    Effect of somatosensory exercise based on artificial intelligence technology in home pulmonary rehabilitation of elderly patients with COPD

    Objective To explore the application effect of multimodal somatosensory exercise based on artificial intelligence technology in home rehabilitation exercise for elderly patients with chronic obstructive pulmonary disease(COPD),so as to promote COPD patients to participate in home rehabilitation exercise. Methods Using the convenient sampling method,80 elderly patients with COPD admitted to the Department of Respiratory Medicine of a tertiary A hospital in Urumqi from November 2023 to February 2024 were selected as the research subjects. According to the random number table method,they were divided into a control group and an experimental group,with 40 cases in each group. The control group adopted the traditional exercise training method,and the experimental group adopted the multi-modal somatosensory movement based on artificial intelligence technology for exercise intervention,with 5 times a week,and the intervention was implemented for 12 weeks. The pulmonary function index,modified Medical Research Council scale score,physical fitness level,Chronic Obstructive Pulmonary Disease Assessment Test scale score and exercise compliance of the 2 groups were compared before intervention and 12 weeks after intervention. Results 77 patients completed the study,with 39 in the experimental group and 38 in the control group. The forced vital capacity,forced expiratory volume in one second,forced expiratory volume in one second to forced vital capacity ratio,physical fitness level and exercise compliance of the experimental group were higher than those of the control group,while the modified British Medical Research Council scale score and Chronic Obstructive Pulmonary Disease Assessment Test score were lower than those of the control group. The differences were statistically significant(P<0.05). Conclusion Somatosensory exercise based on artificial intelligence technology can improve the lung function of the patients with COPD,improve the exercise compliance and physical fitness indicators of elderly patients and improve the quality of life of the patients.

    Objective Taking into account the physical and mental characteristics of patients with COPD,we develop and implement a lung rehabilitation education game software,aiming to explore more effective health education practices for COPD. Methods Drawing on the Health Belief Model theory,the game content was developed by semi-structured interviews,brainstorming sessions,and expert consultations,and it was implemented via a WeChat mini program. Patients with COPD from the respiratory department of a tertiary general hospital of Luzhou city between March and October 2023 were conveniently selected as the study subjects. Among them,40 patients from July to October 2023 were designated as an experimental group,and another 40 from March to June 2023 constituted a control group. The experimental group was provided with routine health education combined with pulmonary rehabilitation games,whereas the control group received standard health education. After the intervention,the researchers compared the COPD knowledge questionnaire scores and pulmonary rehabilitation adherence between the 2 groups,and assessed the satisfaction of both patients and nurses with the software. Results No sample detachment. The score of the COPD knowledge questionnaire in the experimental group was higher than that in the control group,and the difference was statistically significant(Z=5.361,P<0.001). The proportion of patients in the experimental group with good adherence to pulmonary rehabilitation(85%) was significantly higher than it in the control group(25%),(χ2=29.091,P<0.001). The patients’ overall satisfaction rate with the game was 95%,with operational effectiveness receiving the highest satisfaction rating at 97%. Conclusion The pulmonary rehabilitation education game for patients with COPD can improve their knowledge of COPD,enhance their compliance with pulmonary rehabilitation. Patients have high satisfaction with the software. The software enriches the clinical health education methods,which can be used in clinical practice.

    Objective Under the guidance of the Social Ecosystems Theory(SET) framework,this study explores the experience of social isolation among patients with chronic obstructive pulmonary disease(COPD),providing a basis for the development of targeted clinical interventions. Methods This study employed purposeful sampling methods. Semi-structured interviews were conducted from April to July 2024 with patients diagnosed with COPD at a tertiary hospital in Shanxi Province,China,who met the inclusion and exclusion criteria. Data were analyzed using Colaizzi’s seven-step analysis method and NVivo 12.0 software. Results This study involved in-depth interviews with 15 patients suffering from COPD,totaling approximately 405 minutes of interview time and yielding over 40,000 words in transcribed text. Totally 3 main themes and 9 sub-themes were distilled,among which the microsystem level reflects the intricate interplay of individual experiences(physical functional limitations,severe negative emotions,heightened emotional imbalance,and weakened social roles);the mesosystem level pertains to the support and challenges from family and social networks(intense need for familial emotional support,widening social gaps in interactions with friends and relatives,and pronounced interpersonal communication barriers);the macrosystem level involves the integrated influence of societal and policy environments(restricted living conditions and urgent demands for policy support and service accessibility). Conclusion The social isolation experienced by patients with COPD manifests in multiple dimensions of experience. It is necessary to develop interdisciplinary and multi-level comprehensive intervention strategies for the future,to create more social opportunities and emotional connections for patients,thereby improving their quality of life.

