    Construction and application of a spinal cord injury rehabilitation care platform based on patient portrait

    Objective To construct a rehabilitation care platform for spinal cord injury patients,based on patient portrait technology and to validate its application effect. Methods Artificial intelligence technology was used to realize the fusion of data. We constructed a patient portrait rehabilitation nursing system that can provide systematic and personalized information support. By the convenience sampling method,53 perioperative patients with spinal cord injury from July 2023 to December 2023 were selected as the test group to implement the intelligent intervention based on patient portrait technology;53 perioperative patients with spinal cord injury who were hospitalized from January 2023 to June 2023 were selected as the control group to implement the conventional information support intervention. The self-efficacy,self-care ability,and quality of life were compared before the intervention,and 2 weeks,4 weeks,12 weeks after the intervention between the 2 groups. Results 45 cases in the experimental group and 43 cases in the control group reached the endpoint of the study after 12 weeks of intervention. The ANOVA of the 2 groups showed the test group having a greater advantage over the control group,with statistically significant differences in self-efficacy,ability to perform daily living tasks,and quality of life between groups,intergroup effect,and interaction effect(P<0.05). Conclusion The rehabilitation care platform based on patient portraits has a positive effect on improving self-efficacy,self-care ability and quality of life for spinal cord injury patients.

    Objective To construct an intervention programme for breastfeeding among mothers of infants with congenital heart disease and to verify its effect. Methods Taking the capability-opportunity-motivation-behaviour model as the theoretical basis,the first draft of the intervention programme was formed through literature search and semi-structured interviews,and 8 experts in the relevant fields were selected to conduct expert validation and determine the content of the program. By the consecutive sampling method,infants and their mothers who attended the cardiothoracic disease clinic of a tertiary-level children’s specialist hospital in Shanghai from 4 May to 24 June 2023,with a day-old age of ≤14 d and a confirmed diagnosis of CHD,were selected as the study subjects. They were divided into an experimental group and a control group,with 16 cases in each group,by the method of randomized grouping by district. The experimental group received breastfeeding intervention for mothers of infants with CHD on the basis of routine care;the control group received routine care and basic disease education and breastfeeding counselling. At 1 and 3 months of the infants with CHD,the 2 groups were compared in terms of exclusive breastfeeding rate,daily human milk as a percentage of total feeds,weight for age Z-score and height for age Z-score. Results A total of 1 round of expert validation was conducted,with a valid questionnaire recovery rate of 100% and an expert authority coefficient of 0.94. The breastfeeding intervention programme for mothers of infants with CHD is a comprehensive approach that encompasses 3 key dimensions,including capability,opportunity,and motivation. The programme is designed to enhance mothers’ ability to recognize feeding signals,master breastfeeding techniques,and maintain successful breastfeeding practices. It also addresses common breastfeeding challenges and provides effective solutions. Additionally,it includes strategies for breastmilk management and fosters family support for breastfeeding. The programme offers medical information support on breastfeeding and aims to elevate mothers’ understanding of its benefits. It involves systematic monitoring and recording of breastfeeding volumes,as well as thorough assessments and guidance on infant growth and development,including but not limited to the 10 specific entries related to the infant’s progress. The results of the generalized estimating equations and repeated measures analysis of variance revealed statistically significant between-group differences(P<0.05) in the rates of exclusive breastfeeding,the proportion of daily human milk as a percentage of total feeds,and the weight-for-age(WAZ) and height-for-age(HAZ) z-scores of infants across the 2 groups at various time points. The results of simple effect analysis showed that the differences in exclusive breastfeeding rate,the proportion of daily human milk as a percentage of total feeds,WAZ and HAZ between the 2 groups before intervention were not statistically significant(P>0.05). At the age of 3 months of the infants after the intervention,the rate of exclusive breastfeeding in the experimental group was higher than that in the control group,and the difference was statistically significant(P=0.003). The proportion of daily human milk as a percentage of total feeds for infants in the intervention group at 1 and 3 months of age was higher than that in the control group,with a statistically significant difference(P<0.05). At 3 months of age,WAZ of the experimental group was higher than that of the control group,and the difference was statistically significant(P=0.037);HAZ of the experimental group was higher than that of the control group at 1 and 3 months of age after intervention,and the difference was statistically significant(P<0.05). Conclusion The breastfeeding intervention programme for mothers of infants with CHD,constructed on the basis of the COM-B model in this study,was comprehensive,feasible and acceptable. Implementation of the programme has potential positive effects on increasing exclusive breastfeeding rates and the daily human milk as a percentage of total feeds,and may also have a positive impact on WAZ and HAZ of infants.

