    Expert consensus on packaging techniques of reusable medical instruments

    Objective To standardize the packaging techniques of reusable medical instruments in central sterile supply departments. Methods From October 2021 to August 2022,the Professional Committee of the Central Sterile Supply Department of Chinese Nursing Association addresses the key problems of reusable medical instruments packaging. On the basis of clinical practice,literature analysis and the operation experience of the national central sterile supply department experts,we made the initial version of Expert Consensus on Packing Techniques of Reusable Medical Instruments(hereinafter referred to as the Consensus). Experts were invited to complete 2 rounds of Delphi expert consultations and 3 rounds of expert meetings. Each item was adjusted,revised and improved,and the final version of the Consensus was formulated. Results The positive coefficients of experts in 2 rounds of Delphi expert consultations were all 100%,and the degrees of expert authority were 0.957 and 0.973,respectively. The average importance value of each item was higher than 4.00;the variation coefficient of each item was less than 0.25. The Kendall’W were 0.181 and 0.338,respectively(all P<0.001). Finally,an agreement was reached on the terms and definitions,basic principles,personnel requirements,environmental requirements,packaging equipment requirements,packaging materials and sterilization monitoring material requirements,packaging auxiliary tools requirements,pre-packaging evaluation,packaging operation and packaging quality confirmation of the reusable medical devices packaging techniques. Conclusion The Consensus covers all aspects of the reusable medical instruments packaging techniques,which is highly practical and scientific,and it also helps to guide the sterile supply professionals to standardize the implementation of packaging techniques,improve the quality of aseptic supplies,and ensure the quality of medical treatment and the life safety of patients.

    Investigation of the functional testing of reusable medical devices in Central Sterile Supply Departments of 281 hospitals in China

    Objective To investigate the current status of functional testing of reusable medical devices of hospital central sterile supply departments(CSSDs) in China’s 29 provinces(autonomous regions and municipalities),and to analyze the main problems in the practice of functional testing of reusable medical devices,in order to better guide its clinical practice. Methods Using the convenience sampling method,the investigation was conducted by the status quo questionnaire of device function testing. 291 hospitals in 29 provinces(autonomous regions and municipalities) were investigated through the support of Professional Committee of CSSDs from September 13 to 21,2022. The questionnaire included 4 aspects:testing methods of reuse medical device function,required materials or equipment of device function testing,classification of device function,and execution,process and staff training of device function testing. The related influencing factors of systematic device function test was analyzed. Results A total of 291 questionnaires were collected and 281 were valid,the recovery rate of which was 96.56%.The functional testing methods in some CSSDs were not comprehensive.27(9.61%) CSSDs provided all the materials or equipment for functional testing and 204(72.60%) CSSDs partially provided those. 176(73.33%) CSSDs tended to classify device functions as “intact,surface damage,in need of repair,and in need of replacement”. 41(14.59%) CSSDs graded device function. Only 144 CSSDs(51.25%) regularly conducted system function testing. The influencing factors were regionalization of CSSDs(P=0.044),establishment of a standard operation process(P=0.035),provision (P=0.021) or provision(P=0.003) of the materials or equipment required for functional testing of the device,and training(P=0.007). Conclusion The application of functional detection of reuse medical devices in CSSD is still in the initial stage.The device function detection method of the CSSD staff needs to be further improved.The overall situation of equipment and materials required for device function testing are acceptable,but further specification is required.The device function classification and evaluation mechanism have been preliminarily established and needs to be further promoted. CSSD managers can further implement device functional testing for staff from establishing standard operation process,actively providing materials or equipment required for functional testing,and strengthening training.

    A study on the current situation of precision instrument drying technology in 262 hospital sterile supply centers

