    Multi-Dimensional Nursing Practice for High-Risk Pregnancy
    Research on a shared care clinic in pregnant women with hypertensive disorders of pregnancy

    Objective To explore the effect of a shared care applied to pregnancy care among pregnant women with hypertensive disorder of pregnancy(HDP). Methods A total of 88 pregnant women with HDP who received obstetric check-ups in a tertiary A-level maternity hospital in Nanjing City from January to October 2023 were conveniently selected as the research subjects. According to the order of obstetric examinations,44 pregnant women with HDP from June to October 2023 were selected as an experimental group,and 44 pregnant women with HDP from January to May 2023 were selected as a control group. The experimental group received the shared care clinic management model,and the control group received the routine management. The scores of the rate of blood pressure attainment,the incidence of disease progression,the questionnaire on knowledge of HDP,the Hypertension Self-Management Scale,the Pregnancy-Related Anxiety Scale,and the Patient Satisfaction Questionnaire were compared between the 2 groups. Results 41 patients in the experimental group and 39 patients in the control group completed the study. After the intervention,the blood pressure compliance rate of pregnant women with HDP in the experimental group was 95.12%,higher than 79.49% in the control group. The incidence of disease progression was 7.32%,lower than 25.64% in the control group. HDP Knowledge Questionnaire score(25.24±4.07),Hypertension Self-Management Scale score(114.39±14.89), and Patient Satisfaction Questionnaire score(19.56±1.14) were higher than that of the control group. The score of Pregnancy-Related Anxiety Scale was(18.02±2.15),lower than(23.18±9.58) in the control group. The score of Patient Satisfaction Questionnaire was(19.56±1.14),higher than that(17.26±2.25) in the control group. The differences were statistically significant(P<0.001). Conclusion The shared care clinic model can improve HDP pregnant women’s blood pressure control compliance,HDP knowledge,and self-management,reduce the incidence of disease progression,pregnancy-related anxiety,and improve satisfaction,which provides a reference for optimising the HDP management model.

    Analysis of influencing factors of perceived uncertainty in the process of re-pregnancy in patients with recurrent abortion and nursing implications

    Objective To investigate the status of perceived uncertainty in the process of re-pregnancy in patients with recurrent abortion,and to systematically explore the influencing factors based on the health ecology model,to inform a reduction in the level of perceived uncertainty about their re-pregnancy and improve their quality of life. Methods A convenience sampling method was used to investigate 260 patients with recurrent spontaneous abortion during re-pregnancy from 2 reproductive clinics of a tertiary A hospital in Suzhou,from December 2023 to June 2024,and they were investigated by a self-designed general data questionnaire,Intolerance of Uncertainty Scale-12,Generalized Anxiety Disorder Scale and Simple Coping Style Questionnaire. The single factor analysis and multiple linear regression methods were used to analyze the influences factors of perceived uncertainty in patients with recurrent spontaneous abortion during re-pregnancy. Results A total of 256 valid questionnaires were collected,with a recovery rate of 98.5%. The score of intolerance of uncertainty in patients with recurrent spontaneous abortion during re-pregnancy was(35.18±9.23). Multiple linear regression analysis found that average monthly family income,residence,history with assisted reproductive therapy,anxiety and coping were the factors influencing the feeling of perceived uncertainty in patients with recurrent spontaneous abortion during re-pregnancy(P<0.05). Conclusion The level of perceived uncertainty in patients with recurrent spontaneous abortion during re-pregnancy is at a medium level. Medical workers should pay attention to the early identification of the perceived uncertainty in patients,enhance the psychological support for patients,and improve their mental health.

    Facilitators and barriers of physical activity participation among women with gestational diabetes mellitus during pregnancy:a qualitative study

