摘要/Abstract

摘要：

目的探究脊髓损伤患者疼痛与疼痛干扰随时间变化的动态特征及影响因素，为临床疼痛管理提供依据。方法通过便利抽样选取2024年10月—2025年11月在广东省某康复医院门诊就诊或住院的83例脊髓损伤患者作为调查对象，运用生态瞬时评估法，对其疼痛和疼痛干扰进行为期7 d、每日4次的持续动态监测，采用多水平模型分析疼痛干扰的影响因素。结果脊髓损伤患者的疼痛与疼痛干扰呈现昼轻夜重的节律性变化，于19：00达到峰值；并在1 d中的不同时间点（08：00、12：00、19：00和睡前）存在瞬时相关性，其中19：00的相关性最强（P<0.001）。多水平模型显示，在个体层面，疼痛可正向影响脊髓损伤患者的疼痛干扰（P<0.001）。结论脊髓损伤患者疼痛及疼痛干扰存在动态变化特征，护理人员应基于患者疼痛波动特点，制订个体化疼痛管理方案，合理优化镇痛措施的实施时机，从而提高整体疼痛管理效果。

Abstract:

Objective To explore the dynamic characteristics and influence factors of pain and pain interference in patients with spinal cord injury over time，and to provide a basis for more effective pain management. Methods 83 patients with spinal cord injury who were treated or hospitalized in a rehabilitation hospital in Guangdong Province from October 2024 to November 2025 were selected as the survey subjects by convenient sampling. Using the ecological momentary assessment method，The pain and pain interference were monitored continuously and dynamically for 7 days，4 times per day. A multilevel model was used to analyze the influence factors of pain interference. Results The pain and pain interference of patients with spinal cord injury showed rhythmic changes with a lighter pattern during the day and aggravation at night，reaching a peak at 19：00. There was a momentary correlation at different time points（08：00，12：00，19：00 and bedtime） in a day，with the strongest correlation at 19：00（P<0.001）. The multilevel model showed that pain could positively affect pain interference in patients with spinal cord injury at the within-person level（P<0.001）. Conclusion There are dynamic changes in pain and pain interference in patients with spinal cord injury. Nursing staff should formulate individualized pain management plans based on the characteristics of patients’ pain fluctuations and reasonably optimize the implementation time of analgesic measures to improve the overall pain management effect.