    Objective To investigate the current status of frailty in liver transplant candidates and to analyze the influencing factors. Methods The convenience sampling method was used to select 150 liver transplant candidates who were hospitalized in the Department of Liver Transplantation of a tertiary hospital in Hangzhou from July 2022 to July 2023. They were investigated by the general patient data,Fried Frailty Phenotype,Barthel Index,Nutrition Risk Screening 2002,General Health Status Scale. Statistical methods including single factor and multifactor analysis were conducted for influencing factors of frailty in liver transplant candidates. Results The pre-frailty and frailty rates in 145 liver transplant candidates were 42.07% and 45.52%,respectively,and only 12.41% were non-frailty. The results of ordered multiple Logistic regression showed that age,total bilirubin,hemoglobin,and general health status were influential factors in the frailty of liver transplant candidates(P<0.05). Conclusion Liver transplant candidates have a high incidence of frailty. Age,total bilirubin,hemoglobin,and general health status are influential factors in frailty. Nurses should pay attention to the assessment of frailty in liver transplant candidates,and take timely and targeted nursing interventions to prevent or delay the occurrence and development of frailty.

    Status and influencing factors of the death anxiety in patients with primary liver cancer

    Objective To understand the current status of death anxiety in patients with primary liver cancer and analyze its influencing factors,so as to provide basis for developing individualized psychological guidance and intervention measures. Methods A convenient sampling method was used to select 302 patients with primary liver cancer who were hospitalized in a tertiary hospital in Jiangsu Province from March to November 2022. The General Demographic Information Questionnaire,the Templer’s Death Anxiety Scale(T-DAS),the State-Trait Anxiety Inventory(STAI),the Rosenberg Self-esteem Scale(SES) and the Self-perceived Burden Scale(SPBS) were used to conduct questionnaire survey during hospitalization. Single factor analysis and multiple linear regression analysis were applied to analyze the factors influencing the death anxiety in patients with primary liver cancer. Results A total of 298 patients with primary liver cancer were included in the study. The effective recovery rate was 98.68%. The total score of the F-DAS was(7.10±3.43) points;56.38% of the patients had high death anxiety. Multiple linear regression analysis results showed that the factors influencing the death anxiety in patients with primary liver cancer included gender,China liver cancer staging,duration of diagnosis,self-esteem,trait anxiety and self-perceived burden. A total of 49.00% of the variation in death anxiety was explained. Conclusion The death anxiety of patients with primary liver cancer is at a high level. It is suggested that medical staff should pay more attention to female patients with China liver cancer staging IV,diagnosis time﹥3 months and low self-esteem,high levels of trait-anxiety and self-perceived burden,in order to develop personalized intervention measures,carry out psychological counseling and emotional support to relieve patients’ death anxiety.

