    Expert consensus on humanistic care for patients in hospice care

    Objective The purpose of writing the “expert consensus on humanistic care for patients in hospice care”(hereinafter referred to as the “consensus”) aims to standardize the practice of humanistic care in the field of hospice care,ensuring that humanistic care is integrated throughout the entire service process for hospice care patients and their families. Methods A systematic search was conducted in domestic and foreign databases for literature related to hospice care and humanistic care,including guidelines,expert consensuses,systematic reviews or Meta-analyses,and evidence summaries. High-quality evidence was evaluated,extracted,and summarized to form the initial draft of the “consensus”. From June to October 2024,20 experts from the fields of hospice care,nursing humanities,and evidence-based nursing were invited to participate in 1 round of expert consultation. Among them,13 experts were selected for 2 rounds of expert demonstration meetings. After collating and analyzing the experts’ opinions,the initial draft was revised and refined,ultimately resulting in the final version of the “consensus”. Results The effective response rate of the consultation questionnaire was 100%,with expert authority coefficient of 0.880,judgment coefficient of 0.935,and familiarity level of 0.825. The Kendall harmony coefficient of the expert consultation was 0.134(P<0.05). The “consensus” consisted of 13 aspects,including the targets and objectives,principles,institutional guarantees,environmental requirements,etc. Conclusion This “consensus” possesses strong scientific rigor and practicality,which can provide guidance and references for the practice of humanistic care in the field of hospice care,promoting the standardization and humanization of hospice care services.

    Adjustment and preliminary application of a data-driven palliative care outcomes collaboration model

    Objective This study aimed to adapt the data-driven Palliative Care Outcomes Collaboration(PCOC) model to the local context and evaluate its feasibility and preliminary effectiveness in a palliative care unit in China,with the goal of informing its broader integration into national palliative care practice. Methods Based on international experience,a localized implementation protocol for the PCOC model was developed through expert consultations and a pilot study. The protocol incorporated key elements including organizational and managerial support,team training and capacity building,information system integration,supervision and feedback mechanisms,process optimization,and data-driven decision-making. From June to December 2023,the protocol was piloted in the palliative care unit of a tertiary cancer hospital in Changsha,China. Implementation outcomes were assessed by comparing patients’ urgent care response rates,symptom stability rates,and symptom improvement rates between the first 1~3 months and 4~6 months after implementation. Results During the study period,a total of 355 inpatients were enrolled,with the PCOC assessment achieving full coverage(100%) and a completion rate of 97.78%. There was no statistically significant difference in the urgent needs response rate between the first 1~3 months and the 4~6 months after the implementation of the PCOC model(P=0.533). However,compared to the first 1~3 months after implementation,patients in the 4~6 months period showed significantly higher symptom stability rates for pain,psychological/spiritual issues,and family/caregiver problems,as well as a higher improvement rate for pain(P<0.05). Conclusion The localized PCOC implementation protocol facilitates standardized assessment and symptom management,and its application can enhance the quality of palliative care.

    Objective To explore the communication preferences for advance care planning(ACP) among elderly hospitalized patients and to provide references for improving the ACP implementation rate. Methods Convenience sampling was used to select elderly hospitalized patients from a tertiary hospital in Wenzhou City between April and November 2023. Face-to-face surveys were conducted using a general data collection form and a self-designed questionnaire on ACP communication preferences. Mixed Logit Model,willingness-to-pay analysis,relative importance analysis,and subgroup analysis were employed to explore preferences. Results 204 questionnaires were distributed and 200 valid questionnaires were recovered,with a valid questionnaire response rate of 98.04%. The Mixed Logit Model analysis indicated that key attributes—communication timing,communication form,family participation in ACP communication,legal validity of ACP documents,and out-of-pocket costs—significantly influenced preferences(P<0.05). Willingness-to-pay analysis showed that changes in communication timing,communication form,and family participation altered patients’ willingness to pay. Relative importance analysis ranked the top 3 attributes as communication timing,communication form,and family participation. Subgroup analysis revealed that gender,education level,and residence location affected preferences(P<0.05). Conclusion Key attributes of ACP communication affect elderly hospitalized patients’ preferences and willingness to pay,with variations across subgroups. Healthcare professionals should develop optimal communication programs based on patient preferences and individual characteristics.

