Early rehabilitation nursing for a pediatric patient undergoing combined heart-lung transplantation with non-intubated spontaneous ventilation anesthesia：a case report

Abstract:

To summarize the early rehabilitation nursing experience for a pediatric patient undergoing combined heart-lung transplantation with non-intubated spontaneous ventilation anesthesia. Nursing measures included：preoperative pulmonary adaptive rehabilitation；postoperative nursing observation for non-invasive mechanical ventilation and complication recognition；safety management of ECMO circuits；leveraging the advantages of non-intubated anesthesia to facilitate early interprofessional collaborative rehabilitation；comprehensive psychological preparation to strengthen the child’s will to live. Following treatment and nursing，the patient was transferred out of the cardiac and great vessel surgery ICU on postoperative day 12 and discharged home on postoperative day 23. At 3-month follow-up，the child demonstrated good recovery of daily activities.

摘要：

总结1例保留自主呼吸非气管插管麻醉下行心肺联合移植术患儿的早期康复护理经验。护理要点：术前肺康复适应性训练；术后即刻无创机械通气的护理观察与并发症识别；清醒体外膜肺氧合支持下的管路安全管理；依托非气管插管麻醉优势，医护患协同共同促进早期康复；充分进行心理护理，增强患儿生存信念。该患儿经过精心的治疗与护理，术后第12天转出心脏大血管外科ICU，术后第23天康复出院，术后3个月随访患儿日常活动恢复良好。
