    Evidence-based practice of discharge preparation services for hip replacement patients

    Objective To construct an evidence-based practice program of discharge preparation services for hip replacement patients based on the best evidence,and to evaluate its clinical effects,aiming to provide references for the implementation of relevant interventions. Methods Knowledge to Action Framework was used as a guide to summarise the best evidence on discharge preparation services for hip replacement patients and to select services based on clinical context. We selected 38 hip replacement patients and 17 healthcare professionals from the Department of Orthopaedic Surgery at a tertiary A hospital in Shanghai in September-October 2023 to conduct a baseline review, identify facilitating and hindering factors,and develop a practice protocol. We also selected 38 hip replacement patients and 17 healthcare professionals from November-December 2023 on this ward for evidence-based practice. Nurses’ rates of completion of review indicators,levels of knowledge of discharge preparation services,and discharge planning competencies were compared before and after practice. Patients’ readiness for discharge,hip function,and unplanned readmissions were also compared before and after the practice. Results After evidence-based practice,nurses’ completion rates of review indicators,knowledge of discharge preparation services,and discharge planning competencies were higher than baseline levels(P<0.05). Hip replacement patients showed a significant improvement in readiness for discharge and hip function(P<0.001). Conclusion The implementation of an evidence-based practice program of discharge preparation services improves nurses’ knowledge and skills,and also contributes to the discharge readiness and hip function of hip replacement patients.

    The effect of family functioning on exercise adherence in elderly patients with postoperative vertebral compression fractures and adjacent vertebral re-fractures

    Objective To test the longitudinal mediating mechanism of kinesophobia between family functioning and exercise adherence in elderly patients with postoperative adjacent vertebral re-fracture after osteoporotic vertebral compression fracture(OVCF). Methods Convenience sampling method was used to admit to the Department of Orthopaedics of a tertiary hospital in Xuzhou City with adjacent vertebral re-fracture after OVCF were conveniently selected as the survey subjects,and longitudinal investigation was conducted using the APGAR,Functional Exercise Adherence Scale,and TSK Scale. Unconditional potential growth model,structural equation and Bootstrap method were used for statistical analysis. Results A total of 232 valid questionnaires were collected. Longitudinal mediation modeling revealed that the intercept of family functioning significantly negatively predicted the intercept of kinesophobia(β=-0.456,P<0.001) and the slope of family functioning significantly negatively predicted the slope of kinesophobia(β=-0.962,P<0.001). The intercept for kinesophobia significantly negatively predicted the intercept for exercise adherence(β=-0.623,P<0.001) and significantly negatively predicted the slope for exercise adherence(β=-0.354,P=0.013). Conclusion The initial level and rate of development of kinesophobia play a fully longitudinal mediating role in the development of family functioning on exercise adherence. Medical professionals should assess and manage patients’ family functioning and kinesophobia in a timely manner,and rationally utilize their interrelationships to improve the level of exercise adherence as much as possible.

    Experiences of postoperative home-based exercise rehabilitation in elderly patients with hip fracture:a qualitative study

    Objective This qualitative study aimed to explore the home-based exercise rehabilitation experiences of elderly patients after hip fracture surgery,in order to provide references for exercise intervention during rehabilitation period. Methods Objective sampling method was used to select 13 elderly hip fracture patients who were hospitalized in the joint surgery department of a tertiary A hospital in Beijing from August 2023 to June 2024 as the research subjects. Semi-structured interviews were conducted,and the interview data were sorted and analyzed by content analysis method. Results The home-based exercise rehabilitation experience of elderly patients after hip fracture surgery can be summarized into 3 themes,including multi-dimensional self-empowerment,use of dynamic personalized skills,full cycle balance reconstruction. Conclusion Patients after hip fracture surgery have rich experiences in home exercise rehabilitation,including self-empowerment,exercise skills and balance reconstruction. Nursing staff should strengthen long-term follow-up,according to the real experience and feeling of the patient’s home exercise rehabilitation in the whole cycle,actively assist the patients to self-empower,and use various exercise skills to constantly rebuild balance to achieve rehabilitation.

