摘要/Abstract

摘要：

目的探讨医院-家庭过渡期用药偏差管理在老年共病患者中的应用效果，为临床护理实践提供参考。方法采用便利抽样法，选取2023年1月—2024年12月郑州市某三级甲等医院收治的150例老年共病患者为研究对象。按照入院时间，将2023年12月—2024年12月收治的75例患者作为试验组，将2023年1—11月收治的75例患者作为对照组。试验组实施制订的用药偏差管理干预方案，对照组实施标准化出院指导。干预时长为从入院至出院后8周。干预后，比较两组用药偏差发生率、药物素养、用药依从性、药物自我管理能力和用药自我效能的差异。结果该研究期间无样本流失。干预后，试验组用药偏差发生率（10.67%）低于对照组（32.00%），两组比较，差异有统计学意义（P<0.05）。试验组药物素养、用药依从性、药物自我管理能力及用药自我效能均优于对照组，差异有统计学意义（P<0.05）。结论应用医院-家庭过渡期用药偏差管理干预方案可减少老年共病患者用药偏差发生率，提高患者用药依从性及药物素养，增强患者药物自我管理能力和用药自我效能。

Abstract:

Objective To investigate the application effect of medication discrepancy management during the hospital-home transitional period in geriatric patients with multimorbidity，and to provide a reference for clinical nursing practice. Methods A total of 150 cases of patients with multimorbidity who were admitted to a tertiary grade A hospital in Zhengzhou City between January 2023 and December 2024 were selected as research subjects. According to the admission time，75 patients from December 2023 and December 2024 were included in an experimental group，and 75 patients from January 2023 to November 2023 were included in a control group. The experimental group received medication discrepancy management，while the control group followed standard discharge guidance protocols. The intervention period spanned from admission to 8 weeks post-discharge. After the intervention，the incidence rate of medication discrepancy events，medication literacy，medication compliance，medication self-management capabilities，medication self-efficacy were compared between the 2 groups. Results No sample loss occurred during the study period. Following the intervention，the incidence of medication discrepancy in the experimental group（10.67%） was lower than that in the control group（32.00%），with a statistically significant difference between the 2 groups（P<0.05）. The experimental group demonstrated superior medication literacy，medication compliance，medication self-management capability，and medication efficacy compared to the control group，with statistically significant differences（P<0.05）. Conclusion The hospital-home transitional medication discrepancy intervention can effectively reduce the incidence of medication discrepancy among aged patients with multimorbidity，improve patients’ medication compliance and medication literacy，and enhance their ability to self-manage medications and the self-efficacy of rational medication.
