脓毒症患者非计划再入院风险因素及干预策略的研究进展及护理启示

摘要：

脓毒症患者出院后常出现新的功能缺陷和认知障碍，非计划再入院率居高不下，严重影响患者的生活质量，并增加医疗负担。该文综述了脓毒症患者非计划再入院原因、影响因素、风险预测模型及干预措施，旨在帮助护理人员识别高风险人群与潜在诱因，为优化护理方案、构建全程个性化照护体系提供参考。

Abstract:

Patients with sepsis often experience new functional deficits and cognitive impairments after discharge，with a persistently high rate of unplanned readmissions. This severely affects their quality of life and increases the medical burden. This article reviews the causes，risk factors，prediction models，and intervention measures of unplanned readmission in sepsis patients. It aims to help nurses identify high-risk groups，and provide references for optimizing nursing plans and constructing a full-cycle personalized care system.
