    Construction of a practical program for interhospital transfer of critically ill children

    Objective To develop an evidence-based,localized practice protocol for the interhospital transfer of critically ill children. Methods Through a comprehensive evidence summary and semi-structured interviews,a preliminary inter-hospital transfer practice protocol for critically ill children was formulated. A panel of 31 experts from 12 hospitals in China participated in 2 rounds of expert correspondence between May and July 2022,facilitating meticulous revision of the protocol entries. Results The response rate for both rounds of questionnaires was 100%,and the expert authority coefficients ranged from 0.926 to 0.931. In the second round of consultation,the coefficient of variation for the importance score of each entry ranged from 0.036 to 0.226,and the Kendall’s W was determined to be 0.201(P<0.001). Additionally,the coefficient of variation for the feasibility score of each entry fell within the range of 0.070 to 0.314,with Kendall’s W of 0.124(P<0.001). Ultimately,the final interhospital transfer protocol for critically ill children comprised 8 level Ⅰ entries,16 level Ⅱ entries,and 75 level Ⅲ entries. Conclusion The interhospital transfer protocol constructed in this study is grounded in scientific evidence and exhibits practical feasibility. It serves as a valuable reference for organizing and implementing interhospital transfers of critically ill children.

    Investigation of sleep status of preterm infants discharged from NICU

    Objective To investigate the status and influencing factors of sleep in preterm infants at 1 month corrected age. Methods 130 preterm infants admitted to the NICU of 3 tertiary hospitals in Hubei Province were recruited as participants during May 2021 and March 2022. Sleep assessment of preterm infants was conducted at 1 month corrected age. Infants’ sleep was assessed using the Brief Infant Sleep Questionnaire(BISQ). Multivariate linear regression model was used to analyze the influencing factors of sleep status(sleep latency,night awakenings,nocturnal sleep duration,daytime sleep duration,24 h sleep duration) of preterm infants. Results A total of 124 preterm infants completed the follow-up at 1 month corrected age. Multiple linear regression analysis showed that feeding patterns and sleep initiation patterns affected the sleep latency;gestational age,mechanical ventilation duration,maternal education level,and sleep initiation patterns were influencing factors of night awakenings;nocturnal sleep duration was influenced by mechanical ventilation duration and feeding patterns;daytime sleep duration and 24 h sleep duration were influenced by feeding patterns and maternal educational level. Conclusion The preterm infant sleep is influenced by gestational age,duration of mechanical ventilation,feeding patterns,maternal education level,and sleep initiation patterns. Neonatology staff should focus on the preterm infants discharged from NICU and develop targeted intervention plans based on the determined influencing factors to improve the sleep status of preterm infants.

    A qualitative study on the experience and needs of fathers of NICU premature infants in preparation for discharge

    Objective To gain an in-depth understanding of the real experiences,feelings and needs of fathers of premature infants in NICU regarding hospital discharge preparation,and to provide a basis for medical institutions on the development of personalized health education plans and continuous nursing care plans for discharged premature infants. Methods By the purposive sampling method,15 fathers of premature infants in NICU of a tertiary obstetrics and gynecology hospital in Zhejiang Province from April 2022 to July 2022 on the day of discharge and 6 weeks after discharge were selected for semi-structured interviews,and the interview data were analyzed using the Colazzi’s phenomenological 7-step analysis. Results The experiences and needs of NICU premature fathers in discharge preparation are divided into 4 themes. 1. The father’s problems coexist with the growth of premature infants,such as expecting to see premature infants on the day of discharge and having confidence in discharge care,experiencing various negative emotions after discharge,and increasing empathy and sense of responsibility after discharge. 2. Inadequate ability to care for oneself,including a lack of access to professional care knowledge,a need to improve nursing skills,and an overreliance on premature mothers. 3. Social role conflict in the family. 4. Longing for assistance,including professional medical advice and more family support. Conclusion Fathers of premature infants in the NICU coexist with negative emotions and growth experiences as they prepare for discharge,with various needs such as reducing role conflict,enhancing independent care ability,and obtaining psychological and multi-party support. To improve their discharge preparation level,nursing staff should develop a personalized health education plan as well as a continuous nursing plan.

