1例肾移植术后移植物功能延迟恢复合并消化道大出血患儿的护理

Abstract:

To summarize the nursing care of a child with delayed graft function and massive gastrointestinal hemorrhage after renal transplantation. Nursing focuses are as following:to maintain blood perfusion of transplanted kidney,to improve delayed recovery of graft function;to implement scientific immunosuppressive management strategies,to provide accurate medication care;to start highly effective anti-shock treatment,to actively cooperate with endoscopic hemostasis;to implement gastrointestinal protection measures,to achieve gastrointestinal bleeding and immunosuppression coordinated care;implementation of phased nutritional support program to maintain gastroin-testinal homeostasis;to carry out individual rehabilitation nursing,to alleviate the negative emotions of children and their families. After careful treatment and nursing care,the children were discharged 59 days after operation and followed up for 1 year,and the transplanted kidney recovered well and the child returned to normal life.

摘要:

总结1例肾移植术后移植物功能延迟恢复合并消化道大出血患儿的护理体会。护理要点:维持移植肾血液灌注,改善移植物功能延迟恢复症状;执行科学的免疫抑制管理策略,提供精准化用药护理;启动高效抗休克治疗,积极配合内镜下止血;落实胃肠道保护措施,实现消化道出血与免疫抑制协同护理;实施分阶段的营养支持方案,维持胃肠道稳态;开展个体化康复护理,缓解患儿及其家长的负性情绪。经过精心的治疗与护理,患儿术后59 d顺利出院,随访1年,移植肾功能恢复良好,回归正常生活。