    To summarize the nursing experience of 3 patients with severe hypercapnia during the acute exacerbation of chronic obstructive pulmonary disease(COPD) undergoing extracorporeal carbon dioxide removal(ECCO2R),as well as adjustment of oxygen therapy support strategy and implementation of non-pharmacological airway clearance techniques. Additionally,the nursing focus includes all process nursing of ECCO2R,which covers the key aspects such as catheter selection and placement management,pre-flush setting and bubble handling,dual connection with initial parameter setting for patients,heparin pathway management and bleeding monitoring,observation of thrombosis formation and pressure monitoring during operation and self-examination,evaluation of carbon dioxide removal efficiency and implementation of decommissioning tests. After careful treatment and nursing,2 patients recovered and were discharged,and a patient’s condition improved and was transferred to the respiratory ward for further treatment,and was discharged after 5 days.

    Objective To systematically evaluate qualitative studies on the experience of inhalation therapy for patients with chronic obstructive pulmonary disease(COPD),in order to provide a basis for healthcare professionals to optimize the care strategies. Methods A systematic search was conducted for qualitative studies on the experience of inhalation therapy for patients with COPD included in domestic and international databases,with a timeframe from the establishment of the database to September 2024. Literature quality was evaluated using the Australian JBI Centre for Evidence-Based Health Care Literature Quality Assessment Criteria for Qualitative Research,and the results were integrated by a pooled integration approach. Results A total of 12 articles were included,and 84 original findings were extracted and summarized into 10 new categories to form 4 integrated results,including low level of drug literacy,differences in perceived efficacy,multiple medication burdens,and multi-dimensional support for inhalation therapy. Conclusion COPD patients have multiple experiences during inhalation therapy. Medical staff should strengthen drug education,enhance patients’ cognition and medication skills,foster a positive treatment attitude,and integrate multiple resources to improve patients’ inhalation therapy experience.

    Objective To explore the expert consensus on perinatal care management of infants with congenital heart disease(hereinafter referred to as “Consensus”) in order to promote the standardization of integrated nursing. Methods The literature was systematically searched and several discussions were organized within the group to compile the first draft of the Consensus. From January to March 2024,20 experts in the clinical nursing,nursing management,clinical medicine and other fields of congenital heart disease were solicited through 2 rounds of Delphi,and 8 experts were invited to conduct a validation to revise the items to form the final Consensus. Results The recovery rates of the 2 rounds of questionnaires were 100%;the experts’ authority coefficient was 0.89;the Kendall’s W were 0.172,0.211,with statistical significance(P<0.05). The Consensus included 8 first-level subjects,namely prenatal examination and consultation,postpartum screening,standardized referral,preoperative nursing,intraoperative nursing,postoperative nursing,other disease screening,health education and discharge follow-up. Conclusion The Consensus is scientific and rigorous,and it can provide a reference basis for clinical nursing staff to carry out the care and management of newborns with congenital heart disease.

    Objective To establish an expert consensus on home exercise for prevention and rehabilitation of breast cancer related lymphedema. Methods Evidence-based nursing method was used to systematically retrieve and evaluate relevant studies in databases and guideline networks,with a search period from the establishment of the databases to October 2023. Evidence was synthesized to form the expert consensus draft after literature screening,quality evaluation,evidence extraction and evidence integration. Then the final draft was formed through expert discussion meeting and 2 rounds of expert letter consultation. Results The consensus includes 5 dimensions with a total of 22 items,involving evaluation of home exercise,management of home exercise,principles of home exercise,program of home exercise and significance of home exercise. Conclusion This “consensus” was constructed in a scientific and rigorous way,and can provide guidance for promoting the home exercise for prevention and rehabilitation of breast cancer related lymphedema,and ensure the safe and effective implementation of home exercise rehabilitation.

    Construction and application of an early in-hospital temperature management protocol for patients with heat stroke

