    Construction and application of a matching exercise training program for patients after lung transplantation

    Objective To construct a postoperative matching exercise training program for lung transplant patients and to evaluate the clinical application effect. Methods A non-concurrent controlled study was used. By convenience sampling method,patients undergoing lung transplantation in a tertiary A hospital in Hangzhou from December 2021 to March 2022 were included in a trial group with 35 cases,while the patients from April to November 2021 were recruited in a control group with 34 cases. Baseline data was consistent between the 2 groups. On the basis of the intervention program in the control group,the trial group received the matching exercise training program which was constructed based on the evidence and Delphi expert consultation method. It was divided into 4 stages according to the BODE index,including primary exercise,advanced exercise,intensive exercise and stable exercise,covering exercise mode,time,and intensity,etc. The control group received a routine pulmonary rehabilitation training program. The 6MWT distance standard time,deep inhalation volume standard time,hospitalization time,FEV1%,BMI,MBI and the incidence of adverse events during exercise training of 2 groups were compared. Results The standard time of the 6MWT distance and the deep inhalation volume in the trial group were lower than it in the control group(P<0.05). The FEV1% and MBI at discharge in the trial group were lower than those in the control group(P<0.05). There was no significant difference in length of stay,BMI and incidence of adverse events between the 2 groups(P>0.05). Conclusion The matching exercise training program is accurate,safe and feasible,and it can promote the rapid postoperative rehabilitation of lung transplant patients.

    A study on the effects of parent-involved painting therapy in children with leukemia

    Objective To explore the effects of parent-involved painting therapy on psychological state,quality of life and family function for children with leukemia. Methods Using a non-parallel controlled clinical trial design,a total of 40 children with leukemia and their parents who were admitted to a tertiary cancer hospital in Anhui Province from July 2021 to January 2022 were selected as the experimental group by convenience sampling,and the parent-involved painting therapy intervention program was implemented on the basis of routine care. 40 children with leukemia admitted from December 2020 to June 2021 and their parents were selected as the control group with routine care. The intervention time lasted 6 weeks for both groups. The anxiety level,depression level,quality of life and family function of the 2 groups were evaluated by the Screen for Child Anxiety Related Emotional Disorders(SCARED),the Depression Self-rating Scale for Children(DSRSC),the Family Adaptability and Cohesion Scale(PedsQLTM 3.0) and the Pediatric Quality of Life Inventory 3.0 Cancer Module(FACES Ⅱ-CV) before intervention,after 3 weeks and 6 weeks of intervention. Results A total of 6 cases dropped out,and finally 36 cases were included in the experimental group and 38 cases in the control group. The comparison of anxiety level and family function at different time points between the 2 groups showed that the time effect, between-group effect and interaction effect were statistically significant(P<0.05). There was no significant difference in depression level and quality of life among time effect, between-group effect and interaction effect(P>0.05). The simple effect analysis showed that the anxiety level of the experimental group was lower than that of the control group after 3 weeks and 6 weeks, and the family function of the experimental group was better than that of the control group after 6 weeks, and the difference was statistically significant(P<0.05). Conclusion Implementing parent-involved painting therapy can effectively reduce the anxiety of children with leukemia and improve their family function.

    Objective To investigate the current situation of ethical conflict among ICU nurses in tertiary general hospitals in China,and to analyze the influencing factors to provide evidence for the mitigation of ethical conflict. Methods A multi-stage sampling method was used to investigate 1608 ICU nurses from 28 tertiary general hospitals in 7 geographical regions in China from December 2021 to April 2022. The Chinese version of nursing ethics conflict questionnaire-intensive care version was adopted. The univariate analysis and multiple logistic regression were used to analyze the influencing factors. Results A total of 1 516 valid questionnaires were collected. The score of ethical conflict among ICU nurses in tertiary hospitals in China was 75.5(35.0,125.0). Logistic regression analysis showed that region,ICU type,age,education level,position and whether they had received ethical training were the influencing factors of ICU nurses’ ethical conflict(P<0.05). Conclusion The ethical conflict of ICU nurses in China is expected to be alleviated. Nursing managers should identify the population at risk of ethical conflict,pay attention to counseling and psychological support,and develop personalized strategies to abate the ethical conflict of ICU nurses.

