    Effect of exercise intervention based on transtheoretical model on elderly patients with knee osteoarthritis in communities

    Objective The aim of this study is to evaluate the effects of transtheoretical model(TTM) based on exercise intervention mode on adherence of elderly patients with knee osteoarthritis(KOA) in communities. Methods The cluster randomized controlled trial was applied in this study. 14 communities in Beijing were enrolled,and they were randomly divided into 2 groups,and a total of 189 elderly KOA patients who met the inclusion criteria were included. The present study lasted 48 weeks,with the intervention time of 0-24 weeks and follow-up time of 24-48 weeks. The experimental group was given a TTM-based exercise intervention mode,and the control group was given the routine home exercise guidance. The exercise adherence,self-efficacy,decisional balance and pain intensity of knee were compared between the 2 groups. Results A total of 156 patients(87 in the experimental group and 69 in the control group) completed the study. After 24 weeks,the score of exercise adherence was(7.58±1.29) in the experimental group and(5.00±1.53) in the control group. After 36 weeks,the score of exercise adherence was(6.55±1.28) in the experimental group and(3.89±1.53) in the control group. After 48 weeks,the score of exercise adherence was(5.56±1.00) in the experimental group and(3.16±1.31) in the control group. There was significant difference between the 2 groups(P<0.05). Meanwhile,during the study period,the exercise self-efficacy and perceived benefits of patients in the experimental group were significantly higher than those in the control group,and the degree of perceived barriers and pain intensity of knee in the experimental group were significantly lower than those in the control group(P<0.05). Conclusion The TTM-based exercise intervention mode can improve the exercise adherence of elderly patients with KOA for a long time,optimize the exercise self-efficacy and decisional balance,and reduce the symptoms of knee pain.

    Objective To form an expert consensus on parenteral nutrition safety infusion, and standardize the operation process of clinical parenteral nutrition safety infusion,so as to reduce the parenteral nutrition complications. Methods On the basis of the evidence-based method,we searched the high level evidence about the parenteral nutrition safety infusion in databases such as UpToDate,BMJ Best Practice,National Institute for Health and Care Excellence,Guidelines International Network,National Guideline Clearinghouse,Registered Nurses' Association of Ontario,Scottish Intercollegiate Guidelines Network,European Society for Clinical Nutrition and Metabolism,American Society for Parenteral and Enteral Nutrition,Chinese Society for Parenteral and Enteral Nutrition,China Guide Network,Joanna Briggs Institute,China Biomedical Literature Network,VIP,Wanfang,CNKI,PubMed,Web of Science,Scopus,Embase. The retrieval period is from the establishment of the databases to January 2021. Studies were evaluated and collected; relevant recommendations and research results were selected to form the draft of the expert consensus. Experts were invited to complete 2 rounds of Delphi expert consultations and 2 rounds of expert meetings. Each item was revised and improved through 2 rounds of Delphi expert consultations and expert meetings,and the final draft of the consensus was formed. Results The positive coefficient of experts in the 2 rounds of correspondence consultations were 100%; the expert authority coefficients were 81.00% and 89.50% respectively,and the coordination coefficients of expert opinions were 0.13 and 0.15 respectively. Finally,the consensus summarizes the latest evidence on 4 aspects of multidisciplinary team formation,evaluation,operation norms,and complication management. Conclusion The establishment of the consensus can provide a reference for the standardization of safe infusion of parenteral nutrition in clinical practice to ensure patient safety and improve service quality.