    Prevalence and risk factors of falls in patients with knee osteoarthritis:a Meta-analysis

    Objective To clarify the evidence of the frequency and risk factors for falls in knee osteoarthritis(KOA) of adults by meta-analysis. Methods Computerized searches of the CNKI,VIP,Wanfang data,CBM,PubMed,Cochrane Library,Embase,Web of Science were conducted for literature on risk factors for falls in adults with KOA from the inception of the databases to August 2024. After literature screening,data extraction,and quality evaluation,RevMan 5.4 software was used for Meta-analysis. Results A total of 26 articles were involved.Meta-analysis result showed that the rate of falls was 29.0%.Factors associated with increased risk of falls included being female(OR=1.35),decreased lower limb muscle strength(OR=1.72),decreased knee flexion muscle strength(OR=7.05),decreased static posture stability(OR=1.28),opioid use(OR=1.79),antidepressant use(OR=1.69),frequent stair climbing(OR=7.58),combined neurological disease(OR=1.77),history of falls(OR=3.29)and fear of falling(OR=2.54). Conclusion The rate of falls of patients with KOA is high. The adults with KOA who are women,have lower muscle strength of lower limbs and knee flexion muscle strength,poorer static posture stability,use opioids,antidepressant,frequent stair climbing,combined neurological disorders,previous falls in the past year and fear of falls are at higher risk of falls.Healthcare professionals should dynamically assess and detect the risk of falls in the patients with KOA and adopt targeted,individualized interventions to prevent falls.

    Application of gamification programs in exercise rehabilitation for knee osteoarthritis:a scoping review

    Objective To conduct a scoping review of the application of gamification programs in exercise rehabilitation for knee osteoarthritis,examining the application carriers,game content,outcome indicators,measurement tools,and application effects of gamification programs,to provide references for future practice and related research in this field. Methods The search was conducted in PubMed,Web of Science,Cochrane Library,Scopus,CINAHL,China National Knowledge Infrastructure,CNKI,and Wanfang Data from their inception to December 28,2024. The included studies were summarized and analyzed. Results A total of 28 studies were included in the review. The game carriers encompassed virtual reality technology,sensor devices,and mobile applications. The game content covered interactive motion games,aerobic exercises,and goal-motivated games. Outcome indicators included functional performance,physical activity,pain,psychological and health status,and user experience. Gamification programs were found to effectively enhance user engagement,improve physical function,reduce negative emotions,and improve quality of life. However,there remains controversy regarding their efficacy in pain relief. Conclusion Gamification programs have shown positive effects in exercise rehabilitation for knee osteoarthritis. Future efforts should focus on developing gamification programs that are culturally appropriate for China,creating “digital therapeutics”,continuously updating systems,conducting economic evaluations,and ensuring digital equity to enhance patients’ rehabilitation experiences and improve health outcomes.

    Objective To gain an in-depth understanding of influencing factors on the preventive behaviors of operating room nurses regarding hypothermia during cesarean sections,and to provide references for promoting the prevention of hypothermia during cesarean sections. Methods A descriptive qualitative research approach was adopted,based on the theoretical framework of the Precede-Proceed Model to develop an interview outline. Semi-structured interviews were conducted with 12 operating room nurses,and the interview data were analyzed using directed content analysis. Results The influencing factors were categorized into 3 main themes and 8 sub-themes.①Predisposing factors:insufficient theoretical knowledge,low risk awareness among healthcare providers.②Enabling factors:rational allocation of equipment/resources,proficient operational skills,targeted and effective training programs,and standardized preventive procedures.③Reinforcing factors:the need to enhance team cooperation,the need to improve the regulatory mechanism. Conclusion Nursing managers should strengthen education and training to enhance nurses’ awareness and skill levels in preventing hypothermia,improve equipment resources and build an information-based assessment and decision support system to promote the implementation of hypothermia prevention behaviors. Additionally,facilitating team collaboration and utilizing information construction to collect and analyze quality indicators can strengthen quality management and promote continuous improvement in the quality of hypothermia prevention during cesarean sections

