    The development and application of a risk prediction model for postoperative delirium in ICU patients

    Objective To evaluate the effects of a risk prediction model for postoperative delirium in ICU patients after surgery. Methods A total of 240 ICU patients in a tertiary hospital were selected, and indicators of ICU postoperative delirium (ICU-P0D) group n=49) and non ICU-P0D group n=191) were compared. Logistic regression was used to establish a risk prediction model. The area under the R0C curve was used to test the model to predict the effects. Twenty-five patients were selected to evaluate the model’s effects. Results The study finally included POSSUM score (OR=4.259), acid-base imbalance (OR=28.731), diabetes (OR=2.699), hypertension (OR=2.055),and transient coma before admission(OR=10.719) to construct the risk prediction model. The model formula was:Z=0.121 × POSSUM + 2.519 × transient coma before admission +1.157 × hypertension + 1.004 × diabetes + 0.26 × acid-base imbalance -6.957,the area under the ROC curve of this model was 0.832,with the sensitivity of 0.639,the specificity of 0.886, the youden of 0.526. The model verification results showed the sensitivity of 60%,the specificity of 100%,and the accuracy of 92%. Conclusion The risk prediction model has satisfactory prediction effects. The risk prediction can be completed at the beginning of ICU stay,which can provide reference for preventative treatment and nursing measures for high-risk patients.

    Objective To explore trends of the National Early Warning Scores (NEWS) 24h prior to in-hospital cardiac arrest (IHCA) for patients with acute coronary syndromes (ACS). Methods We conducted a retrospective case-control study in 3 tertiary hospitals in Fujian Province,patients with ACS who experienced cardiac arrest were in the case group. We randomly selected patients with ACS who had not experienced cardiac arrest as the control group. The NEWS was calculated at 24h,16h,8h,1h and 0.5h before cardiac arrest and the trends were analyzed. Results The NEWS were significantly different between the case group and the control group at 24h,16h, 8h, 1h and 0.5h before cardiac arrest, the scores in the case group were higher than those in the control group, and the trend was continuingly rising (P<0.001). The proportion of high-risk NEWS group was enlarged before cardiac arrest (from 24h to 0.5 h,NEWS increased from 11.6% to 36.62), and the proportion of low-risk NEWS group was smaller (from 24h to 0.5h,NEWS decreased from 67.7% to 21.4%). From 24h to 0.5h prior to cardiac arrest, compared to low-risk group, the incidence of cardiac arrest of high-risk group was increased from 7.00 times to 56.57 times. Conclusion The NEWS can properly predict cardiac arrest among patients with ACS, and can better identify high-risk patients.

    Objective To explore interventions based on best evidence for reducing physical restraint among patients in ICUs and to evaluate effects of interventions. Methods The best evidence-based interventions were obtained by adopting evidence-based nursing procedures and were implemented in the ICUs with 110 beds. This project was guided by the Joanna Briggs Institute (JBI) Practice Application of Clinical Evidence System (PACES). The rates of using physical restraint and incorrect practice of physical restraint before and after the implementation were compared. Results After implementation of the evidence-based interventions (the year 2015 v.s. the year 2017),the rate of using physical restraint was decreased from 40.89% to 30.23%,and the difference was statistically significant (P<0.05). The rate of incorrect practice of physical restraint was reduced from 33.12% to 7.48%,and the difference was statistically significant(P<0.05). Meanwhile,the rate of unplanned extubation before and after implementation was 3.12% and 2.94% "respectively. Conclusion The evidence-based nursing interventions can further standardize the usage of physical restraint,and effectively reduce the rates of using physical restraint and incorrect practice of physical restraint among patients in ICUs without increasing the risk of unplanned extubation.