    Objective To understand the current status of precision instrument drying technology in hospital sterile supply centers and to provide references for the development of a standardized drying process for precision instruments. Methods From June to August 2022,the sterile supply centers of 262 hospitals of different levels in 27 provinces (autonomous regions and municipalities) were surveyed by convenient sampling method to conduct questionnaire surveys on issues related to the configuration of precision instrument drying equipment,the current status of precision instrument drying and the evaluation method of drying effect,and the data was collected and analyzed. Results 272 questionnaires were distributed and 262 valid questionnaires were collected. The precision instruments centrally processed by the hospital sterile supply center include rigid endoscopy,flexible endoscope,microscopic instruments,power system equipments of various specialties,transplant surgical instruments and robotic surgical instruments. The configuration of drying instruments was 258 hospitals (98.47%) equipped with pressure air guns,215(82.06%) equipped with medical hot air drying cabinets,and 143 (54.58%) equipped with medical low temperature vacuum drying cabinets;146(55.73%) received training from precision instrument manufacturers;140 hospitals(53.44%) established precision instrument drying processes;230 (87.79%) used 2 or more kinds of drying equipment and supplies in combination for drying methods;248 (94.66%) used visual inspection to evaluate the drying effect of precision instruments. Conclusion There are many types of centralized treatment of precision instruments,and most sterile supply centers adopt a variety of drying methods combined with drying technologies,but the configuration proportion of medical low-temperature vacuum drying cabinet is not high,the management and use of pressure air guns is not standardized;the training and drying process is insufficient;the evaluation methods of drying effect are not uniform. It is recommended that hospital managers pay more attention to the drying of precision instruments,formulate drying operation procedures and drying effect evaluation methods with different precision instruments specifications,extend the service life of precision instruments,and ensure the quality of sterile goods.

    Objective To develop and test the reliability and validity of the Questionnaire on Prevention and Control Competence of Nursing Staff for Infectious Disease Emergencies. Methods Guided by translated Infection Prevention Society Competency Framework,a questionnaire draft was constructed through the literature review,Delphi expert consultation,and a pilot survey. By convenience sampling method,303 nursing staff were investigated for item analysis,exploratory factor analysis,and reliability analysis of the pre-test questionnaire from a tertiary A hospital in Hubei in June 2022;another 561 nurses were selected for confirmatory factor analysis from 3 tertiary A hospitals in Hubei in July 2022. Results The formal questionnaire included 35 items in 5 dimensions(clinical practice,learning and education,leadership and management,quality improvement and research,and professional ethics). Exploratory factor analysis extracted 5 common factors,and the cumulative variance contribution rate was 74.587%. Confirmatory factor analysis showed that the model fit well and the questionnaire’s factor structure was stable. The scale-level content validity index of the questionnaire and the item-level content validity index of each item were 1.00. The overall Cronbach’s α coefficient was 0.975. The overall split-half reliability was 0.874. The overall retest reliability was 0.877. Conclusion The Questionnaire on Prevention and Control Competence of Nursing Staff for Infectious Disease Emergencies has good reliability and validity,which could be used to assess nurses’ competence in infectious disease emergencies prevention and control.

    Objective To summarize the best evidence for the prevention and control of nosocomial infection in patients with acute respiratory infectious diseases during high-flow nasal cannula oxygen therapy. Methods A literature retrieval strategy was formulated to search BMJ best practice,Up To Date,National Guideline Clearinghouse,National Institute for Health and Care Excellence,Scottish Intercollegiate Guidelines Network,World Health Organization website,American Association for Respiratory Care website,European Respiratory Society website,Yimaitong Database,Cochrane Library,PubMed,CINAHL,Web of Science,Wanfang database,CNKI. The search time limit is from the establishment of the database to August 2022. The literature related to this study were selected according to the inclusion criteria,and evidence was extracted after literature quality evaluation. Results A total of 11 pieces of the literature were included,including 9 expert consensuses,1 systematic review and 1 original study. A total of 26 pieces of evidence were extracted,including 6 categories as personnel management,environmental management,medical staff protection,medical care operation,terminal disinfection,and emergency management. Conclusion This study has guiding significance for the prevention and control of nosocomial infection in patients with acute respiratory infectious diseases during high-flow nasal cannula oxygen therapy. The application of evidence should be dialectically adopted and implemented in combination with the characteristics of pathogens and the strength of transmission.