    Objective To investigate the factors that promote or impede physical activity participation in pregnant women with gestational diabetes mellitus(GDM),aiming to offer recommendations and evidence for optimizing blood glucose control in this patient population. Methods The purposive sampling method was used to select 12 pregnant women with GDM from July to December 2023 in a tertiary hospital in Shanghai for semi-structured in-depth interviews. Content analysis and the COM-B model were used to analyze and refine the theme. Results This study categorizes and synthesizes factors influencing physical activity during pregnancy among women with GDM into 3 facilitators:capability factors(positive attitudes towards physical activity,internalization of health consciousness,effective storage of health management knowledge);opportunity factors(family support and companionship,guidance and supervision from healthcare professionals,motivational effects of virtual peer support);motivation factors(desire to ensure fetal safety,motivation from achieving milestones,positive feedback from physical activity). Additionally,3 barriers were identified:capability factors(physiological burden and physical limitations of pregnancy,lack of self-efficacy in changing past habits,insufficient cognitive abilities);opportunity factors(inadequate support from healthcare professionals,difficulty in accessing information);motivation factors(feelings of fatigue during physical activity,resistance to engaging in physical activity). Conclusion The participation of pregnant women with GDM in physical activities is affected by their capability,opportunities,and motivation. GDM patients exhibit a strong dependence on authoritative guidance. Through the utilization of “virtual peer support”,healthcare professionals can stimulate intrinsic motivation towards fetal well-being,enhance positive engagement and compliance with physical activities by incorporating a system of incremental achievements and positive reinforcement. This approach can ultimately assist GDM patients in achieving improved blood glucose control outcomes.

    This paper summarizes the nursing experience of a pregnant woman with congenital heart disease who developed pulmonary artery hypertension crisis after cesarean section. Nursing key points:monitoring and management of inhaled nitric oxide therapy to reduce pulmonary hypertension;providing sequential respiratory support care and dynamically adjusting oxygen therapy regimen;precise volume regulation and alertness to serious complications;optimizing puerperal supervision strategy to reduce the risk of infection;preventing and controlling stress bleeding and implementing a stepped dietary management strategy;implementing phased rehabilitation training to accelerate the recovery process;carrying out personalized psychological care and health education. With comprehensive nursing care,the patient recovered well and was discharged smoothly after an 18-day hospital stay.

    Objective To form an expert consensus on nasogastric tube and intestinal tube placement in children (hereinafter referred to as the “consensus”),and provide a reference for pediatric medical workers regarding the operation of gastrointestinal tube placement in children. Methods A “consensus” formulation group was established. The “5.0” EBHC Pyramid Model was employed to systematically search,evaluate,summarize and extract relevant evidence to form the initial draft of the “consensus”. The RAND-UCLA expert consensus method was utilized. Through a round of expert inquiries by correspondence and a round of face-to-face expert discussions,the data were collated,analyzed,refined,and modified to form the final version of the “consensus”. Results The effective recovery rate of the expert inquiry questionnaire was 100%. The coefficient of expert authority(Cr) was 0.891. The Kendall’s concordance coefficient of the inquiries was 0.692(P<0.01),which was statistically significant. The median of the “RAND-UCLA” suitability score were 7-9 points,and the disagreement index(DI) value was<1.00. The final “consensus” encompasses 9 aspects,including qualification requirements for the tube placement personnel,indications and contraindications,preoperative assessment,preoperative preparation,measurement of tube length,key points of tube placement,methods for confirming the position,tube flushing,fixation,and recording,with 47 recommendations. Conclusion This “consensus” is scientific,rigorous,and practical,covering all links of the gastrointestinal tube placement process in children,providing reference and guidance for the safe and standardized implementation of gastrointestinal tube placement in children.

    Objective To compile the “expert consensus on preventive and management of exposure keratopathy”(hereinafter referred to as “consensus”),aiming to standardize and promote the prevention of exposure keratopathy(EK) in medical institutions at different levels. Methods The evidence-based methods were used to retrieve,evaluate and summarize evidence in this field according to the level of evidence,and relevant recommendations and research conclusions were extracted,and the first draft of consensus was formed. After 2 rounds of Delphi expert letters and expert meetings,combined with evidence and expert opinions,the consensus content is written,adjusted and modified,and recommendations were made. Results 58 experts were invited to participate in the consultation. The expert positive coefficient was 100%;the expert judgment basis of the 2 rounds were 0.940 and 0.936;the degree of familiarity of the 2 rounds were 0.779 and 0.797;the coefficient of authority of the 2 rounds were 0.859 and 0.866. The Kendall W coefficients of the 2 rounds of inquiry were 0.099 and 0.117,and the difference was statistically significant(P<0.05). The consensus includes 5 aspects of EK:risk factors,assessments,protection measures,symptom management and training and management. Conclusion The consensus was based on the existing evidence and clinical practical requirements and recommendations of experts in the field. Consensus can provide guidance for clinical nursing staff in prevention,nursing and management of EK effectively.