    Objective To explore the effects of different lengths of supine position after intrathecal chemotherapy on occurrence of complications in children with leukemia,in order to determine an optimum length of supine position for improving comfort. Methods Clinical experimental design was adopted, and children patients were assigned into 3 groups according to the sequence of admission using random table number, including supine position for 2 hours,3 hours,and 4 hours after intrathecal chemotherapy. Complications that occurred during supine position and 1 week after intrathecal chemotherapy were recorded. The level of tolerance and activities during supine position were also recorded. Results Totally 180 children were included in this study with 60 cases in each group. There was no significant difference in the incidence of complications among three groups during supine position and in the following week(P>0.05). Difference in the level of tolerance among groups had significant difference (P<0.05),and the 2-hour group had the best tolerance. There was no difference in activities during supine position among three groups(P>0.05). More physiological needs were observed for longer length of supine position. Conclusion Two hours for supine position after intrathecal chemotherapy does not increase the incidence of complications in children with leukemia, and the level of tolerance would also be improved, which would be recommended.

    Objective To explore the application effects of targeted sedation nursing for children with severe pneumonia in bedside bronchoscopy lung lavage, Methods From January to September 2016,in pediatric intensive care unit in a tertiary hospital in Shenyang,40 cases of children who underwent bedside alveolar lavage for severe pneumonia and received routine sedation nursing were selected as the control group. From January to September 2017 in the same hospital, 40 cases of children who received bedside alveolar lavage and targeted sedation nursing for severe pneumonia were selected as the observation group. Focused on the specific conditions of children with severe pneumonia, according to Ramsay sedation score, the individualized sedation target of each stage of bedside bronchoscopy lung lavage was identified! preoperative time of bedside bronchoscopy lung lavage(T?),the time of fiber bronchoscope into the glottis (T?), the first time of sputum suction lavage (T?), and postoperative time of alveolar lavage (T?), and the target Ramsay sedation score values were 4,5,5, and 4 points, respectively. By comparing the Ramsay sedation scores before (T?) and during (T??T?) percutaneous alveolar lavage between two groups,the children’s vital signs and the occurrence of intraoperative adverse reactions were recorded. Results There were no statistically significant differences in Ramsay sedative scores and vital signs between two groups at the time of T? (P>0.05). The differences in Ramsay sedative scores and vital signs between two groups at T??T? were statistically significant (P< 0.05). The incidence of intraoperative adverse reactions was significantly low in the observation group (P<0.05). Conclusion Through targeted sedation nursing,the dynamic evaluation of sedative effects was achieved,the intraoperative vital signs in children maintained stable,and the occurrence of adverse reactions due to lack of sedation were preventatively reduced.

    Objective To develop Nursing Practice Guideline for Emergency Percutaneous Coronary Intervention so as to provide guidance for nursing of emergency percutaneous coronary intervention. Methods Based on World Health Organization Handbook for Guideline Development,the recommendations were established according to best evidence and expert consensus. Appraisal of Guidelines for Research and Evaluation (AGREE) and Reporting Items for Practice Guidelines in Healthcare (RIGHT) were used to finalize full text of the guideline. Results The guideline contained 20 recommendations for 7 clinical questions about identification and assessment,condition monitoring and assessment,oxygen inhalation,safe transport,rehabilitation,skin preparation,and iodine allergy test. Conclusion This guideline based on best evidence and expert consensus can provide guidance and reference for nursing of emergency percutaneous coronary intervention.

    Objective To establish a predictive model suitable for emergency medical staff to evaluate the severity of patients with acute exacerbation of chronic obstructive pulmonary disease (AEC0PD), so as to better identify critical patients and provide timely treatment and nursing intervention. Methods The patients with AEC0PD who visited the emergency department from March 2017 to January 2018 were enrolled using convenience sampling method and divided into stable group and critical group. The data of the two groups were analyzed by univariate analysis and logistic regression analysis to establish the model and simplify it. Results A total of 142 patients were included in the study. Three factors including NEWS,CCI and age,entered the model. The AUC predicted by the model was 0.903[95%CI(0.845,0.960)]. With further simplifying the scoring rules,then AUC was 0.900. Conclusion The predictive model in this study can better predict the prognosis of patients with AEC0PD, and all the indicators are simple and easy to obtain, which is convenient for emergency clinical nursing to use.
    Development of continuing nursing evaluation system for patients with breast cancer

