Research progress on transitional care handover of ICU transferred patients

Abstract

Abstract:

Transitional care handover of ICU transferred patients affects patient safety and continuity of care. This article reviews the current situation of transitional care handover for ICU transferred patients at home and abroad，and discusses in depth the problems，influencing factors and intervention strategies in the handover process. It aims to provide references for the improvement of the transitional care handover process of ICU transferred patients in China，development of intervention strategies based on meeting the needs of all parties and centred on patients and families，and enhance the quality of ICU transferred transitional care and patient safety.

摘要：

ICU转出患者过渡期护理交接影响患者安全及护理的连续性。该文对国内外ICU转出患者过渡期护理交接现状进行综述，深入探讨交接过程中存在的问题、影响有效交接的因素及提高交接质量的干预策略，旨在为国内ICU转出患者过渡期护理交接流程的改进、以满足各方需求及以患者和家庭为中心干预策略的制订、提升ICU转出过渡期的护理质量和患者安全提供参考。
