1例先天性腹裂伴疟疾和心脏病早产儿的围手术期护理

Perioperative nursing care for a premature infant with congenital gastroschisis,malaria,and heart disease

摘要:

总结援非医疗中1例先天性腹裂伴疟疾和心脏病早产儿的围手术期护理经验。护理要点:术前观察和保护脱出的肠管,预防肠损伤及感染,并及时识别和处理心律失常、急性缺氧发作;术中密切监测生命体征,配合医生制作“人工腹腔”;术后加强病情观察及腹内压监测,规范用药、合理输血,以预防腹裂修复术相关并发症、控制疟疾相关症状,同时给予营养支持,并加强健康宣教。经多学科团队的协作诊治和精心护理,2周后患儿好转出院。术后2个月随访,患儿喂养正常,手术切口愈合良好,无急性缺氧发作,心前区仍可闻及收缩期杂音。

Abstract:

This paper summarizes the perioperative nursing experience of a premature infant with congenital gastroschisis accompanied by malaria and heart disease during a medical aid mission in Africa. Nursing key points include:preoperative observation and protection of the protruding intestines to prevent intestinal injury and infection,timely identification and management of arrhythmias and acute hypoxic episodes;intraoperative close monitoring of vital signs and assisting the surgeon in creating an "artificial abdominal cavity";postoperative intensification of condition monitoring and intra-abdominal pressure surveillance,standardized medication administration and rational blood transfusion to prevent complications related to gastroschisis repair,and to control malaria-related symptoms,while providing nutritional support and reinforcing health education. Through collaborative diagnosis and treatment by a multidisciplinary team along with meticulous nursing care,the infant showed improvement and was discharged after 2 weeks. A follow-up visit 2 months postoperatively revealed normal feeding,good surgical wound healing,no recurrence of acute hypoxic episodes,and a persistent systolic murmur auscultated over the precordium.