    Objective To develop continuing nursing evaluation system for patients with breast cancer based on Omaha System. Methods The draft of evaluation system was based on literature review and expert meeting. Delphi method was used to identify continuing nursing evaluation system by consulting with 29 experts, and 61 breast cancer patients were enrolled to provide comments regarding the draft. The application of the system was completed among 30 discharged breast cancer patients. Results The response rates of two rounds consultation were 100% and 93.5% ,respectively. The authority coefficient of experts in the second round was 0.844,and the coordination coefficients of two levels of indicators were 0.300,0.195, respectively (P<0.001). Experts and patients tended to have similar opinions on the contents. The evaluation system consisted of 8 sections and 63 indicators. The rate of usefulness on the evaluation system was 96.7% among breast cancer patients and 100% among nurses who worked in the referral institutions. Conclusion The continuing nursing evaluation system for patients with breast cancer is scientific and practical. It can be promoted further in clinical practice to guarantee continuous nursing.
    Analysis on features and related factors of physical growth in very low birth weight preterm infants in 12 months of corrected age

    Objective To identify the Geatures o£ catch-up growth o£ weight, length and head circumGerence in very low birth weight (VLBW) preterm infants in 12 months of corrected age and to explore the influencing factors. Methods We conducted a retrospective review o£ medical records o£ the VLBW preterm in£ants who were in compliance with inclusion and exclusion criteria and admitted to neonatal intensive care unit (NICU) from January 2014 to March 2016 in one maternity and infant hospital in Shanghai. Infants’ growth chat from Fenton (2003) and Centers for Disease Control and Prevention (CDC) were used for development outcome comparison. We also analyzed the influencing factors of physical growth outcome at 6 and 12 months corrected age. Results A total of 113 VLBW preterm infants were included in this study!of which 106 cases were followed up to 3 months. The outcomes of physical growth in VLBW preterm infants showed significant difference (P<0.05), comparing with other preterm and term infants. For male VLBW preterm infants at 6 months corrected age!longer length of stay, initiating oral feeding later!achieving full oral feeding later and initiating oral feeding with breast milk later were associated with non-optimal length growth & non-optimal weight growth was associated with prolonged length of stay and initiation of oral feeding later (P<0.05). For female VLBW preterm infants at 12 months corrected age!older maternal age and feeding with preterm formulas had negative association with the growth of head circumference (P<0.05 )& older maternal age in NICU was factors influencing weight growth (P<0.05). For male VLBW preterm infants at 12 months corrected age! initiating oral feeding and achieving full oral feeding later negatively impacted on the growth of head circumference (P<0.05) while exclusively feeding with breast milk in NICU was negatively related to length growth (P<0.05). Conclusion Transition to full oral feeding earlier and providing mixed feeding both in-hospital and post-discharge period played important role in VLBW infants’ physical growth. The results suggested feeding counselling in NICU and during follow-up periods is essential for successful catch-up growth in VLBW preterm infants.

    Objective To explore the nursing practice of reducing nipple necrosis after the corrective surgery for congenitally inverted nipple using the tractor. Methods Through clinical observation and literature review, improved postoperative nursing countermeasures were developed by medical cooperation. From 2014 to 2018,104 patients (196 inverted nipples) underwent surgeries and received the new nursing countermeasures. Results There was no nipple necrosis after operation. Skin wound occurred on 43 nipples,and was completely healed after 7-34 days with dressing change. Conclusion The rate of nipple necrosis was zero in the past four years,and the improved postoperative nursing countermeasures were effective.

