    Application of nurse-led early activity procedure in patients with mechanical ventilation

    Objective To analyze the safety,compliance and effect of nurses-led early activity procedure in mechanically ventilated patients with severe neurological diseases. Methods Totally 158 patients from January to March 2019 were selected as the experimental group,and the early activity procedure led by nurses was implemented;147 ICU patients with mechanical ventilation in a tertiary hospital from October to December 2018 were selected as the control group and given routine nursing of mechanical ventilation. The safety and compliance of the experimental group were observed. Recovery and complication rate were compared between the two groups. Results There were no significant differences in intracranial pressure,heart rate and oxygen saturation before exercise(5 min,10 min),during exercise(5 min,10 min) and after exercise(5 min,10 min) between the experimental group and the control group(P>0.05),but there was significant difference in mean arterial pressure(P<0.05). Compliance was 98.1% in the experimental group and 74.8% in the control group. There was significant difference between the two groups (P<0.05). Length of mechanical ventilation,length of ICU stay and overall length of hospital stay in the experimental group were significantly lower than those in the control group(P<0.05),and the scores of daily living ability of the experimental group were significantly higher than those of the control group(P<0.05). The incidence of ventilator-associated pneumonia and deep venous thrombosis in the experimental group was significantly lower than that in the control group(P<0.05). There was no significant difference in the incidence of delirium in ICU between the two groups(P>0.05). Conclusion Nurse-led early activity procedure for mechanical ventilated patients with severe neurological diseases has high safety and compliance,which can improve the recovery of patients,reduce complications and promote early rehabilitation of patients.

    A longitudinal study of anxiety,depression,fatigue,and their impact on health status in renal transplant recipients

    Objective To investigate the anxiety,depression and fatigue in patients after renal transplant in one month(T1),six months(T2) and one year(T3),and to explore their impact on health status. Methods A longitudinal study was conducted among 180 patients admitted to the department of organ transplantation of a tertiary hospital for renal transplantation from May 2016 to June 2018. The questionnaire included general information questionnaire,Patient Health Questionnaire-9(PHQ-9),Generalized Anxiety Disorder-7(GAD-7),the Multi-dimensional Fatigue Inventory(MFI) and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC-QLQ-C30). Results ①The incidence of anxiety at T1,T2,T3 was 28.9%,29.4%,33.9%,and depression was 39.4%,36.3%,37.8%,severe fatigue was 14.4%,11.3%,and 14.3%,respectively. ②There was no statistically significant change in anxiety and depression scores after kidney transplantation(P>0.05),however,fatigue scores decreased over time(F=7.667,P<0.001),and overall health status scores decreased significantly one year after kidney transplantation(F=3.746,P<0.028). ③The results of the generalized estimation equation showed that depression negatively affected the overall postoperative health status(Wald χ 2=21.155,P<0.001).④Sensitivity analysis showed that results of multiple imputation and deletion were consistent,and depression could still predict the overall health status(Wald χ 2=12.386,P<0.001). Conclusion The incidence rates of anxiety,depression and severe fatigue were 33.9%,37.8%,and 14.3%,respectively one year after transplantation. The overall health status at T3 was lower than that at T1 and T2. Depression can negatively affect postoperative health status,which should be concerned by health care workers.

    Objective To explore the effects of Heart to Heart Café for death education among 77 oncology nurses. Methods Heart to Heart cards designed by the Chinese American Coalition for Compassionate Care,were used to complete Heart to Heart Café activity under the guidance of 2-3 facilitators among 77 oncology nurses. Heart to Heart Café participants evaluation form was used to evaluate the effects. Results Totally 231 Heart to Heart cards were selected from 77 oncology nurses:68 cards for social concerns(29.4%),66 for physical concerns(28.6%),49 for financial concerns(21.2%),43 for spiritual concerns(18.6%),and 5 for special wishes(2.2%). Among investigated nurses,98.7% believed that Heart to Heart Café was meaningful and worth promoting,and they had a positive attitude towards medical desire. Sixty-three oncology nurses(81.8%) stated that Heart to Heart Café activity had surprising effects:open their mind,release their emotions,confide in each other,and talk about death openly. Sixty-nine(89.6%) believed that Heart to Heart Café helped them to think about death,face death,understand their inner thoughts and live in the moment. Conclusion Heart to Heart Café is an effective way to promote death education for oncology nurses.