    Objective To explore the impact of Forebrain verbal and auditory feedback cognitive training on cognitive function and daily living ability of patients with brain trauma. Methods From October 2021 to May 2022,80 patients with cognitive impairment after traumatic brain injury who met the inclusion and exclusion criteria were selected from the neurosurgery department of a tertiary hospital in Tangshan City,and they were randomly divided into a treatment group and a matched group according to the "RAND-function". 40 patients in the treatment group received routine nursing on the first day of enrollment,and Forbrain training was added on the second day while receiving rehabilitation nursing. The intervention time was for 2 weeks. At the same time,40 patients in the matched group received neurosurgery rehabilitation nursing measures for 2 weeks. The researchers collected the patients’ general and disease data on the first day of admission. MoCA and BI are used to complete the evaluation of the patient’s cognitive function and daily life ability,and the same scales were used for re-evaluation after 2 weeks of intervention. Results The baseline indexes of the 2 groups were comparable(P>0.05). After 2 weeks,regarding the cognitive function,the researchers found that the cognitive score and difference score of the treatment group were significantly higher than those of the matched group(P<0.001);the treatment group had higher differences in all 5 cognitive dimensions than the matched group,including visuospatial function,attention,language,abstraction and memory(P<0.05). Regarding activity of daily living,the researchers found that the BI score and difference score of the treatment group were significantly higher than those of the matched group(P<0.001). Conclusion Forbrain training can improve cognitive function and daily living ability of patients with brain injury,and promote the recovery of attention,memory and language functions.

    Comparison of 2 questionnaires in evaluating the quality of life of patients with chronic wounds

    Objective To compare the reliability and validity of the Short Form-36 Health Survey(SF-36) and Euro Quality of Life Five-Dimensional Questionnaire(EQ-5D) in assessing the quality of life of patients with chronic wounds and the similarities and differences of their assessment on different dimensions. Methods Using convenience sampling,chronic wound patients treated in the wound center of a tertiary A hospital in Beijing from July 2019 to February 2022 were selected. The demographic data and disease related data were collected by a self-designed questionnaire. The patients’ quality of life was investigated by SF-36 and EQ-5D. Current situation of chronic wound patients’ quality of life were explored. By comparison of the 2 questionnaires,internal consistency,score correlation and score differences of patients grouped by the wound characteristics were compared and analyzed. Results A total of 144 patients were included in this study. The Physical Component Summary of SF-36 was 224.52±94.36,and the Mental Component Summary was 282.88±90.77. EQ-5D utility score was 0.73±0.23,and the score of EQ-5D Visual Analogue Scale was 72.90±20.60. The Cronbach’s α of SF-36 subscales and EQ-5D were 0.694~0.957 and 0.757,respectively,and the scores were significantly correlated(P<0.001). Significant differences in SF-36 and EQ-5D scores were found among patients with different self-reporting wound changes(P<0.05). There were also statistically significant differences in EQ-5D scores among patients with different wound types,wound exudation volume and pain grades(P<0.05). Conclusion The quality of life in patients with chronic wounds is low,and the physical health is more significantly impaired. Both SF-36 and EQ-5D have good reliability and validity in assessing the quality of life of patients with chronic wounds. SF-36 is more comprehensive in assessing both physical and mental dimensions,and EQ-5D focuses mainly on physical dimension. Besides,EQ-5D is shorter and thus time-saving,and could better reflect different wound characteristics. Researchers are recommended to make the selection according to their research purposes and specific needs.

    Establishment and application of an evaluation index system for health literacy among gestational diabetes mellitus patients

    Objective We aimed to establish an evaluation index system for health literacy among gestational diabetes mellitus(GDM) patients. As a theoretical basis,this system can be applied by the clinical nurses to carry out targeted health guidance among GDM patients and to evaluate health literacy level. Methods Initial indexes were generated through the literature review and semi-structured interviews. 2 rounds of expert consultations and analytic hierarchy process were conducted to establish the evaluation index for health literacy among GDM patients. The convenience sampling method was used to select 15 GDM patients and 15 obstetric medical staff members from a tertiary A hospital in Qingdao as the research subjects in March 2022,and the application effect of the constructed evaluation index system was preliminarily verified by semi-structured interviews. Results The response rates of the 2 rounds of expert consultations are 100% and 94.74%. The expert authority coefficients are 0.924 and 0.942,respectively. The Kendall harmony coefficients of the 2 rounds of expert consultations are 0.123~0.154 and 0.312~0.327(P<0.01). As a result,we built up an evaluation index system of health literacy which comprised 7 first-level indexes,49 second-level indexes for GDM patients. 86.7% of GDM patients thought that the content of index system was clear,easy to understand,and instructive for daily self-management. 100% of obstetric medical staff were satisfied with the index system,and thought that the content was comprehensive,professional,with applicability and necessity of application. Conclusion The evaluation index system for health literacy among GDM patients is reasonable,scientific and reliable. The index content is specialized and specific,in line with the physical and psychological characteristics of GDM patients.