    Objective To explore the facilitators and barriers of palliative care volunteering,and to provide references for further advancement of palliative care volunteering. Methods Purposeful sampling was used to select 12 volunteers from a palliative care ward in Hangzhou,Zhejiang Province,between April and September 2024. Semi-structured interviews were conducted,and directed content analysis was applied to organize and analyze the data,followed by theme analysis. Results Facilitators and barriers for volunteers’ participation in palliative care volunteering were extracted. The 5 sub-themes of facilitators include motivating factors and perceived benefits,support and collaboration among volunteers,professional training and healthcare recognition,increased social awareness and public acceptance,and government support and institutional safeguards. The 5 sub-themes of barriers include limitations in individual capacity,challenges in collaboration with patients,families and healthcare workers,inadequate management and service mechanisms,uneven development of palliative care and insufficient public attention to psychological problems,and inadequate relevant laws and incentives. Conclusion There are more factors affecting the development of palliative care volunteering,and healthcare professionals should adopt targeted strategies to promote the active participation of volunteers in order to promote the sustainable development of palliative care volunteering.

    Application of foot massage based on the brain-gut axis theory in hemodialysis and insomnia patients with spleen-kidney yang deficiency type

    Objective To explore the application effect of foot massage based on the brain-gut axis theory in patients with spleen-kidney yang deficiency syndrome undergoing maintenance hemodialysis and suffering from insomnia,and to provide references for improving patients’ sleep quality and gastrointestinal symptoms. Methods A total of 88 patients with spleen-kidney yang deficiency syndrome undergoing maintenance hemodialysis and suffering from insomnia,who were treated at a Blood Purification Center of a tertiary-A general hospital in Hangzhou,Zhejiang Province from April to September 2024,were selected as a study subjects. They were randomly divided into an experimental group and a control group using the random number table method,with 44 cases in each group. The experimental group received foot massage based on the guidance of the brain-gut axis theory in addition to conventional treatment and nursing care,while the control group received conventional treatment and nursing care only. The intervention lasted for 3 months. The differences in the Pittsburgh Sleep Quality Index(PSQI) scores,Gastrointestinal Symptom Rating Scale(GSRS) scores,traditional Chinese medicine(TCM) syndrome scores,and serum 5-hydroxytryptamine and melatonin levels between the 2 groups before and after the intervention were compared. Results During the study period,no cases were lost in the experimental group,while 5 cases were lost in the control group. Ultimately,44 cases were included in the experimental group and 39 cases in the control group. After the intervention,the PSQI scores,GSRS scores,and TCM syndrome scores in both groups were lower than those before the intervention,and those in the experimental group were lower than those in the control group. The serum 5-hydroxytryptamine and melatonin levels were higher than those before the intervention,and the levels in the experimental group were higher than those in the control group,with statistically significant differences (all P<0.001). Conclusion Foot massage based on the brain-gut axis theory can improve gastrointestinal symptoms and TCM syndromes in patients with spleen-kidney yang deficiency syndrome undergoing maintenance hemodialysis and suffering from insomnia,and enhance their sleep quality.

    Application of multiple needle puncture technique in autonomous arteriosclerosis fistula puncture in maintenance dialysis patients

    Objective To compare the application effects of different puncture techniques in the autonomous arteriovenous fistula puncture of patients undergoing hemodialysis,providing a reference for nurses in selecting puncture methods. Methods A total of 135 patients undergoing hemodialysis at a tertiary hospital in Changzhou from September 2023 to September 2024 were selected and randomly divided into 3 groups,including a multiple puncture group,a rope ladder puncture group,and a buttonhole puncture group. Follow-up was conducted every 3 months,and the first puncture success rate,puncture pain,diameter of blood scab at the puncture site,hemostasis time,and complications of arteriovenous fistula(infection,embolism,aneurysm,stenosis,intimal hyperplasia) were compared among the 3 groups. Results A total of 114 patients completed the study,with 40 in the multiple puncture group,42 in the rope ladder puncture group,and 32 in the buttonhole puncture group. The single-puncture success rate was higher in the multiple single-puncture group(99.4%) and the buttonhole puncture group(99.6%) than it in the rope-ladder puncture group(93.3%),with all P<0.001. The scab diameter in the buttonhole puncture group(2 mm) was larger than that in the multiple single-puncture group(1 mm) and the rope-ladder puncture group(1 mm),with all P<0.001. The incidence of intimal hyperplasia in the buttonhole puncture group(34.3%) was higher than that in the multiple single-puncture group(10.0%) and the rope-ladder puncture group(11.9%),with all P<0.05. Conclusion Each of the 3 arteriovenous fistula puncture techniques has its advantages and disadvantages. The multiple puncture group technique can improve the first puncture success rate,and it does not significantly increase the complications of arteriovenous fistula compared to the rope ladder and buttonhole techniques,making it suitable for widespread use.