    Objective To improve quality of death outcomes of patients with terminal tumors in palliative care demonstration ward. Methods Totally 69 patients with terminal tumor admitted to the Anxin ward were randomly divided into two groups(the control group and the experimental group) by random number table method. Both groups were evaluated,planned,and provided with symptom management of Chinese medicine and caring time,and evaluated for spiritual care by a hospice care team including medical workers. The experimental group had cancer volunteers participating in the“Reading the Classics of Sinology”,“Meeting a Wish”,“Voice of the Mind Theme Event” and “Festival Culture” activities. The patients’ quality of life,spiritual well-being and anxiety and depression were evaluated by FACT-G,FACT-Sp-12 and HAD. Results Patients in the experimental group had significant better scores in social/family status dimensions of quality of life,spiritual comfort,anxiety and depression than patients in the control group(P<0.05). There were no differences in three dimensions of quality of life (physical condition,emotional condition,and functional condition) and total score for quality of life between two groups(P>0.05). Compared with those before intervention,quality of life,spiritual comfort,anxiety and depression in two groups were significantly different(P<0.05). Conclusion Compared with the group composed entirely of medical workers,the activities of palliative care with participation of cancer volunteers can better improve high quality death of patients with terminal tumors.

    Expectation and needs of a good death in patients with end-stage chronic diseases:a systematic review

    Objective To systematically review the expectation and needs of a good death in patients with end-stage chronic diseases. Methods We searched databases including PubMed,Embase,Web of Science,PsycINFO,Cochrane Library,CNKI,Wanfang,CBM from inception to March 2019. The Joanna Briggs Institute’s Critical Appraisal Tool for qualitative studies was used to evaluate the quality of included studies. The results were synthesized by integrating methods. Results A total of 14 studies were finally included,and 52 results were extracted,and then 4 synthesized results were summarized from the 10 categories. The integrated results were physiological needs(physiological needs at the end-stage,physiological needs at the time of death),psychological needs(desire fulfillment,positive psychology),spiritual needs(religious belief,maintenance of dignity,other spiritual needs),and societal needs(interpersonal,economic,and support systems). Conclusion The expectation and needs of a good death in patients with end-stage chronic disease involve physiological,psychological,spiritual and societal aspects,with characteristics of individuality and subjectivity,and through the whole process of end-stage. Different expectation and needs may overlap and influence each other. Healthcare providers should pay more attention to patients’ expectation and needs regarding a good death,and provide individualized nursing care for whole human and whole process,as well as spread the knowledge of good death to the public. It is crucial for social resources and health policy to improve the hospice care for those with end-stage chronic diseases.

    Objective To reduce the unplanned ICU readmission rate of patients due to pulmonary infection after surgery. Methods We established ICU-ward extension nursing team. With reference to the expert consensus on multidisciplinary perioperative airway management in China(2018 edition),through lung assessment of high risk factors among patients transferred out of ICU,individualized pulmonary nursing plan was formed,specialized and systematic lung care was performed,procedure risk screening mechanism was established and implemented. Based on standard airway grading management,effect evaluation and quality control of pulmonary nursing from four aspects were performed before and after transfer to ensure effects of extension nursing,and reduce ICU readmission rate due to pulmonary infection. Results The ICU readmission rate due to pulmonary infection in the experimental group and the control group were 0% and 11.06%,respectively. Conclusion Through the implementation of pulmonary extension nursing plan for patients transferred out of ICU,we can provide continuous lung management for patients transferred to the general ward,reduce ICU readmission rate,and achieve nursing goals of improving clinical indicators and patient experience.

    Objective To explore the effects of application of grading management scheme for chemotherapy induced nausea and vomiting(CINV) in breast cancer patients. Methods Cluster sampling method was adopted to recruit 158 subjects in the control group and the experimental group,respectively. The control group was given routine care of CINV,while the experimental group was given grading management scheme for CINV. The incidence of CINV and the impact of CINV on the quality of life of the two groups were observed and compared. Results In the experimental group,the incidence of CINV in patients with high and medium risk of emetogenic chemotherapy was significantly reduced compared with the control group(P<0.05),however,there was no statistically significant difference in the incidence of CINV between the two groups of patients receiving low risk of emetogenic chemotherapy. The score of Functional Living Index-Emesis in the experimental group was significantly higher than that in the control group(P<0.05). Conclusion The grading management scheme for CINV can reduce the incidence and grade of CINV,and reduce the impact of CINV on the quality of life among breast cancer patients.