    Objective To analyze the application of a nursing outcome classification system in postoperative nursing of liver transplantation patients and its influence on the quality of clinical nursing records. Methods A total of 44 patients who underwent liver transplantation in a tertiary A general hospital in Zhejiang Province from March to August 2023 were selected by convenience sampling method. According to the North American Nursing Diagnosis Association International-Nursing Outcomes Classification linked scale of liver transplantation,nurses used the five-level classification method to score and review the liver transplantation patients on the day after surgery,3d after surgery,7d after surgery,14d after surgery,and on the day of discharge. The nursing outcome classification scores at 5 time points were statistically analyzed. The nursing records of 44 patients with liver transplantation from June to December 2022 were retrospectively collected by convenient sampling,and 44 nursing records before Nursing Outcomes Classification application and 44 nursing records after Nursing Outcomes Classification implementation were measured by Quality of Documentation of Nursing Diagnoses,Interventions and Outcomes. Results The Nursing Outcomes Classification scores of 15 nursing diagnoses were analyzed by single factor repeated measurement,and 9 of them showed continuous improvement and significant difference at 4 measurement time points(P<0.05). After Nursing Outcomes Classification implementation,the total score of nursing record quality,nursing diagnosis and nursing outcome record quality scores were higher than those before implementation,with statistically significant differences(P<0.05). Conclusion The nursing outcome classification system can be used to evaluate the postoperative health status and nursing effect of adult liver transplantation patients. It has high specialty specificity and can improve the quality of nursing records.

    To summarize the nursing experience of a patient with bilothorax after autologous liver transplantation. The key points of nursing include:early identification of reexpansion pulmonary edema,the start of the hospital rapid response team to give efficient rescue;early identification of bilothorax,and maintenance of effective drainage combined with accurate anti-infection treatment;individual and stage lung rehabilitation training to promote patients lung rehabilitation;step anticoagulant therapy is safely administered under dynamic assessment of bleeding risk. Through careful treatment and nursing,the patient was discharged 63 days after surgery.

    Objective To systematically evaluate the measurement properties and methodological quality of the liver cancer patients specific self-reported outcome assessment tools,and to provide a basis for healthcare professionals to select a high-quality scale. Methods Systematic search of PubMed,Embase,CINAHL,Web of Science,Cochrane Library,China National Knowledge Infrastructure,VIP Database,Wanfang Database,and China Biomedical Literature Database was conducted for studies related to the evaluation of the measurement properties of liver cancer specific self-reported outcome assessment tools. The search period was from the date of creation to February 1,2024,and 2 researchers independently conducted literature screening and data extraction according to the COSMIN selection criteria,and used the COSMIN Risk of Bias Inventory and Quality Criteria Scale to measure the methodological quality and measurement properties,and the final recommendations were formed. Results A total of 12 studies involving 8 specific self-reported outcome assessment tools for liver cancer patients were included. Finally,TSM-PLC was recommended at level A;EORTC QLQ-HCC18,QOL-LC,QLICP-LI were recommended at level C;FACT-HEP,MDASI-Periop-Hep,Quality of Life Scale for Liver Cancer Patients,and FHSI-8 were recommended at level B. Conclusion Compared to other scales,TSM-PLC has relatively good methodological quality and measurement properties,and is recommended for use as a priority. Due to its evaluation content mainly focusing on the patient’s physical symptom distress,it can be used in conjunction with other scales to enhance the comprehensiveness of the evaluation.

    Objective To evaluate the effect of prone ventilation in children with severe pneumonia at high altitude. Methods By convenience sampling method,80 children with severe pneumonia hospitalized in intensive care department of a tertiary A children’s hospital in Xining,Qinghai Province from June 2021 to June 2023 were selected as the study subjects,and the regional randomization group method was used to divide into a test group and a control group with 40 cases in each group. On the basis of routine care,the test group received prone ventilation once a day for 6 to 12 h;the control group received supine ventilation. Respiratory mechanical parameters(oxygenation index,oxygen saturation,arterial oxygen partial pressure,arterial CO2 partial pressure),mechanical ventilation duration and safety parameters (incidence of unplanned extubation,stress injury) at 6 h and 12 h of mechanical ventilation were compared. Results There were no shedding cases. The interaction between oxygenation index,blood oxygen saturation and arterial oxygen partial pressure(P<0.05) in the 2 groups was compared(P>0.05). The results of simple effect analysis showed that at 6 h,the oxygen saturation and oxygenation index in the test group were higher than those in the control group,and the difference was statistically significant(P<0.05). At 12 h of mechanical ventilation,the oxygenation index,blood oxygen saturation and arterial oxygen partial pressure in the test group were higher than those in the control group,and the difference was statistically significant(P<0.05). The time of mechanical ventilation in the test group was102.00(60.00,153.00) h and 126.00(108.00,156.00) h in the control group,and the difference was statistically significant(P=0.013). The incidence of unplanned extubation and pressure injury were compared,and the differences in 2 groups were not significant(P>0.05). Conclusion Prone ventilation in children with severe pneumonia at high altitude is safe and feasible,which is helpful to improve the respiratory function and shorten the time of mechanical ventilation.