    The status and influencing factors of demoralization syndrome in patients with maintenance hemodialysis disease

    Objective To investigate the status of demoralization syndrome in patients with maintenance hemodialysis disease(MHD) and to analyze the influencing factors. Methods Convenience sampling method was used to select MHD patients from 16 hospitals in Xiangyang,Hubei Province from October 2021 to April 2022. The general information questionnaire,Demoralization Scale-Ⅱ,Sense of Coherence-13,Mishel Uncertainty in Illness Scale,and Medical Coping Modes Questionnaire were used for investigation. Multiple linear regression was used to analyze the influencing factors of demoralization syndrome in patients with maintenance hemodialysis. Results A total of 1094 MHD patients were included in this study,and the DS-Ⅱ score was(12.72±7.10),of which 57.04% were moderately demoralization and 12.43% were severe demoralization. Multiple linear regression analysis revealed that medical payment method,occupational status,sleep quality,quantity of long-term oral medication,sense of coherence,uncertainty in illness,and medical response were the influencing factors of demoralization syndrome(P<0.05). Conclusion MHD patients have a high incidence of demoralization syndrome,and their psychological distress state needs to be improved. Medical staff should regularly assess the level of demoralization syndrome in patients,and formulate targeted intervention measures according to relevant influencing factors,so as to reduce the level of demoralization syndrome and improve the prognosis and psychological status of patients.

    Analysis of rehabilitation exercise compliance and its influencing factors in young and middle-aged patients with hemorrhagic stroke

    Objective To investigate and analyze the current situation of rehabilitation exercise compliance and its influencing factors in young and middle-aged patients with hemorrhagic stroke. Methods By convenient sampling method,young and middle-aged patients with hemorrhagic stroke were selected from 3 hospitals in central,eastern and northern China from July 2021 to January 2022. According to the Health Action Process Approach(HAPA),a cross-sectional survey was conducted with the general information questionnaire,Exercise Adherence Questionnaire (EAQ),the Self-Efficacy for Exercise(SEE) Scale,exercise plan scale,stroke health belief scale and self-care ability scale. Results The total score of exercise adherence of young and middle-aged patients with stroke was 43.46±9.58,and the average compliance rate was 77.60%. Multivariate regression analysis showed that the total scores of exercise self-efficacy,exercise plan,perceived susceptibility,perceived severity,perceived benefits,perceived disorders,health motivation and self-efficacy were the influencing factors of exercise compliance in young and middle-aged stroke patients(P<0.05). Conclusion Clinical medical staff should accurately evaluate the rehabilitation exercise stage of patients according to HAPA model,formulate and implement stroke rehabilitation exercise strategies according to patients’ actual conditions and various possible influencing factors,so as to improve patients’ exercise compliance,their functional status and further improve their quality of life.

    Establishment and validation of a risk prediction model for oral mucositis in children undergoing hematopoietic stem cell transplantation

    Objective To explore the influencing factors of oral mucositis in children undergoing hematopoietic stem cell transplantation(HSCT),and to develop a nomogram model to predict these risks. Methods A total of 286 children who underwent allogeneic HSCT were recruited between January 2019 and June 2022 in a tertiary A children’s hospital in Chongqing. Multivariate logistic regression was conducted to identify independent risk factors of oral mucositis in children undergoing HSCT. A nomogram was developed by R 3.6.1 software and validated to predict the risk of oral mucositis in children undergoing HSCT. Results Logistic regression analysis showed that body mass index<16,oral pH≤6.5,oral cleanliness score(2~3 points),myeloablative or reduced intensity in conditioning regimen,standard methotrexate dose GVHD prophylaxis and neutrophil count<1.5×109 /L were inde-pendent risk factors of oral mucositis after HSCT in children(P<0.05). The model passed Hosmer-Lemeshow goodness of fit test( χ2=0.969,P=0.998)and the area under the ROC curve of this model was 0.881,with the optimal critical value of 0.7,accuracy of 80.36%,sensitivity of 83.59% and the specificity of 74.88%. The concordance index for predicting oral mucositis was 0.881,and the adjusted concordance index was 0.862. Conclusion The risk prediction model constructed in this study has good discrimination and accuracy. It can be used to predict the risk of oral mucositis in children undergoing HSCT,providing the reference for management and preventative intervention strategies for high-risk children.

