,and to explore their influencing factors,so as to provide a basis for formulating targeted interventions for patients with oral frailty and sarcopenia. Methods A total of 515 elderly patients hospitalized in 3 tertiary hospitals in Zhengzhou City and 1 second-class hospital in Shangqiu City from January 2024 to April 2024 were selected as the research subjects by convenience sampling,and the general information questionnaire,Oral Frailty Index-8,Sarcopenia Screen Questionnaire,Mini Nutritional Assessment Short Form,Social Support Rating Scale and Hospital Anxiety and Depression Scale(HADS) were used for investigation. Logistic regression was used to analyze the influencing factors of oral frailty and sarcopenia comorbidities in elderly inpatients. Results 508 valid questionnaires were collected. The prevalence of sarcopenia and oral frailty in elderly inpatients was 22.8%,and age,dysphagia,number of chronic diseases,and history of hypertension or stroke,multiple drug use,depression,bad nutrition and low level of social support were the risk factors(P<0.05). Conclusion The incidence of oral frailty combined with sarcopenia was higher in elderly hospitalized patients. Nurses should take nursing measures according to its influencing factors to prevent and control the development of oral frailty combined with sarcopenia.

    Objective To systematically evaluate the factors that promote and hinder exercise in frail older adults,and to provide references for the formulation of exercise intervention programs. Methods We searched PubMed,Web of Science,Embase,CINAHL,Cochrane library,CNKI,Wanfang,VIP database,and China Biomedical Literature Database for qualitative studies on exercise facilitators and hindrances in frail older adults,and the search time period was from the establishment of the databases to September 1,2024. The quality of the literature was evaluated using the Joanna Brigg Institute Australian Centre for Evidence-Based Health Care Quality Assessment Criteria for Qualitative Research(2016),and the results were integrated by integrating methods. Results A total of 18 studies were included,and 72 research results were extracted,and 10 categories were summarized. The final synthesis included 2 integrated results:the facilitators included personal exercise motivation,physical and psychological benefits,positive interpersonal interactions,multiple social support systems and person-centred exercise programme;the impediments included underlying diseases and somatic functional limitations,negative psychological status,low health literacy,family role conflicts and limits of the environment. Conclusion Exercise for frail older adults is affected by a variety of factors. Healthcare professionals should improve the positive perception of exercise for frail older people and help them overcome psychological barriers;establish an all-round support system to enhance the sense of social contact of the frail elderly;formulate a personalised exercise programme with a human-centred approach to enhance the motivation of the frail elderly.

    With the accelerating global aging process,falls among older adults have emerged as a significant health problem,which not only significantly impacts their quality of life,but also brings a heavy social burden. Cognition-motor dual-task training,an important intervention for preventing falls,can significantly enhance the cognitive and motor functions in older adults. This article reviewed the implementation forms,mechanism and application effects. It aims to provide references for the implementation of cognition-motor dual-task training in older adults.

    Objective To construct a perioperative blood pressure management plan for patients undergoing carotid artery stenting(CAS) and evaluate its application effectiveness,and to provide references for clinical practice. Methods The relevant literature on perioperative blood pressure management for CAS patients was retrieved from domestic and foreign databases,and a preliminary draft of a perioperative blood pressure management plan for CAS patients was constructed. The preliminary draft was discussed and modified through expert meetings,and 2 rounds of expert inquiries were conducted using the Delphi method from June 2023 to August 2023 to determine the final plan. 32 patients who received CAS in the neurology department of a tertiary comprehensive hospital in Zhejiang Province were selected from May to July 2024 as an experimental group,and 33 patients who received CAS in the same ward from January to March 2024 as a control group. The experimental group applied the constructed CAS patient perioperative blood pressure management plan,while the control group applied routine perioperative nursing measures. The incidence of postoperative hypertension,postoperative hypotension,and postoperative adverse events was compared between 2 groups. Results A total of 13 experts completed 2 rounds of inquiries. The effective questionnaire response rate for the second round of inquiry was 100%,with an authority coefficient of 0.897. The coefficients of variation for the importance and feasibility scores of each item were 0~0.19 and 0~0.18,respectively,and the Kendall harmony coefficients were 0.224 and 0.201,respectively(P<0.05). The final construction plan includes 3 primary indicators,15 secondary indicators,and 40 tertiary indicators. Ultimately,32 cases were included in the experimental group and 33 cases in the control group. The incidence of postoperative hypertension,postoperative hypotension,and adverse events in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05). Conclusion The perioperative blood pressure management plan for CAS patients constructed in this study has good scientificity and practicality,which can improve the stability of patients’ perioperative blood pressure and reduce the occurrence of adverse events.

