Symptom clusters in patients with gynecologic cancer receiving chemotherapy：a scoping review

Abstract:

Objective To identify the characteristics and research status of symptom clusters in patients with gynecologic cancer receiving chemotherapy. Methods This scoping review was conducted under the Joanna Briggs Institute guidelines. A literature search was performed in the PubMed,Embase,CINAHL,Web of Science,Medline,Scopus,CNKI,Wanfang,and CBM databases to identify relevant studies about symptom clusters in patients with gynecologic cancer from inception until November 29,2021. The included studies were summarized and analyzed. Results A total of 12 articles were included. The elements of identifying symptom clusters included symptom assessment and analysis methods. There were 13 kinds of symptom clusters during chemotherapy. The psychological symptom cluster and gastrointestinal symptom cluster are the most serious symptom cluster before chemotherapy and after 1st chemotherapy respectively. The body image symptom cluster is the most serious symptom cluster after 3rd and 6th chemotherapy. Conclusion Gynecologic cancer patients receiving chemotherapy experienced multiple symptom clusters with dynamic change. Future research should combine specific symptom measurement for gynecologic cancer patients with advanced machine learning techniques,and explore the construction of an accurate symptom cluster management scheme.

摘要：

目的对妇科恶性肿瘤患者化疗期间症状群的研究进行范围综述,以确定症状群特点及研究现状,为今后症状群研究提供指导。方法以乔安娜布里格斯研究所范围综述指南为方法学框架,检索PubMed、Embase、CINAHL、Web of Science、Medline、Scopus、中国知网、万方数据库和中国生物医学文献数据库中妇科恶性肿瘤患者化疗期间症状群的相关研究。检索时限为建库至2021年11月29日。对纳入文献进行数据提取及汇总分析。结果纳入12篇文献,症状群的识别要素涵盖症状评估及分析方法。妇科恶性肿瘤患者化疗期间症状群的种类涉及13种,化疗前心理症状群最严重,第1次化疗后消化道症状群最严重,第3、6次化疗后身体形象症状群最严重。结论妇科恶性肿瘤患者在化疗期间存在多种症状群,并呈动态变化。未来研究可探索妇科肿瘤患者特异性症状评估工具,结合机器学习等先进分析技术,逐步改进症状群的识别,探究精准化症状群管理方案的构建。
