    Effect of mindfulness-based stress reduction combined with exercise training on sleep in elderly patients with stable chronic obstructive pulmonary disease

    Objective To investigate the effects of Mindfulness-based Stress Reduction(MBSR) combined with exercise training on sleep structure and sleep quality in elderly patients with stable chronic obstructive pulmonary disease(COPD),and to provide an objective reference for improving the sleep quality of patients with stable COPD in old age. Methods Using the convenient sampling method,23 elderly stable COPD patients who were admitted or re-diagnosed at a tertiary A hospital in Beijing from April 2019 to July 2020 were selected as the study subjects. Through self-control,patients were given a 6-week MBSR combined with exercise training interventions. Bracelet-type sleep monitoring devices,watch-based blood oxygen meters,and the Pittsburgh Sleep Quality Index(PSQI) were used to assess differences in sleep structure,nighttime oxygen saturation parameters,and sleep quality before and after the intervention. Results After intervention,the proportion of NREM 3 stage in patients increased from(9.26±2.39)% to(13.08±3.33)%;the lowest oxygen saturation at night increased from(84.05±5.40)% to (87.45±4.17)%;the Oxygen Desaturation Index decreased from(16.14±10.60) cycles/h to(9.95±6.12) cycles/h;the low oxygen ratio increased from[4.70(0.67,11.60)]% to[1.25(0.15,4.02)]%;the PSQI score reduced from(6.65±2.85) scores to(4.55±2.11) scores;the differences were statistically significant(P<0.05). Conclusion MBSR combined with exercise training can optimize the sleep structure and improve night oxygenation and sleep quality in patients with elderly stable COPD.

    Objective To develop the management information system of recognition and prevention of flap vascular crisis,and to explore its application effect in the prevention and nursing management of the vascular crisis in patients with flap transplantation. Methods A research group of flap vascular crisis was established to construct the management information system of flap vascular crisis identification and prevention and control. The man-machine interaction technology was used to develop 5 modules,including addition of patient and basic information,flap labeling,flap observation,early warning of crisis,and nursing documents. It was officially put into clinical application in December 2019. The incidence of timely detection rate of vascular crisis,the incidence of vascular crisis,and survival rate of flap were compared before and after the application of the system. The satisfaction of medical staff was investigated. Results After the application of the information system,the timely detection rate of vascular crisis increased from 33.33% to 92.31%(P=0.007);the incidence of vascular crisis increased from 8.18% to 10.24%(P=0.587);the survival rate of flap increased from 94.55% to 99.21%(P=0.083). Conclusion The interface of management information system for the recognition and prevention of flap vascular crisis is simple and easy to operate. It can provide nurses with accurate and objective decision-making basis,improve the timely detection rate of vascular crisis,and help to further ensure patient safety.

    Objective To explore the effect of hospital-home continuum nursing based on a cloud follow-up platform in elderly patients with radical prostatectomy. Methods Convenience sampling method was used to select 80 prostate cancer patients who planned to receive radical surgery in a tertiary A a hospital in Xinjiang from July 2020 to February 2022 as the research subjects,and they were divided into an experimental group and a control group according to the operation time(40 cases in each group). The experimental group received hospital-home continuous care based on a cloud follow-up platform,while the control group received routine perioperative care and discharge follow-up.The degree of urinary incontinence and pelvic floor muscle strength were compared between the 2 groups on the day,2 weeks,1 month,3 months,and 6 months after extubation,and the quality of life and health knowledge of urinary incontinence at 6 months after catheter removal were compared. Results With the help of the cloud follow-up platform,the hospital-home continuous care was implemented at 2 weeks,1 month,3 months,and 6 months after the removal of the urinary catheter. The pelvic floor muscle strength scores of the experimental group were higher than those of the control group,and the urinary incontinence scores were all lower than the control group.The difference was statistically significant(P<0.001);the scores of quality of life and urinary incontinence health knowledge in the experimental group at 6 months after extubation were 93.50(88.25,100.00) and 20.00(20.00,20.00),which were higher than those in the control group of 57.00(42.75,77.00) and 1.00(9.00,11.00). The difference was statistically significant(P<0.001). Conclusion The application of hospital-home continuum nursing based on the cloud follow-up platform in elderly patients with radical prostatectomy can improve the patient’s pelvic floor muscle strength and urinary incontinence health knowledge,improve the patient’s urinary incontinence problem,and improve their quality of life.