    Descemet’s stripping with automated endothelium keratoplasty is a new surgical method,which is mainly used to treat various serious corneal endothelium lesions. This paper summarized the nursing experience of 20 children with Descemet’s stripping with automated endothelium keratoplasty. Key points of nursing included:using psychological nursing methods appropriate for children, implementation of preoperative fasting, ensuring face-up position 4h after operation through the cooperation of nurses and family members, conscientiously observation of condition, medication, postoperative diet and safety nursing.     Objective To explore the efficacy of active heat preservation(heating blanket combined with self- heating paste) in elderly patients undergoing Transurethral Resection of the Prostate (TURP). Methods From January to December 2017,100 elderly patients undergoing TURP were recruited and randomly divided into the experimental group (50 cases) and the control group (50 cases) using random number table method. Both groups were treated with heating blanket for active heat preservation from entering the operating room to leaving the postoperative room. Cotton blanket for passive heat preservation was applied for lower extremities during operation with lithotomy position. In addition#the experimental group used self-heating paste under the feet outside the socks for active heat preservation. Results Core body temperature decreased slightly and tended to be stable in the experimental group compared to the control group at 1h#2h#and 3h after anesthesia(P<0.05),and the incidence of hypothermia and shivering was lower in the experimental group than those in the control group(12% v.s. 54%,2% v.s. 32%,P<0.05); the thermal comfort score of the experimental group (8.44 ± 0.56) was higher than that of the control group (6.25 ± 0.78),the difference was statistically significant (P<0.05)..Conclusion Active heat preservation (heating blanket combined with self-heating paste) can provide effective body temperature protection for elderly.

    Objective To establish an evaluation index system of traditional Chinese medicine nursing project for breast cancer,in order to offer reference for evaluating the effects of traditional Chinese medicine nursing project. Methods Through literature review and clinical investigation,the evaluation index system was preliminarily established. Delphi technique was adopted to enroll 21 experts for 2 rounds of consultation. Combined with experts’ advice, the evaluation index system of traditional Chinese medicine nursing project for breast cancer was established finally. Results The average age of 21 experts were (43.62 ±5.56) years,and the average length of work was (19.90 ±7.79) years,66.67% of the experts had high professional title. The response rates of expert consultation were 95.5% and 100%,the authority coefficients were 0.840 and 0.852,and Kendall’s coefficient of concordance was 0.430. The average importance value of indexes ranged from 4.19 to 5.00,and variation coefficients of each item ranged from 0.00 to 0.20. The index system consisted of 4 first-level indexes, 12 second-level indexes and 25 third-level indexes. Conclusion The evaluation index system of traditional Chinese medicine nursing project for breast cancer is scientific, and all the indexes meet the requirements and the content is reasonable.

    Objective To establish a dynamic allocation scheme for nursing performance appraisal,so as to provide an innovative method for hospital nursing performance evaluation. Methods Based on the data extracted from the hospital information system,90 specific indexes in four dimensions (quantity,quality,efficiency and performance of nursing work) in all ward units were analyzed by chi-square automatic interactive detection method. The ward units were grouped into different performance grade clusters to complete the categorization for nursing performance appraisal of the ward units. Satisfaction among nurses and nursing managers were investigated from six aspects'applicability ,fairness,objectivity,acceptability,efficiency and overall satisfaction. Results After the application of this method,nursing managers’ satisfaction for performance appraisal was increased from(2.69±1.01) to (4.66±0.69) with statistically significant difference (t=-12.221,P<0.01). Nurses’ satisfaction for performance appraisal was increased from (3.39±0.89) to(4.60±0.52) with statistically significant difference(t=-23.836,P<0.01). Conclusion The dynamic allocation scheme for nursing performance appraisal can accurately and quickly complete the classified evaluation of ward unit nursing performance, which is an effective method to provide scientific decision-making basis for nursing performance evaluation in ward units.
    Establishment and application of informationized performance management scheme in stomatological outpatient nurses