    To summarize the nursing experience of an adolescent with early repolarization syndrome who had implanted a subcutaneous implantable cardioverter defibrillator(ICD) and experiences frequent ventricular fibrillation. The main nursing care were as follows:recognizing a ventricular fibrillation attack and emergency care;risk prevention and handling in transportation;prevention of iatrogenic infection and strengthening protective isolation;promoting the recovery of brain function;personalized health education;pre-hospital first aid skills training for families. After careful treatment and nursing,the patient did not recur arrhythmia,and heart function had improved significantly,and the pulmonary inflammation disappeared. No medical-acquired infections occurred during hospitalization. The patient’s motor ability and language expression ability recovered to the baseline,and was discharged on the 20th day of admission. A month after discharge,the patient recovered well and could study and live normally.

    To summarize the nursing experience of a child with pulmonary rejection after hematopoietic stem cell transplantation who underwent double lung transplantation and was complicated with bronchiolitis obliterans after lung transplantation and underwent second lung transplantation. The key points of nursing included:the implementation of step-wise lateral position and prone position alternate non-invasive mechanical ventilation to improve lung function;sequential airway clearance was used to control pulmonary infection;precise volume management to prevent pulmonary edema;to strengthen the observation and medication management,prevent hospital infection and rejection;to provide goal-oriented personalized nutritional support to improve nutritional status;psychological nursing measures were implemented to improve the treatment confidence and compliance of children;health education and follow-up management should be strengthened to improve the long-term quality of life. After active treatment and careful nursing,the child recovered and was discharged 26 days after surgery.

    Application of comprehensive cognitive reinforcement intervention in patients with spinal cord injury

    Objective To use the cognitive reinforcement comprehensive intervention program constructed by our team to intervene in patients with spinal cord injury and evaluate its clinical application effect. Methods A non-randomized trial design was adopted to select 97 patients with spinal cord injury from November 2021 to September 2022. Forty-four patients from March to September 2022 in a Grade A hospital in Hefei City were included in the experimental group,and 53 patients from November 2021 to February 2022 were included in the control group. The cognitive reinforcement comprehensive intervention program was used to intervene in the experimental group,and the conventional rehabilitation nursing was used to intervene in the control group.The intervention lasted for 12 weeks in both groups. The Changsha Montreal Scale,Social Support Rating Scale,Rehabilitation Exercise Self-efficacy Scale,Spinal Cord Injury Independence Rating Scale and Hamilton Anxiety Scale were used to measure the two groups before intervention,1 month after intervention and 3 months after intervention. Results 40 cases in the experimental group and 48 cases in the control group completed the study.Repeated measurement ANOVA showed that the temporal,interactive and intergroup effects of cognitive function scores and anxiety scores were statistically significant(P<0.05). The time effect and interaction effect of the subjective support dimension score,coping self-efficacy dimension score of the two groups were statistically significant(P<0.05). One month after the intervention,the cognitive function scores of test group were higher than before intervention and control group,and the anxiety scores were lower than before intervention and control group(P<0.05). Three months after the intervention,the scores of cognitive function,subjective support dimension and coping self-efficacy dimension of experimental group were higher than those before intervention and control group,and the scores of anxiety level were lower than those before intervention and control group(P<0.05). Conclusion Comprehensive intervention of cognitive reinforcement can improve the cognitive function of patients with spinal cord injury,delay the process of cognitive impairment,enhance self-confidence,relieve anxiety,and promote physical and mental rehabilitation of patients.

    Objective To explore the application of cluster nursing mode in perioperative period of elderly patients with hip fracture. Methods A retrospective cohort study design was used.Through convenience sampling method,50 elderly patients admitted to the hospital for hip fractures from January 2018 to December 2019 were selected as the control group. Meanwhile,47 elderly patients admitted to hospital for hip fractures from January 2020 to December 2021 were selected as the experimental group. The experimental group received cluster nursing,while the control group received routine nursing measures. The outcomes compared between the 2 groups included the effective rate of pain intervention,functional exercise compliance,and the incidence of complications. Results The effective rate of pain intervention(χ2=5.922,P=0.015) and functional exercise compliance(χ2=9.685,P=0.008)in the experimental group were higher than those in the control group. The number of perioperative complications(χ2=7.600,P=0.006) in the experimental group were lower than those in the control group. Conclusion The application of cluster nursing mode in the perioperative period of elderly patients with hip fractures can effectively control pain,improve functional exercise compliance,and reduce complications.

