1例利妥昔单抗治疗重症天疱疮患者并发皮下血肿的护理

摘要：

总结1例利妥昔单抗治疗重症天疱疮患者合并多处皮下血肿的护理经验。护理重点：运用伤口管理原则控制皮肤感染并促进皮肤伤口愈合,尽早输注利妥昔单抗;做好利妥昔单抗输注全过程护理防范,确保输注安全;多学科会诊评估、分析皮下巨大血肿原因,积极应对,实施综合护理促进血肿消退。患者住院33 d,皮肤伤口完全愈合,血肿缩小,好转出院。出院30 d随访,皮下血肿消退,天疱疮未复发。

Abstract:

This article aims to summarize the nursing experiences of a severe pemphigus patient complicated with multiple subcutaneous hematomas treated with rituximab. The nursing points：to control skin infection and promote skin wound healing according to the principle of wound TIME management;to create opportunities to apply rituximab in time;to take care of the whole process of rituximab infusing to ensure safety;multidisciplinary consultation evaluates and analyzes reasons of huge subcutaneous hematoma forming,and comprehensive nursing promotes hematoma regression. After 33 days of hospitalization,the patient’s skin wound healed completely;the hematoma shrank;the patient got better and was discharged. After 30 days follow-up,subcutaneous hematoma vanished and pemphigus did not recur.