    Decision tree analysis of the trajectories in cognitive function in elderly hemodialysis patients and the core influencing factors

    Objective To explore the potential categories and influencing factors of cognitive function trajectories in elderly hemodialysis patients. Methods A convenience sampling method was used to select elderly patients undergoing hemodialysis in the blood purification center of a tertiary hospital in Beijing,China,from June 2022 to June 2024. A general information questionnaire,the Short Physical Performance Battery,the Patients Health Questionnaire-9,the Impact on Participation and Autonomy Questionnaire,the Hemodialysis Self-Management Instrument,and the Mini-Mental State Examination,were used to collect data at the time of enrollment. Cognitive function was evaluated at the time of enrollment and 6,12,18,and 24 months after enrollment,using latent category growth model to identify trajectory categories,and logistic regression,and decision tree models to analyze the influences on cognitive function trajectories. Results Finally,304 patients were included. A total of 2 trajectories of cognitive function in elderly hemodialysis patients were identified,namely,high-level stable group(63.2%) and low-level declining group(36.8%). Both logistic regression and decision model results showed that the type of dialyzer,body mass index,physical function,and depression status were the factors influencing potential categories of cognitive function for elderly hemodialysis patients. The areas under the receiver operating characteristic curves for logistic regression and decision tree models were 0.823[95%CI(0.775,0.870)] and 0.816[95%CI(0.765,0.867)],respectively. Conclusion There is heterogeneity in the cognitive function trajectories of elderly hemodialysis patients,and healthcare professionals can develop targeted interventions according to the influencing factors of the trajectory categories to improve their cognitive function.

    Objective To investigate the current status of productive engagement of elderly patients undergoing unilateral total hip arthroplasty in postoperative rehabilitation period. Methods From May to Novermber 2024,242 elderly patients undergoing the first total hip arthroplasty from the joint surgery department of Nanjing Medical University Kangda College First Affiliated Hospital were selected as investigation subjects. They were investigated in postoperative rehabilitation period after receiving rehabilitation training for a week at getting out of bed for the first time after surgery by the Basic Information Questionnaire,Productive Engagement Scale,Improved Barthel Index Rating Scale,Self-Rating Anxiety Scale,Self-Rating Depression Scale,Fall Risk Perception Scale for Community-dwelling Older Adults,Tampa Rating Scale(17 Item Version),Perceived Social Support Scale and Social Isolation Scale for the Elderly. Results In the 230 valid questionnaires,the score of productive engagement of elderly patients undergoing unilateral total hip arthroplasty was(31.13±3.69) points in postoperative rehabilitation period. The Linear regression analysis showed that age,education level,place of residence,normal BMI,number of coexisting chronic diseases,time after surgery,ability of self-care,fall risk perception,Tampa,perceived social support and social isolation were all significant influencing factors for productive engagement of elderly patients undergoing unilateral total hip arthroplasty in postoperative rehabilitation period(P<0.05). Conclusion The level of productive engagement of elderly patients undergoing unilateral total hip arthroplasty in postoperative rehabilitation period is relatively low. Their productive engagement were influenced by many factors. Clinical medical staff should develop intervention plans based on such influence factors to improve their level of productive participation.