    Objective To describe the status and influencing factors of psychological distress in older people living with HIV in Shanghai. Methods Convenience sampling was used to select HIV patients with the age of 50 years old and above who attended follow-up visits at the voluntary counselling & testing clinic in Shanghai Public Health Clinical Center from December 2022 to October 2023. Data were collected using a general information questionnaire,HIV disclosure questionnaire,Distress Thermometer and Lubben Social Network Scale-6. Binary logistic regression was employed to analyze the factors influencing psychological distress. Results The prevalence of psychological distress among 332 participants was 105 cases(31.6%). The top 3 items selected by participants in Psychological Distress Problem List were insurance/financial issues,worry and sleep,relationship with children. Factors associated with a higher level of psychological distress included lower monthly income,not HIV disclosure and higher levels of social isolation. Conclusion Psychological distress among older people living with HIV remains to be further reduced,and it is affected by many factors. Health care providers should pay more attention to psychological distress among older HIV patients,and develop targeted intervention according to relevant influencing factors,aiming to reduce their psychological distress.

    Objective To understand the real experience of kinesiophobia in patients after bone transport technique,providing references for taking targeted nursing interventions to alleviate kinesiophobia of patients. Methods Purposive sampling method was employed to select 15 patients who underwent bone transport technique in the Department of Traumatic Orthopedics in a tertiary A hospital in Guangdong Province from October to December 2023 as the research subjects. Phenomenological research method was utilized to conduct semi-structured interviews with the patients,and Colaizzi 7-step analysis method was applied for data analysis and theme extraction. Results A total of 3 themes and 11 sub-themes were extracted,including the existence of negative psychological experience(fear and concern regarding exercise,excessive alarm in response to pain,helplessness and sadness about the change of life,persistent reflection on past experiences,anxiety and confusion about the future),facing the dilemma of physiological symptoms(pain and discomfort,fatigue and disturbed sleep),taking diversified coping approaches(selecting avoidance strategies,conducting self-adjustment,seeking kinesiophobia related knowledge and exercise guidance,acquiring social support). Conclusion The experience of kinesiophobia in patients after bone transport technique is complex and varied. Medical and nursing staff should prioritize the psychological relief of patients after bone transport technique,pay attention to the assessment and management of kinesiophobia related symptom,provide professional guidance and assist with multi-dimensional support to help patients reduce the experience of kinesiophobia and promote recovery of patients.

    Objective To explore the dyad coping experience of stress in pregnant women with undifferentiated connective tissue disease and their spouses,providing a basis for developing dyadic coping intervention measures. Methods By purposive sampling,15 pairs of pregnant women and their spouses who visited the outpatient clinic for pregnancy complicated with immune diseases in a maternity hospital in Jiangsu Province from April to August 2023 were selected as the research subjects for semi-structured interviews. Content analysis was conducted using the Colaizzi 7-step method,and an interview outline and integrated themes were developed based on the Developmental Contextual Coping Model. Results 3 themes(coexistence of positive and negative stress experiences;diverse approaches to dyadic coping with stress;growth and challenges following stress adaptation) and 11 subthemes were identified. Conclusion Healthcare professionals should attach importance to the stress coping issues of pregnant women with undifferentiated connective tissue disease and their spouses,take measures to regulate the negative emotions of couples,ensure multi-faceted support,help the couple adopt positive coping strategies,and promote good stress adaptation.