    The current status and influencing factors of the coexistence of multiple frailty domains in patients with chronic heart failure

    Objective To investigate the status and influencing factors of the coexistence of multiple frailty domains in patients with chronic heart failure(CHF),and to provide a reference for formulating targeted intervention measures. Methods The patients with CHF hospitalized in the cardiovascular department of a tertiary general hospital in Tianjin from November 2021 to December 2022 were conveniently selected as the study subjects. The self-designed general information questionnaire,FRAIL scale,Montreal Cognitive Assessment(MoCA) scale,HALFT Scale,Social Support Rating Scale and Self-Rating Depression Scale were used to investigate the coexistence of physical,cognitive and social frailty in CHF patients,and single factor analysis and binary logistic regression analysis were used to analyze the influencing factors. Results A total of 275 questionnaires were issued and 269 valid questionnaires were collected. Among 269 patients with CHF,12(4.5%),67(24.9%),122(45.4%) and 68(25.3%) patients had 0,1,2 and 3 frailty domains,respectively. According to the patients with CHF whether coexists physical,cognitive, social frailty,divided into coexistence and non-coexistence group. In univariate analysis,there were statistically significant differences in education,residence,family history of cardiovascular disease,NYHA cardiac function grade,atrial fibrillation,BMI,social support score and depression score(P<0.05). Binary logistic regression analysis showed that high education level(OR=0.567,95%CI 0.397~0.810,P=0.002) was the protective factor for multiple frailty domains in CHF patients,and atrial fibrillation(OR=1.904,95%CI 1.054~3.442,P=0.033) and depression(OR=1.038, 95%CI 1.006~1.071,P=0.018) were risk factors. Conclusion The prevalence of the coexistence of multiple frailty domains in CHF patients is high. Education level,atrial fibrillation and depression are potential intervention targets for multiple frailty domains in CHF patients. Targeted intervention measures should be carried out to reduce the incidence of multiple frailty domains and to improve quality of life for CHF patients.

    A path analysis of the impact of death attitudes on negative emotion among nurses engaged in hospice care

    Objective To investigate the current status of rumination and professional grief among hospice nurses,and to explore the mediating effect of death attitude between the two. Methods A total of 323 nurses in hospitals with hospice services and hospice wards in Hangzhou City were surveyed from April to July 2024 by convenience sampling method. They were investigated with general information questionnaire,Event Related Rumination Inventory,Death Attitude Profile-Revised,and Grief State Scale for Nurses by convenience sampling method. The structural equation model was used to analyze the mediating effect of death attitude between rumination and professional grief among hospice nurses. Results A total of 331 questionnaires were collected,among which 8 invalid questionnaires were deleted,and 323 valid questionnaires were retrieved,with the valid questionnaire recovery rate of 97.58%. The score of intrusive rumination dimension for hospice nurses was 13.34(7.00,20.00),and the score of deliberate rumination dimension was 16.92(11.00,24.00). The negative attitude towards death scored 38.35(29.00,48.00),while the positive attitude towards death scored 62.88(49.00,78.00). The total score of Grief State Scale for Nurses was 47.05(40.00,53.00). Hospice nurses’ intrusive rumination was positively correlated with negative attitudes toward death,professional grief,and negatively correlated with positive attitudes toward death(all P<0.05);deliberate rumination was negatively correlated with negative attitudes toward death,professional grief,and positively correlated with positive attitudes toward death(all P<0.05). Negative death attitudes and positive death attitudes partially mediated between intrusive rumination and professional grief,with mediation effects of 0.053 and 0.037,accounting for 12.70% and 9.25% of the total effect. Negative death attitudes and positive death attitudes similarly partially mediated between deliberate rumination and professional grief,with mediation effects of -0.033 and -0.080,accounting for 10.03% and 21.33% of the total effect. Conclusion Death attitudes are a mediating variable between hospice nurses’ rumination and professional grief. Nursing managers can take effective measures to improve hospice nurses’ ruminative contemplation ability and positively guide positive attitudes toward death,thereby reducing their professional grief levels.