    Objective To explore the current status of organizational silence among nurses,and to analyze its latent profiles and population characteristics. Methods Convenience sampling was adopted to select nurses from secondary and above hospitals in Northeast,South,Southeast,Central South,East,Southwest,and Northwest China in June 2020 by the demographic characteristics questionnaire,Employee Silence Scale(ESS) and Peer Support Scale (PSS). A latent profile analysis(LPA) was performed on characteristics of nurses’ organizational silence,and the influencing factors of the latent profile were identified by single-factor analysis and logistic regression analysis. Results A total of 3 974 nurses were included,and their organizational silence can be divided into 4 categories,namely “outsiders”(15.60%),“followers”(22.60%),“middlers”(37.75%),and “protagonists”(24.06%). Logistic regression analysis showed that family financial burden,health status,job title,hospital level,head nurse support and colleague support were the influencing factors of the latent profile of organizational silence(P<0.05). Conclusion There was heterogeneity in the characteristics of organizational silence,which could be divided into 4 latent profiles. Nursing managers can conduct individualized interventions according to the influencing factors of different organizational silence profiles to reduce the level of organizational silence of nurses.

    Objective To investigate the status quo of work readiness of standardized training nurses and to analyze its influencing factors,so as to provide a scientific basis for improving the standardized training nurses’ work readiness and improving the effect of standardized training. Methods From March to April 2022,944 nurses undergoing standardized training in 8 tertiary A hospitals in Xinjiang Uygur Autonomous Region were selected by the convenience sampling method as the research subjects. Questionnaire survey were conducted through the general information questionnaire,the Chinese version of the graduate nurse job readiness scale and the New Nurse Transformation Impact Assessment Scale. Results The total score of nurses’ work readiness in standardized training was(265.72±47.79). Multiple linear regression analysis showed that educational background,marital status,training time and transition impact were the main influencing factors for the job readiness of standardized training nurses(P<0.05). Conclusion The job readiness of standardized training nurses is at a moderate level. Nursing managers should pay attention to the needs and career expectations of standardized training nurses with different educational backgrounds,and make an assessment of their job readiness. In order to improve job readiness,it is necessary to formulate training goals and standards in stages according to the training rotation time,dynamically assess their psychological state and feelings,and implement interventions at the right time to reduce the impact of transformation.

    Objective To develop the Holistic Human Caring Perception Scale for clinical nurses and to test its reliability and validity. Methods On the basis of documents retrieval and interviews,the pre-test scale was constructed through Delphi method. In April 2022,20 nurses in Wuhan were selected for a pre-survey to test the readability of the scale. From April to May 2022,a total of 501 clinical nurses in hospitals in Xianning and Tai’an were selected as the investigation subjects. Item analysis and reliability and validity test of the scale were conducted. Results The formal holistic human caring perception scale for clinical nurses with 6 dimensions and 40 items was formed. 6 common factors were extracted by exploratory factor analysis and the cumulative variance contribution rate was 79.05%;confirmatory factor analysis showed that the fitting index of the scale structure model was good. The S-CVI of the scale was 0.94 and the I-CVI was 0.78~1.00;the Cronbach’s α coefficient of the scale was 0.98 and the test-retest reliability was 0.87. Conclusion The reliability and validity of the Holistic Humanistic Caring Perception Scale for clinical nurses are good,which can comprehensively reflect the caring perception of clinical nurses.

    At present,the level of psychological resilience of domestic nurses is generally low. The construction of a scientific and effective resilience intervention program for clinical nurses is an urgent need to promote their physical and mental health and the stable development of the nursing group in China. This paper reviews the current research progress of resilience intervention for clinical nurses at home and abroad,summarizes the intervention form,intervention content and intervention outcomes,analyzes the shortcomings of current research,and puts forward the outlook for future intervention research. It aims to provide guidance for proposing scientific and reasonable intervention programs in the future.