    Objective To establish a set of informationized performance appraisal management scheme,which reflected the characteristics of stomatological outpatient nurses. Methods According to the hospital actual situation and the content of stomatological nursing service,the scheme was established by group discussion and questionnaire survey. The scheme was applied in clinical for 1 year. Results The scheme included 3 first-level indicators,7 second-level indicators and 21 third-level indicators. Ater implementation,nurses’ performance satisfaction score was increased from (86.19 ± 1.26) to (88.04 ± 1.45)(P<0.001 ),while nursing performance appraisal calculating time was decreased from (246.17 ± 9.38) min to (113.17 ± 10.75) min (P<0.001). Conclusion The improved outpatient nurse performance appraisal management scheme can reflect the characteristics of stomatological outpatient nurses. It can improve nurse satisfaction and efficiency of nursing management.

    Objective To study the working conditions of nursing staff using the overall nursing efficiency model in order to provide reliable evidence for the optimal allocation of nursing human resources. Methods A total of 28 departments of four tertiary first-class general hospitals in Tianjin were selected to investigate the level of working ability,workload and quality of nursing service by convenience sampling. Excel was used to build database and statistical analysis was done according to the formula. Results The overall nursing efficiency in Tianjin was 39.91% ,the level of working ability was 43.63% ,the workload was 95.11% ,and the nursing service quality was 96.15%.The workflow level is 0.764 and the operational level is 0.395 to 0.834.Conclusion The overall nursing efficiency in tertiary first-class general hospitals in Tianjin needs to be improved,the level of nursing workflow is acceptable, the level of nursing operation needs to be improved,the overall workload is heavy, and the time of effective direct care are less.

    In order to provide better mental health services to hospitalized patients,our hospital set up a post of full-time psychological nurses in May 2015. Through formulating the work guide for psychological nurses,the management assessment system and clinical practice standards of full-time psychological nurses were clarified. From August 1st,2015,to December 31st,2017,psychological nurses conducted psychological assessment and screening on a total of 9734 hospitalized patients in the pilot departments,carried out 8931 cases of mental health education for patients and 2874 cases of individual psychological services, 1129 cases with serious psychological problems were referred,and 145 cases received referrals,1384 patients received group psychotherapy,and 612 cases of psychological rescue in public health emergencies and disaster events, responded to psychological hotline 56 times, accepted 645 patients in psychological nursing outpatient clinic. The average service satisfaction of psychological nurses in the pilot departments was 98.36%,which was higher than other departments(96.64%). The difference was statistically significant x2=8.232,P=0.004). The establishment and management of the post of full-time psychological nurse play a positive role in the mental health management of hospitalized patients.

    Objective To perform hierarchical training for clinical nursing research group in order to improve nursing research ability of clinical nurses with different levels in the hospital. Methods A clinical nursing research group was established in 2017,including management group,expert group,core group,interest group and general group. Hierarchical training and practice of nursing research was conducted based on the results of pre-training demand survey between March and December in 2017. Results The nursing research ability of nurses was increased from(3.28±0.82)to(3.43±0.77)(P<0.01),and the self-efficacy was increased from (2.45±0.60)to(2.56± 0.63)(P<0.01 ),and the differences were statistically significant. Conclusion The hierarchical training for clinical nursing research group is beneficial to improvement of overall nursing research ability and self-efficacy of nurses.

    Objective To investigate the current status of nursing burden and its influencing factors of caregivers of patients with severe mental disorders in one district in Chongqing. Methods A total of 1218 patients with severe mental disorders included in public security bank and managed by national information system of severe mental disorders were re-diagnosed and evaluated by various forms. At the same time,their educational level,family income ,guardian relationship,guardian care burden,drug compliance were investigated. Nursing burden was explored by Zarit Caregiyer Burden Interview (ZBI). The data were analyzed by multivariate stepwise regression analysis. Results The ratio of high nursing burden (score of 46.38±5.61) was 76%,and low nursing burden(score of 31.80± 5.91)was 24.0%. There were only 375 patients (30.8%) who were regularly treated with drugs. Multivariate stepwise regression analysis showed that the drug compliance, risk classification and disease category were the main factors affecting the nursing burden of patients with severe mental disorders. Conclusion The ZBI score of caregivers of patients with severe mental disorders in Chongqing is relatively high. The percentage of schizophrenia patients with high burden of care was the largest proportion. Poor drug compliance increases nursing burden. The risk burden of patients with grade 4 or above is especially obvious.