    Objective To explore the remote management based on “cloud platform” and its effect on home mechanical ventilation. Methods Totally 162 home mechanical ventilation patients who were managed remotely by cloud platform were enrolled from May 2015 to May 2018,and all patients used noninvasive ventilators with GPRS network connection. Respiratory symptoms,quality of life,mental status,quality of sleep,action status and compliance were assessed in all patients. Results After application,respiratory symptoms,quality of life and action status tended to be improved in 162 patients without statistical significance(P>0.05). There were significant improvement in mental status,sleep quality and treatment compliance after application of the remote management(P<0.05). Conclusion Application of modern internet of things technology can realize the remote medical management in home mechanical ventilation patients,and treatment compliance,mental status and quality of sleep can be obviously improved.

    Objective To understand the status of sexual and intimate relationship experience of postoperative patients with cervical cancer,in order to provide reference for the development of sexual health guidance program. Methods Using the phenomenological method,10 postoperative patients with cervical cancer who were followed up in the outpatient department were recruited in three tertiary hospitals using purposive sampling method and a semi-structured interview was conducted with them. Following Colaizzi’s method of analysis,data were analyzed with QSR Nvivo 12.0 software. Results The sexual and intimate relationship experience of postoperative patients with cervical cancer were classified into four themes:sexual distress,female role trauma,changes in marital relationships,changes in self-perception. There was a certain relevance between the themes. Conclusion The sexual and intimate relationship experience of postoperative patients with cervical cancer is poor. In order to improve the sexual relationship and intimacy between patients and their partners timely,and improve their overall quality of life,medical staff should take the initiative to provide sexual health guidance for patients and their partners.

    Objective To understand the maternal experience of Early Essential Newborn Care (EENC) in order to provide theoretical basis for the effective development of EENC technology. Methods A qualitative research method was used to conduct semi-structured interviews with 12 pregnant women who met the inclusion criteria on their EENC experience. Colaizzi’s phenomenological data 7-step analysis method was used to analyze the data. Results Four themes of maternal experience of early essential health care for newborns were extracted:understanding of social support,overwhelming love and responsibility,calm and worrying heart and the family contradiction and doctor-patient contradiction. Conclusion It is necessary to pay attention to maternal EENC perception,strengthen the publicity of EENC knowledge,expand the publicity group,consolidate the professional foundation of medical staff,strengthen effective communication,give full play to the advantages of traditional Chinese nursing,and reduce the contradiction between doctors and patients.

    Objective To explore the effects of hospital patient safety culture on distress of nurses as second victims. Methods This study used convenience sampling method to investigate 2 897 nurses who had experience of patient safety incidents in 6 tertiary hospitals in central and west China from October 2018 to January 2019. The Hospital Survey on Patient Safety Culture(HSOPSC),and the Second Victim Experience and Support Tool (SVEST) were used. Results The hospital patient safety culture had significant impact on psychological distress,physical distress,and professional self-efficacy of nurses as second victims. The associated factors of nurses’ psychological distress were non-punitive response to error,handoffs and transitions,teamwork across units,feedback and communication about error,overall perceptions of patient safety. The associated factors of physical distress were non-punitive response to error,handoffs and transitions,teamwork across units,staffing and nurses’ position. The associated factors of professional self-efficacy were non-punitive response to error,teamwork across units,overall perceptions of patient safety,handoffs and transitions,and nurses’ working years. Conclusion Distress of nurses as second victims is affected by the hospital patient safety culture,among which the non-punitive safety culture,teamwork across units and patient handoff were important influencing factors.