    Effects of mindfulness training based on WeChat live broadcast on subthreshold depressive symptoms after PCI

    Objective To investigate the effect of mindfulness training based on WeChat live broadcast on subthreshold depressive symptoms and quality of life in patients with subthreshold depression after PCI. Methods A total of 64 patients with subthreshold depression after PCI in the department of cardiology of a tertiary hospital in Tangshan city from October 2020 to April 2021 were randomly selected by convenience sampling method as the research subjects. 2 out of 4 wards were randomly selected by drawing lots,and the experimental group and the control group were determined by coin toss,with 32 cases in each group. The experimental group was given mindfulness training on the basis of routine nursing,and the control group was given routine nursing. Subthreshold depressive symptoms,mindfulness level and quality of life were assessed by the Chinese version of Cardiac Depression Scale(C-CDS) and the Chinese version of Five Facet Mindfulness Questionnaire(Ch-FFMQ) and the Seattle angina questionnaire(SAQ) before the intervention(T1),after the intervention(T2) and 3 months after the intervention(T3). Results Before the intervention,there was no statistical significance in the level of mindfulness,subthreshold depressive symptoms and quality of life(P>0.05). After the intervention and 3 months after the intervention,compared with the control group,the scores of subthreshold depressive symptoms in the experimental group decreased(LS mean difference of -18.241 and -10.270,respectively),and the scores of mindfulness level increased (LS mean difference of 31.148 and 13.856,respectively),with statistical significance(P<0.001). The quality of life score was higher than that of the control group,but the difference was not statistically significant(P>0.05). 3 months after the intervention,there was statistically significant difference in the incidence of subthreshold depressive symptoms between the 2 groups(OR=0.564,P=0.032). Conclusion Mindfulness training can effectively relieve subthreshold depression symptoms of patients after PCI,but it cannot improve their quality of life.

    Objective To explore the effect of virtual reality(VR) intervention on improving preoperative anxiety of adult patients. Methods We searched PubMed,Embase,Web of Science,Cochrane Library,CNKI,Wanfang Data from the inception to February,2022. There were 2 researchers who independently screened the literature,extracted information. RevMan 5.4 was used to analyze the data after the quality of literature was assessed. Results 12 randomized controlled trials were finally included,with a total of 1247 patients. The results of meta-analysis showed that the preoperative exposure based on VR technology[SMD=-0.45,95%CI(-0.61,-0.29),P<0.01] and distraction[SMD=-0.24,95%CI(-0.44,-0.04),P<0.05] could effectively reduce the preoperative anxiety of adult patients. Conclusion The preoperative VR intervention can effectively alleviate the preoperative stress of adult surgical patients by VR distraction or VR exposure.

    Atrial fibrillation is one of the main causes of stroke,heart failure,dementia and sudden death worldwide,with high rate of disability and mortality. Mobile health has a significant effect on the health management of patients with atrial fibrillation. This paper introduces the definition,form and application of mobile health in the health management of patients with atrial fibrillation,including atrial fibrillation screening,risk assessment,self-monitoring,health education,medication management and risk factor intervention. It also summarizes the existing problems and puts forward proposals,so as to provide references for the development of professional and high-quality mobile health technology for the health management of patients with atrial fibrillation in China.

    Construction and validation of risk prediction model of psychological distress in patients with cancer

    Objective To explore the influencing factors of psychological distress in patients with cancer,to analyze the risk index of patients with psychological distress,and to develop a nomogram model to predict these risks. Methods In this cross-sectional study,a total of 223 patients with cancer from the oncology department of a tertiary A hospital in Jingmen were selected as the research subjects from December 2020 to July 2021,and the independent risk factors of psychological distress through univariate analysis and Logistic regression analysis were explored. According to these selected independent risk factors,a risk prediction model was established and incorporated into the nomogram,and the model power and its accuracy were evaluated by receiver operating characteristic. Results The risk prediction model was constructed based on 8 influencing factors,including pathological stage,primary caregivers,Pittsburgh sleep quality index score >9.5,numerical rating scale score (1 point for each increase),temperament type,coping style,financial problems and exercise. The area under the predicted ROC curve was 0.924. When the Yoden index was 0.699,the optimal critical value was 0.513,the sensitivity was 0.866,and the specificity was 0.833. The slope of the calibration plot was close to 1. Conclusion The prediction model constructed in this study has good prediction effects which can provide references for medical staff to quickly identify the risk of psychological distress in cancer patients and timely take preventive management measures.

