运动想象型脑机接口引导的瘫痪患者康复护理流程的构建与初步验证

Construction and preliminary validation of a standard operating procedure for active rehabilitation nursing in patients with paralysis guided by motor imagery-based brain-computer interface

摘要:

目的 构建运动想象型脑机接口引导的瘫痪患者主动康复护理标准作业程序并进行初步验证,为脑机接口在康复护理中的规范化应用提供参考。方法 通过文献回顾、临床实地观察及院内专家小组讨论,构建护理标准作业程序初稿;2026年2—3月采用德尔菲法对16名专家进行2轮函询,对条目进行修订与优化,形成终稿;采用前瞻性单组试点设计,选取江西省南昌市某三级甲等医院康复医学科5例瘫痪患者开展初步临床应用验证。结果 2轮函询问卷有效回收率均为100%,专家权威系数分别为0.897和0.906;第2轮专家函询中,各级条目重要性评分变异系数为0~0.142,肯德尔和谐系数为0.183~0.396,均具有统计学意义(P<0.001)。最终形成包含5个一级条目、11个二级条目、16个三级条目的标准作业程序。初步验证结果显示,标准作业程序流程执行率为95.6%,训练完成率为100%,未发生严重不良事件,患者、家属及护理人员满意度均较高,患者运动想象参与度良好。结论 构建的运动想象型脑机接口引导的瘫痪患者主动康复护理标准作业程序科学合理、内容全面、专科性强,可为脑机接口在康复护理中的规范化应用提供参考。

Abstract:

Objective To construct and preliminarily validate a standard operating procedure(SOP) for active rehabilitation nursing in patients with paralysis guided by motor imagery-based brain-computer interface(MI-BCI),providing a reference for the standardized application of brain-computer interfaces in rehabilitation nursing. Methods An initial draft of the nursing SOP was developed through literature review,clinical field observation,and expert panel discussion. From February to March 2026,16 experts were consulted using the Delphi method in 2 rounds to revise and optimize the SOP. A prospective single-arm pilot study was subsequently conducted,and 5 paralysis patients from the rehabilitation medicine department of a tertiary A hospital in Nanchang,Jiangxi Province,were selected for preliminary clinical application validation. Results The effective response rates of the 2 rounds of expert consultation questionnaires were both 100%,and the expert authority coefficients were 0.897 and 0.906,respectively. In the second round of expert consultation,the coefficients of variation of item importance scores ranged from 0 to 0.142,and Kendall’s coefficients of concordance ranged from 0.183 to 0.396,all showing statistical significance(P<0.001). The final SOP consisted of 5 first-level items,11 second-level items,and 16 third-level items. Preliminary validation results showed that the SOP implementation rate was 95.6%,the training completion rate was 100%,and no serious adverse events occurred. Patients,family caregivers,and nurses all reported high satisfaction,and patients demonstrated good participation in motor imagery training. Conclusion The constructed SOP for MI-BCI-guided active rehabilitation nursing in patients with paralysis is scientific,comprehensive,and highly specialized,and it may provide a reference for the standardized application of brain-computer interface technology in rehabilitation nursing.