    The current status of nurses’ career anchors in tertiary general hospitals in Sichuan Province

    Objective To investigate and analyze the status quo of nurses’ career anchors in tertiary general hospitals in Sichuan Province,so as to provide references for nurses’ career planning. Methods We used the stratified random sampling method to survey 1 408 nurses from 11 tertiary general hospitals in Sichuan Province using Schein’s Career Anchors Self-Assessment. Results The top two type career anchors of nurses were Lifestyle and Security/Stability,while the last two types were General Managerial Competence and Entrepreneurial Creativity. Scores of the General Managerial Competence and Pure Challenge in male nurses were higher than those in female nurses,while the Security/Stability and Lifestyle scores were lower than female nurses(P<0.05). As the growth of the age and working years,the scores of the nurses’ Technical/Functional and General Managerial Competence career anchors increased,while the scores of Autonomy/Independence and Entrepreneurial Creativity gradually reduced (P<0.05). As the raising of the title levels,the scores of nurses’ Technical/Functional and General Managerial Competence career anchors increased,while the scores of Autonomy/Independence,Entrepreneurial Creativity and Lifestyle gradually reduced(P<0.05). Conclusion The nurses’ career anchors in tertiary general hospitals were mainly Security/Stability and Lifestyle,and different demographic data had different effects on nurses’ career anchors. Therefore,nursing managers should consider the characteristics of nurses’ career anchor,and implement scientific and effective career orientation,so as to improve the efficiency of nursing human resource management.

    Objective Based on the family medical platform(referred to as the platform),a continuity nursing model for discharged patients was constructed and its application effects were evaluated in order to improve the quality of home care. Methods The platform include monitor platform and operation platform. Operation platform included:patient APP,providing service items and personal information pages,supporting user registration,project selection,service reservation and evaluation;nurse APP,providing service items,service application display,security center and personal information pages,supporting nurse registration,order receiving and security monitoring;and monitoring platform,responsible for the management of uses,service item maintenance,data management and safety monitoring. The platform was used to implement home care services for discharged patients. Data from October 2018 to March 2019 were collected including patients’ satisfaction and health knowledge mastery rate of patients to evaluate the platform. Results A total of 231 nurses and 109 patients were registered on the platform,164 times of home care and 8 times of accompanying visits were completed. The patients’ satisfaction rate was 93.9%,which was significantly higher than 86.2% of patients at discharge(χ 2=4.623,P=0.032). Patients’ mastery rate of health knowledge was 97.7%,which was significantly higher than 87.6% of inpatients’ health knowledge(χ 2=14.545,P<0.001). No complaints and adverse events occurred. Conclusion Constructing a family medical platform for continuity nursing has brought into the professional value of clinical nurses,provided systematic,standardized,safe and high-quality home nursing services for discharged patients,significantly improved the satisfaction of patients at home and the mastery rate of health knowledge,and provided an efficient and convenient way for patients to seek medical treatment.

    Objective To establish an evaluation index system and an effectiveness index for PICC and PORT. Methods The Delphi method was adopted to identify the indicators and their weight coefficients in the evaluation index system for evaluating PICC and PORT through two rounds of expert consultation. Then the effectiveness index for evaluating the effect of PICC and PORT was established based on the synthetical index method. Results The responses rates of two rounds of Delphi consultation were all 100%. The experts’ authoritative coefficient was 0.87. The coordination coefficients of the two rounds of Delphi consultation were 0.283 and 0.306(both of them P<0.001). The evaluation index system for evaluating the effect of PICC and PORT was established,including 3 primary indicators,9 secondary indicators,and 13 tertiary indicators. Then the effectiveness index for evaluating the effect of PICC and PORT was established based on exponential formula and synthetical index formula. Conclusion The established evaluation index system and the effectiveness index for evaluating the effect of PICC and PORT are scientific,reliable and feasible,providing a quantitative method for decision makers and administrators to make holistic comparison of the effect of PICC and PORT.

    Objective To select and obtain the best evidence of prevention and management of feeding intolerance(FI) in critically ill patients with enteral nutrition support so as to provide guidance for clinical practitioners to reduce the incidence rate of FI. Methods Using the method of evidence-based nursing,clinical question of FI prevention and management in critically ill patients with enteral nutrition support was raised. Relevant research was retrieved by computer search. JBI evidence appraisal tools and recommendation system were used to evaluate quality of studies and level of evidence. Results Combined with the judgment of clinical professionals,totally 22 items of evidence were selected,including FI assessment,risk factors of FI,enteral formula management,enteral feeding pathway,position management,gastric residual quantity monitoring,drug administration,intra-abdominal pressure monitoring and traditional Chinese medicine therapy,etc. Conclusion This study summarized the best evidence for prevention and management of FI in critically ill patients with enteral nutrition support and help clinical nurses and nursing administrators with decision making,ultimately through adapting the best evidence and using scientific nursing methods to cope with the clinical problems and improve nursing quality.