    Objective According to the food hierarchical standard of the International Dysphagia Diet Standardisation Initiative(IDDSI),the food management plan for patients with swallowing disorders was constructed based on the swallowing function classification. Methods From January to March 2021, clinical practice guidelines,manuals,expert consensuses,evidence summaries,and systematic evaluation of dietary nutrition management in patients with swallowing disorders were systemically retrieved,and then the evidence was extracted and integrated. The first draft of the diet management plan was formulated by the research group,using Delphi method for 2 rounds of expert consultations,and expert opinions were modified by group discussion. Results 15 experts participated in 2 rounds of consultations,and the effective recovery rate of questionnaire was 100%. The expert authority scores of 2 rounds were 0.88,and the importance and operability of the second round of expert opinions Kendall harmony were 0.396 and 0.412. The diet management plan for patients with swallowing disorders,includes 5 first-level items(failed,level 4,level 3,level 2,level 1) and 41 second-level items. Conclusion There were high levels of coordination,enthusiasm,and authority in experts. The diet management plan of patients with swallowing disorders is scientific and feasible,which can provide references for the diet management of patients with swallowing disorders.

    A survey on the current situation of occupational protection in central sterile supply departments in 46 hospitals in Shanghai

    Objective The purpose of this survey was to investigate the occupational protection status of the staff in the central sterile supply department of Shanghai hospitals,in order to provide references for strengthening the occupational safety of the staff in the central sterile supply departments. Methods A cross-sectional study was designed based on the questionnaire information collected on the staff from 46 hospitals in Shanghai between June 7,2021 to June 20,2021. The questionnaire content included basic information of the staff,types and protection of occupational exposure,frequency,time period,cause of occupational exposure and occupational protection training information etc. Results 588 staff members from 46 hospitals participated in the questionnaire and 207 staff members had occupational exposure with an incidence of 35.20%. Among them,the highest incidence of acute device injury was 26.36%,followed by hearing loss and muscle soft tissue injury or sprain,with 22.28% and 20.07%. 90.82% of occupational exposures occurred during busy working hours. The occurrence of hearing loss was positively correlated with working years. Nurses were significantly at high risk of muscle and soft tissue injury or sprain,compared with sterilizers and workers. A total of 548 staff members(93.20%) have received the occupational safety-related protection knowledge training. Conclusion The incidence of occupational exposure of central sterile supply departments in 46 hospitals in Shanghai is high. The management personnel should shift flexibly,standardize the operation process,strengthen occupational protection measures,and strengthen training to reduce the occurrence of occupational exposure.

    Systematic evaluation of a fall risk prediction model in convalescent stroke patients

    Objective To systematically analyze and evaluate the prediction model of fall risk in convalescent stroke patients,in order to provide references for clinical practice. Methods Original articles in Chinese and English were systematically searched from Chinese databases(CNKI and Wanfang) and English databases(PubMed,Embase,Cochrane) published prior to June 2021. There were 2 researchers who independently screened the literature,extracted information,and analyzed and compared the characteristics of the population,study types,predictive factors,model construction methods and model prediction results. Results This systematic review included 12 pieces of the literature on the development of predictive models,including 15 models. In these models,10 models reported the area under the curve and 4 models simultaneously reported the confidence interval of the area under the curve;4 models showed good differentiation in the internal verification population. In addition,only 6 models were tested for goodness of fit test,4 of them reported P value,and the results showed that the consistency was good. All included studies have low concerns regarding applicability,but with a certain bias,which mainly due to insufficient number of dependent events in the field of analysis,neglect of missing data,screening predictors based on univariate analysis,lack of model performance evaluation and over-simulation of the model. Conclusion There are still some shortcomings in the fall risk prediction model of convalescent stroke patients. Future research should improve the research design and research report,and carry out internal and external verification.

