摘要/Abstract

摘要：

目的构建择期全麻手术患者围手术期安全自检清单并进行临床验证，为手术患者参与围手术期安全管理提供工具支持。方法通过文献回顾、半结构式访谈，初步拟订自检清单初稿；于2025年3—5月，邀请来自全国8所三级甲等医院的16名相关领域专家进行2轮德尔菲法专家函询，结合专家意见对清单条目进行修改、完善，形成预试版清单。采用便利抽样法，选取2025年7—11月重庆市某三级甲等医院6个外科科室的择期全麻手术患者作为调查对象，进行内容效度指数、组内相关系数和Cronbach’s α系数分析。结果 2轮专家函询的有效问卷回收率均为100%；专家权威系数分别为0.90、0.94；肯德尔和谐系数分别为0.356、0.365（P<0.001）。最终形成包含3个一级指标、7个二级指标、32个条目的自检清单。信效度验证结果显示，总体平均内容效度指数为0.886、组内相关系数均值为0.883，95%CI为0.857~0.905（P<0.001），清单总体的Cronbach’s α系数为0.894、各维度Cronbach’s α系数为0.768~0.915。结论构建的择期全麻手术患者围手术期安全自检清单内容全面，具有科学性和临床适用性，可为开展手术患者参与围手术期安全管理提供参考。

Abstract:

Objective To construct and clinically verify a perioperative safety self-checklist for elective general anesthesia surgical patients，so as to provide a scientific and effective tool for surgical patients to participate in perioperative safety management. Methods Based on literature review and semi-structured interviews，a preliminary draft of the self-checklist was initially formulated. From March to May 2025，16 experts from the relevant field in 8 tertiary hospitals across the country were invited to conduct 2 rounds of Delphi expert consultation. The list items were revised and improved by incorporating expert opinions，and a pilot version of the checklist was formed. Using convenience sampling，patients undergoing general anesthesia for elective surgery from 6 surgical departments in a tertiary hospital in Chongqing from July to November 2025 were selected as the survey subjects. Content validity index，intra-class correlation coefficient（ICC） and Cronbach’s α analysis were conducted. Results The recovery rates of valid questionnaires in both rounds of expert consultation were 100%. The expert authority coefficients were 0.90 and 0.94，respectively. The Kendall’s coefficient of concordance was 0.356 and 0.365，respectively（P<0.001）. Finally，a self-checklist with 3 first-level indicators，7 second-level indicators，and 32 items was formed. The results of reliability and validity verification showed that the Total Scale-Content Validity Index/Average（S-CVI/Ave） was 0.886，and the mean value of ICC was 0.883，with a 95% confidence interval of （0.857~0.905），and P<0.001. The overall Cronbach’s α coefficient of the checklist was 0.894，and the Cronbach’s α coefficients for the dimensions ranged from 0.768 to 0.915. Conclusion The constructed self-checklist for perioperative safety of elective general anesthesia surgical patients is comprehensive in content，scientific and clinically practical，providing a basis for carrying out surgical patients’ participation in perioperative safety management.