    This article summarized the nursing care experience of a pediatric patient with kawasaki disease after off-pump coronary artery bypass grafting. The key points of nursing care include:①the emergency treatment plan of chest pain was formulated before operation to prevent and identify myocardial infarction as soon as possible;early respiratory function training was carried out to improve cardiopulmonary function;risk management was improved to prevent rupture of coronary artery aneurysm. ②The circulatory system was closely monitored to ensure stable hemodynamic performance during operation;fluid balance was maintained to prevent renal function injury.③Posto-perative pain and irritation were managed to prevent atrial fibrillation;the coagulation function was monitored,and coagulation associated complications,such as epistaxis,should be treated timely;non-pharmacological intervention measures were taken to relieve postoperative delirium;individualized exercise rehabilitation care plan was made to enhance recovery;individualized diet plan was made to improve the nutritional status;structural education was enhanced to make the patient and his parents more cooperative. After careful treatment and nursing care,the patient recovered and was discharged from the hospital at 16 days after the surgery without any complications in one-year follow up.

    To summarize the nursing experience of a pregnant woman with primary mediastinal large B-cell lymphoma complicated with atmospheric tract stenosis. Nursing key points include inventory management to ensure the safety of ECMO two-way transport;setting up an exclusive nursing team to ensure the continuity of nursing process and effective implementation of decision-making;staged coagulation monitoring and management during ECMO and CRRT operation;doctor-nurse cooperation was carried out according to the patient’s condition to predict the related complications and formulate the process plan,to provide a safe guarantee for the prevention of airway obstruction,tumor lysis syndrome,catheter-related bloodstream infection and other related complications. At the same time,patients are provided with refined pregnancy management and individualized psychological intervention. The patient was hospitalized for 9 days. After careful treatment and nursing,the patient’s condition was and improved transferred to the department of obstetrics for further treatment.

    To summarize the nursing care of a child with vascular anastomotic thrombosis after living small bowel transplantation. Essentials for nursing care:early identification of vascular anastomotic thrombosis and emergency care;negative pressure occlusion drainage therapy to timely treatment of incision infection;implementing individualized nutrition support programmes to improve nutritional status;dynamic monitoring of immunosuppressant plasma concentration,and being vigilant for rejection reaction;paying attention to the mental health of children and providing psychological support;developing a follow-up plan for continuity of care. After 29 days of careful treatment and nursing care,the child’s condition improved and was successfully transferred to the general ward,and discharged 67 days after surgery. At 2 months of follow-up after discharge,the intestinal function of the child recovered well,and the child can eat by himself presently.

    Summary of best evidence for management of fatigue in patients with hematopoietic stem cell transplantation

    Objective To retrieve,evaluate and summarize domestic and foreign available evidence on the management of fatigue in patients with hematopoietic stem cell transplant(HSCT),so as to provide references for clinical evidence-based practice. Methods We systematically searched domestic and foreign computer decision support systems,guideline websites,databases,and relevant society websites for evidence on the management of fatigue in patients with HSCT,including guidelines,clinical decision-making,recommended practices,evidence summaries,systematic reviews,and expert consensuses published from the establishment of the databases or websites to February 10,2022. There were 2 researchers who evaluated the quality of the included literature independently,and combined the professional judgement with information extraction. Results A total of 24 pieces of literature were eligible for inclusion,including 4 clinical guidelines,2 evidence summaries,1 expert consensus and 17 systematic reviews. There were 24 pieces of best evidence with 8 dimensions being summarized,namely assessment,exercise management,psychosocial interventions,health education,nutritional therapy,complementary and alternative therapies,rational drug use,and shared decision-making and multidisciplinary collaboration. Conclusion Nursing staff should pay attention to the management of fatigue in patients with HSCT,and manage patients’ fatigue in a standardized,effective,and safe way according to characteristics of clinical environment and patients’ preferences,so as to improve their prognosis and quality of life.