    Objective To search and evaluate the systematic review of telerehabilitation management in stroke patients,providing references for clinical decision making and continuous improvement of rehabilitation exercise intervention strategies. Methods Systematic reviews/meta-analysis of telerehabilitation management of motor function in stroke patients was searched in Cochrane Library,Embase,PubMed,Web of Science,CINAHL,Joanna Briggs Institute(JBI),China National Knowledge Infrastructure,China Science and Technology Journal Database,Wanfang Database,and Chinese Biomedicine Literature Database. The search time limit was from the inception of the database until September 19,2021. 2 researchers trained in evidence-based knowledge system applied AMSTAR (Assessment of Mutiple Systematic Reviews) to evaluate the methodological quality of the literature. GRADEE (Grades of Recommendations Assessment,Development and Evaluation) system was used for evidence summary and grading. Results There were 124 preliminary studies in this study,of which 14 were included. AMSTAR quality evaluation results showed that there were 5 studies of medium quality,and the rest were all of high quality,indicating that the overall research methodology of the included literature was of high quality. According to the evidence quality evaluation results of GRADE system for 67 main outcome indicators of 14 articles,22 pieces of evidence were medium,29 pieces of evidence were low,and 16 pieces of evidence were extremely low. Conclusion The methodological quality of systematic evaluation included in this study is high,but the evidence quality of outcome indicators is low. Telerehabilitation exercise management is beneficial to improve patients’ motor function,self-care ability and quality of life,and improve patients’ satisfaction and participation. However,the design,operation and evaluation of the outcome index of telerehabilitation management still need to be improved.

    Objective To retrieve,evaluate and summarize the best evidence of safety management of intra-hospital transport in patients on extracorporeal membrane oxygenation(ECMO),and to provide a reference for clinical practice. Methods The BMJ Best Practice,UpToDate,Registered Nurses’ Association of Ontario,National Guideline Clearinghouse,National Institute for Health and Care Excellence,Yimaitong,Joanna Briggs Institute Library,Cochrane Library,CINAHL,PubMed,CNKI,Wanfang Database and Extracorporeal Life Support Organization Website were researched to collect the literature,including guidelines,consensuses,evidence summaries,systematic reviews,and well-designed original studies. The time limit for the retrieval is from the inception of databases until August 2021. Results 11 pieces of the literature were retrieved,including 5 consensuses,2 system reviews,1 randomized controlled trial,1 quasi-experimental study,1 cohort study and 1 cross-section survey. Totally 30 pieces of evidence were summarized,including team composition and ability requirements,transport preparation,monitoring during transport,prevention and control of adverse events and management of handover. Conclusion It is suggested to build a professional ECMO intra-hospital transport team based on the actual hospital situation,to apply the best evidence to standardize the intra-hospital transport process of patients on ECMO,and to establish an adverse event prevention and control system and information communication mechanism to ensure patient safety.

    Standard precautions is an important strategy to prevent nosocomial infection. Scientific and effective compliance with standard precautions measures can help to reduce the probability of patients and medical service providers acquiring nosocomial infection and to ensure patient safety. This paper summarized the characteristics and application of the self-report assessment tools for standard precautions compliance and influencing factors of nursing staff. We also proposed and analyzed the existing problems and put forward related suggestions for assessment tools,so as to provide references for clinical nurses to select appropriate assessment tools.

    Objective To explore the effect of the empowerment intervention on patients with biliary calculi carrying T tube. Methods From June 2019 to August 2021,102 biliary calculi patients undergoing surgery carrying T tube in our hospital were randomly assigned to a control group or an experiment group,with 52 cases in each group. Traditional nursing was for the control group,while the empowerment nursing intervention was implemented in the experiment group. Readiness for hospital discharge scale(RHDS) was used to evaluate the discharge readiness of the 2 groups at discharge. The 2 groups were compared in terms of rehabilitation indicators such as self-care ability,intubation time of T-tube and postoperative complications. Anxiety and depression of all patients were assessed by the hospital anxiety and depression scale(HADS) at 3 times of admission,discharge and 1 month after discharge. Results This study was completed among 48 patients in control group and 47 patients in experiment group. The scores for RHDS and self-care ability in the experiment group were higher than those in the control group(P<0.05),while T-tube complications were lower in the experimental group(P<0.05). The scores of anxiety and depression in the control group were higher than those in the experimental group at discharge and 1 month after discharge(P<0.05). There was no significant difference in intubation time of T-tube,hospitalization cost,postoperative hospitalization days and readmission rate within 3 months after operation between 2 groups. With the prolongation of postoperative time,anxiety and depression scores in each group decreased gradually,and there were time interaction and intergroup interaction between 2 groups(P<0.05). Conclusion The empowerment strategy is effective for improving the readiness for hospital discharge and self-care ability of biliary calculi patients carrying T tube,and it may reduce complications and negative emotions during catheterization.

