    Expert consensus of suicide care interventions for psychiatric inpatients with depression

    Objective To form an expert consensus on suicide nursing interventions for inpatients with depression in psychiatric departments,standardize the contents of suicide nursing interventions for depression patients in psychiatric departments. Methods Evidence-based methods and literature analysis were used to extract recommendations and research conclusions of suicide nursing interventions for inpatients with depression,and the consensus draft was formed. After 2 rounds of expert letter consultation and 2 expert discussions,the draft was adjusted,modified and improved,and the final draft of the consensus was formed. Results The positive coefficients of the experts in 2 rounds of letter consultation were 100%;the authority degree of the experts was 0.924;the mean values of importance of each index were >3.5 points;the coefficient of variation was<0.25;the Kendall harmony coefficients of the experts were 0.182 and 0.260,respectively(P<0.01). Finally,the operational definition,suicide risk assessment,intervention form and time,theoretical basis of intervention,general principles of intervention,suicide impression intervention,suicidal behavior intervention,key points of intervention in special populations,evaluation of intervention effect and post-discharge evaluation of suicide care in hospitalized patients with depression were analyzed. A total of 11 components of the health education program were agreed upon. Conclusion The consensus provides the guidance for the suicide nursing interventions of psychiatric inpatients with depression,so that the suicide nursing intervention of psychiatric inpatients with depression is more standardized.

    Objective To develop a self-management scale for patients with bipolar disorder and to test the reliability and validity of the scale. Methods This study uesd Lorig and Holman’s self-management theory and Bandura’s social cognition theory as theoretical framework,combined with the results of semi-structured interviews with 20 patients,carers and medical personnel. The pretest scale was formed through literature research method and 2 rounds of expert evaluations. A convenience sample of 397 patients with bipolar disorder was selected for item analysis,the pretest scale of the reliability and validity. Results The self-management scale for patients with bipolar disorder includes 3 dimensions and 18 items. The overall Cronbach’s α coefficient of the scale was 0.906;the split-half reliability was 0.853;the retest reliability coefficient of the scale was 0.658. There were 3 factors being extracted from exploratory factor analysis,explaining 54.814% of the total variances. The correlation coefficient between the self-management scale and the Brief version of Quality of Life in Bipolar Disorder was 0.535(P<0.001). Conclusion The self-management scale for patients with bipolar disorder has good reliability and validity,and it can effectively assess the status of self-management.

    Mediating effect of automatic thoughts on the relationship between self-compassion and mental pain in patients with depression

    Objective To explore the relationship between mental pain,self-compassion and automatic thoughts in depressed patients. Methods A total of 263 depressed inpatients were selected and tested by the Chinese version of the Orbach & Mikulincer Mental Pain Scale(OMMP),Self-Compassion Scale-Chinese Version(SCS-C),Automatic Thought Questionnaire(ATQ) between 2020 and 2021 from a tertiary psychiatric hospital through convenient sampling. SPSS 25.0 was employed for Pearson correlation analysis and multiple linear regression analysis,and we selected the Model 4 of SPSS PROCESS for mediating effect analysis. Results Mental pain was significantly positively correlated with automatic thoughts(P<0.001) and negatively associated with self-compassion(P<0.001). The results of Bootstrap test showed that the mediating effect of automatic thoughts on the relationship between self-compassion and mental pain was -0.274(95%CI:-0.356~-0.203),accounting for 59.6% of the total effect. Conclusion The automatic thoughts play a partial mediating role on the relationship between self-compassion and mental pain in depressed patients. Based on this path of mental pain,nurses should actively constitute intervention plans,in order to achieve the final goal of decreasing the incidence rate of suicide or self-injury in patients with depression.