    The status and influencing factors of acceptance of disability in young and middle-aged spinal cord injury patients

    Objective To investigate the current status of acceptance of disability(AOD) among young and middle-aged spinal cord injury patients and analyze the factors influencing it,in order to provide a basis for nursing managers to develop targeted interventions. Methods From February to July 2024,a convenience sample of 330 young and middle-aged spinal cord injury patients who were hospitalized in 3 tertiary-level hospitals in Henan Province was selected. The survey was conducted using a general information questionnaire,the Brief Adaptation to Disability Scale-Revised,Perceived Social Support Scale,Connor-Davidson Resilience Scale-10 item,Brief Illness Perception Questionnaire,Medical Coping Modes Questionnaire. Results A total of 322 patients were included in the final analysis,yielding a valid response rate of 97.58%. The AOD score for young and middle-aged spinal cord injury patients was(25.99±6.68). Multiple linear regression analysis revealed that gender,education level,type of paralysis,injury duration,pain intensity,social support,psychological resilience,disease perception,and coping styles (confrontation and submission) were influencing factors of AOD(P<0.05). Conclusion AOD among young and middle-aged spinal cord injury patients is at a moderate level. Healthcare professionals should pay particular attention to male patients,those with low literacy,quadriplegia,shorter injury durations,high pain intensity,low social support,weak psychological resilience,strong disease perception,and yielding coping styles. Timely interventions are recommended to enhance AOD,strengthen rehabilitation outcomes,and improve prognosis.

    Influencing factors of family caregivers’ caregiving capacity for children with Hirschsprung’s disease:a mixed-method study

    Objective To evaluate the caregiving capacity of family caregivers of children with hirschsprung’s disease(HD) and to establish an evidence base for developing family-empowered home care interventions. Methods An explanatory sequential mixed-method design was employed. A total of 308 family caregivers of HD patients admitted to the Pediatric Surgery Department of a tertiary children’s hospital in Shanghai,were enrolled from July 2019 to July 2024. Quantitative data were collected in 2024 using a demographic questionnaire,the Zarit Caregiver Burden Interview and the Family Caregiver Task Inventory. Semi-structured interviews were conducted in August to December 2024 with 15 caregivers of children with HD with poor caregiving skills. Quantitative data were analyzed using nonparametric tests and multiple linear regression,and qualitative information was refined through Colaizzi’s 7-step analysis of themes. Results In the end,281 valid questionnaires were recovered. The total caregiving competence was 12.00(6.00,20.00). Multivariate analysis identified significant predictors of caregiving competence,including children in the postoperative dilation period,frequency of enterocolitis episodes,place of residence monthly household income and burden of caregiving were the factors influencing the ability of family caregivers to take care of their children(P<0.05). Qualitative analysis revealed 4 themes:persistent multidimensional adaptive challenges,disparity in clinical caregiving skills,systemic support limitations,and role multiplicity strain. Conclusion Family caregivers of children with HD face multidimensional stressors,necessitating a dynamic,family-centered support ecosystem. Strategic interventions should focus on enhancing clinical caregiving literacy while improving access to socioeconomic resources and establishing role-specific support networks.

    Qualitative research on the barriers of chronic wound linkage management in medical consortium

    Objective To explore the barriers of implementing collaborative chronic wound management within medical consortium and provide references for ensuring continuous care for chronic wound patients. Methods Using purposive and snowball sampling methods,14 specialist nurses participating in chronic wound collaborative management across medical consortium in 7 provinces/municipalities(Beijing,Shanghai,Zhejiang,Sichuan,etc.) were selected for semi-structured interviews between December 2023 and January 2024. Data were analyzed via content analysis. Results Totally 4 themes and 8 subthemes were identified,including management and organizational level(the management system needs improvement;the function of medical information mutual recognition has not been fully realized),resource and technology level(there are insufficient nursing resources at the grassroots hospitals;the specialized nursing capabilities of wound nurses at grassroots hospitals need to be enhanced),patient and demand level(patients’ trust is insufficient;patients’ concepts are rigid),information technology and support level(the informatization construction of grassroots hospitals lags behind;the intelligence of information systems is insufficient). Conclusion Wound specialist nurses encounter various difficulties when participating in the coordinated management of chronic wounds within the medical consortium. The management and organizational system constraints hinder the effectiveness of the nursing coordination;the resource and technology gap affects the implementation of graded wound care in the region;the lack of patient trust and misconceptions intensify the resistance to coordination;the lagging in information technology construction limits the innovative development of nursing coordination.