    Objective To summarize the best available evidence of discharge follow-up of patients after knee arthroplasty,and to provide reference for clinical practice. Methods We searched National Guideline Clearinghouse,National Institute for Health and Care Excellence,the Scottish Intercollegiate Guidelines Network,Canadian Medical Association Infobase(CMA),Guidelines International Network(GIN),and National Health And Medical Research Council(NHMRC),Registered Nurses’Association of Ontario(RNAO),Cochrane Library,JBI Library,CINAHL,Uptodate,BMJ best practice,Embase,Pubmed,CNKI,Wanfang Data,China Biology Medicine disc(CBM) and the VIP Database(VIP) for all evidence of discharge follow-up of patients who underwent knee arthroplasty,including guidelines,systematic reviews,evidence summary,recommended practices,and original studies. Results A total of 9 articles were selected in the literature,including 1 clinical decision,2 guidelines,3 systematic reviews,1 randomized controlled trial,2 expert consensuses. Finally 17 items of best evidence were summarized and categorized into 5 dimensions of recommendations,including follow-up time and method,physical assessment,drug management,discharge rehabilitation,and health education. Conclusion This study summarized the best evidence of discharge follow-up after knee arthroplasty,and nurses should receive relevant training to apply the best evidence to clinical practice. When applying the best evidence,medical staff should consider the patient’s wishes and specific clinical conditions,and combine professional judgment to form an effective and feasible follow-up strategy to improve the quality of nursing quality.

    Objective To select and obtain relevant evidence on postpartum optimal nursing management for older second-birth parturients,and summarize the best evidence. Methods Computer search of BMJ Best Practice,Joanna Briggs Institute Evidence-based Health Care Center Database,Up to date,Cochrane Library,British Guide Network,American Guide Network,China Guide Network,Evidence-based Medicine Database,PubMed,EMbase,Ontario Registered Nursing Association of Canada,China Biomedical Database,CNKI and American Association of Obstetricians and Gynaecologists was conducted for all relevant evidence on optimal maternal care and management,including guidelines,evidence summary,best clinical practice information sheet,systematic review,recommended practice and original research. Quality evaluation and data extraction of included literature were carried out by two researchers and documents that met quality standards were summarized. The retrieval period was from the establishment of the database to February 2019. Results A total of 15 documents including 6 guidelines,6 systematic reviews,1 cohort study and 2 randomized controlled trials were included. Totally 11 items of best evidence were summarized. Conclusion This study summarized the best evidence forolder second—birth parturients of postpartum optimal nursing and management. When applying evidence in clinical practice,it is necessary to carry out individualized evaluation of parturients. Attention should be paid to the renewal of the best evidence. Postpartum problems of second-birth parturients should be solved by scientific nursing approaches to improve quality of nursing.

    Evidence summary for exercise therapy programs in patients with type 2 diabetes mellitus

    Objective To retrieve,appraise and integrate available evidence on exercise therapy programs in patients with type 2 diabetes mellitus so as to provide evidence for clinical development of exercise therapy programs for patients with type 2 diabetes. Methods Using the method of evidence-based nursing,questions about exercise therapy programs for patients with type 2 diabetes were raised,relevant literatures were searched. JBI evidence appraisal and recommendation system were used to evaluate quality of studies and level of evidence. Results A total of 10 articles were included,including 4 guidelines,2 systematic reviews,1 recommended practice,1 consensus and 2 randomized controlled trials. A total of 17 best evidences were summarized from 5 items of exercise evaluation,exercise mode,exercise intensity,exercise time,and exercise safety. Conclusion This study summarized best evidence on exercise therapy programs in patients with type 2 diabetes mellitus,and formed recommendations,which can provide evidence for nurses and nursing managers to establish relevant nursing schemes,and have significance in guiding exercise therapy.