    Objective To systematically evaluate the disease experience of patients with schizophrenia in rehabilitation period,and to provide the evidence for strengthening standardized management in rehabilitation period and improving the rehabilitation effect and mental health level of this population. Methods A computer search was conducted on PubMed,Web of Science,Cochrane Library,CINAHL,Embase,PsyINFO,CNKI,Wanfang Database,VIP Science and Technology Journal Database and China Biomedical Literature Database for qualitative research on mental experience,real experience and feelings of schizophrenia patients in convalescence. The search time was from the establishment to February 2022. The literature quality evaluation was carried out by the quality evaluation standard of qualitative research of Joanna Briggs Institute(JBI),Australia(2016 edition),and the research results were summarized and integrated by the convergent integration method. Results Totally 13 articles were included;86 topics were extracted;10 categories were summarized. 4 integrated results were formed as following. Schizophrenia is a complex mystery,which is often accompanied by many negative disease experiences;social isolation and alienation from the outside world;the attempt to rebuild the life;rehabilitation support needs to be completed together. Conclusion Medical staff should pay attention to the emotional experience of patients,fully explore the positive feelings,meet the reasonable needs of patients,and give patients systematic rehabilitation support. The public should reduce the stigma of patients and create a good social environment.

    Shared decision-making is considered as a fundamental part of "patient-centered care",which has raised great interest in mental healthcare abroad over the last decade. Shared decision-making,focusing on the needs of patients,strengthens doctor-patient information exchange and enhances patients’ status in decision-making. Shared decision-making can raise the quality of health care service and patients’ satisfaction,treatment compliance,and outcome. This article reviews the concept,model,content,influencing factors,and effects of shared decision-making in patients with severe mental disorders,and puts forward suggestions on the implementation of shared decision-making in this group of patients,to provide references for the application of shared decision-making for patients with severe mental disorders in China.

    Objective To construct a platform of post-hospital health management for mothers and infants based on “Internet +”,and to provide corresponding evidence. Methods The post-hospital health management for mothers and infants based on “Internet+” included 2 parts:①Patient APP included service items page and personal information page,supporting personal information registration,previous health file query,team selection and appointment,service evaluation,etc;②Medical APP included personal work station,appointment service application display and consultation communication interface,supporting medical staff registration,viewing appointment services,and consultation. Data from January 25 to August 31,2021 were collected,including the use of platform,online medical team-patient interaction,and maternal satisfaction with the platform. Results The platform has served 990 mothers,and the repeated use rate was 38.28%(379/990). The consultation content covers many aspects of maternal and child care during the puerperium period,of which a total of 42 cases were solved by medical and nursing staff (4.24%);251(25.36%) problems were solved off regular working hours. Conclusion Post-hospital health management platform for mothers and infants based on “Internet +” is convenient,efficient and professional,and it works well. Solving maternal and infant problems in a timely,professional and efficient manner,it improves the participation of mothers and meets their diversified health needs.

    Construction and validation of a risk prediction model for central venous catheter-associated deep venous thromboses in children with congenital heart disease after surgery

    Objective To construct a risk prediction model for central venous catheter-associated deep venous thrombosis(CADVT) in children with congenital heart disease after surgery,and to verify the prediction effect of the model. Methods A total of 234 children hospitalized in the cardiac care unit of a tertiary first-class pediatric hospital in Zhejiang Province from January to November 2021 were conveniently selected as the model building subjects. According to whether CADVT occurred,they were divided into a CADVT group(53 cases) and a non-CADVT group(181 cases). Logistic regression analysis was used to establish the risk prediction model. Hosmer-Lemeshow test was used to determine the goodness of fit of the model. The receiver operator characteristic curve (ROC) was used to test the prediction effect of the model,and a nomogram model was drawn. 60 children with congenital heart disease after surgery in the same hospital from December 2021 to February 2022 were selected as the validation group,and Hosmer-Lemeshow test and ROC curve were used for external verification of the model. Results The incidence of CADVT in children with congenital heart disease after surgery was 22.65%. Intubation duration(OR=1.172),D-dimer concentration(OR=1.169),fibrinogen concentration(OR=3.888),days of sedation(OR=1.538),using of vasoconstrictor(OR=3.554) and pediatric critical illness score(OR=3.308) were its risk factors(P<0.05). The Hosmer-Lemeshow test showed that χ2=4.456,P=0.814,the area under the ROC curve was 0.957(95%CI:0.930~0.984),the optimal critical value was 0.813,the sensitivity was 0.962,and the specificity was 0.851. In the external validation,the sensitivity,specificity and accuracy of the model were 0.857,0.870,86.67%,respectively. Conclusion The risk prediction model constructed in this study has good effects,which can provide a reference for clinical evaluation of the risk of CADVT in children with congenital heart disease after surgery.