    Development and reliability and validity test of a Discharge Readiness Scale for Patients with PICC

    Objective To develop a Discharge Readiness Scale for Patients with PICC and to test its reliability and validity. Methods Based on the Meleis transition theory,the initial version of the scale was formed through literature review,qualitative interview,expert enquiry and pre-investigation. Patients with PICC in 2 tertiary hospital in Hangzhou from December 2023 to March 2024 were selected to investigate the reliability and validity of the scale by convenience sampling. Results 352 questionnaires were distributed,and 340 valid questionnaires were recovered,and the validity recovery rate was 96.59%. The exploratory factor analysis extracted 4 common factors with the cumulative variance contribution rate of 80.568%. The confirmatory factor analysis showed that all evaluation indicators of the scale were within ideal standards. The item-level content validity index ranged from 0.79 to 1.00,and the scale-level content validity index was 0.96. The total Cronbach’s α coefficient of the scale was 0.963,and the split-half reliability coefficient was 0.889. The Chinese version of the Discharge Readiness Scale was used as a calibration tool,and the correlation coefficients between the total score of the scale and the scores of each dimension and the calibration tool were 0.788 and 0.567~0.878,respectively(all P<0.05). The Discharge Readiness Scale for Patients with PICC included 26 items in 4 dimensions including physiological and psychological status,knowledge of catheter maintenance,catheter maintenance capability and social support. Conclusion The Discharge Readiness Scale for Patients with PICC has good reliability and validity,and can be used to evaluate the level of discharge readiness among patients with PICC.

    Latent profile analysis of dyadic psychological capital among stroke patients and their spouses and nursing countermeasures

    Objective To explore the potential categories of dyadic psychological capital and related factors among stroke patients and their spouses,in order to inform the improvement of their mental health. Methods Stroke patients and their spouses were recruited in the Neurology Department of 3 tertiary general hospitals from March,2023 to August,2023 by convenience sampling method. The general information questionnaire,Modified Rankin Scale(mRs),Psychological Capital Questionnaire,The Kessler Psychological Distress Scale(K10) and Perceived Social Support Scale(PSSS) were used for investigation. Latent profile analysis was adopted to explore the categories of dyadic psychological capital,and multiple Logistic regression analysis was used to assess the effect of various factors on different categories. Results A total of 232 returned questionnaires(out of the 235) were valid,resulting an effective response rate of 98.7%. Stroke patients and their spouses were divided into 3 categories,including dyadic low psychological capital-low self-efficacy group(6.0%),dyadic high psychological capital-low resilience group(15.9%) and dyadic medium psychological capital group(78.1%). The stroke patients’ education level,mRs scores,occupational status after the illness,level of perceived social support,and spouses’ working condition were the influencing factors on potential profiles of dyadic psychological capital(P<0.05). Conclusion There are heterogeneity and interdependence in the level of dyadic psychological capital of stroke patients and their spouses. Healthcare professionals should promote dyadic mental health by developing targeted interventions for stroke patients and their spouses with different psychological capital characteristics from a dyadic holistic perspective.

    Objective To analyze the current situation and latent profiles of nurses’ perceived organizational support,and explore the influencing factors of different types. Methods Through convenience sampling,a total of 1,860 nurses from the Pearl River Delta,Northern Guangdong,Eastern Guangdong and Western Guangdong regions of Guangdong Province were selected for the survey between October and December 2023. The demographic questionnaire,the Perceived Organizational Support Scale,and the Psychological Resilience Scale were used for investigation. The latent profiles of the nurses’ perceived organizational support,the influencing factors of latent profiles were identified by single-factor analysis and multivariate Logistic regression. Results The score of the items of the perceived organizational support in 1,819 nurses is(3.47±0.88) scores. Nurses perceived organizational support is divided into 3 latent classes,namely “poor group”(10.1%),“balanced group”(44.4%),and “rich group”(45.5%). The influencing factors include recruitment type,hospital level,and psychological resilience(all P<0.05). Conclusion The perceived organizational support among nurses is moderately high and heterogeneous,which could be divided into 3 latent profiles. Nursing managers should focus on nurses from the poor group and balanced group and intervene with nurses according to the influencing factors of the different latent profiles to improve the level of nurses perceived organizational support.

    Establishment and comparison of 3 compassion fatigue risk prediction models for obstetrics and gynaecology nurses based on machine learning

    Objective To compare the performance of 3 risk prediction models based on machine learning in predicting the risk of compassion fatigue among obstetrics and gynaecology nurses. Methods Using the convenience sampling method,1 323 obstetrics and gynaecology nurses from 11 tertiary hospitals in 9 cities were selected from December 2022 to March 2023,and were randomly divided into a training set and a test set according to 7∶3 ratio. Compassion Fatigue Scale,Five Facet Mindfulness Questionnaire and Emotional Intelligence Scale were used for the survey. A total of 3 types of risk prediction models were constructed for the factors affecting the compassion fatigue of obstetrics and gynaecology nurses,namely,Logistic Regression,Decision Tree,and Random Forest. The accuracy,precision,specificity,sensitivity,F1 score and AUC were used to compare the predictive performance of the 3 models. Results Finally 1 276 maternity nurses completed the survey. All 3 models showed that nature of employment,years of experience,mindfulness and emotional intelligence were independent risk factors for compassion fatigue in obstetrics and gynaecology nurses (P<0.05). The results of model comparison showed that the accuracy of Logistic regression,decision tree and random forest were 0.804,0.806,0.796;the precision was 0.821,0.827, 0.823;the sensitivity was 0.956,0.949,0.939;the F1 score was 0.883,0.884,0.877;the AUC was 0.704(95%CI:0.701~0.713),0.760(95%CI:0.751~0.771),0.742(95%CI:0.723~0.762) respectively. Conclusion The risk prediction model of factors affecting compassion fatigue among obstetrics and gynaecology nurses constructed by decision tree performed the best,and the predictive performance was better than that of the random forest and logistic regression models. The multi-model effectively predicts the risk of compassion fatigue in obstetrics and gynecology nurses,explores the interaction of influencing factors in multiple dimensions,and it can inform the early identification and prevention of compassion fatigue and the development of related interventions.