    Objective To investigate the knowledge and attitude of clinical nurses on Panvascular Disease (PVD) care in Zhejiang Province,and to analyze the influencing factors,so as to provide references for constructing a systematic PVD training program. Methods The convenient sampling method was used to investigate 454 nurses from 36 general hospitals in 11 prefectures and cities in Zhejiang Province in June 2024 with a self-designed questionnaire on general information and a questionnaire on PVD nursing knowledge and attitude questionnaire,and multiple stepwise linear regression was used to analyze the impact factors. Results 452 valid questionnaires were collected and the valid recovery rate was 99.56%. Nurses scored(8.57±2.02) in knowledge dimension,(60.82±6.45) in attitude dimension. The results of multiple stepwise linear regression analysis showed that hospital grade was the factor influencing the score of nurses’ knowledge towards PVD nursing in Zhejiang Province(P<0.05). Hospital grade,whether they had received systematic knowledge training related to PVD were the factors influencing the score of nurses’ attitude towards PVD nursing(P<0.05). Conclusion Nurses in Zhejiang Province are positive about PVD nursing,but their knowledge level needs to be improved. Nursing managers should pay full attention to the training intention of clinical nurses,establish a PVD knowledge training system,and strengthen the theoretical and practical training of clinical nurses on PVD nursing,so as to promote the development of PVD nursing work.

    Objective To develop a continuous care program for patients with cardiac implantable electronic devices(CIED) utilizing the Omaha system,and to assess its effectiveness. This study aims to provide a reference for the continuous care of patients with CIED. Methods Using the Omaha system as the theoretical framework,based on literature search and semi-structured interviews,a continuity of care program for CIED patients based on the Omaha system was constructed through 2 rounds of expert inquiry. Convenience sampling method was used to select CIED implant patients from the cardiovascular department of a tertiary hospital in Wuhan as the research subjects. 61 patients admitted from April to June 2024 were assigned as an experimental group with a continuity of care program based on the Omaha system;61 patients admitted from December 2023 to March 2024 were designated as a control group and received routine nursing measures. The Omaha performance indicators and quality of life at 3 months after discharge were compared between 2 groups upon admission and 1 month after discharge. Results The final 2 groups each included 59 patients. A month after discharge,the Omaha efficacy index in the experimental group was higher than that in the control group,and 3 months after discharge,the quality of life in the experimental group was higher than that in the control group(P<0.001). Conclusion The CIED patient continuity of care program based on the Omaha system could improve patients’ cognitive level,promote healthy behavior,enhance their self-efficacy,and improve their quality of life.

    Analysis of influencing factors on the trajectories of psychological symptom clusters in pregnant women with assisted reproductive technology and nursing revelation

    Objective To explore the trajectory of psychological symptom clusters in pregnant women with assisted reproductive technology(ART),and analyze the influencing factors of each trajectory subgroups,in order to provide a theoretical basis for the management of psychological health during pregnancy in pregnant women with ART. Methods A total of 205 pregnant women who had conceived using ART were sampled from the obstetrics clinic of a tertiary hospital in Nanjing from August 2023 to April 2024 using a convenient sampling method. The baseline data were assessed by general information questionnaire,Symptom Checklist-90,Distress Disclosure Index and Positive Psychological Capital Questionnaire at 10-14 weeks gestation,and the follow-up information was assessed by Symptom Checklist-90 at 22-26 weeks of gestation and 34-38 weeks of gestation. Exploratory factor analysis was used to extract symptom clusters;the latent class growth mixture model was used to identify the track categories;the multiple logistic regression analysis was used to analyze the influencing factors of the track. Results 180 cases were finally included. By exploratory factor analysis,5,4 and 5 factors were extracted at 3 time points respectively. Trajectories of psychological symptom clusters in pregnant women with ART is divided into 3 potential classes:low level-slow relieving group(28.89%),high level-significant increasing group(6.11%),medium level-slow increasing group(65.00%). Logistic regression analyses showed that duration of infertility,number of ART,literacy,pain self-expression and positive psychological capital were influential factors in the potential categories of psychological symptom clusters in pregnant women conceived with ART(all P<0.05). Conclusion The trajectory of psychological symptom clusters in pregnant women with ART was divided into 3 potential classes. Medical workers could develop corresponding interventions based on the influencing factors and implement comprehensive and efficient symptom management.