    Development and validation of a Behavioral Decision-making Scale for Glycemic Management in Pregnant Women with Gestational Diabetes

    Objective To develop a Behavioral Decision-making Scale for Glycemic Management in pregnant women with gestational diabetes and to test its reliability and validity. Methods Based on the trans-theoretical model and behavioral decision theory,the test version of the scale was formed through literature review,semi-structured interview,brainstorming,2 rounds of expert consultation and cognitive interview. A total of 560 pregnant women with gestational diabetes mellitus were recruited from 10 hospitals in Quanzhou,Fujian Province by convenience sampling method from 21 July to November 2023. The data were divided into 2 parts by random number method for exploratory factor analysis and confirmatory factor analysis. Results The scale included 4 dimensions of “behavioral decision-making motivation”“behavioral decision-making influencing factors”“behavioral decision-making intention” and “behavioral decision-making effectiveness” with 34 items. The Cronbach’s α coefficient of the total scale was 0.971;the split-half reliability was 0.919;the test-retest reliability was 0.863;the content validity index of the scale was 0.853. The exploratory factor analysis extracted 4 common factors,and the cumulative variance contribution rate was 78.28%. The confirmatory factor analysis showed that the factor structure of the scale was stable. Conclusion The scale has ideal reliability and validity,which can be used to measure the level of glycemic management behavior decision-making of pregnant women with gestational diabetes mellitus.

    Objective To translate the Misscare Survey Operating Room(OR) Scale into Chinese,and to test its reliability and validity. Methods The improved Brislin model is used for translation. The Chinese version scale was determined after cross-cultural adaptation and pre-experiment. A convenient sampling method was used to investigate 310 operating room nurses from 2 tertiary A hospitals in Liaoning province from October to December 2023 to analyze the reliability and validity of the scale through questionnaire survey. Results The Chinese version scale of the Misscare Survey OR included 28 OR nursing activities in part A and 16 items of missed nursing care in part B. The Cronbach’s α coefficient was 0.949 and 0.886,respectively;the test-retest reliability was 0.887 and 0.784,respectively. In part A,the exploratory factor analysis extracted 5 factors,including law,preparedness,security,communication and closure procedures,with a cumulative variance contribution of 85.718 %. In Part B,4 factors (urgency,staffing,materials,teamwork) were extracted by exploratory factor analysis,and the cumulative variance contribution rate was 82.965%. In Part A,the content validity and the content validity index of the item level of the scale was 0.80~1.00 and 0.98;in Part B,the content validity and the content validity index of the item level of the scale was 0.80~1.00 and 0.96. Conclusion The Chinese version of the Misscare Survey OR Scale has good reliability and validity,which can be used to measure the missing items of the OR nurses in China and reflect the missing reasons,and it can be used as an evaluation tool to evaluate the missed care of OR nurses.

    To summarize the nursing care of a case with perioperative sudden electrical storm of heart valvular disease treated with stellate ganglion block. Nursing points are as follows:to avoid triggering factors before surgery,with early identification of electrical storms;to implement emergency management of electric storm and improve rescue quality;to improve the operation preparation,intraoperative cooperation and observation;to conduct rapid and accurate assessment and pre-control of postoperative complications;to formulate rehabilitation strategies to promote postoperative rehabilitation;to strengthen psychological intervention to ensure emotional stability. The patient was transferred to the department of cardiac surgery from the department of cardiology to install a permanent cardiac resynchronization pacemaker in 6 days after surgery,and discharged successfully 10 days after surgery. The results were good at 14 d and 28 d post-discharge follow-up.

    To summarize the nursing experience of a patient with cardiac resynchronization therapy defibrillator (CRT-D) implantation who was about to deplete battery due to repeated discharge after implantation and underwent left ventricular assist device implantation combined with cryoablation. Key points of care were as follows. To begin with,we actively managed arrhythmias and electric storm. Secondly,we carried refined capacity management to avoid water and sodium retention. Thirdly,we formulated personalized anticoagulation programs to reduce the risk of bleeding and thrombosis. Then,the pump line infection should be prevented as early as possible. At the whole stage,we provided psychological counseling to improve emotional state. After discharge,regular follow-up was carried out through online and offline method to ensure quality of life in the long term. After careful treatment and nursing care,23 days after surgery,the patient was discharged smoothly,and good follow-up after discharge.