    Summary of the best evidence for perioperative management of oral nutritional supplementation in patients with gastric cancer

    Objective To search,select,evaluate and summarize the best evidence of perioperative oral nutritional supplement management in patients with gastric cancer,so as to provide references for the formulation of standardized oral nutritional supplement management program in clinical practice. Methods Guidelines,evidence summaries,systematic reviews,expert consensuses and group standards were systematically and completely retrieved from NGC,NICE,SIGN and other websites,as well as Cochrane Library,JBI and other databases. The retrieve time limit was from the database construction to March,2024. Totally 2 researchers trained in evidence-based nursing independently evaluated the quality of the literature,extracted the evidence,and classified the evidence level. Results A total of 21 articles were involved,including 4 guidelines,6 evidence summaries,5 expert consensuses and 6 systematic reviews. 28 pieces of best evidence on 6 topics were summarized,including the establishment of multidisciplinary teams,health education,type selection and target requirement of oral nutritional supplement,preoperative oral nutritional supplement management,postoperative oral nutritional supplement management,and discharge oral nutritional supplement guidance. Conclusion This study summarized the comprehensive and scientific best evidence of oral nutritional supplement management for patients with perioperative gastric cancer,providing evidence-based support for medical staff to manage the oral nutritional supplement in perioperative gastric cancer patients systematically and standardly.

    A scoping review of functional prognosis prediction models for stroke patients

    Objective To conduct a comprehensive review of functional prognosis prediction models for stroke patients,offering insights for clinical nursing practice and research. Methods A search was conducted across various databases including PubMed,Web of Science,Embase,CINAHL,Cochrane Library,China National Knowledge Infrastructure,Wanfang Database,VIP Database,and China Biomedical Literature Database from their inception to April 1,2024. There were 2 researchers who independently screened the literature,extracted data,and assessed the quality of the studies. Results The review included 19 studies on the development of functional prognosis prediction models for stroke patients,resulting in a total of 57 models. The area under the receiver operating characteristic curve ranged from 0.730 to 0.974. External validation was performed for 3 of the models. Age,gender,NIHSS,and history of stroke emerged as the principal predictors in the functional prognosis prediction models for stroke patients. Conclusion Research on functional prognosis prediction models for stroke patients is still in its early stages,with models demonstrating reasonable predictive power but carrying a high risk of bias. Future studies should standardize model development and validation. The aim should be to create user-friendly models with strong predictive capabilities,thereby providing healthcare professionals with solid foundations for clinical decision-making.

    In recent years,there has been an increase in the prevalence and incidence of acute myocardial infarction. Despite advances in diagnostic and therapeutic technologies related to the disease,challenges remain in the management of the disease after patients are discharged from the hospital. Home-based cardiac rehabilitation has been demonstrated to be an effective means of improving the prognosis of the disease. The utilization of digital health technology has the potential to enhance patient compliance and quality of life in the context of home-based cardiac rehabilitation for those who have experienced an acute myocardial infarction. This article reviews the forms of intervention,specific applications and effects of digital health technologies in home-based cardiac rehabilitation, It analyses the existing problems and puts forward nursing countermeasures. The aim is to further help nursing staff to guide the practice of home cardiac rehabilitation for patients with acute myocardial infarction.

    The iterative advancement of digital technology has enhanced the integration and interaction efficiency of resource information within healthcare institutions,providing solutions to issues such as data silos and poor communication prevalent in traditional nursing quality management settings. This paper reviews the digital empowerment of nursing quality management based on the Value Co-Creation theory and expounds the conceptual connotation of digital empowerment,the path of realization,the specific application and effect,and the potential risks and challenges,to provide references for healthcare professionals to understand the digital transformation of nursing quality management as well as to explore the directions that can be developed in the future.