    Objective To investigate the current status of decision fatigue of substitutive decision-makers in ICU patients,and to analyze its influencing factors. Methods By the convenient sampling method,314 substitutive decision-makers of ICU patients were selected from February to October 2022 in a tertiary hospital in Shandong. General information questionnaire,decision fatigue scale,preparation for decision making scale,family decision making self-efficacy scale,and social support rating scale were used for investigation. Results The score of decision fatigue scale for substitutive decision-makers in ICU patients was(12.33±4.88). The results of multiple linear regression analysis showed that the number of days patients stayed in the ICU,gender of the substitutive decision-makers,education level,per capita monthly income of the family,occupational status,whether they knew about the disease,decision-making readiness,decision-making self-efficacy and subjective support were the influencing factors of decision fatigue. Conclusion The decision fatigue of substitutive decision-makers in ICU patients was at a middle level. Patients with long stay in ICU,female substitutive decision-makers,high education level,low per capita monthly income of families,being employed,and not knowing about the disease have a high level of decision fatigue tended to have a higher level of decision fatigue. The improvement of decision-making readiness,decision-making self-efficacy,and subjective support level is conducive to reducing the decision-making fatigue level of substitutive decision-makers and improving their decision-making quality.

    Objective To investigate the current situation of disordered eating behaviors in adolescents with type 1 diabetes mellitus,and to analyze the influencing factors of disordered eating behaviors. Methods A total of 187 adolescents with type 1 diabetes mellitus were recruited from a tertiary hospital during follow-up by the convenient sampling method from December 2021 to September 2022. The General Information Questionnaire,the Diabetes Eating Problem Survey-Revised,the Problem Areas in Diabetes Scale-5,the Diabetes Strengths and Resilience Measure for Adolescents,and the Dual-Mode of Self-Control Scale were used to explore the current status of disordered eating behaviors in adolescents with type 1 diabetes. We adopted the multiple linear regression model to analyze the influencing factors of disordered eating behaviors. Results 174 participants completed the questionnaires. The scores of disordered eating behaviors were(21.25±10.93) points. The incidence of disordered eating behaviors was 54.60%. The multiple linear regression analysis showed that the influencing factors of disordered eating behaviors involved gender,BMI,mental distress,resilience,and self-control. Conclusion High disordered eating behaviors exist in adolescents with type 1 diabetes. Medical practitioners should regularly evaluate patients’ disordered eating behaviors,especially female,obese or overweight patients. Through correcting the patients’ problematic body image cognition,and enhancing their confidence in disease control,medical staff could help them cope with stress positively,reduce negative emotional experiences,and further decrease the occurrence of disordered eating behaviors.

    Objective To analyze the effect of somatosensory movement combined with percutaneous acupoint electrical stimulation rehabilitation nursing program on rehabilitation training after neck lymph node dissection for thyroid cancer. Methods The convenient sampling method was used to select 90 patients with thyroid cancer who were admitted to the head and neck surgery department of a tertiary A hospital in Zhejiang Province from May 2021 to January 2022 after neck lymph node dissection. According to the random number table method,they were divided into a test group and a control group,with 45 patients in each group. The test group implemented so-matosensory movement combined with percutaneous acupoint electrical stimulation rehabilitation nursing program,and patients in the control group received routine rehabilitation nursing training,with a total duration of 12 weeks. The differences of the neck range of motion,shoulder joint function assessment and cancer patients’ quality of life between the 2 groups before the intervention and at the 1st,4th and 12th weeks after the intervention were com-pared. Results Before the intervention,there was no significant difference between the 2 groups in the scores of the functional dimension and symptom dimension in the cervical range of motion,shoulder function rating scale and the quality of life scale for cancer patients(P>0.05). At the 1st,4th and 12th weeks after the intervention,there was an interaction effect between the 2 groups in the scoring time of the functional dimension and symptom di-mension in the cervical range of motion,shoulder function rating scale and the quality of life scale for cancer pa-tients(P<0.05). Conclusion Somatosensory exercise combined with percutaneous acupoint electrical stimulation re-habilitation nursing program can improve the neck mobility and neck shoulder joint function of patients with thy-roid cancer after surgery,and improve the quality of life.

