Abstract:

To summarize the perioperative nursing experience of a pediatric patient with aortic atresia combined with ventricular septal defect undergoing Yasui surgery. Peroperative nursing points are as follows：implementing prehabilitation interventions to optimize the preoperative physical status；adopting the principle of shared decision-making can reduce the fear and anxiety associated with a second surgery；closely monitoring cardiac function，standardizing fluid management and preventing heart failure. Postoperative nursing points are as follows：precisely regulating blood pressure，reducing afterload，and protecting left ventricular function；implementing goal-directed anticoagulation management to prevent bleeding and thrombosis；carrying out early and staged cardiac rehabilitation training to accelerate postoperative recovery. After careful treatment and nursing care，the pediatric patient recovered and was discharged from the hospital at 36 days after the surgery without any complications in one-year follow up.

总结1例主动脉闭锁合并室间隔缺损患儿行Yasui手术的围手术期护理经验，护理要点如下。术前护理：实施术前预康复，优化身体状态；采用共享决策原则，降低对二次手术的恐惧及焦虑；严密监测心功能，规范液体管理，防治心力衰竭。术后护理：精准调控血压，降低后负荷，保护左心室功能；目标导向性抗凝管理，防止出血与血栓的发生；早期分阶段实施心脏康复训练，加快康复进程。经过精心的治疗与护理，患儿于术后36 d康复出院，随访1年，恢复良好。