    Objective This study aims to develop an early in-hospital temperature management protocol for heat stroke patients and assess its effectiveness,providing guidance for rapid cooling and precise target temperature control. Methods The protocol was developed through a Delphi expert consultation combined with expert panel meetings. A multi-center,non-randomized,historical control study was conducted,utilizing convenience sampling to select heat stroke patients from the emergency departments of 7 tertiary hospitals in Zhejiang Province,China,between June and August 2024 as an experimental group. The protocol was implemented in this group,while the control group consisted of heat stroke patients treated between June and August 2022,prior to protocol implementation. Cooling rates,target temperature attainment rates,and clinical outcomes were compared between the 2 groups. Results The final protocol included 6 primary indicators,23 secondary indicators,and 56 tertiary indicators. After protocol implementation,the experimental group achieved a cooling rate of 0.08(0.05~0.09) °C/min within 0.5 hours,significantly higher than the control group,which had a rate of 0.04(0.02~0.06) °C/min(P<0.001). The target temperature attainment rates at 0.5 hours and 2.0 hours were 55.93% and 98.31%,respectively,significantly higher than the rates of 15.87% and 61.11% in the control group(P<0.001). The mechanical ventilation rate,hospitalization rate,ICU admission rate,and mortality rate in the experimental group were 25.42%,61.02%,44.07%,and 8.47%,respectively. Logistic regression analysis revealed that the early in-hospital temperature management protocol significantly reduced the risk of mechanical ventilation and hospitalization in heat stroke patients,with odds ratios(ORs) of 0.294 and 0.300,respectively(both P<0.05). Conclusion The developed protocol for early in-hospital temperature management in heat stroke patients is scientific,systematic,and practical. It improves cooling rates and target temperature attainment,thereby enhancing the prognosis of heat stroke patients.

    Objective In order to understand the current status and influencing factors of perioperative pulmonary rehabilitation knowledge,attitude,and practice among Chinese thoracic surgery nurses,and to provide references for managers to take targeted measures to improve their pulmonary rehabilitation knowledge,attitude,and practice level. Methods From August to October 2023,a convenience sampling method and cross-sectional descriptive survey were used to anonymously survey 1 167 thoracic surgery nurses from tertiary-level A cancer specialty hospitals and tertiary-level A general hospitals in 24 provinces through the Wenjuanxing platform. A general information questionnaire and a knowledge,attitude and practice questionnaire for lung cancer perioperative pulmonary rehabilitation nurses were used,including knowledge dimension(14 items),belief dimension (6 items),and behavior dimension(14 items),totaling 34 items. Univariate analysis and linear regression multivariate analysis were used to determine the influencing factors of the knowledge,attitude and practice level of thoracic surgery nurses in perioperative pulmonary rehabilitation. Results 1 089 nurses completed the investigation. The total standard score and the dimension scores on knowledge,attitude,and behavior of the perioperative pulmonary rehabilitation questionnaire for thoracic surgery nurses were(80.45±11.84)(73.72±14.68)(89.61±11.38) and(83.46±14.48),respectively. The nature of the hospital,whether there is an administrative position,whether the department carries out ERAS,and whether they have received pulmonary rehabilitation training are the influencing factors of the perioperative pulmonary rehabilitation knowledge,attitude,and practice level of thoracic surgery nurses(P<0.05). Conclusion The knowledge,attitude,and practice of perioperative lung rehabilitation among thoracic surgery nurses are generally at a moderate level. However,their knowledge of perioperative lung rehabilitation is relatively insufficient,and there are many influencing factors. Managers should strengthen pulmonary rehabilitation training for thoracic surgery nurses,improve their knowledge reserves and evidence conversion capabilities,and coordinate cross-integration among multidisciplinary teams to improve their knowledge,attitude and practice in perioperative pulmonary rehabilitation.

    Objective To understand the facilitating and obstacle factors for pediatric nurses in implementing interventions for children’s medical fears and to provide a foundation for exploring intervention strategies for pediatric nurses to effectively tackle children’s medical fears. Methods By purposive sampling,face-to-face interviews were conducted with 20 pediatric nurses in a tertiary-level A children’s hospital in Wenzhou City from December 2023 to February 2024. Traditional content analysis was used for data analysis. Results Facilitators and obstacle factors to the implementation of medical fear interventions by pediatric nurses were extracted. The 4 sub-themes of the facilitator theme include awareness of the importance of medical fear intervention,the drive of individual empathy,positive peer support,and the construction of a suitable hospital environment. The 3 sub-themes of the obstacle theme include the lack of individualized intervention skills for medical fear,heavy workload,interference from negative emotional behavior of family members. Conclusion There are many factors influencing paediatric nurses to implement medical fear interventions for children. It is recommended that clinical nursing administrators should strengthen the systematic training of pediatric nurses’ knowledge about medical fear interventions for children,unite with multidisciplinary experts,and recruit medical social workers or volunteers to collaborate with nurses and children’s families to cope with children’s medical fears together and to promote physical and mental health of hospitalized children.