    Construction and preliminary application of a self-management question prompt list in adult liver transplant recipients

    Objective To develop a postoperative self-management question prompt list for adult liver transplant recipients and conduct preliminary application,aiming to provide an effective tool for facilitating their engagement in postoperative self-management. Methods From August to September 2024,the first draft of the postoperative self-management question prompt list for adult liver transplant recipients was developed through literature search and qualitative interviews,including 9 primary items and 50 secondary items. From October to November 2024,16 experts from Qingdao,Jinan,Beijing,and Fuzhou were interviewed on the Delphi method for 2 rounds to revise the question prompt list. From February to March 2025,19 patients after liver transplantation were selected for the preliminary application of the question prompt list. Results The response rates in the 2 rounds of consultations were both 100% and the authority coefficients of experts were both 0.88. The Kendall’s W in the 2 rounds was 0.336 and 0.344(P<0.001),respectively. The final question prompt list includes 9 primary items and 49 secondary items. The study showed that QPL demonstrated high clinical practicability in helping patients systematically understand the self-management framework after liver transplantation, promoting doctor-patient communication, and enhancing the initiative of self-management. Conclusion The question prompt list of postoperative self-management for adult liver transplant recipients established in this study is scientific,reliable,and practical,which is helpful for patients to obtain information about self-management from medical staff.

    Research on the current status and influencing factors of consumer health activation index in hospitalized elderly patients with multiple chronic conditions

    Objective To explore and analyze the current situation of consumer health activation index and its influencing factors in hospitalized elderly patients with multiple chronic conditions,and to provide reference basis for clinical intervention. Methods Convenience sampling method was used to select 332 hospitalized elderly patients with multiple chronic conditions from a tertiary hospital in Hefei City from July to December 2024 as the study subjects,and general information questionnaire,Consumer Health Activation Index,Health Literacy Management Scale,Scale of the Elderly Self-neglcet(Rural),and Perceived Social Support Scale were used to investigate the current situation of consumer health activation index of hospitalized elderly patients with multiple chronic conditions. Univariate analysis and multiple linear regression analysis were employed to explore the influencing factors of consumer health activation index in patients with multiple chronic conditions. Results A total of 332 questionnaires were distributed,and 325 valid questionnaires were retrieved. The score of the consumer health activation index of hospitalized elderly patients with multiple chronic conditions was(61.68±15.12). The results of multiple linear regression showed that literacy,average monthly household income,number of chronic diseases,medication frequency,self-care ability,health literacy management,perceived social support and self-neglect were the influencing factors for the positivity of hospitalized elderly patients with multiple chronic conditions(P<0.05),which could explain 59.9% of the variance of the consumer health activation index of hospitalized elderly patients with multiple chronic conditions. Conclusion The consumer health activation index of hospitalized elderly patients with multiple chronic conditions is at a low level. In the future,precise and systematic intervention measures for enhancing positive health status can be formulated by taking into account relevant influencing factors,in order to promote the transformation of patients from passive health to active health,and thereby facilitating positive aging.

    Objective To develop a Self-Report Scale of Symptoms for Post Intensive Care Syndrome-Family(PICS-F) and validate its reliability and validity,providing a scientific tool for healthcare professionals to comprehensively assess post intensive care syndrome-family. Methods Based on the symptom experience dimension of Symptom Management Theory,a preliminary version of the scale was developed through literature analysis,semi-structured interviews,expert consultations,and a pilot survey. A convenience sampling method was used to conduct the pre-trial version of the scale questionnaire survey on 442 family members of ICU patients at a tertiary A comprehensive hospital in Wuhan from June to September 2024. The test of the validity and reliability was conducted. Results The developed scale consists of 4 dimensions and 20 items. Exploratory factor analysis identified 4 factors,with a cumulative variance contribution rate of 70.921%. The content validity index at the scale level was 0.941,and the content validity indices for individual items ranged from 0.809 to 1.000. Confirmatory factor analysis indicated that the model fit indices were within acceptable ranges. The overall Cronbach’s α coefficient for the scale was 0.941,and the split-half reliability was 0.832. Conclusion The Self-Report Scale of Symptoms for PICS-F demonstrates good reliability and validity. It can comprehensively reflect the primary symptoms of PICS-F,and can serve as an effective measurement tool for clinicians to assess PICS-F in family members of ICU patients.

