腹腔镜阑尾切除术后患儿快速康复游戏运动棋盘的设计与应用研究

摘要：

目的设计一款腹腔镜阑尾切除术后患儿快速康复游戏运动棋盘，并评价其应用效果，旨在通过游戏提升患儿术后早期下床活动的依从性，为儿童快速康复临床护理实践提供多样化的促进方法。方法采用便利抽样法，选取2024年1—12月在武汉市某三级甲等儿童医院行腹腔镜阑尾切除术的378例患儿作为研究对象，随机分为试验组（n=185）与对照组（n=193）。试验组采用常规健康宣教结合快速康复游戏运动棋盘，指导患儿参与游戏并完成游戏任务；对照组以常规健康宣教方式督促指导患儿下床活动。比较两组术后早期下床情况、术后疼痛、下床活动满意度、术后并发症发生率及不良事件发生率的差异。结果两组均无样本脱落。试验组术后早期下床情况优于对照组（P<0.001）；术后24、48 h疼痛评分较对照组低（P<0.001）；下床活动满意度高于对照组；术后并发症发生率低于对照组（P=0.015），两组均无跌倒、引流管非计划拔管等不良事件发生。结论快速康复游戏运动棋盘通过游戏任务激发儿童手术后对下床活动的兴趣，可促进阑尾切除术后患儿术后早期下床活动、减轻术后疼痛、减少术后并发症，提升满意度，具有临床推广价值。

Abstract:

Objective To design and evaluate a gamified exercise board for enhancing recovery after laparoscopic appendectomy in pediatric patients，with the aim of improving compliance with early postoperative ambulation through game-based interventions and providing an innovative supportive approach for clinical nursing practice within the enhanced recovery after surgery（ERAS） framework. Methods Using convenience sampling，378 children who underwent laparoscopic appendectomy at a tertiary A children’s hospital in Wuhan between January and December 2024 was recruited and randomly assigned to an experimental group（n=185） and a control group（n=193）. The experimental group received routine health education combined with the use of the gamified exercise board，which involved guided completion of game-oriented tasks. The control group received routine health education and standard encouragement for ambulation. Outcome measures included levels of early ambulation，pain intensity，satisfaction with mobilization，incidence of postoperative complications，and occurrence of adverse events. Results No samples were lost in either group. The experimental group showed significantly better early postoperative ambulation than the control group（P<0.001）. The experimental group showed significantly better postoperative pain outcomes at 24 and 48 hours compared to the control group（P<0.001）. The experimental group had higher satisfaction with ambulation than the control group. The incidence of postoperative complications was lower in the experimental group than it in the control group（P=0.015）. No adverse events such as falls or unintended removal of drainage tubes occurred in either group. Conclusion The rapid gamified exercise board for enhancing recovery stimulates children’s interest in early ambulation after surgery through game tasks，effectively promoting early ambulation，reducing postoperative pain，minimizing postoperative complications，and enhancing satisfaction in children undergoing laparoscopic appendectomy. It demonstrates significant clinical value and is worthy of widespread application.