    The nursing experience of a pregnant woman with stillborn fetus with a septic shock to cardiac arrest at 30 weeks of pregnancy was summarized. The critical points of nursing include first aid nursing of cardiac arrest during pregnancy,preparation of transport plan to ensure ECMO combined with CVVH to support cesarean section,anti-infection nursing of fetal death complicated with septic shock,precise implementation of target temperature management,individualized anti-coagulation nursing under multiple factors interference. After 31 days of careful treatment and nursing,the patient’s condition was stable,discharged smoothly,and the patient recovered well after 5 months of follow-up.

    The nursing experience of a severe crusted scabies patient with multiple underlying diseases based on the multidisciplinary treatment(MDT) mode was reported. To avoid the nosocomial scabies outbreaks and promote disease control,the key points of nursing included:precise treatment and care for underlying diseases based on MDT model;application of 20% sulfur ointment to smear whole body skin to kill scabies mites and strict contact isolation;respiratory support and airway management to improve respiratory failure;alleviation of itching and pain to enhance patient comfort;quantitative assessment of mental state and sleep condition,implementation of psychological nursing to improve sleep quality;to implement individualized nutrition,blood glucose management,pulmonary rehabilitation program and targeted follow-up guidance. After 28 days of meticulous care,the patient’s condition was stable with controlled scabies and gradually alleviated severe itching. Sleep quality was improved,and no scabies transmission occurred in the hospital. The patient was transferred to the community hospital for further rehabilitation.

    Pregnant women are affected by various biological,psychological and social pressures,and the incidence of perinatal vulnerability is relatively high. The existence of perinatal vulnerability seriously affects the physical and mental health of pregnant women and infants. Attention to perinatal vulnerability can help reduce the risk of adverse maternal and infant outcomes. This paper reviews the concept,classification,assessment tools,influencing factors,intervention measures,limitations and prospects of perinatal vulnerability,providing references for formulating management programs of perinatal vulnerability.

    Peristomal pyoderma gangrenosum(PPG) is a subtype of pyoderma gangrenosum(PG) characterized by ulcerations surrounding an enterostomy or continent stoma. The disease progresses rapidly,and the cause is still unknown. As a result of the challenging diagnosis of PPG and the high rate of misdiagnosis,along with the severe pain experienced by patients,the vulnerability of wounds to infection,and the prolonged healing time,it has a substantial impact on the patient’s quality of life and presents significant challenges to medical professionals. This paper reviews the diagnostic tools for PPG,factors influencing wound healing,and integrated wound care methods. The aim is to provide a reference for developing wound care strategies for PPG and improving the quality of clinical nursing practices in China.

    Objective To understand the current status of implicit theory derivation forms and applications in the health field at home and abroad,and to provide references for clinical practice and future research. Methods In accordance with the scoping review framework,PubMed,Embase,Web of Science,Cochrane Library,CINAHL,PsycINFO,CNKI,Wanfang,VIP,SinoMed were searched for relevant studies. The time limit for the search was from the establishment of the databases to 10 March 2024. Results A total of 45 articles were included in the study,including 24 articles on health behaviour and 21 articles on mental health,covering 14 derivations of implicit theories such as implicit health theory,implicit weight theory and implicit emotion theory. The main intervention process of implicit theories can be summarised as 3 major steps:belief-led intervention,attention examination and outcome testing. Conclusion The research value of implicit theory in the field of health has been gradually explored,and it is an important antecedent to improve individual health behaviors and mental health,and its brief and effective intervention model is more compatible with the characteristics of nursing work,so domestic and foreign nursing researchers can learn from the relevant research results to further optimize the strategies of health education,preventive guidance,and psychological care,and to carry out high-quality studies to validate them.