    A qualitative study on barriers to self-management behaviors in postoperative pituitary adenoma patients

    Objective To explore the barriers to postoperative self-management behaviors in patients with pituitary adenoma and provide a theoretical basis for developing nursing intervention strategies. Methods Using a purposive sampling method,16 postoperative pituitary adenoma patients admitted to a tertiary hospital in Zhengzhou from July 2024 to January 2025 were selected for semi-structured interviews. The interview data were analyzed and refined through Colaizzi’s phenomenological analysis method. Results A total of 4 themes and 12 subthemes were extracted,including postoperative physiological dysfunction(sensory dysfunction,difficulty in managing diabetes insipidus,fatigue syndrome),psychosocial adaptation barriers(self-image distress,fear of disease recurrence,restricted information delivery),challenges in maintaining health behaviors(environment-behavior conflicts,insufficient adherence to medication and activity guidelines,inadequate complication management),deficiencies in support systems(non-specific postoperative guidance,poor communication channels,insufficient coordination of medical resources). Conclusion Postoperative self-management behaviors in pituitary adenoma patients are influenced by multidimensional factors,necessitating comprehensive intervention strategies addressing physiological,psychological,behavioral,and systemic support levels.

    Influencing factors of health-related quality of life in patients after ileal ureteral replacement:a qualitative study

    Objective To explore the influencing factors of health-related quality of life in patients after ileal ureteral replacement,and to provide a reference for formulating effective management plans. Methods Based on the conceptual model of health-related quality of life,16 patients who underwent ileal ureteral replacement in the Department of Urology of a tertiary A hospital in Beijing were selected for semi-structured interviews using descriptive research methods and purposive sampling method. The data were analyzed using directed content analysis. Results There are 6 themes and 10 sub-themes:symptom factors(distress of tube-related symptoms,distress of gastrointestinal symptoms);individual characteristics factors(obvious negative emotions,health behavior development);environmental factors(support from relatives and colleagues,lack of primary medical services);functional status factors(physical function rehabilitation,social function recovery);physiological function factors(test abnormal test index,risk of primary disease recurrence);general health perception factors. Conclusion The health-related quality of life of patients undergoing ileal ureteral replacement surgery was affected by symptoms,negative emotions,health behaviors,social support,functional status et al. In view of the above factors,medical staff should carry out effective evaluation and intervention in time,so as to improve the quality of life of patients.

    A longitudinal study on short-term changes in gastrointestinal symptoms and influencing factors in patients undergoing bariatric surgery

    Objective To explore the changes and influencing factors in patients’ gastrointestinal symptoms and food tolerance before and after laparoscopic sleeve gastrectomy(LSG),and to provide a reference for the development of targeted gastrointestinal symptoms management program. Methods A convenience sampling method was used to select 125 patients who underwent LSG in the bariatric and metabolic surgery department of a tertiary-level hospital in Xuzhou City from June to November 2023 for a prospective observational study. The general information questionnaire,food tolerance questionnaire and Gastrointestinal Symptom Rating Scale (GSRS) were used to investigate at preoperative,3 months after surgery,and 6 months after surgery. Two-factor repeated measures variance analysis and generalized linear mixed model(GLMM) was used to analyze the influencing factors of gastrointestinal symptoms. Results 116 valid questionnaires were collected,and the GSRS scores of LSG patients at 3 time points were 21.67±5.80,23.28±4.33,and 21.22±3.18 respectively;the incidence rates of food intolerance were 0,32.75%,and 28.45% respectively. The patients’ symptoms of bowel sounds,dysphagia,constipation,and dry stool were worse after surgery than before surgery(all P<0.05),and the symptoms of bad breath,diarrhea,loose stools,and the need to defecate immediately were alleviated compared with symptoms before surgery(all P<0.05). GLMM results showed that short meal times,poor food tolerance,and high school/technical secondary school education were risk factors for increasing gastrointestinal symptoms(all P<0.05). Conclusion LSG could increase the prevalence of gastrointestinal symptoms such as dysphagia,dyspepsia,constipation and food intolerance. Medical staff should pay attention to the assessment of gastrointestinal symptoms in LSG patients,especially for patients with short meal time and poor food tolerance,strengthen postoperative follow-up,build targeted treatment and care plan,and prevent and reduce such uncomfortable symptoms.