    Objective To retrieve,evaluate and synthesize the best evidence for nutritional management in patients with acute-on-chronic liver failure. Methods Evidence on nutritional management of patients with acute-on-chronic liver failure was systemically retrieved from the clinical decision support systems,guideline websites,professional association websites and databases,such as Cochrane Library,Joanna Briggs Institute,PubMed,Embase,CINAHL,Web of Science,VIP database,CNKI and CBM,including clinical decisions,guidelines,expert consensuses,evidence summaries and systematic reviews from inception to July 16,2024.Researchers trained in evidence-based nursing used authoritative tools to evaluate the methodological quality of the literature,and extracted and synthesized evidence according to the theme. Results A total of 13 articles were included,including 1 clinical decision,8 guidelines,and 4 expert consensuses. The best evidence included 26 pieces of evidence in 6 areas,namely the nutrition support team,nutritional assessment,energy and protein intake,oral nutrition support,enteral and parenteral nutrition support,and management of complications. Conclusion This study summarizes the best evidence for nutritional management in patients with acute-on-chronic liver failure,which can provide a basis for their standardized management.Medical staff should evaluate the availability of evidence from multiple perspectives when applying it.

    Survey on the current situation of management of sterilization and supply centers in response to emerging and outbreaks of infectious diseases in 442 hospitals and analysis of influencing factors

    Objective To investigate the current situation of emergency response management during new outbreaks of infectious diseases in central sterile supply department(CSSD) of hospitals in 18 provinces(autonomous regions and municipalities),and to provide a reference basis for the establishment of a sound emergency response management system. Methods A convenience sampling method was used to investigate and study the CSSDs in 447 hospitals from June to December 2023 using a self-developed questionnaire. The questionnaire included 3 aspects,including general information,emergency disposal management situation(including environment management,personnel management,material management,and process management),and satisfaction with emergency disposal management,and the relevant factors affecting the satisfaction with the emergency disposal management of CSSDs responding to new outbreaks of infectious disease devices were analyzed. Results A total of 447 questionnaires were recovered,with 442 valid questionnaires and a valid questionnaire recovery rate of 98.88%. In terms of environmental management,CSSDs in 88(19.91%) hospitals set up independent instrument pre-sterilization rooms for infected device disposal. In terms of material management,CSSDs in 42(9.50%) hospitals conducted daily emergency material management. In terms of personnel management,CSSDs in 236(53.39%) hospitals conducted emergency response knowledge assessments. In terms of process management,CSSDs in 225(50.90%) hospitals conducted information traceability system full-process closed-loop management. The results of multiple linear regression analysis showed that whether or not to carry out pre-treatment,emergency supplies stockpiling and management,emergency disposal knowledge assessment,establishment of standard operating procedures,and full-process closed-loop management of the traceability system were the influencing factors affecting the satisfaction of CSSD’s emergency disposal management(P<0.05). Conclusion The current situation of CSSD’s emergency disposal management of new outbreaks of infectious diseases needs to be improved,and the environmental management of the decontamination area should be strengthened to improve the staff’s ability of emergency disposal,guarantee the reserve of emergency supplies,and strengthen the informatization of the whole process management,to reduce the risk of hospital infections and safeguard the quality of medical care.

    Investigation and nursing countermeasures on the current situation of oral instrument cleaning,disinfection,and sterilization management in 348 medical institutions in Henan Province

    Objective To investigate the current status of cleaning,disinfection,and sterilization management of dental instruments in medical institutions in Henan Province,analyze potential issues in hospital infection control,and propose targeted nursing strategies. Methods A convenience sampling method was used to survey the cleaning,disinfection,and sterilization management of dental instruments in 352 medical institutions in Henan Province from February to April 2024. A self-made questionnaire was designed,covering aspects such as the handling model and personnel configuration for dental instruments,training for cleaning,disinfection,and sterilization personnel,configuration and maintenance of cleaning and disinfection equipment,use and management of small pressure steam sterilizers,and reprocessing status of dental instruments. Results A total of 352 questionnaires were distributed,with 348 valid responses. Among the 34 primary medical institutions,only 10(29.41%) had a centralized cleaning,disinfection,and sterilization model for dental instruments;13(38.24%) had dedicated personnel for cleaning,disinfection,and sterilization of dental instruments;25(73.53%) were equipped with pressure water guns. Compari-sons among medical institutions of different levels showed statistically significant differences(P<0.001). In the 348 medical institutions,194(55.75%) arranged pre-job training for nursing personnel;143(41.09%) performed mainte-nance on cleaning and disinfection equipment once a year;52(14.94%) did not perform pre-treatment on contaminated dental instruments after use. Among the 104 institutions using small pressure steam sterilizers,21(20.19%) used Type N small pressure steam sterilizers. Conclusion The centralized management rate of dental instruments and the basic equipment configuration rate in primary medical institutions in Henan Province are relatively low. There are issues such as insufficient personnel training,neglect of equipment maintenance,improper management of small pressure steam sterilizer usage,and incomplete reprocessing procedures in medical institutions at all levels. It is recommended that nursing managers further strengthen the training of nurses in the disinfection supply center,standardize the cleaning,disinfection,and sterilization workflow for dental instruments,in order to prevent and control hospital infections.