    Objective To identify the multi-dimensional needs of symptom management for oral cancer patients based on the journey map,and provide references for optimizing the symptom management of oral cancer patients. Methods From September 2023 to March 2024,the purposive sampling was used to select 15 perioperative oral cancer patients from a tertiary A general hospital in Zhengzhou for semi-structured interviews. The content analysis method was used to analyze the data and create a patient journey map. Results According to the time axis of diagnosis and treatment,the symptom management of patients with oral cancer was subdivided into 22 themes including identifying abnormal symptoms,clarifying diagnosis,anxiety,fear and uncertainty,guilt,high-risk behavioral inertia solidification, diagnostic trust crisis and so on from 3 dimensions of task,emotion,and pain point,and a journey map was formed. Conclusion The journey of symptom management for patients with oral cancer is long and complex,and the needs of patients’ physiological and psychological symptom management are dynamically changing. In the future,the digital intelligence of big data technology can be combined to achieve whole-process,personalized and precise symptom management to improve the quality of life of oral cancer patients.

    The medication experience of adolescents with mood disorders:a qualitative research

    Objective The aim of this study is to explore the medication experience of adolescents with mood disorders,providing references for promoting their medication adherence. Methods Descriptive qualitative research was utilized. A total of 16 teenager patients with mood disorders from a tertiary A hospital in Lanzhou City,were selected for semi-structured interviews by purposive sampling. The data was analyzed by the Colaizzi 7-step analysis method. Results A total of 4 themes were extracted. ①Cognitive experiences(Confusion and rejection at the initial stage;acceptance and compromise with time;self-stigmatization of medication). ②Emotional experiences (expectations of medication effects at the initial stage;concerns and fears about side effects;anxiety of long-term medication;disappointment of the effect;desire for medication reduction). ③Behavioral experiences(helpless persistence;poor medication behavior;coping with medication stigma on campus;traumatic experience of suicide by taking an overdose). ④Multiple obstacles to the medication(family prejudice and misconceptions about medication;lack of acceptance and understanding by peers;lack of care and communication from health professionals;shortage of mental healthcare resources;limited access to medication information). Conclusion In the process of medication,the cognitive and emotional experience of adolescent patients with mood disorders showed multiple and changing characteristics,and there were behavioral experiences of poor medication compliance and poor coping style. At the same time,adolescents expressed diverse needs for medication. In the future,we should actively explore patient-centered personalized nursing according to the dynamic experience and demand of adolescents for drug treatment.

    Validation and application of Chinese Clinical Context Assessment for Community Health in evidence-based hypertension medication management

    Objective To translate the Context Assessment for Community Health(COACH) scale into Chinese,thereby providing a standardized measurement tool for context assessment in clinical evidence-based practice. Methods The scale was translated following the Brislin translation model,ultimately forming the Chinese version of the COACH scale. Using convenience sampling,584 healthcare professionals were recruited from 3 tertiary hospitals in Jiangxi,Henan,and Liaoning provinces and 2 community hospitals in Liaoning province from May to September 2024 to evaluate the reliability and validity of the scale. In October 2024,the Chinese version of COACH was applied to 44 healthcare professionals in a hypertension specialist ward of a provincial tertiary hospital. Results Of the 584 distributed questionnaires,519 valid questionnaires were collected(the effective response rate of 88.87%). The Chinese version consists of 7 dimensions with 42 items. The overall Cronbach’s α coefficient was 0.975;the McDonald’s omega coefficient was 0.979;split-half reliability was 0.808,and the test-retest reliability was 0.917. The scale-content validity index/average was 0.950,and the scale-content validity index/universal agreement was 0.980,with item-content validity index ranging from 0.775 to 1.000. Exploratory factor analysis extracted 7 factors,accounting for 79.260% of the cumulative variance,with factor loadings ranging from 0.569 to 0.954. Confirmatory factor analysis showed good model fit. The preliminary application results showed that the total score of the scale was 185.18±17.82. The correlation coefficients between dimensions and total score ranged from 0.116 to 0.905. Conclusion The Chinese version of the COACH scale demonstrates good reliability and validity,making it suitable as a context measurement tool for implementation research at different stages in clinical and public health settings.