    Objective To gain insights into the supportive care needs of liver transplant recipients,in order to provide references for the formulation of effective and targeted intervention strategies. Methods Phenomenological research method was employed to conduct semi-structural in-depth interviews with 16 liver transplant recipients in the liver transplant center of a tertiary hospital in Shanghai from November 2021 to April 2022. Colaizzi 7-step analysis method and Nvivo software were used to analyze and extract the themes. Results 5 themes were extracted,including symptom management needs,longing for the perception and avoidance of recurrence risk,disease information management needs,emotional and psychological support and the return to social roles. Conclusion The physical distress and burden of liver transplantation recipients coexist. Although the initiative to seek medical treatment is high,the perception level of disease recurrence risk is not high. In order to meet the needs of liver transplant recipients and improve their quality of life,it is necessary to strengthen the health education of disease symptom management and related risk factors,pay attention to psychological counseling and strengthen social support,and provide targeted treatment care.

    Objective To construct a structural equation model to analyze the effect of various factors on the quality of life in stroke patients. Methods In this cross-sectional study, a total of 657 stroke patients from 2 tertiary A hospitals in Shanghai were enrolled as investigation subjects via convenience sampling methodology. General information questionnaire,Sense of Coherence Scale-13,Perceived Social Support Scale,Medical Coping Modes Questionnaire,National Institutes of Health Stroke Scale,Self-Esteem Scale and Stroke-Specific Quality of Life were utilized as the investigating tools;Mplus 7.4 was used to construct a structural equation model. Results The average score of quality of life in stroke patients was(178.30±30.30);neurological status,self-esteem were negatively cor-related with quality of life(P<0.05);sense of coherence,social support,coping style were positively correlated with quality of life(P<0.05);sense of coherence,taking a confrontational approach for coping,yielding and neurological status had a direct impact on quality of life(path coefficients were 0.471,0.169、0.135,-0.089,P<0.05);social sup-port,self-esteem,sense of coherence and neurological status had an indirect impact on quality of life(path coefficients were 0.232,0.108,0.058,-0.058,P<0.001). Conclusion The quality of life in stroke patients is influenced by multi- ple factors interactively,and sense of coherence is an important factor affecting the quality of life;adopting a confron-tational approach as coping styles,good social support and high self-esteem can improve the level of quality of life.

    Objective To describe the status of the post-discharge coping difficulties in women with hypertensive disorders of pregnancy(HDP),and to explore its influencing factors. Methods A total of 280 women with HDP from a tertiary A maternity hospital in Nanjing City were investigated with the general information questionnaire,the post-discharge coping difficulty scale-new mother form,the readiness for hospital discharge study-new mother form,the family APGAR index. The factors influencing post-discharge coping difficulties in women with HDP were analyzed using univariate analysis and multiple linear regression. Results 238 valid questionnaires were collected,with a response rate of 85.0%. The score of the post-discharge coping difficulties in women with HDP was 5.07±1.69,which is at a middle level. The results of multiple linear regression analysis showed that parity,primary caregiver during the postpartum period,HDP type,readiness for hospital discharge,family APGAR index were influencing factors of post-discharge coping difficulties in women with HDP(P<0.05). Conclusion Post-discharge coping difficulties in women with HDP remains to be further improved,and it is affected by many factors. Medical staff should pay more attention to post-discharge coping difficulties of such parturients,formulate targeted continuation care programs according to relevant influencing factors,comprehensively improve their coping ability,and reduce their coping difficulties after discharge.

