4 907例老年髋部骨折患者术后并发症发生现状及影响因素研究

摘要:

目的 调查老年髋部骨折患者术后并发症发生现状,分析其影响因素,为制订临床干预措施提供参考。 方法 采用便利抽样法,于2023年9—12月选取31个省(区、市)594所医院的老年髋部骨折术后患者作为调查对象,采用一般资料调查表、术后并发症发生情况调查表进行调查,采用单因素分析及二元Logistic回归分析明确老年髋部骨折患者发生术后并发症的影响因素。 结果 共调查4 907例老年髋部骨折患者,其中309例患者术后发生并发症,发生率为6.30%。性别、受教育程度、是否有基础疾病、术前生活自理能力、入院原因、健康教育方式(医护人员讲解、视频、微信小程序、公众号)、骨折史是老年髋部骨折患者术后并发症的影响因素(P<0.05)。 结论 老年髋部骨折患者术后并发症受多种因素影响,应重点关注风险人群,强化有效干预措施,以预防或减少术后并发症。

Abstract:

Objective To investigate the prevalence of postoperative complications in elderly patients with hip fractures and analyze the influencing factors,thereby providing a reference for developing clinical intervention strategies. Methods Using a convenience sampling method,elderly patients who underwent surgery for hip fractures were selected from 594 hospitals across 31 provinces(autonomous regions and municipalities) of China between September and December 2023. Data were collected using a general information questionnaire and a postoperative complications questionnaire. Univariate analysis and multivariate logistic regression were used to identify factors associated with postoperative complications in elderly patients with hip fracture. Results A total of 4,907 elderly hip fracture patients were surveyed. Among them,309 patients developed complications after surgery,with an incidence rate of 6.30%. The influencing factors for postoperative complications in elderly patients with hip fractures included patient gender,education level,presence of underlying diseases,preoperative self-care ability,reason for hospital admission,methods of health education(guidance by medical staff,video education,WeChat mini-program/public account),and history of previous fractures(P<0.05). Conclusion The occurrence of postoperative complications in elderly hip fracture patients is influenced by multiple factors. Clinical practice should focus on high-risk populations,actively address modifiable risk factors,and reinforce effective interventions to prevent or reduce postoperative complications.
