Abstract:

To summarize the nursing experience of a child with hypoplastic left heart syndrome undergoing modified total vena cava pulmonary artery anastomosis operation. Preoperative nursing points：to establish a multidisciplinary team to formulate the best surgical plan；implementation of pre-rehabilitation nursing to reduce the risk of surgery. Postoperative nursing points：precise volume management to ensure effective pulmonary circulation perfusion；to reduce pulmonary artery pressure and maintain effective cardiac output；early warning of high risk to prevent thrombosis；nutritional support should be strengthened and protein-losing enteropathy should be alert for its incidence. Phase I cardiac rehabilitation training should be strengthened to improve cardiopulmonary function. After careful treatment and nursing care，the child recovered and was discharged on the 28 th day after surgery.

总结1例左心发育不良综合征患儿行改良全腔静脉-肺动脉吻合术的护理体会。术前护理要点：组建多学科团队，制订最佳手术方案；实施预康复护理，降低手术风险。术后护理要点：精准容量管理，保证肺循环有效灌注；降低肺动脉压力，维持有效心排血量；启动高危预警，预防血栓形成；加强营养支持，警惕蛋白丢失性肠病的发生；强化Ⅰ期心脏康复训练，改善心肺功能。经过精心的治疗与护理，患儿于术后第28天康复出院。