    Objective To explore the posttraumatic growth experience of grandparents of children with developmental disabilities under 6 years old,so as to provide a basis for formulating intervention programs to promote their psychological growth. Methods The purposive sampling method was adopted to select 12 grandparents of children with developmental disabilities who received the rehabilitation treatment in a tertiary A hospital in Chongqing from October 2021 to January 2022 as research subjects. Data were collected by semi-structured interviews and face-to-face nonverbal behavior observation method,and data were sorted out with the help of GooSeeker software. The data were analyzed by Colaizzi 7-step analysis method. Results 5 themes were extracted.①The positive psychological derivations:perceived progress of children,looking forward to a better future,positive coping styles.②New life perception:accepting reality calmly,shifting focus of life,valuing health more. ③The enhancement of personal strength:the enhancement of resilience,the enhancement of self-efficacy,optimism and prospect. ④The enhancement of support system:the enhancement of emotional support,the enhancement of social support.⑤Interpersonal harmony:altruistic behavior,improvements in relationships with others. Conclusion Grandparents of children with developmental disabilities under 6 years old have various experiences of posttraumatic growth. Medical staff can help them get more social support through group psychological counseling,setting up psychological support groups and strengthening disease publicity,so as to help them effectively cope with stress and promote posttraumatic growth.

    Objective To learn about the current situation of fear of progression(FoP) in patients with cervical cancer,and to analyze the influence path of FoP in cervical cancer patients by establishing a structural equation model based on Social Cognitive Processing Theory. Methods From August to October 2020,the convenience sampling method was adopted to investigate 222 cervical cancer patients who were hospitalized in Oncology and Gynecology Departments of 10 tertiary hospitals in Hunan province with the general information questionnaire,Fear of Progression Questionnaire-Short Form(FoP-Q-SF),Impact of Event Scale Revised(IES-R),Social Constraint Scale(SCS-15),and Distress Disclosure Index(DDI),while their spouses were surveyed by Fear of Progression Questionnaires for Partners(FoP-Q-SF/P). Results The incidence of FoP mental dysfunction in cervical cancer patients was 43.69%. Social constraint,negative cognitive processing and FoP of the spouses positively affected the FoP of cervical cancer patients with a total effect of 0.427,0.631,0.468,respectively. Marital intimacy and distress disclosure negatively affected the FoP of cervical cancer patients with a total effect of -0.279,-0.454,respectively. Conclusion The incidence of FoP psychological dysfunction in cervical cancer patients is high. In clinical practice,medical staff should pay attention to the impact of social constraints,cognitive processing,distress disclosure,marital intimacy and FoP of the spouses on the FoP in cervical cancer patients,and take targeted nursing measures to reduce the level of FoP in cervical cancer patients.

    Objective To construct a scientific and systematic core competence evaluation index system for nurses participating in ECMO technology,in order to provide references for the training,assessment and competence evaluation of nurses. Methods Through literature review and semi-structured interviews,the preliminary draft of the core competence evaluation index system for nurses participating in ECMO technology was formed in this study. From February to April 2021,25 experts from 6 provinces in China were consulted by Delphi method,and the weight of indexes at all levels was determined by analytic hierarchy process. Results The effective recovery rates of the 2 rounds of expert consultations were 90.00% and 92.59%,respectively;the expert authority coefficient was 0.886,and the Kendall harmony coefficient was 0.205(P<0.001) in the second round. The final core competence evaluation index system for nurses participating in ECMO technology includes 7 first-level indexes,19 second-level indexes and 70 third-level indexes. The first-level indexes include professional knowledge,clinical practice ability,critical thinking ability,professional development ability,communication and coordination ability,management ability,and personal traits. Conclusion The core competence evaluation index system for nurses participating in ECMO technology in this study was scientific and reliable,which can provide valuable references for the training,assessment and competence evaluation of nurses.