    Objective To establish an exercise rehabilitation program for patients treated with lower limb bone transport that is suitable for our country,to standardize the management and guide clinical nursing practice,and to promote the rehabilitation of patients. Methods Based on evidence-based methods to retrieve domestic and foreign literature,combined with content analysis,the first draft of the exercise rehabilitation program for patients treated with lower limb bone transport was developed. From April to June 2021,21 experts were consulted by the Delphi method for 2 rounds of correspondence,and the final program was revised and established according to the experts’ opinions. Results The positive coefficients of 2 rounds of expert inquiries were 84.00% and 100%,respectively;the authority coefficient was 0.89;the coordination coefficients were 0.10 and 0.17,respectively(P<0.001). The final program contained 5 primary indicators,15 secondary indicators and 28 tertiary indicators. Conclusion The constructed exercise rehabilitation program for patients treated with lower limb bone transport is scientific and practical,which can provide a theoretical basis and practical direction for the rehabilitation and nursing of patients treated with lower limb bone transport.

    Risk analysis of readmission of preterm infants with bronchopulmonary dysplasia within 2 years of age based on the family participatory care model in the NICU

    Objective To explore the influencing factors of re-admission of premature infants with bronchopulmonary dysplasia(BPD) within 2 years under the family participatory care model. Methods BPD premature infants with gestational age of 32 weeks or less admitted to the neonatal intensive care unit of a tertiary hospital in Wenzhou from January 2017 to December 2018 were selected as the research subjects. They were divided into a non-readmission group and a readmission group according to whether they were readmitted between the first discharge to 2 years old. Their clinical data during hospitalization and follow-up data under family participatory care were retrospectively analyzed,and the influencing factors of readmission were analyzed. Results A total of 178 BPD preterm infants were followed up,of which 98(55.06%) were not readmitted and 80(44.94%) were readmitted. Logistic regression analysis showed that the number of receiving nursing education was over 3 persons(OR=5.806) and severe BPD(OR=2.725) were the risk factors for BPD premature infants to be readmitted to hospital within 2 years of age(P<0.05). Receiving home oxygen therapy(OR=0.311) was a protective factor for read-mission(P=0.001). Conclusion During the period of carrying out family participatory care in the hospital,the number of receiving nursing education should not be too much,and the contents should be homogenized and standardized. Families of premature infants with moderate to severe BPD need to receive necessary training in home oxygen therapy,and the conducting of home oxygen therapy should be paid attention during discharge follow-up.

    Construction of a risk prediction model for patients with post-intensive care syndrome after cardiovascular surgery and its prediction effect

    Objective To investigate the risk factors of post-intensive care syndrome(PICS) in patients after cardiovascular surgery,and to establish a risk prediction model and conduct internal validation to verify the prediction effect. Methods A total of 304 patients after cardiovascular surgery in 2 tertiary A hospitals in Changsha were selected by convenience sampling method from July to October 2019. The patients were followed up 3 months after they were transferred out of ICU. According to the occurrence of PICS during follow-up,the patients were divided into a PICS group (n=103) and a non-PICS group (n=201). Risk factors of PICS were screened and a predictive model was established based on Logistic regression results. The area under receiver operating characteristic(ROC) curve was used to test the prediction effect of the model,and Bootstrap method was used to re-sample 1 000 times for internal verification of the model. Results The final model was Logit P=0.722×gender+0.903×age+0.968×score of CD-RISC+1.073×pre-operative left ventricular ejection fraction+0.706×length of operation+0.797×length of ICU stay-3.212. The area under ROC curve of the prediction model was 0.817;the sensitivity was 59.2%;the specificity was 85.6%. The results of internal verification by Bootstrap method showed that the C index was 0.804,indicating good prediction effect. Conclusion The risk prediction model for patients with PICS after cardiovascular surgery constructed in this study has good predictive efficacy,which helps to predict the risk of PICS and provides a reference for the implementation of relevant intervention measures in the future.

