三级综合医院急诊科护士置信职业行为指标的构建与验证

Construction and verification of entrustable professional activities for nurses in emergency departments of tertiary general hospitals

摘要:

目的 构建三级综合医院急诊科护士置信职业行为指标并进行初步验证,为评价三级综合医院急诊科护士岗位胜任力提供参考。方法 通过文献研究、研究小组会议及专家咨询法确定急诊科护士置信职业行为指标,并于2022年5月使用三级综合医院急诊科护士置信职业行为评价指标满意度问卷对无锡市某三级综合医院40名急诊科护士进行问卷调查,以初步验证指标的应用效果。结果 对21名专家进行2轮咨询,咨询问卷的有效回收率均为100%;专家权威系数分别为0.881、0.898;肯德尔和谐系数分别为0.509、0.535(P<0.01),最终形成了18个三级综合医院急诊科护士置信职业行为指标条目及说明,并确定各指标在不同年限的预期置信级别。急诊科护士对置信职业行为评价指标较满意,满意度得分为(22.25±3.04)分。结论 该研究形成的三级综合医院急诊科护士置信职业行为指标具有科学性和实用性,可为评价三级综合医院急诊科护士岗位胜任力提供参考。

Abstract:

Objective To construct indicators of nurse entrustable professional activities(EPAs) in emergency departments of tertiary general hospitals,and to conduct preliminary validation,so as to provide references for evaluating the competence of nurses in emergency departments. Methods EPAs indicators for emergency department nurses were identified through literature research,research group meetings and Delphi consultation. A questionnaire survey was conducted in May 2022 on 40 nurses in an emergency department of a tertiary general hospital in Wuxi using Entrustable Professional Activities Evaluation Indicators Satisfaction Questionnaire to verify the effectiveness of the indicator application. Results 2 rounds of Delphi consultations were conducted with 21 experts. The effective recovery rates for the 2 rounds of Delphi consultation questionnaires were 100%. The authority coefficient values were 0.881 and 0.898,respectively. Kendall concordance coefficients were 0.509 and 0.535,respectively(P<0.01). Finally,a set of 18 indicators and descriptions of EPAs in emergency department nurses were formed,and the expected entrust level of each indicator in different years were determined. Emergency department nurses were satisfied with the indicators of EPAs evaluation,with a score of(22.25±3.04). Conclusion The indicators of nurse EPAs in emergency department are scientific and practical,which can provide a better reference for the evaluation of nurses’ competence in emergency department of tertiary hospitals.
