Abstract: Objective To analyze the acceptance of disability for initial stroke patients and its influencing factors，and to provide theoretical evidence for psychological intervention and clinical rehabilitation. Methods A total of 150 hospitalized initial stroke patients were investigated with Adaptation of Disability Scale-Revised，Medical Coping Modes Questionnaire，Functional Activities Questionnaire，Self-Rating Depression Scale and Perceived Social Support Scale. Results The mean score for Adaptation of Disability Scale-Revised was（73.56±15.44），and dimensions in descending order were as follows：enlargement，transformation，containment，and subordination. Multiple linear regression analysis showed that confrontation，understanding of disease control，depression，functional activity，and perceived family support were major factors affecting acceptance of disability among initial stroke patients，which explained 51.8% of the total variance. Conclusion Nurses should detect initial stroke patients who are in low level of disability acceptance. Oriented by each dimension and influencing factors，proper psychological interventions should be provided to enhance the level of disability acceptance and psychosomatic rehabilitation.

摘要： 目的 探讨首发脑卒中患者残疾接受度及其影响因素，以期为首发脑卒中致残患者的康复心理干预提供理论依据。 方法 运用残疾接受度量表（修订版）、医学应对问卷、社会活动功能问卷、抑郁自评量表和领悟社会支持量表对150例首发脑卒中住院患者进行问卷调查。 结果 残疾接受度得分为（73.56±15.44），各维度得分排序依次为：扩大维度、转变维度、包容维度、从属维度。 多元线性回归分析结果显示，“面对”的应对方式、对疾病防治的认识、抑郁状态、社会活动功能和家庭内支持是首发脑卒中患者残疾接受度的影响因素，解释总变异的51.8%。 结论 及时发现残疾接受水平较低的首发脑卒中患者，以残疾接受度各维度及影响因素为切入点，实施针对性的康复心理干预，改善残疾接受水平，促进身心康复进程。
