Research progress of discharge readiness assessment tools in family with hospitalized children

Abstract

Abstract:

The evaluation of the family discharge readiness of hospitalized children can timely understand the care needs of the family,help nursing staff to formulate and implement personalized follow-up plans for the children after discharge,and reduce the rate of readmission and unplanned visit of children. This paper reviews the major contents,characteristics,application and limitations of the discharge readiness assessment tools in family with hospitalized children at home and abroad. It aims to provide references for the localization of the development of discharge readiness assessment tools in family with inpatient children and clinical nurses to select appropriate assessment tools. Meanwhile,it provides some useful information for the improvement of the discharge planning for hospitalized children.

摘要：

对住院患儿家庭出院准备度进行评估可及时了解患儿家庭的照护需求,有助于护理人员针对出院后的患儿制订并实施个性化的随访方案,降低患儿再入院率及非计划性就诊率。该文对国内外开发的住院患儿家庭出院准备度评估工具进行综述,包括评估工具的主要内容、特点、应用现状及局限性,以期为国内住院患儿家庭出院准备度评估工具的开发及合理化选用提供参考,为住院患儿出院准备服务方案的完善提供依据。