    Effect of computerized cognitive training on patients with mild cognitive impairment: an overview of systematic reviews

    Objective To evaluate the systematic reviews of the effect of computerized cognitive training on patients with mild cognitive impairment. Methods PubMed,Cochrane Library,Embase,Joanna Briggs Institute,Wan Fang Data,CNKI,CBM and VIP were searched by the computer to collect systematic reviews or meta-analysis of the effect of computerized cognitive training intervention on mild cognitive impairment patients. 2 researchers independently conducted 1iterature search and data extraction. AMSTAR 2 scale and GRADE system were used to evaluate the methodological quality and evidence quality of the included literature. We excluded the overlapping randomized controlled trials (RCTs) and performed a re-meta-analysis of the primary RCTs. Results A total of 10 systematic reviews/meta-analyses were included. The methodological quality evaluation by AMSTAR 2 showed that the quality of studies was generally low,with a low study and 10 extremely low ones. The results showed that computerized cognitive training is beneficial to improve patients’ holistic cognition,while the efficacy of executive function,memory function and attention needs to be further confirmed. Conclusion Computerized cognitive training can provide positive influences on cognitive function of patients with mild cognitive impairment. Limited by the quantity and quality of the included studies,a large sample and high-quality studies are still needed to further confirm the reliability of this conclusion.

    Objective To describe the scope and breadth of primary research regarding the implementation of the Safewards Model and its interventions in psychiatric wards. Methods A comprehensive search of the electronic databases,such as PubMed,Embase,CINAHL,PsycINFO,Web of Science,Scopus,Cochrane Library,JBI Evidence-Based Practice,JBI Evidence Synthesis,CNKI,WANFANG,VIP,and SinoMed,from database inception to August 2022,has been conducted to retrieve all the potentially relevant articles. 2 researchers independently screened and extracted information for the retrieved literature,and the evidence was then synthesized and summarized. Results The included 19 articles comprehensively evaluated 14 safewards implementation studies in terms of its effectiveness,intervention fidelity,and feasibility. Conclusion The safewards interventions,if implemented with high fidelity,can effectively reduce the incidence of conflict events in psychiatric wards and the use of restrictive practices,and improve the ward atmosphere. Safewards provide an opportunity and platform by which trauma-informed and recovery-oriented practices in psychiatric setting can be achieved. For the future implementation of safewards,it is necessary to strengthen the training of staff,to help staff develop positive attitudes and beliefs toward safewards,and to seek support from hospital management level.

    Loneliness is common in patients undergoing hematopoietic stem cell transplantation (HSCT) during protective isolation,which could affect their health outcomes. This article reviews the latest studies on loneliness,including concept,theory,influencing factors,assessment tools,and interventions. Currently,there are few studies on loneliness in HSCT patients in protective isolation in China and other countries. We propose to conduct some research to investigate the current status of loneliness,the influencing factors,and the relationship with patients’ mental health problems in the future,which could provide a theoretical basis for developing interventions to reduce the loneliness of the HSCT patients in protective isolation.

    Objective To translate the Inflammatory Bowel Disease Distress Scale(IBD-DS) and verify reliability and validity of the Chinese version of IBD-DS. Methods According to the Brislin translation model,the English IBD-DS was translated into Chinese and the cultural adaptation was conducted. From February to June 2022,178 patients with inflammatory bowel disease from the Department of Gastroenterology,Zhongshan Hospital Xiamen University,were selected by convenience sampling. We verified the reliability and validity of the Chinese version of the scale. Results The Chinese version of the IBD-DS contains 28 items in a dimension. The Cronbach’s α coefficient of the scale was 0.978,and the interclass correlation coefficient was 0.68(95%CI:0.42~0.84). The scale-level content validity index(S-CVI)was 0.964 and the item-level content validity index(I-CVI) ranged from 0.800 to 1.000. The Chinese version of IBD-DS was significantly positively correlated with the Hospital Anxiety and Depression Scale(r=0.712,P<0.001;r=0.601,P<0.001). Conclusion The Chinese version of IBD-DS has good reliability and validity,and it can be used to evaluate the disease distress in patients with inflammatory bowel disease.
