11例体外膜肺氧合桥接双肺移植术患者的护理

Nursing care of 11 patients undergoing extracorporeal membrane oxygenation bridging to double lung transplantation

摘要:

总结11例体外膜肺氧合桥接双肺移植术患者的护理经验。护理要点:肌力风险分层管理与早期功能锻炼、膈肌功能动态监测与阶梯式撤机干预、吞咽功能早期筛查与渐进式康复训练、组织灌注监测与急性皮肤衰竭综合防护、创伤心理干预与自我身份重建、分阶段随访管理与延续性康复指导。经过精细化护理,所有患者均顺利转出ICU。2例分别于转出ICU后第125、48天重返ICU,最终死亡;9例康复出院。在出院随访中,除1例因肺部感染死亡外,其余8例随访至术后2~9个月,肺功能发挥正常,日常生活能力恢复正常。

Abstract:

This study summarizes the nursing experience of 11 patients undergoing extracorporeal membrane oxygenation(ECMO) bridging for double lung transplantation. Key nursing strategies included:muscle strength risk stratification management with early functional exercises,dynamic monitoring of diaphragmatic function with stepwise mechanical ventilation weaning,early screening of swallowing function with progressive rehabilitation training,tissue perfusion monitoring with comprehensive protection against acute skin failure,psychological intervention for trauma patients along with self-identity reconstruction,and staged follow-up management with continuous rehabilitation guidance. Through meticulous nursing care,all patients were successfully transferred out of the ICU. 2 patients were readmitted to the ICU on days 125 and 48 after being transferred out and ultimately died,and 9 patients recovered and were discharged. During follow-up,a patient died from pulmonary infection,and the remaining 8 patients were followed up for 2~9 months postoperatively,with normal pulmonary function and restored daily living abilities.
