Development and validity test of Knowledge，Attitude and Practice Scale for home-based pulmonary rehabilitation caregivers of patients with chronic obstructive pulmonary disease

Abstract:

Objective To develop a Knowledge，Attitude，and Practice（KAP） scale for home-based pulmonary rehabilitation caregivers of patients with chronic obstructive pulmonary disease（COPD），and to evaluate its reliability and validity，so as to provide a tool for assessing the caregiving ability of such caregivers. Methods Guided by the KAP theory，the initial version of the scale was developed through literature review，semi-structured interviews，and 2 rounds of expert consultation. From May 2025 to December 2025，413 home-based pulmonary rehabilitation caregivers of COPD patients followed up in the respiratory department of a tertiary hospital in Shanghai were selected as survey subjects using convenience sampling. Item analysis，exploratory factor analysis，and confirmatory factor analysis were used to evaluate structural validity. Content validity was assessed by 5 experts. Reliability was evaluated using Cronbach’s α coefficient，split-half reliability，and test-retest reliability. Results A total of 400 valid questionnaires were collected. The final KAP scale for home-based pulmonary rehabilitation caregivers of COPD patients consists of 3 dimensions，7 factors，and 42 items. Exploratory factor analysis extracted 7 common factors，with a cumulative variance contribution rate of 86.408%. Confirmatory factor analysis indicated a good model fit. The scale-level content validity index was 0.856. The Cronbach’s α coefficient of the total scale was 0.820；the split-half reliability was 0.955；the test-retest reliability was 0.827. Conclusion The developed KAP scale for home-based pulmonary rehabilitation caregivers of COPD patients demonstrates good reliability and validity，and it can be used as a tool to assess the competence of these caregivers.

摘要：

目的编制慢性阻塞性肺疾病患者照护者居家肺康复知信行量表，并检验其信效度，为测评慢性阻塞性肺疾病患者照护者的居家肺康复相关能力提供工具。方法以知信行理论为框架，通过文献分析、半结构式访谈和2轮专家函询形成量表初稿。于2025年5—12月，采用便利抽样法，选取在上海市某三级医院呼吸科随访的413名进行居家肺康复的慢性阻塞性肺疾病患者的照护者作为调查对象。采用项目分析及探索性、验证性因子分析评价量表的结构效度；邀请5名专家评定量表的内容效度；采用Cronbach’s α系数、折半信度及重测信度评价量表的信度。结果共回收有效问卷400份。最终形成的慢性阻塞性肺疾病患者照护者居家肺康复知信行量表包括3个维度、7个因子，共42个条目。探索性因子分析提取7个公因子，累计方差贡献率为86.408%。验证性因子分析提示模型拟合良好。量表水平的内容效度为0.856。量表总的Cronbach’s α系数为0.820，折半信度为0.955，重测信度为0.827。结论编制的慢性阻塞性肺疾病患者照护者居家肺康复知信行量表具有良好的信度和效度，可作为评估慢性阻塞性肺疾病患者照护者居家肺康复能力水平的工具。