    To summarize the nursing experience of a rectal cancer patient with secondary to multiple necrotizing soft tissue infection of abdominal wall. Nursing points include the close monitoring of the changes of the condition,the early identification and effective control of the septic shock and necrotizing soft tissue infection,and the control of the recurrence of systemic infection through multidisciplinary cooperation;cutting and draining the necrotizing soft tissue infection as early as possible to remove the necrotic tissue;using multiple negative pressure combined with continuous drip irrigation and drainage technology to nurse the wound;implementing the individualized analgesia mode and nutrition support program dynamically by stages;carrying out the whole process psychological intervention. After 122 days of careful nursing care,all 8 wounds healed and the patient recovered well.

    To summarize the first aid and nursing experience of an ICU patient with stress cardiomyopathy after kidney transplantation. Nursing points:focus on changes in the condition and implement efficient first aid measures; rapid and accurate assessment,safe and stable transport;strict hemodynamic measurement,goal-oriented fluid management;precise drug administration,observation of drug effects and adverse reactions; emphasis on psychological counseling and rehabilitation training and doing a good job of continuous nursing are keys to the smooth transfer of patients. After careful treatment and care,the patient was transferred to the kidney transplant ward for continuous specialist treatment on the 16th day after the operation. On the 38th day after the operation,he recovered and was discharged from the hospital.

    The nursing care experience of 7 patients with cystic fibrosis is summarized in this article. The critical points of nursing care are recapitulated as followed:collecting the sweat chlorine test specimen to assist diagnosis;promoting airway secretion discharge to improve respiratory function;strengthening electrolyte monitoring to correct electrolyte disorder;promoting rational medicine use to alleviate patient symptoms;developing a nutritional plan to improve patient nutritional status;strengthening discharge education to improve patient quality of life. The result is desirable that symptoms of all patients were alleviated and they were discharged from our center after active treatment and diligent care.

    Objective To extract,summarize and evaluate the best evidence of nutrition management in the elderly with sarcopenia,and to provide bases for guiding nutrition in the elderly with sarcopenia. Methods We searched all evidence about nutrition management for elderly patients with sarcopenia in the UpToDate,Joanna Briggs Institution,Cochrane Library,DynaMed,National Guideline Clearinghouse,Registered Nurses Association of Ontario,Scottish Intercollegiate Guidelines Network,National Institute for Health and Care Excellence,Guidelines International Network,WHO Guidelines,CPG,Yimaitong,PubMed,Web of Science,CNKI,CBM and YIIGLE. The types of the literature include clinical decision-making,guidelines,evidence summaries,expert-consensuses and systematic reviews. The retrieval time is from the establishment of the databases to January 20,2021. Results Finally,18 articles were included in this summary,including 1 guideline,6 expert consensuses,1 clinical decision-making and 10 systematic reviews. 32 pieces of evidence were summarized in 4 categories,including nutrition screening and assessment,nutrient intake,nutrition monitoring and health education. Conclusion Nutrition management can effectively improve sarcopenia. The evidence summary process of this study is scientific,rigorous and comprehensive. Clinical staff should select the best evidence according to the specific situation and individual factors of patients in the process of evidence transformation,so as to guide the elderly patients with sarcopenia to safely and effectively improve their nutritional status and quality of life.