    Objective To explore the challenges in sedentary behavior modification among patients with lower extremity arteriosclerosis obliterans(ASO),with the goal of informing the development of personalized physical acti-vity interventions to address sedentary behavior patterns. Methods A purposive sample of 16 patients diagnosed with lower extremity ASO was recruited as participants from the department of vascular surgery at a tertiary hospital in Sichuan province between August and November 2024. Semi-structured interviews were conducted to collect data and the Colaizzi 7 steps was used to analyze the data based on Capacity-Opportunity-Motivation-Behavior model. Results 3 themes and 12 sub-themes were identified,namely capacity(lower extremity and somatic dysfunction,psychological perception constraints,disease literacy deficits),motivation(inadequate perceived value of physical activity,diminished self-efficacy,kinesiophobia,negative cognitive framing),opportunity(generalization of guidance on disease knowledge,constrained healthcare resources,the normalization of sedentary entertainment practices,financial burden constraints,inadequate family support). Conclusion The factors influencing sedentary behavior in ASO patients are complex and diverse. Healthcare professionals need to comprehensively assess the multifaceted factors contributing to patients’ difficulties in changing sedentary behavior,and develop targeted strategies accordingly to promote improvements in their sedentary habits.

    The development of a Self-Management Demand Scale for infertile patients with obese polycystic ovary syndrome and the test of its reliability and validity

    Objective To develop a Self-Management Demand Scale for infertile patients with obese polycystic ovary syndrome(PCOS) suitable for domestic socio-cultural background,and to test its validity and reliability. Methods Based on the self-management model,the item pool of the scale was constructed through literature review,semi-structured interviews,and Delphi expert consultation. Using convenience sampling method,560 obese PCOS combined with infertility patients from a tertiary-level hospital in Wuhan City were selected for questionnaire survey from June to November 2023,and they were divided into 2 parts according to the time of survey for exploratory factor analysis and validation factor analysis. Results Finally,554 valid questionnaires were collected. The Self-Management Demand Scale for infertile patients with obese PCOS contains 3 dimensions and 37 entries. Exploratory factor analysis extracted 3 factors with the cumulative variance contribution rate of 78.785%. The results of the validation factor analysis showed a good model fit. The content validity indices at the scale entry level ranged from 0.830 to 1.000,and the content validity indices at the scale level were 0.920. The Cronbach’s α coefficients for the scale as a whole were 0.967,and those for the dimensions ranged from 0.890 to 0.971;the split-half reliabilities of the overall scale as a whole were 0.856 and the split-half reliabilities of the dimensions were 0.802~0.895. Conclusion The scale has ideal validity and reliability,which provided a scientific tool to measure accurately the self-management demands of the infertile patients with obese PCOS.

    This report summarizes the nursing experience of a patient with acute myocardial infarction complicated by cardiogenic shock and frequent electrical storms. The key nursing interventions are outlined as follows. Formation of a multidisciplinary management team:a collaborative treatment plan was developed by assembling a multidisciplinary team of healthcare professionals. Activation of the hospital emergency response system:the hospital’s emergency response protocols were promptly activated to ensure efficient emergency nursing care. Dynamic hemodynamic monitoring:circulatory support strategy guided by hemodynamic monitoring. Establish-ment of a malignant arrhythmia warning system:a warning system for malignant arrhythmias was implemented to standardize the emergency procedures for electrical cardioversion and defibrillation,thereby maximizing the time available for life support. Sequential antiplatelet and anticoagulation management:a sequential approach to antiplatelet and anticoagulation therapy was employed to effectively prevent the risks of thrombosis and bleeding. Focus on psychological care and rehabilitation training:emphasis was placed on psychological support and rehabilitation exercises to enhance the patient’s comfort and exercise tolerance. The patient was hospitalized for 33 days and was discharged in improved condition. This translation is structured to reflect the conventions of academic writing,making it suitable for inclusion in a clinical report or research paper.

    This article summarizes the nursing experience of a patient with cold agglutinin syndrome who experienced 2 sudden cardiac arrests. The nursing key points included:balancing the contradiction between hypothermia rewarming and head therapeutic hypothermia,thereby optimizing the temperature management strategy; controlling the entire process to ensure the safety of blood transfusion; conducting dynamic assessment and monitoring of coagulation function,preventing both bleeding and thrombosis; timing of nutritional therapy and progressive feeding at the right time;multidisciplinary collaboration was implemented to carry out a rehabilitation activity plan during continuous renal replacement therapy; preventing ICU delirium. With comprehensive treatment and meticulous nursing care provided by the multidisciplinary team,the patient was discharged in good condition after 22 days of hospitalization,and has resumed his daily working life after 3 months of follow-up.

