Abstract:

Objective Based on the perspective of Chinese fertility culture，the study explored the impact of cultural factors on the decision-making of pregnant women regarding physical activity. It aimed to provide insights into the underlying mechanisms behind the insufficient physical activity of pregnant women in China and promote higher levels of physical activity during pregnancy. Methods The research method of grounded theory was adopted，and a purposive sampling approach was employed to select 15 pregnant women from a tertiary hospital in Nantong City as research subjects between December 2024 and March 2025. Semi-structured interviews were conducted from the perspective of fertility culture，using the proceduralized grounded theory to analyze the data through 3 levels of coding. Results The analysis revealed that fertility culture influenced pregnant women’s physical activity decision-making in 4 aspects：the implicit norms of intergenerational fertility culture on decision-making，the risk-avoidance decision logic in traditional fertility culture，the reshaping of decision-making among working women under modern fertility culture，and the decision-making strategies for physical activity amid the cultures integration. Conclusion Fertility culture deeply participates in pregnant women’s physical activity decision-making by influencing their cognitive beliefs，behavioral choices，and activity patterns. Healthcare professionals should pay attention to the impact of cultural factors on pregnant women’s physical activity. They should enhance support for pregnant women through diverse channels and create a reproductive-friendly social atmosphere to improve the quality of prenatal care.

摘要：

目的 基于国内生育文化视角，探讨生育文化对孕妇孕期体力活动决策的影响，为解析国内孕妇孕期体力活动不足背后的深层机制提供参考，以提升孕妇孕期体力活动水平。 方法 采用扎根理论的研究方法，通过便利抽样法，于2024年12月—2025年3月选取南通市某三级甲等医院产检的15名孕妇作为研究对象，对其进行半结构式访谈，采用扎根理论，通过三级编码分析资料。 结果 生育文化对孕妇孕期体力活动决策的影响体现在4个方面：代际生育文化对孕期体力活动决策的隐性规范、传统生育文化中的风险规避型决策逻辑、新型生育文化对职业女性孕期体力活动决策的重塑、文化调适下的孕期体力活动决策策略。 结论 生育文化通过影响孕妇孕期体力活动的认知信念、行为选择、活动模式，深度参与孕妇孕期体力活动决策。医护人员应重视文化因素对孕妇孕期体力活动的影响，通过多元化渠道加强对孕妇的支持，营造生育友好的社会氛围，以提高孕期保健质量。