    Objective To evaluate and summarize the evidence of pregnancy guidance for young breast cancer patients,and provide the basis for medical staff to provide pregnancy guidance. Methods The guidelines,expert consensuses,evidence summaries,clinical decisions and systematic evaluations of pregnancy guidance of breast cancer patients were systematically searched from BMJ Best Practice,UpToDate,Cochrane Library,JBI,Pubmed,CINAHL,Embase,US National Guidelines Library,International Guidelines Collaborative Network,Scottish Intercollegiate Guidelines Network,Canadian Ontario Registered Nurses Association website,Web of Science,Yimai Tong,China Guidelines.com,CNKI,Wanfang Medical Network,China Biomedical Literature Service and VIP database from the establishment of the database to November 2021. There were 2 researchers with evidence-based nursing research background who independently completed the quality evaluation of the literature,and combined with the opinions of professionals to extract and summarize the literature that met the quality standards. Results A total of 9 articles were included,including 1 clinical decision,1 guideline,1 systematic evaluation and 6 expert consensuses. Conclusion This study summed the best evidence to provide pregnancy guidance to young breast cancer patients. In clinical practice and application,healthcare professionals need to take the clinical context into account and fully consider the patient’s wishes to maximize patient benefits,in order to further improve the quality of life of patients.

    Objective To search and obtain relevant evidence of leg cramps management in pregnant women,and to provide the evidence for clinical practice. Methods We systematically searched relevant guidelines websites,professional associations websites,as well as Cochrane Library,PubMed,Embase,CNKI,CBM and other databases for all evidence on management of leg cramps during pregnancy,including guidelines,clinical decision-making,evidence summaries,system reviews,expert consensuses,randomized controlled trials from the inception of databases to December 2021. The literature was evaluated and rated for quality by 2 researchers independently. Results Totally,13 articles were enrolled,including 1 guideline,4 clinical decisions,5 systematic reviews and 3 randomized controlled trials. A total of 24 pieces of evidence were summarized,including evaluation and diagnosis,risk factors,basic treatment principles,acute treatment,non-drug intervention and drug intervention. Conclusion This study summarized and formed the management plan of leg cramps in pregnant women,including its assessment and diagnostic points,risk factor management,interventions,etc. Evidence translation should be combined with clinical context and pregnant women’s willingness to select the best evidence to reduce the incidence of leg cramps in pregnant women and improve the quality of life during pregnancy.

    Objective To identify the characteristics and research status of symptom clusters in patients with gynecologic cancer receiving chemotherapy. Methods This scoping review was conducted under the Joanna Briggs Institute guidelines. A literature search was performed in the PubMed,Embase,CINAHL,Web of Science,Medline,Scopus,CNKI,Wanfang,and CBM databases to identify relevant studies about symptom clusters in patients with gynecologic cancer from inception until November 29,2021. The included studies were summarized and analyzed. Results A total of 12 articles were included. The elements of identifying symptom clusters included symptom assessment and analysis methods. There were 13 kinds of symptom clusters during chemotherapy. The psychological symptom cluster and gastrointestinal symptom cluster are the most serious symptom cluster before chemotherapy and after 1st chemotherapy respectively. The body image symptom cluster is the most serious symptom cluster after 3rd and 6th chemotherapy. Conclusion Gynecologic cancer patients receiving chemotherapy experienced multiple symptom clusters with dynamic change. Future research should combine specific symptom measurement for gynecologic cancer patients with advanced machine learning techniques,and explore the construction of an accurate symptom cluster management scheme.

    According to different clinical manifestations,delirium can be divided into 3 subtypes,including hypoactive delirium,hyperactive delirium,and mixed delirium. Each subtype has its own special nursing priorities and clinical outcomes according to different clinical significance and prevalence. This review has summarized the progress of delirium subtypes and assessment tools,in order to provide scientific advice for the selection of appropriate assessment tools for delirium subtypes in the future.

    This paper reviewed the assessment tools of readiness for Advance Care Planning(ACP),summarized the main content,scoring criteria,reliability and validity of the healthy group,patient population and other aspects. We also analyzed and compared the existing problems of the readiness for ACP assessment tools and put forward related suggestions,so as to provide references for the localization development of the readiness for ACP assessment tools,the selection of appropriate assessment tools by clinical nurses and implementation of targeted interventions.