    Development and validation of a safety assessment scale for postoperative early mobilization for lung transplant patients

    Objective To develop and validate a safety assessment scale for early mobilization in lung transplant patients after surgery. Methods From September 2022 to April 2023,a preliminary scale was constructed using literature analysis,expert meetings,and the Delphi method. From June 2023 to May 2024,a convenience sampling was used to select 135 patients admitted to the lung transplant center of a tertiary general hospital in Jiangsu Province. Item analysis,reliability analysis,and validity analysis were applied to adjust the items,forming the final scale. Results The post-operative early mobilization safety assessment scale for lung transplant patients includes 5 dimensions(consciousness level,vital signs,disease-related factors,mobility,and nutritional status) and 18 items. The inter-rater reliability of the scale is 0.985;the overall content validity index is 0.853;the split-half reliability is 0.716. The area under the receiver operating characteristic curve for predicting adverse events within 72 hours of mobilization is 0.845,with a 95% confidence interval of 0.767 to 0.917(P<0.001). The optimal cutoff value is 18.5,with a sensitivity of 0.735 and a specificity of 0.782. Conclusion The developed safety assessment scale for early mobilization in lung transplant patients after surgery demonstrates good reliability and validity,and it can serve as a clinical tool to assess the safety of early mobilization activities in these patients.

    Objective To explore the influencing factors of family participation in medication safety of aged patients with multi-morbidity,and to provide references for clinical practice. Methods Based on capability,opportunity and motivation-behavior model,17 family caregivers of aged patients with multi-morbidity at 8 communities of Zhengzhou,Henan Province from September to November 2024 were selected for semi-structured interviews using purposive sampling method. The data were analyzed using directed content analysis. Results Facilitators of family participation behavior included ability factors(strong self-reflection and emotional regulation ability),opportunity factors(family and peer support,professional support from medical staff),motivational factors (par-ticipation in medication safety importance perception,high level of participation self-efficacy,strong sense of responsibility). Barriers of family participation behavior included ability factors(the family caregivers’ own aging and disease limitations,and the family caregivers’ medication safety management ability is insufficient),opportunity factors(the excessive physical and mental independence of the patients,the multiple role conflict of the family caregivers). Conclusion There are some facilitators and barriers in the process of family participation in medication safety of elderly patients with multi-morbidity. Medical staff should attach importance to family participation experience,adopt targeted strategies to promote family participation in medication safety of elderly patients with multi-morbidity,so as to promote disease recovery.

    Objective To explore the factors affecting social frailty in elderly patients with mild ischemic stroke and conduct interpretable analysis to provide references for the implementation of targeted intervention. Methods Using convenience sampling method,elderly mild ischemic stroke patients treated in a tertiary A hospital in Tangshan from September 2022 to August 2023 were selected as the survey subjects according to whether social weakness and no social weakness occurred 6 months after discharge. General Information Questionnaire,Chronic Illness Resources Survey,Family Care Index Questionnaire,Instrumental Activities of Daily Living Scale,Mini-Mental State Examination, International Physical Activity Questionnaire Short Form,and Social Frailty Scale were uesd to investigate. Univariate analysis,random forest-feature recurrence elimination screening variables were used to construct random forest models,and SHAP values were introduced for influencing factor analysis. Results A total of 818 elderly patients with mild ischemic stroke were included,and 110 patients were lost at 6 months,and 708 patients(86.55%) finally completed the survey. The prediction accuracy of random forest model was 0.828 and 0.807 in the training set and test set. The SHAP interpretability analysis results showed that impaired self-care ability,sedentary behavior,and the number of combined chronic diseases were positively associated with social weakness,ideal utilization of chronic disease resources,good family care and social weakness;the importance of characteristics was self-care ability,chronic disease resource utilization,family care,sedentary behavior,and combined chronic diseases. Conclusion The influencing factors of social frailty in elderly patients with mild ischemic stroke are complex,so medical staff should pay attention to the social function of elderly patients with mild ischemic stroke,and reduce the risk of social frailty by formulating personalized intervention measures.

    To summarize the perioperative nursing experience of a patient with thoracic and abdominal aortic dissection aneurysm after Stanford type A aortic dissection surgery,who underwent intra-abdominal aortic aneurysm endovascular isolation,in situ fenestration branch covered stent placement,and artificial blood vessel bypass. The nursing points include regular evaluation of the intra-abdominal pressure of the patient,being on guard against the rupture of the dissection;specialist nurses participated in the preoperative multidisciplinary discussion;establishing an emergency plan for the rescue of ruptured aortic dissection;monitoring the vital signs of patients closely to prevent critical complications during surgery. After multidisciplinary treatment and meticulous care,the patient’s surgery went smoothly and the postoperative recovery was good. The patient was discharged 8 days later.