    To summarize the nursing experience of a patient with uremia on maintenance hemodialysis complicated by recurrent ventricular tachycardia and treated with transcatheter radiofrequency ablation. Key nursing interventions included:dynamically assessing the patient’s coagulation and bleeding status,being vigilant against the occurrence of deep vein thrombosis,and preventing local and major organ bleeding;implementing goal-oriented volume management strategies to prevent electrolyte disorders;the strengthened management of vascular access to reduce risks of stenosis or occlusion in the arteriovenous fistula;conducting precise assessment and comprehensive intervention to reduce the patient’s psychological and mental burden. After careful treatment and nursing care,the patient was stable and discharged on the 6th postoperative day. During the 2-month outpatient follow-up,cardiac function indicators were normal,and the fistula was unobstructed,and the patient recovered well.

    Objective To synthesize qualitative studies on the experiences of postoperative lung cancer patients participating in pulmonary rehabilitation,aiming to comprehensively understand the barriers and facilitators influencing their engagement and to provide evidence-based insights for improving rehabilitation adherence. Methods Qualitative studies were retrieved from databases including Web of Science,PubMed,Cochrane Library,Embase,CNKI,Wanfang Data,VIPand Sinomed,with a search period spanning from database inception to January,2025. The methodological quality of eligible studies was assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Qualitative Research. Extracted findings were mapped and synthesized using the Theoretical Domains Framework(TDF) and the Capability-Opportunity-Motivation-Behavior mode(COM-B). Results Ultimately,a total of 11 studies were included,yielding 48 findings. Using the Theoretical Domains Framework(TDF) and COM-B model,facilitators and barriers to pulmonary rehabilitation in postoperative lung cancer patients were mapped and consolidated into 14 categories,culminating in 2 synthesized findings. Conclusion Participation in pulmonary rehabilitation among postoperative lung cancer patients is influenced by multiple factors. Clinicians should develop patient-centered pulmonary rehabilitation plans by considering these factors and incorporating patients’ attitudes and perspectives.

    Objective To systematically evaluate the methodological quality and measurement properties of assessment tools for maladaptive eating behaviors in bariatric surgery patients. Methods A systematic search of the CNKI,Wanfang database,VIP,CBM,PubMed,Web of Science,Embase,and Cochrane Library was conducted from inception to March 15,2025. Totally 2 researchers independently screened the literature and extracted the data. The consensus-based standards for the selection of health measurement instruments checklist and quality criteria were employed to evaluate the methodological quality and psychometric properties of the included assessment tools,and combined with a modified version of the GRADE to form recommendations. Results A total of 14 articles were included,involving 2 specific assessment tools for bariatric surgery patients and 10 universal assessment tools. The methodological quality of content validity was rated as “unclear” for all scales,with moderate certainty of evidence. Structural validity and internal consistency generally performed well(Cronbach’s α>0.7). However,evidence for stability,criterion validity,and cross-cultural validity remains insufficient. A total of 9 instruments were recommended as Grade B,while 3 were recommended as Grade C. Conclusion The disordered eating after bariatric surgery and the food craving questionnaire-trait are recommended for clinical use,but further validation of their psychometric properties is still needed.

    Home parenteral nutrition is an important way of nutritional support for patients with short bowel syndrome at home.It can not only improve the nutritional status of patients,enhance their quality of life,but also shorten their hospital stay,save medical expenses and facilitate their return to family life. This study reviews the current situation of home parenteral nutrition,the implementation methods and follow-up management of home parenteral nutrition for patients with short bowel syndrome,and the countermeasures to promote the implementation of home parenteral nutrition for patients with short bowel syndrome,with the aim of providing references for promoting the implementation and standardizing the management of home parenteral nutrition for patients with short bowel syndrome in China.

    Caregivers for chronic disease patients of caregiver activation emphasizes the subjective initiative and preparedness of caregivers in managing patients’ health conditions and medical needs. Caregivers with high activation levels can participate more effectively in disease management,significantly improve the quality of care,and have a positive impact on their own mental health and quality of life. This article reviews the concept,assessment tools,current situation,influencing factors,and intervention strategies of caregiver activation,and proposes future research directions,aiming to provide references for conducting research and practical exploration related to caregiver activation.