    Caring experience of donors and caregivers of pediatric living related liver transplantation:a qualitative study

    Objective To understand the caring experience of donors and caregivers of pediatric living related liver transplantation. Methods Using descriptive phenomenological research method,in-depth interviews were conducted with 11 donors and caregivers of pediatric living related liver transplantation from liver transplantation center of a tertiary hospital in Hangzhou from May to August 2021. Colaizzi 7-step analysis method and Nvivo software were used to analyze and extract the topics. Results 4 themes were extracted,including obvious physical and mental distress(significant physical symptoms and heavy psychological burden),adaptation conflict of multiple roles (adaptation conflict of donor and caregiver role,adaptation conflict of child caregiver and other family member caregiver role,adaptation conflict of family and social role),contentment and coping positively,gratitude and feedback to the society. Conclusion Donors and caregivers of pediatric living related liver transplantation have both burden and positive feelings throughout the care process. Medical staff should take targeted intervention measures to reduce the burden of feeling,inspire positive experience,guide coping positively,so as to improve the quality of life of donors and caregivers of pediatric living related liver transplantation,and then improve the quality of care.

    Development of a social resource support scale for pregnant women with gestational diabetes and its test of validity and reliability

    Objective To develop a social resource support scale of pregnant women with gestational diabetes and to test its reliability and validity. Methods Based on the social-ecological model,the initial scale was formed through literature analysis,open-model expert consultation,and 2 rounds of the Delphi method. The convenience sampling was adopted to select 301 pregnant women with gestational diabetes from January to August 2021 in 3 hospitals in Fujian Province of China. The items screening and the test of reliability and validity were carried out. Results The scale includes 7 dimensions,38 items,and 7 additional evaluation items of various dimensional importance,which come to 45 items in total. The positive coefficients of 2 rounds of letter consultations were 100% and 85.71%;the authority coefficient were all 0.879;the coordination coefficients were 0.218 and 0.298. The Cronbach’s α coefficient was 0.935;the test-retest reliability was 0.724;the content validity was 0.970. The exploratory factor analysis identified 7 principal factors,and the accumulative contribution of variance was 63.210%. Conclusion The social resource support scale of pregnant women with gestational diabetes has good reliability and validity. It can be used as a tool for medical staff to evaluate the social support of pregnant women with gestational diabetes.

    Objective To explore the formation process of career capital of specialist nurses,and to provide bases for the training and management of specialist nurses. Methods Using the qualitative research method of grounded theory,16 nurses were interviewed by semi-structured in-depth interviews. The data were analyzed by the research method of grounded theory,and the core categories were extracted to form the theoretical category. Results The main categories of story lines are “know how”,“know why” and “know who to be ”,and the core category that the career capital construction of specialist nurses is a self-actualization oriented career embedding process is obtained. Conclusion The composition of professional capital of specialist nurses integrates the process of improving professional ability,increasing psychological resources and constructing role identity. It is suggested that nursing managers should pay attention to career capital construction of specialist nurses in order to promote the transformation of career capital to job performance.

    Congenital tufting enteropathy is rare to see in clinic,and it could be life-threatening due to intractable diarrhea in the early stage. The nutrition management was the most important and difficult part of nursing care. This study aimed to summarize the nutritional support experience of a case with congenital tufting enteropathy. In this case,we provide the personalized nutrition support based on nutrition screening and evaluation,and carry out parenteral nutrition support as soon as possible to help the child get fully potential of growth,as well as to get the metabolism balance. Besides,the timely enteral nutrition support was also used to improve the nutritional status of the child. Prevention and timely intervention for common complications were used to achieve clinical benefits. After a 60-day nursing care,the child’s condition was well-controlled with steady increasing in height and weight.