    To summarize the postoperative nursing experience for a child with Uhl’s anomaly undergoing complex heart surgery. Nursing key points include implementing goal-oriented liquid negative balance management to promote the recovery of cardiac function;carrying out pressure load management and closely monitoring the heart rate and rhythm to maintain the stability of the circulatory system;closely monitoring the central venous pressure in the upper chamber and paying attention to the changes of the pulmonary artery pressure in real time;adopting a refined integrated management to avoid an abnormal rise in the pulmonary artery pressure;withdrawing invasive mechanical ventilation as early as possible and carrying out the non-invasive ventilator deconditioning training in an orderly manner;the synergistic management of bleeding and anticoagulation;the adoption of various measures to actively prevent complications;the prevention and control of infections in an all-round and multi-angle manner;the formulation of individualized cardiac rehabilitation training and nutritional intervention programs. The child was transferred from the pediatric intensive care unit back to the pediatric ward on postoperative day 9,transferred back to the pediatric intensive care unit on postoperative day 11 for 13 d for right atrial thrombus, stabilized and transferred back to the pediatric ward,and discharged after 10 d for recovery. The prognosis showed good recovery after 1 month and 3 months.

    To summarize the nursing experience of a patient with anthrax meningoencephalitis caused by cerebrospinal fluid rhinorrhea post-trauma. Nursing points include strict implementation of isolation and hospital infection prevention and control measures;to standardize the use of antibiotics throughout the process to prevent and manage antimicrobial-related diarrhea;to pay attention to the nutritional status of patients and develop individualized nutritional support programs based on evidence;to implement refined airway care to prevent pulmonary complications;to strengthen disease observation,identify intracranial hemorrhage in time;to provide patient and family-centered psychological empowerment in the whole process. After 66 days of careful treatment and nursing care,the patient recovered and was discharged. A follow-up visit after 2 months of discharge showed a good recovery.

    Objective To conduct a scoping review of studies related to gamified physical activity interventions for pediatric cancer patients,and to extract the gamification elements and application effects within physical activity intervention programs. Methods A computer-assisted search was conducted in CNKI,Wanfang Data,VIP Database,China Biology Medicine disc,Cochrane Library,PubMed,Embase,and Web of Science for studies on gamified physical activity interventions in pediatric cancer patients,with a search period from database inception to December 31,2023. The included literature was screened,summarized,and analyzed. Results A total of 18 articles were included,including 9 randomized controlled trials,4 quasi-experimental studies,and 5 mixed-method studies. The gamified intervention programs for physical activity in children with cancer integrated 7 gamification elements,including goal setting,capacity to overcome challenges,providing feedback on performance,reinforcement,progress monitoring,social connectivity,and fun and playfulness. The types of physical activity in the intervention programs included aerobic exercise,balance training,strength training,endurance training,etc. The intensity of the activities was mainly low to moderate;the duration was mostly 30~60 minutes per session;the intervention duration ranged from 5 weeks to 1 year. Numerous research findings indicate that gamified physical activity interventions for children with cancer can help improve physical function,quality of life,and fatigue levels. However,there is signi-ficant controversy regarding their impact on improving physical activity levels. Conclusion The gamified inter-vention for physical activity in children with cancer was safe and feasible. It is recommended that in the future,personalized and phased gamified intervention programs should be developed to evaluate the intervention effects.

    Occupational health issues among nurses significantly impact their quality of life and the quality of nursing services. Wearable devices,equipped with functions such as real-time monitoring,intelligent assessment,and early warning feedback,can provide technical support for occupational health surveillance among nurses. This paper reviews the concept of nurses’ occupational health,the advantages of wearable devices in monitoring nurses’ occupational health,and the current applications of wearable devices in monitoring both physiological and psychological health among nurses. It also analyzes the challenges and proposes countermeasures,aiming to provide insights for optimizing occupational health management for nurses.