    Objective To analyze the influencing factors of inadequate bowel preparation in elderly patients undergoing colonoscopy,to construct a risk score model,and to provide the evidence for targeted intervention measures. Methods A total of 610 elderly patients undergoing colonoscopy in a tertiary A hospital in Taiyuan from May 2021 to October 2021 were enrolled by convenience sampling method,and they were divided into a development group (406 cases) and a validation group (204 cases) by random number table method. The patients in the development group were divided into a successful group and a failed group according to the quality of bowel preparation. Then the Logistic regression analysis was used to analyze the risk factors and construct risk score model. Hosmer-Lemeshow χ2 test and the area under the ROC curve were used to evaluate the predictive effect of the model. Results Logistic regression analysis showed that age>65 years,BMI≥24,constipation,diabetes,history of colorectal surgery,and the first experience of colonoscopy were independent risk factors for inadequate bowel preparation in elderly patients,and the corresponding risk score were 2,2,3,2,5,4 points,respectively (high-risk group included patients scored≥8 points). In the development group,the results showed that Hosmer-Lemeshow χ2=8.969,P=0.255;the area under ROC curve was 0.717(P<0.001),with the sensitivity and specificity of 50.89% and 79.59%. In the validation group,the results showed that Hosmer-Lemeshow χ2=4.188,P=0.840;the area under the ROC curve was 0.720(P<0.001),with the sensitivity and the specificity of 48.38% and 81.69%,and the accuracy was 71.57%. Conclusion The risk score model constructed in this study has good predictive effects and it can provide the reference for clinical assessment of inadequate bowel preparation risk in elderly patients undergoing colonoscopy.

    Objective To explore the perceptions and suggestions of nurses in long-term care of older patients with mild cognitive impairment(MCI) on the participation of these patients in shared decision making,and to provide references for promoting the implementation of shared decision making. Methods Using the descriptive phenomenological research method,semi-structured interviews were conducted from November 2020 to February 2021 with nurses from 3 tertiary hospitals in Beijing who had the experience of providing health-care services to older patients with MCI,and Colaizzi 7-step analysis method was used to analyze the data. Results Perception of nurses on elderly MCI patients’ participating in shared decision making can be summarized into 5 themes:①respect for patients’ preferences,②guarantee of comprehensive improvement of nursing quality,③limitations by patients’ decision-making ability,④negative emotional coping affected patients’ participation,⑤clinical implementation was restricted. Suggestions from nurses were as follows:①further inspire patients’ decision-making awareness,②call for family support,③expect to improve the implementation conditions. Conclusion Shared decision making has important practical significance,but there were some obstacles in its clinical implementation. In the future,it is necessary to strengthen the decision-making consciousness of patients,cultivate the decision-making ability of nurses,improve the implementation conditions of shared decision making,so as to give full play to the guiding role of nurses.

    Objective To develop the questionnaire of community nursing needs assessment tool for middle and advanced Parkinson’s disease and to test its reliability and validity. Methods Based on Maslow theory of hierarchical needs and Johnson behavior theory,using literature review,semi-structured interviews and Delphi,the primary questionnaire was initially formulated on community nursing needs for middle and advanced Parkinson’s disease,and the questionnaire was adjusted and revised through pre-survey. 400 patients with middle and advanced Parkinson’s disease in Chongqing from March to October 2021 were investigated in the form of face-to-face survey,and the reliability,validity and needs were further analyzed. Results The community nursing needs questionnaire for middle and advanced Parkinson’s disease contains 39 items in 6 dimensions,including daily life care needs,fall prevention needs,autonomic dysfunction needs,emotional support needs,disease treatment needs and continuing care needs. The Cronbach’s α coefficients of the questionnaire was 0.922,and the split-half reliability of the questionnaire was 0.702. The total content validity index of the questionnaire was 0.907 and each item ranged from 0.78 to 1.00 Exploratory factor analysis extracted 6 main factors and the cumulative variance contribution rate was 68.952%. Confirmatory factor analysis showed that the model fit well and the factor structure was stable. Conclusion The questionnaire has good reliability and validity,which can be used as a specific tool to comprehensively reflect the needs of patients for community nursing support,and it can also be used as an auxiliary tool for dynamic evaluation of disease symptom control to guide nurses to carry out health assessment,management and education.

