银屑病患者自我报告结局评估工具测量学属性的评价及护理启示

Systematic review of measurement properties of psoriasis patient specific self-reported outcome assessment tools and nursing implications

摘要:

目的 对银屑病患者自我报告结局评估工具进行测量学属性的评价和研究方法学质量评价,为临床推荐适用的评价工具提供依据。 方法 检索PubMed、Embase、CINAHL、Web of Science、Cochrane Library、中国知网、维普数据库、万方数据库和中国生物医学文献数据库等,检索建库至2025年6月1日发表的关于银屑病患者自我报告结局评估工具开发或验证的研究,由2名研究人员独立检索和提取文献,对纳入文献的方法学和评估工具的测量学属性等进行评价,并得出推荐意见。 结果 共纳入12项研究,涵盖10种银屑病患者自我报告结局评估工具。其中银屑病关节炎疾病影响问卷和银屑病与银屑病关节炎甲病变生活质量量表在内容效度与内部一致性方面均被评为“充分”,A级推荐;其余8种工具评为B级推荐。 结论 银屑病关节炎疾病影响问卷和银屑病与银屑病关节炎甲病变生活质量量表的整体方法学质量与测量学属性较好,建议在临床中优先选用,但仍需进一步开展验证研究。

Abstract:

Objective To conduct a systematic performance evaluation and methodological quality assessment of patient-reported outcome measures(PROMs) specific to psoriasis,thereby providing evidence to support clinical recommendations for appropriate tools. Methods Databases including PubMed,Embase,CINAHL,Web of Science,Cochrane Library,CNKI,VIP,Wanfang,and CBM were searched from inception to June 1,2025 for studies on the development or validation of psoriasis-specific PROMs. A total of 2 researchers independently conducted the literature search and extraction. The COSMIN bias risk checklist and quality assessment criteria were employed to evaluate the methodological quality of the included studies and psychometric properties of the instruments,with recommendations subsequently formulated. Results 12 studies covering 10 psoriasis-specific PROM tools were included. Among these,the Psoriasis Arthritis Disease Impact Questionnaire(PsAID) and the Nail Psoriasis and Psoriatic Arthritis Quality of Life Scale(NAPPA-QoL) were rated as ‘adequate’ for both content validity and internal consistency,receiving an ‘A’ grade recommendation. The remaining 8 tools were rated as ‘B’ grade. Conclusion PsAID and NAPPA-QoL demonstrate favourable overall methodological quality and measurement performance,warranting priority use in clinical practice. However,further validation studies remain necessary.