    Objective This study aims to explore the formation of the dilemma of “giving care” for disabled elderly people in nursing homes from the perspective of active health. Methods Purposive sampling was used to select caregivers and disabled elderly people from a nursing home in Chongqing as research subjects from July 2022 to December 2022. One-on-one in-depth interviews were conducted and Colaizzi’s 7-step thematic analysis method was employed to collect and analyze the interview data. Results 4 themes were identified:①the dominance of the “giving” care concept,including the cultural thoughts of filial piety,passive acceptance of care characteristics and consumer psychology regarding paid services;②insufficient “participation” care ability,including lack of knowledge regarding active health and a weakening of skills to promote participation;③the hindrance to “transformation” of care models,including objective limitations in terms of human resources and delays in adapting aging environments;and ④the decline in “utilization” of internal abilities,including excessive avoidance of potential risks,a heavy workload of care and poor quality of individual care. Conclusion The formation of the dilemma of “giving care” for disabled elderly people in nursing homes is affected by multiple factors such as social background,service system,supply resources,and management mode. Transforming disabled elderly individuals from a state of “passive giving care” to “active participation in their health” is an important measure to realize the concept of positive aging and healthy aging.

    Latent class analysis of psychological distress among older adults with multiple chronic conditions in communities

    Objective To explore latent class of psychological distress among the elderly with multiple chronic conditions in the community and to analyze differences in demographic and alexithymia characteristics across class. Methods A total of 685 urban and rural elderly with chronic disease co-morbidities in 6 prefecture-level cities in Jiangsu Province were selected for the study using the General Information Questionnaire,the Depression Anxiety and Stress Scales-21,and the Toronto Alexithymia scale from November 2022 to March 2023 using a convenience sampling method. Results A total of 662 older adults with multiple chronic conditions were finally investigated with a total psychological distress score 30.00(16.00,52.00). Psychological distress manifested itself in 4 different potential categories named “stress-depressive”(19.9%),“low distress”(26.0%),“stress-anxiety”(23.4%),and “overall high distress”(30.7%). The comparison of the different psychological distress categories with respect to age group,marital status,residential status,financial stress of illness,acute events within 1 year,and self-perceived health status showed statistically significant differences(P<0.05). The comparison of alexithymia scores for the 4 potential categories showed statistically significant differences(F=136.544,P<0.001). Conclusion Psychological distress in older adults with multiple chronic diseases has distinct categorical features,and alexithymia varies across different categories of psychological distress. Healthcare professionals can adopt targeted interventions according to different categories of chronic disease co-morbidities in order to reduce their psychological distress,and at the same time they should focus on older adults with the alexithymia of “stress-depressive” and “overall high distress”.

    Objective To systematically evaluate the medication experience of patients with chronic multimorbidities and to provide a reference for developing precise intervention programs to improve the medication experience of patients with chronic multimorbidities. Methods PubMed,Cochrane Library,Web of Science,Embase,CINAHL,PsycINFO,CNKI,Wanfang database,VIP database,and Chinese biomedical literature database were searched from the year of database establishment to October 2022,and qualitative studies on medication experiences of patients with chronic multimorbidities were retrieved. The Joanna Briggs Institute Critical Appraisal Tool for qualitative research(2016) in Australia was used to evaluate quality of the studies. A meta-synthesis method was used to integrate the results. Results A total of 11 articles were included,and 56 research results were extracted and integrated into 12 new categories,which were summarized into 4 integrated results:poor sensory experience of drugs,negative emotional and related experiences,reflective and behavioral experiences,and multi-dimensional debugging to improve medication experience. Conclusion The medication experience of patients with chronic multimorbidities should receive extensive attention from society and medical workers,and future research should be conducted to improve the level of patients’ medication experience in 5 dimensions,including sensory experience,emotional experience,reflective experience,behavioral experience and associative experience,so as to provide a basis for further improving the medication experience of patients with chronic multimorbidities in China.