    Objective To explore the effect of the horticultural therapy in the elderly with mild cognitive impairment in elderly care institutions. Methods A convenient cluster sampling method was used. The study was conducted among the elderly with mild cognitive impairment in 2 nursing homes with 5A-level in Guangzhou,from March to September 2024. Using a lottery method,subjects from 2 nursing areas across 2 elderly care institutions were allocated to an experimental group,with the other 2 nursing areas serving as a control group,each group comprising 55 cases. The experimental group participated in horticultural therapy on the basis of control group interventions,while the control group was given routine care and daily leisure activities. The cognitive function,basic psychological needs and quality of life were compared between the 2 groups after the intervention. Results Eventually,37 cases in the experimental group and 38 cases in the control group completed the study. After the intervention,the cognitive function,basic psychological needs and quality of life in the experimental group were all better than those in the control group,and the differences were statistically significant(P<0.05). Conclusion The horticultural therapy program can delay the progression of cognitive decline in the elderly with mild cognitive impairment in nursing homes,meet their basic psychological needs and improve their quality of life.

    Objective To assess the current status and influencing factors of high-risk populations for osteosarcopenia in elderly community residents,and to construct a health portrait for this group,revealing population characteristics and differences. Methods A convenience sampling method was employed to recruit 800 community-dwelling older adults in Hangzhou,China,between August and December 2024. Data were collected using the following validated instruments,namely the General Information Questionnaire,Age-adjusted Charlson Comorbidity Index,Chinese version of the Nutritional Form for the Elderly,Osteoporosis Self-Efficacy Scale,Social Frailty Screening Tool,Osteoporosis Risk One-Minute Test,and Osteoporosis Self-Assessment Tool for Asians. Additionally,objective physical measurements were obtained,including calf circumference,handgrip strength,and five-times sit-to-stand test time. The influencing factors were analyzed using univariate analysis and binary logistic regression,and two-step cluster analysis was employed for clustering to construct group profiles. Results 770 valid questionnaires were collected. The detection rate of high-risk osteosarcopenia in the elderly community is 27.53%. Older age,low BMI,prolonged sitting,chronic pain,low intake of soy products,low osteoporosis self-efficacy,and high social frailty are associated with a higher risk of osteosarcopenia. Totally 3 population portraits were constructed through clustering,including health-maintenance type,risk-ignoring type,and active management type. Conclusion The detection rate of high-risk elderly individuals with osteosarcopenia in communities is relatively high,with significant differences across groups. Intervention strategies can be proposed based on different group characteristics,providing a reference for precise interventions.

    A qualitative study on the management dilemma of coexisting chronic diseases among elderly patients in the communities

    Objective To understand the challenges faced by elderly patients with multiple chronic conditions in community settings during the process of disease management,and to provide a basis for developing intervention strategies for managing comorbidities in the elderly. Methods A phenomenological research approach was adopted,using purposive sampling. From July to October 2024,16 elderly patients with multiple chronic conditions from 5 community service centers in Shenzhen were selected as study participants. Semi-structured in-depth interviews were conducted,and the interview data were analyzed using Colaizzi’s seven-step method. Results Totally 4 themes and 8 sub-themes were extracted,including the disorder of multi-source health information structure and environment (insufficient systematic integration of health guidance,uneven quality of knowledge under multiple information channels),the exhaustion of self-management efficacy among chronic disease patients(the coexistence of multiple diseases aggravates the burden of physical and mental symptoms;negative emotions weaken the efficacy of active management),the absence of an integrated management mechanism for multiple diseases(repeated medical visits lead to poor treatment experience;multiple medications pose potential health risks),and the lack of in-depth and dynamic social support practices(formal operation of community support systems,family support facing the dilemma of filial piety and control). Conclusion Elderly patients with multiple chronic conditions in community settings face numerous challenges in disease management. Current chronic disease management in China primarily focuses on single diseases. It is recommended that healthcare professionals shift their perspective,improve the identification,assessment,nursing,and treatment of comorbidities in the elderly,and provide integrated chronic disease management approaches. This would enable a "one-stop" solution to the management challenges of multimorbidity in elderly patients,thereby enhancing their quality of life.