    Objective To develop a recurrence risk perception scale for patients with ischemic stroke and to test its reliability and validity. Methods With the risk perception theory and health belief model as the theoretical framework,the initial scale was developed through literature analysis,qualitative interviews,and expert consultation. A questionnaire survey was conducted on 430 patients with ischemic stroke who were treated or reviewed in a tertiary hospital in Xi’an from June 2020 to October 2021 to test the reliability and validity of the scale. Results The recurrence risk perception scale for patients with ischemic stroke included 25 items in 4 dimensions. In the critical ratio analysis,the difference in scores between high score group and low score group was statistically significant(P<0.05). The exploratory factor analysis extracted 4 common factors with the cumulative variance contribution rate of 70.164%. The results of confirmatory factor analysis showed that the main evaluation indicators were all within the acceptable range of the judgment standard. The item-level content validity index ranged from 0.867 to 1.000;the scale-level content validity index/universal agreement was 0.821;the scale-level content validity index/average was 0.979. In the criterion-related validity analysis,the total score of the scale was positively correlated with the total score of the calibration scale(r=0.522,P<0.01). The Cronbach’s α coefficient,half reliability and retest reliability of the overal scale were 0.905,0.763,and 0.897,respectively. Conclusion The recurrence risk perception scale for patients with ischemic stroke has good reliability and validity,which is suitable for assessment of recurrence risk perception in related populations.

    Objective To explore the role of constructing the prehospital management department in the operation of surgical departments. Methods 177 patients who underwent early liver cancer resection,knee replacement and hip replacement from September 2020 to March 2021 were selected as the experimental group. The experiments were conducted by the prehospital management team. 261 patients with the same disease who received the routine admission process from February 2020 to August 2020 were included in the control group. The average preoperative waiting time,length of stay and hospitalization expenses of the 2 groups were compared. Results The average preoperative waiting time and hospital stay in the experimental group were significantly less than those in the control group(P<0.001). The average hospitalization cost in the experimental group of hip replacement was significantly lower than that in the control group(P<0.05),but there was no significant difference in the early liver cancer resection and knee replacement between the 2 groups(P=0.058). Conclusion The intervention of the prehospital management department can apparently improve the efficiency of surgical departments,making full use of medical resources and greatly alleviating the shortage of medical resources. The management mode is worth of promoting.

    Investigation of nursing prevention and control practice of central line-associated bloodstream infections in ICU of 183 tertiary hospitals

    Objective To understand the clinical practice of prevention and control of ICU central line-associated bloodstream infections(CLABSI) of tertiary hospitals in China,to evaluate the weakness in nursing practice and explore the common problems. Methods From September 1st to 15th,2021,the convenience sampling method was adopted,and the self-designed “Questionnaire on current status of nursing practice for prevention and control of CLABSI” was used in 183 tertiary hospitals in 26 provinces/autonomous regions/municipalities to select ICU nurses in the hospitals. The questionnaire consists of 3 parts and 29 items. Results A total of 336 questionnaires were collected,with an effective rate of 100%. Among them,294(87.50%) ICUs filled in the incidence of CLABSI,and the incidence ranged from 0 to 20.01‰ with a median of 0.77‰. In the central vascular catheterization process,275(81.85%) ICUs had catheterization inspectors;77(22.92%) ICUs used the sterile towel to cover the whole body during the puncture;48(14.29%) ICUs used Statlock to fix the catheter;52(15.48%) ICUs selected chlorhexidine alcohol as skin disinfectant;only 23(6.85%) departments had the standard of disinfection scope during catheterization. In the central line maintenance process,148(44.05%) used pre-flushing catheter flushing fluid to flush the tube;123(36.61%) used pre-flushing catheter flushing fluid to seal the tube;71(21.13%) ICUs adopted chlorhexidine for sponge bath;253(75.30%) ICUs have procedures for using infusion connectors;162(48.21%) ICUs wiped the infusion connectors with an alcohol cotton pad;195(58.04%) ICUs wipe time is more than or equal to 15 s. Different ICUs have great differences in the implementation of key process with statistically significant differences. Conclusion At present,the relevant systems and procedures to prevent CLABSI have been improved,but the prevention and control measures in the aspects of central line catheterization,maintenance,and the use of infusion joints need to be further detailed. It is recommended to further strengthen and implement the training related to the guidelines,and to formulate specific measures to promote the implementation of the guidelines.

