A qualitative study on illness perception in patients with cancer-associated venous thromboembolism

摘要:

目的 剖析肿瘤相关性静脉血栓栓塞症(cancer-associated venous thromboembolism,CAT)患者疾病感知的特征,分析现存问题,为后续开展基于疾病感知评估的精准干预提供依据。 方法 以自我调节常识模型为理论指导,采用描述性质性研究设计,于2025年7—8月通过目的抽样法选取在北京市某三级甲等医院住院的17例CAT患者进行半结构式访谈,采用Colaizzi 7步分析法对访谈资料进行分析。 结果 共提炼出4个核心主题及12个亚主题。①疾病感知偏差:对疾病症状的感知偏差、对病因的感知偏差、对病程预期的感知偏差、对后果的感知偏差、疾病控制信念缺失。②疾病感知影响下的应对策略存在个体化差异:低疾病控制信念驱动的信息回避、疾病控制责任外置驱动的被动接受、高疾病控制信念驱动的积极应对。③疾病感知影响下的多维负担体验:多重躯体症状叠加、多维负性情绪交织。④疾病感知影响下的治疗决策困境:多病共存下对各疾病危害的排序影响治疗决策、多重用药下的认知冲突影响抗凝治疗依从性。 结论 CAT患者存在疾病感知偏差,应对行为存在个体化差异,整体疾病负担重且面临多重治疗决策冲突。未来应基于疾病感知评估开展精准干预,以促进患者积极应对、改善预后。

Abstract:

Objective To describe the illness perceptions of patients with cancer-associated thrombosis(CAT),to gain an in-depth understanding of the characteristics and existing problems of their illness perceptions,and to provide a basis for subsequent precision interventions based on illness perception assessment. Methods Guided by the Common-Sense Model of Self-Regulation as the theoretical framework,a descriptive qualitative study design was adopted. Purposive sampling was used to select 17 patients with CAT for semi-structured interviews. Data were analyzed using Colaizzi’s seven-step method. Results A total of 4 themes and 12 sub-themes regarding the patients’ illness perceptions of venous thromboembolism(VTE) were extracted:①cognitive biases in illness perceptions(to identity biases in perception of disease symptoms,biases in causal attribution,biases in disease course expectation,biases in consequence perception,and perceived lack of control);②individual differences in coping behaviors influenced by illness perception(information avoidance driven by low perceived control,passive compliance driven by externalized control,and active self-management driven by high perceived control);③mul-tidimensional burden experience influenced by illness perception(superimposed perception of multiple physical consequences and multidimensional intertwining of negative emotions); ④treatment decision-making dilemmas influenced by illness perception(impact of consequence prioritization under multimorbidity on anticoagulation therapy,impact of cognitive conflict under polypharmacy on acceptance of anticoagulation). Conclusion CAT patients exhibit cognitive biases in VTE illness perceptions,significant individual differences in coping behaviors,multidimensional burden,and treatment decision-making conflicts. Future interventions based on illness perception assessment and precision strategies should be implemented to promote positive coping and improve prognosis.