    Objective To collect and summarize the best evidence of bathing for premature infants. Methods We systematically searched the British Medical Journal best practice,UpToDate,the Agency for Healthcare Research and Quality(AHRQ),Registered Nurses’ Association of Ontario(RNAO),National Institute for Health and Care Excellence(NICE),Canadian Medical Association(CMA),Guidelines International Network(GIN),WHO,American College of Physicians(ACP),Joanna Briggs Institute(JBI),Association of Women’s Health,Obstetric and Neonatal Nurses (AWHONN),Cochrane Library,PubMed,CNKI and Wanfang database to collect the literature including best practice,evidence summaries,guidelines,systematic reviews,expert consensuses,etc. The retrieval period was from January,2011 to April,2021. 2 researchers independently evaluated the quality of the literature,combining with the judgment of professionals,to extract the data from the literature meeting the standards. Results A total of 5 articles were incorporated,including 3 guidelines,1 expert consensus,and 1 evidence summary. The best evidence included 27 pieces of evidence in 6 aspects,including bathing assessment,bathing supplies,bathing timing,bath management,rewarming after bath and vernix protection. Conclusion The research summarized the best evidence of bathing for premature infants. The best evidence should be selected based on the environment,conditions and patient’s willingness,in order to reduce complications and adverse events and to ensure bath safety.

    Objective To search,evaluate,and summarize the best evidence of non-pharmacological interventions to improve cooperation during anesthesia induction in children undergoing surgery,providing evidence-based evidence for clinical practice. Methods According to the “6S” evidence resource pyramid model,the domestic and foreign guide networks and databases including UpToDate,Cochrane Library,PubMed and Yimai Tong Guide were systematically searched. All the evidence of non-pharmacological interventions to improve anesthesia induction coordination in children undergoing surgery was involved,including guidelines,systematic reviews,evidence summaries,and expert consensuses. The retrieval time limit was from the establishment of the database to October 1,2021. Researchers conducted independent quality evaluation of the literature according to the relevant literature quality evaluation criteria,and they standardized data extraction of the final included literature based on professional judgment. Results Totally 15 pieces of the literature were included,including 7 clinical practice guidelines,7 systematic reviews and 1 expert consensus. Finally,20 pieces of best evidence about preoperative education,operating room environment setting,distraction technology,social intervention and the use of breathing masks were summarized. Conclusion The study summarizes the best evidence of non-pharmacological interventions to improve the anesthesia induction cooperation in children with surgery. Non-pharmacological interventions have high safety and operability for nurses,and clinical medical staff can combine the clinical situation and specific policies to carry out the evidence for clinical transformation and application,so as to improve the degree of anesthesia induction cooperation in surgical children.

    Translation of the Breastfeeding Motivation Scale and the test of its reliability and validation

    Objective The purpose is to translate the Breastfeeding Motivation Scale(BMS) based on Self-Determination Theory,and to test its reliability and validity. Methods According to Brislin model,the original scale was translated and back-translated;after cultural adjustment and pretest,the Chinese version of the scale was determined. 312 primiparas were investigated by convenience sampling method in a tertiary hospital in Nanjing to evaluate the reliability and validity of the scale from December 2020 to May 2021. 270 primiparas were investigated by convenience sampling method for confirmatory factor analysis from June to October 2021. Results The Chinese version of BMS was divided into 4 dimensions with a total of 24 items,namely enjoyment(with 8 items),bonding with the baby and maternal self-perception(with 8 items),pressure from significant others(with 4 items),and instrumental needs(with 4 items). The item level content validity index of the Chinese BMS was 0.830~1.000;the level validity index was 0.901. The scale did not calculate the total score,and the Cronbach’s α coefficient of each dimension was 0.709~0.934;the split-half reliability was 0.717~0.930. Confirmatory factor analysis showed that the scale had good construct validity. Conclusion The Chinese version of BMS has good reliability and validity,and it can be used for breastfeeding motivation assessment and nursing research,which can improve the positive psychology of breastfeeding.