    Objective To evaluate and summarize the relevant evidence of exercise intervention in peritoneal dialysis patients,so as to provide evidence-based bases for making the exercise guidance flow chart of peritoneal dialysis patients. According to the best evidence,the exercise guidance flow chart of peritoneal dialysis patients is made to facilitate clinical nurses to guide the exercise of peritoneal dialysis patients. Methods The computer search includes the World Health Organization,BMJ(best practice),the website of the National Institute for health and clinical optimization,the international guidelines collaboration network,the Scottish inter school guidelines website,the Joanna Briggs Institute(JBI),the U.S. national guidelines library,up to date,Embase,PubMed,Web of Science,Cochrane Library. Relevant literature on exercise of peritoneal dialysis patients in China biomedical literature database,Yimaitong,China knowledge network,Wanfang database and VIP database. The retrieval time is from the establishment of the database to November 1,2021. The quality of literature was evaluated and the evidence was extracted. The expert group meeting method was conducted to integrate the evidence and the flow chart of sports guidance was made. Results Finally,5 pieces of the literature were included,among which 4 were systematic reviews and 1 was expert consensus.     Objective To summarize the best evidence of intermittent pneumatic compression(IPC) therapy in patients with breast cancer-related lymphedema,and to provide references for clinical decision making. Methods According to “6S” model,we systematically searched the domestic and foreign databases to collect the evidence on IPC treatment for patients with breast cancer-related lymphedema,including guidelines,expert consensuses,systematic reviews,evidence summaries and recommended practice. The retrieval period was up to October 2021. The retrieved evidence was screened according to inclusion and exclusion criteria,and 2 researchers independently evaluated the quality of included literature,extracted and summarized the available information. Results A total of 16 articles were involved,including 5 guidelines,1 evidence summary,7 systematic reviews and 3 expert consensuses. 25 pieces of evidence were synthesized into 6 categories,including indications,contraindications,treatment programs,treatment effects,monitoring and points for attention during IPC treatment. Conclusion IPC can be used as an adjunctive therapy for breast cancer-related lymphedema. Contraindications should be fully evaluated before IPC treatment,and recommended treatment programs should be followed. Potential complications should be monitored and prevented during the treatment.

    Training transfer refers to the application of specific behaviours learned from training to the actual work of trainees and the continued maintenance of the behaviour after a period of time. This paper reviewed the concept and common models of training transfer,and summarized the research progress on the influencing factors and implementation strategies of training transfer. Based on the training transfer model,we compared the problems in the process of training transfer of specialist nurses in China,and put forward preliminary suggestions to promote the effectiveness of training transfer of specialist nurses in China,including the urgent need to build a positive training transfer climate and reconstruct the migration design link,so as to provide references for furthers improving the effectiveness of training of specialist nurses in China.

    The nasoenteric tube is one of the important approaches of clinical enteral nutrition support,and the correct position of the tube is the prerequisite for usage. The current clinical use of nasoenteric tube positioning technology mainly includes in vitro measurement technology,body fluid measurement technology,abdominal auscultation technology,ultrasound-assisted positioning technology,digital subtraction angiography(DSA),electromagnetic navigation technology,etc. There are huge differences of the application status for different positioning technologies. This article intends to review the related research on these positioning techniques,aiming to provide the basis and references for the selection of the positioning method of the clinical nasointestinal tube.

    Objective To design a modified one-piece ostomy bag used for abdominal drainage tube nursing in post-Kasai biliary atresia infants,and to assess its clinical application effects. Methods The modified one-piece ostomy bag consisted of an extra thin hydroactive dressing and a one-piece ostomy bag. From January,2018 to December,2020,all 79 post-Kasai biliary atresia patients were chosen. According to admission date,patients were divided into 2 groups. From January,2020 to December,2020,20 infants in the experimental group received modified one-piece ostomy bag nursing for abdominal drainage tube;from January,2018 to December 2019,59 patients in the control group received common dressing change nursing for abdominal drainage tube. The dressing change counts,costs,incidence of dermatitis and tube dislocation were compared between the 2 groups,and clinical nursing effects were observed. Results During abdominal drainage carriage,compared with it in the control group,the median counts of dressing change in experimental group were 1.4 counts less(P<0.001) and costs were ¥46.1 higher(P=0.002),and the incidences of dermatitis were lower in the experimental group(5% vs. 22%,P=0.102). There were no unexpected drainage tube dislocation in the experimental group,and 2 cases in the control group. Conclusion The clinical use of modified one-piece ostomy bag for abdominal drainage tube nursing in post-Kasai biliary atresia infants are feasible,which can reduce counts of dressing change and the incidence of dermatitis,with precise measurement of drained ascites volume.