    Objective To systematically evaluate the qualitative studies on the care experience of caregivers for primary malignant bone tumors patients,in order to provide references for the construction of bone tumor support care system. Methods The Cochrane Library,PubMed,Embase,CNKI,Wanfang Database,VIP database,and China Biomedical Literature Database were searched by computer to collect qualitative studies on the care experience of caregivers of malignant bone tumors patients from the establishment of the databases to November 2022. The quality of the literature was evaluated using the Joanna Briggs Institute(JBI) Quality Evaluation Criteria for Quality Research in Evidence-Based Health Care Centers(2016),and the results were integrated by a pooled integration approach. Results A total of 12 studies were included;48 themes were extracted and summarized into 9 categories,which were combined into 3 integrated results. Integration result 1 is obvious physical and mental disturbance. Integration result 2 is multiple role maladaptation. Integration result 3 is positive growth after adjustment. Conclusion Caregivers of patients with malignant bone tumors have serious physical and mental burden and are eager for multiple support. It is suggested that medical staff pay attention to the multi-dimensional needs of patients,formulate personalized support strategies,help caregivers adapt and transform their roles,and promote the post-traumatic growth of caregivers.

    Effect of solution-focused brief therapy on anxiety and depression in patients with HIV/AIDS:a meta-analysis

    Objective Meta-analysis was used to evaluate the effect of the solution-focused brief therapy on improving the anxiety and depression status of patients with HIV/AIDS. Methods Computer search of PubMed,Embase,Web of Science,Cochrane Library,CINAHL,PsycINFO,Chinese Biomedical Literature Database,China National Knowledge Infrastructure,Wanfang Database,CQVIP were conducted,and the search time frame was from the establishment of databases until April 9,2023. There were 2 investigators who independently screened the literature according to inclusion and exclusion criteria,extracted data and performed quality evaluation,and performed Meta-analysis using RevMan 5.4 software. Results A total of 11 publications were included,including 9 randomized controlled trials and 2 quasi-experimental studies,with a total of 1 219 patients with HIV/AIDS. Meta-analysis results showed that solution-focused brief therapy reduced anxiety scores(SMD=-1.89;95%CI:-2.79~-0.99,P<0.001),depression scores(SMD=-2.45;95%CI:-3.51~-1.39,P<0.001). Subgroup analysis showed that improved anxiety(SMD=-4.16;95%CI:-7.97~-0.35,P<0.001),depression(SMD=-5.69;95%CI:-11.20~-0.19,P<0.001)in pregnant HIV/AIDS patients was significantly better than that in ordinary patients. Conclusion Solution-focused brief therapy is effective in improving anxiety and depression levels in patients with HIV/AIDS,and the application of this model in pregnant patients with HIV/AIDS has a more significant improvement effect,but high-quality,multicenter,large-sample clinical trial studies are needed to further confirm this conclusion in the future.

    Hypertension has a high global prevalence rate,which is the main preventable risk factor for cardiovascular disease. Home blood pressure monitoring plays an important role in diagnosis,adjustment of treatment strategies and long-term follow-up of hypertension. However,there are still enormous challenges for the popularization and standardized application of home blood pressure monitoring. Therefore,this review article describes indication,standardized implementation protocol,current status and influencing factors of implementation and management forms of home blood pressure monitoring. It can provide a reference for promoting the implementation of practical guidelines for home blood pressure monitoring and optimizing the application of home blood pressure monitoring.

    Perinatal loss is one of the most serious childbirth trauma,which seriously affects maternal physical and mental health. At present, the management strategy of maternal lactation can be divided into two aspects: milk return and milk donation. Breast milk donation may build new values of perinatal loss,which can promote maternal physical and mental recovery. This article reviewed the conception,status,motivations,facilitators,barriers and interventions of lactation management in women following perinatal loss,so as to provide bases for improving the awareness of obstetric nursing staff,quality of obstetric care and future research on lactation management in women following perinatal loss in China.

    Incontinence-associated dermatitis is one of the common complications in critically ill patients. This paper reviews the research progress of risk prediction models for incontinence-associated dermatitis in critically ill patients,introduces and compares the characteristics and application effects of different risk prediction models. The purpose is to provide ideas for constructing a localized risk prediction model and provide evidence for medical staff to identify risk factors of incontinence-associated dermatitis at an early stage and take preventive measures.