    Objective To develop a guidance for applying pressure on puncture site after neonatal radial artery sampling (hereinafter referred to as the “guidance”),in order to standardize the procedure,improve the efficiency of compression,and reduce the incidence of complications after radial artery blood sampling. Methods Using literature review,questionnaire investigation and clinical practice,a standardized guidance was established with regard to dressing material,operator’s finger position,pressure intensity and duration. All nurses were trained to apply a “sandwich” point pressure with non-woven pure cotton fabric over puncture site for 5 minutes, based on neonatal systolic blood pressure. To evaluate the effectiveness of the guidance, success rate of hemostasis, degree of palm pulse oxygen desaturation during compression,and bleeding rate of puncture site or subcutaneous tissue were documented. Results After applying the guidance,success rate of hemostasis rose from 69.89% to 97.80%,palm desaturation rate fell from 18.28% to 5.49%,puncture site bleeding rate fell from 35.48% to 6.59%,and subcutaneous hemorrhage rate fell from 33.33% to 9.89%. Conclusion The application of our guidance after neonatal radial artery sampling can effectively achieve hemostasis.

    Objective To adapt the Clinical Practice Guideline (CPG) related to gestational diabetes mellitus. Methods The draft of gestational diabetes mellitus clinical practice guideline was developed through the ADAPTE method,and revised by expert consultation and patients review. According to the opinions of experts and patients, the final document was completed. Results A total of 14 guidelines and 49 systematic reviews were included,and the recommendations of the CPG involved 5 aspects: screening for GDM,antenatal care,intrapartum care,neonatal care,postpartum care. The CPG contained 21 clinical questions and 69 items of recommendations. Conclusion The adaptation of gestational diabetes mellitus is effective and feasible,and can provide evidence and support for professional decision-making in clinical practice.

    Objective To translate the Essentials of Magnetism Scale(EOM) into Chinese,and test the reliability and validity of the scale for initially establishing a Chinese version of the Essentials of Magnetism Scale. Methods A total of 1 389 clinical nurses were tested from a tertiary comprehensive hospital in Qingdao using the Chinese version of EOM which was developed through the process of translation,back-translation and cultural adaptation to verify its reliability and validity according to Brislin model. Results The Chinese version scale was extracted for 7 factors and 45 items by the factor analysis using the maximum orthogonal rotation,the cumulative contribution rate was 72.328%. Cronbach’s alpha coefficient for the overall scale was 0.980, the split-half reliability coefficient was 0.919 and the test-retest reliability coefficient was 0.889. Conclusion The Chinese version of the Essentials of Magnetism Scale has been proved to be reliable and valid which can be used to objectively evaluate the magnetic level and health status of nursing work environment in China.

    Objective To translate Functional Assessment of Cancer Treatment-Bone Marrow Transplant (FACT-BMT) Version 4 into Chinese,and test its reliability and validity in Chinese patients. Methods FACT-BMT Version 4 was translated according to Brislin’s method and culturally adapted in 137 patients underwent bone marrow transplant in Peking University People Hospital for testing internal consistency,and content validity was explored in experts. Results The Cronbach’s a coefficient was 0.71~0.90, and 0.83 for the total. The S-CVI was 86.9%,and 22 items’ I-CVI was above 0.80. Conclusion Domestic patients underwent bone marrow transplant can understand FACT-BMT Version 4 well. This scale has high internal consistency and content validity, and is capable to measure physical health, psychological emotion and social role of patients in China.
