Abstract

Abstract:

Objective To explore the experiences of pregnant patients with end-stage renal disease （ESRD） undergoing multidisciplinary precision management，providing evidence to optimize clinical multidisciplinary management protocols. Methods This study employed purposeful sampling methods. Semi-structured interviews were conducted from September 2021 to February 2025 with pregnant patients with ESRD who received full multidisciplinary precision management in a tertiary-level hospital in Chongqing，China. Data were analyzed using Colaizzi’s 7-step analysis method. Results This study involved in-depth interviews with 8 pregnant patients with ESRD，totaling approximately 370 minutes of interview time and yielding over 26，000 words in transcribed text. Totally 3 core themes and 6 sub-themes were extracted，namely the motivation to continue the pregnancy，multidimensional pregnancy management experience（comprehensive risk assessment and counseling，informed communication，continuous pregnancy monitoring，personalized nutritional support，individualized exercise guidance，continuous psychological support and companionship），transformation and maintenance of healthy behaviors. Conclusion The pregnancy journey for patients with ESRD is fraught with uncertainty and challenges. Future efforts should focus on achieving full-cycle empowerment management through continuous health education and family support mechanisms，ensuring the safety of both mothers and babies while enhancing patients’ quality of life.

摘要：

目的剖析多学科管理下终末期肾病患者的妊娠体验，为优化多学科管理方案提供参考。方法采取目的抽样法，选取2021年9月—2025年2月在重庆市某三级甲等医院接受多学科管理的终末期肾病妊娠患者进行半结构式访谈，运用Colaizzi 7步分析法对资料进行分析。结果共访谈8例终末期肾病妊娠患者，访谈总时长为370 min，转录2.6万余字。共提炼出3个核心主题及6个亚主题：①坚持妊娠的动机；②多维妊娠管理体验，包括全面的风险评估与咨询、充分的知情沟通、连续的妊娠管理、个性化的营养支持、差异化的运动指导、持续的心理支持与陪伴；③健康行为的转变与维持。结论终末期肾病患者在妊娠过程中存在较大风险与挑战，未来需进一步加强健康教育与家庭支持，实现全周期的赋能管理，以保障母婴安全、提升患者的生活质量。