    Objective To systematically evaluate and synthesize the experience and coping process of bowel symptoms in patients after sphincter-preserving surgery for rectal cancer,and to provide the evidence for the subsequent development of bowel symptom management strategies. Methods A comprehensive search was conducted across the Pubmed,Cochrane Library,CINAHL,Embase,Web of Science,CNKI,Wanfang Database,VIP Database and CBM Database for qualitative studies on the experience of bowel symptoms in post-sphincter-preserving surgery patients with rectal cancer. The search period was from database inception to October 2024. The quality of the included literature was assessed according to the Australian Joanna Briggs Institute Center for Evidence-Based Health Care Quality Assessment Criteria for Qualitative Research,and the results were synthesized through the aggregative integration method. Results 14 studies were included,yielding 52 research findings,which were grouped into 10 subcategories and further synthesized into 4 results:the physical and psychological experiences of patients with bowel symptoms;the impact of bowel symptoms on patients’ daily lives;coping styles for bowel symptoms in patients;facilitators of patients bowel symptom coping. Conclusion Bowel symptoms have significant negative impacts on the lives of patients following sphincter-preserving surgery for rectal cancer,and healthcare professionals should address these patients’ needs by developing effective symptom management strategies and supporting patients in enhancing self-management abilities to improve quality of life.

    Objective To evaluate the methodological quality and measurement attribute quality of the evaluation tool for pediatric palliative care quality assessment tools,and to provide references for medical staff to select the best assessment tools. Methods The PubMed,Embase,Cochrane Library,Web of Science,CINAHL,Scopus,China National Knowledge Infrastructure(CNKI),Wanfang Database,VIP Database,Chinese Biomedical Literature Database,GIN,NGC,NICE,NRAO,medlive,WHO,AAHPM,WHPCA,APHN were searched from inception to March 28,2024. Data were screened and extracted independently by 2 researchers. The consensus-based standards for the selection of health measurement instruments(COSMIN) checklist and quality criteria were employed to evaluate the methodological quality and psychometric properties of the included pediatric palliative care quality assessment tools. Finally,recommendations were formulated based on these evaluations. Results A total of 13 articles were included,involving 9 pediatric palliative care quality assessment tools. Among them,the PICU-QODD,PaPEQu and QCPCI demonstrated good content validity and internal consistency,and are recommended as Grade A. The remaining assessment tools are recommended as Grade B or C. Conclusion The PICU-QODD,PaPEQu and QCPCI are recommended for use,but further validation of their psychometric properties is still needed.

    Just-in-time adaptive intervention is an emerging technology driven type of behavioral change intervention that can provide tailored behavioral support to participants when they are most likely to change negative behavior. This article reviews the application of just-in-time adaptive intervention in chronic illness sedentary and physical activity from 3 aspects:overview,application,and intervention effect. The aim is to provide theoretical references for improving sedentary behaviors and enhancing physical activity.

The model supports personalized interventions,aiming to achieve precision care and ultimately improve patient health. This study reviews the development and scope of the symptom science model,analyzes its current applications in chronic kidney disease,and explores its implications for symptom management in this condition. The aim is to offer insights and references for the scientific management of chronic kidney disease symptoms.

    The majority of shift work nurses have poor sleep quality,and chronic low quality sleep can adversely affect their physical and mental health,work performance,and patient safety. Non-pharmacologic interventions have a high safety profile and can be effective in improving sleep quality. This article reviews the factors affecting sleep quality,the theoretical basis,non-pharmacological interventions,and their effects among nurses at home and abroad,and proposes an outlook for the future,aims to provide theoretical guidance for the development of well-targeted and scientifically sound intervention programmes in the future.

    Chronic disease patients often experience negative emotions such as anxiety and depression,and the accurate assessment of these emotions is crucial for developing effective care plans and interventions. Traditional assessments of negative emotions are limited by subjectivity,error and accessibility issues,making precise identification and evaluation difficult. In recent years,vision technology has demonstrated unique advantages in emotion recognition due to its accuracy,speed,consistency,objectivity nature. This paper reviews the development process of computer vision technology,the content and effects of its application in negative emotion assessment for chronic disease patients,and the current challenges,in order to provide references for the evaluation of chronic disease patients’ negative emotions.