    This case study summarizes the nursing experience of a patient with right forearm damage who underwent total shape combined replantation reconstruction after heterotopic limb salvage. Key nursing points included:preoperative assessment of the patient’s injury and emergency treatment,formulation of a personalized surgical plan in collaboration with multiple departments,and complete preoperative preparation. Postoperatively,implement phased individualized nursing care postoperatively,strengthen monitoring and prevention of vascular emergencies,control and treat wound infections, implement multidimensional pain management strategies,provide comprehensive psychological care, and conduct stepwise functional rehabilitation training. After 2 stages of surgery and 112 days of treatment and nursing care,the patient’s right forearm showed good functional recovery,and the patient was discharged successfully.

    The nursing experience of buttonhole cannulation for an autogenous arteriovenous fistula with an Eluvia drug-eluting stent in a patient undergoing maintenance hemodialysis was summarized. Key nursing points included:developing a scientific and rational buttonhole cannulation plan for the autogenous arteriovenous fistula stent based on its characteristics post-stent implantation;conducting preoperative ultrasonic evaluations of the arteriovenous fistula and stent;establishing and maintaining a buttonhole tunnel at the stent site;regularly monitoring and following up on the cannulation site and the condition of the autogenous arteriovenous fistula;strengthening the observation and prevention of potential complications. Through rigorous and standardized assessments,procedures,and nursing care,the patient maintained good autogenous arteriovenous fistula function without stent-related complications such as fracture,stenosis,or infection over a 14-month follow-up period.

    Objective To systematically evaluate the factors affecting taste alteration in patients undergoing cancer chemotherapy. Methods PubMed,Cochrane Library,Web of Science,CINAHL,Embase,CBM,CNKI,WanFang Data and VIP were searched from the establishment until August 1,2024. The retrieved literature was independently screened,evaluated and the data were extracted by 2 researchers,and statistical analysis was performed using Stata 17.0 software. Results A total of 18 studies were included,involving 4 686 patients. The incidence of taste changes is 73.46%. Totally 9 influencing factors were extracted through quantitative analysis,including oral mucositis(OR=1.98),dry mouth(OR=1.82),nausea(OR=3.05),loss of appetite(OR=2.41),use of triple antiemetic drugs(OR=2.45),gynecological cancers(OR=0.67),lung cancer(OR=0.57),paclitaxel types of chemotherapy drugs(OR=2.86),and smoking(β=9.38). Conclusion The alteration of taste in cancer chemotherapy patients is in-fluenced by multiple factors. Nurses should regularly and dynamically assess changes in taste and implement individualized and refined nursing interventions in clinical practice to prevent or delay the development of taste alterations,thereby improving patients’ quality of life and treatment adherence.

    Objective To systematically integrate the qualitative studies on body image experience in breast cancer patients,in order to provide references for the clinical formulation of corresponding intervention programs. Methods Systematical search was conducted for qualitative studies on the body image experiences of breast cancer patients in databases including PubMed,Web of Science,Cochrane Library,CINAHL,PsycINFO,CNKI,WanFang Database,VIP and CBM. The search period was from the establishment of the database to July 2024. The quality of the literature was evaluated using the 2016 edition of the qualitative research appraisal tool from the Evidence-Based Health Care Center of the Joanna Briggs Research Institute in Australia,and the results were integrated using the pooled integration method. Results A total of 14 pieces of the literature were included,and 36 research results were extracted and summarized into 7 new categories. Totally 3 integrated results were obtained,namely the negative impact of changes in body image,patients’ self-adjustment and growth,and patients’ need for external support. Conclusion Breast cancer patients are plagued by multiple body images. Medical staff should pay attention to the psychological state of patients,do a good job in health education,improve the understanding and support of the society,provide targeted professional guidance,promote the psychological adaptation of patients,meet the emotional needs of patients,establish multiple support systems,and improve the body image and quality of life of patients.
