User experiences with virtual reality in patients with mild cognitive impairment:a Meta-synthesis

摘要:

目的 系统评价和整合轻度认知障碍患者对虚拟现实技术使用体验的质性研究,为制订针对性的干预措施提供参考。 方法 计算机检索10个国内外数据库,收集关于轻度认知障碍患者对虚拟现实技术使用体验的质性研究,检索时限为建库至2025年10月。采用澳大利亚乔安娜布里格斯研究所循证卫生保健中心质性研究文献质量评价标准对纳入文献进行质量评价,运用汇集性整合法整合结果,并对整合结果进行质量评级。 结果 共纳入8篇文献,提炼出31个研究结果,归纳出12个类别,最终形成3个整合结果:①多种复杂的使用体验交织,包括感到技术形式新奇与有趣、身临其境的沉浸感、技术壁垒造成使用困扰;②感知机体功能改善并逐渐掌控生活,包括感知认知功能提高与躯体机能优化、身心愉悦感增强、学会使用应对策略、自觉在日常生活中进行认知锻炼、社会参与积极性提升、感知自我价值复现;③多种技术使用的需求交织,包括需要多方支持与保障、希望进一步优化虚拟现实训练、存在未来持续使用技术的意愿。质性研究整合证据评估工具评分显示,整合证据体质量等级为中等水平。 结论 护理人员应关注患者使用虚拟现实技术的体验,识别患者的认知康复需求,制订针对性的干预策略,预防轻度认知障碍向痴呆发展。

Abstract:

Objective To systematize and synthesize qualitative evidence regarding the experiences of individuals with mild cognitive impairment(MCI) using virtual reality(VR) technology,thereby informing the development of targeted interventions. Methods A systematic computer-based search was conducted across 10 domestic and international databases for qualitative studies focusing on the VR user experiences of MCI patients,covering the period from the inception of each database to October 2025. The methodological quality of the included studies was assessed using the Joanna Briggs Institute(JBI) Critical Appraisal Checklist for Qualitative Research. Findings were synthesized via a meta-aggregation approach,and the quality of the synthesized evidence was evaluated. Results A total of 8 studies were included,from which 31 findings were extracted. These findings were categorized into 12 subcategories,ultimately converging into 3 synthesized findings:1) A complex interplay of multifaceted user experiences encompassing perceptions of novelty and enjoyment,an immersive sense of presence,and challenges arising from technological barriers;2) Perceived improvements in physical and cognitive functioning coupled with an enhanced sense of control over daily life,including perceived cognitive remediation and optimized physical functioning,increased psychological and physical well-being,acquisition of coping strategies,proactive engagement in cognitive exercises in daily routines,heightened motivation for social participation,and a restored sense of self-worth;3) An intertwined set of requirements for technology use,including the need for multifaceted support and assurances,desires for further refinement of VR training programs,and an expressed willingness for sustained future engagement with the technology. The ConQual score indicated an overall moderate level of quality in the synthesized evidence. Conclusion Healthcare providers should closely pay attention to patients’ experiences with VR technology,identify their specific cognitive rehabilitation needs,and formulate tailored intervention strategies to prevent the progression from MCI to dementia.