    To summarize the nursing experience of 39 patients undergoing transcatheter tricuspid annulus repair via jugular vein. The main contents of nursing care are as follows:to carry out accurate evaluation and optimize the preoperative preparation strategy;to conduct multi-disciplinary team cooperation during the operation to develop emergency plans and pre-control the risks during the operation;to implement postoperative goal-oriented hemodynamic monitoring,prevention of bleeding,and close monitoring of postoperative cardiac complications;to implement progressive limb rehabilitation exercise to accelerate the rehabilitation process of patients. The case management files were established to ensure the implementation of continuity of care. After careful treatment and nursing,39 patients recovered and was discharged from the hospital. They were followed up for 3 to 6 months and recovered well.

    To summarize the successful care of a patient with acute liver failure who formed an obstructive fibrinous tracheal pseudomembrane during mechanical ventilation,and whose tracheal pseudomembrane annular abscised after extubation. Key points of nursing include early identification of pseudomembrane abscission,and team collaboration in emergency management;individualized airway care after re-intubation to prevent aggravation of airway injury;active infection control to reduce airway inflammation;strengthening anticoagulation care to promote the recovery of traumatic mucosa;goal-directed sedation and analgesia to assist mechanical ventilation;aspiration prevention during nasal feeding and swallowing function recovery after extubation. The patient was successfully extubated 3 days after re-intubation.

    Objective To systematically evaluate the psychological experience of adolescent suicide attempters. Methods Computer search was conducted on Pubmed,CINAHL,Embase,PsychInfo,Web of Science,Cochrane Library,CNKI,Wanfang Database,VIP database,and Chinese Biomedical Literature database to retrieve qualitative studies on the psychological experience of adolescent patients with suicide attempts. The retrieval period was from the establishment of the databases to July 2022. Qualitative research quality evaluation criteria(2016 edition) of the Joanna Briggs Institute(JBI) Center for Evidence-based Health Care in Australia were used for literature quality evaluation,and meta-integration method was used for integrated analysis of the results. Results A total of 11 studies were included,and 61 research results were extracted,which were summarized into 9 new categories and integrated into 4 integrated results. ①mental and emotional storms,including mental and emotional breakdown,feeling of love and regaining confidence;②obstructed life and study,including lack of life goals,heavy academic pressure;③conflicted intimate relationships,including family friction,love failure,friendship failure;④adjustment and coping,including multiple external support,self-adjustment and perception. Conclusion Nursing staff should pay attention to the psychological experience of adolescent patients with suicide attempts,guide them to relieve the pressure of life and study,relieve family conflicts,assist patients to establish a positive view of friendship and love,and jointly develop a long-term intervention program of multidisciplinary,in order to prevent patients from suicide again.

    Objective To evaluate and summarize the best evidence for ultrasound-guided nasointestinal tube placement in adult critically ill patients,and to provide references for clinical practice. Methods The relevant evidence of ultrasound-guided nasointestinal feeding tube placement in adult critically ill patients was systematically searched from domestic and foreign databases and websites,including guidelines,expert consensuses,clinical decision-making,evidence summaries and systematic reviews,and the search time limit is from the establishment of the databases to June,2022. There were 2 researchers who independently assessed the quality of the literature and identified the level of evidence. Results A total of 22 articles were included,including 5 guidelines,6 expert consensuses,1 group standard,2 clinical decision-making,3 evidence summaries and 5 systematic reviews. 26 pieces of evidence were summarized from 5 themes:clinical situation,assessment,preparation,key points of catheterization,and nursing essentials. Conclusion The best evidence of ultrasound-guided nasointestinal tube placement in adult critically ill patients was summarized to provide evidence-based references for the implementation of bedside ultrasound-guided nasointestinal feeding tube placement and ensure patient safety.

    Objective To search,evaluate and integrate relevant evidence on the management of eating behaviors in patients with anorexia nervosa(AN) at home and abroad,so as to provide references for clinical practice. Methods We systematically searched the domestic and foreign databases to collect the evidence on the management of eating behaviors in patients with AN,including clinical decisions,recommended practice,evidence summaries,clinical practice guidelines,systematic reviews and expert consensuses. The time for the retrieval is from the inception of databases until February 2022.There were 3 researchers who evaluated the quality of the literature,and extracted and summarized the evidence based on this subject. Results A total of 16 articles were included,including 2 clinical decisions,2 evidence summaries,9 guidelines,and 3 systematic reviews.Totally 26 pieces from 6 aspects of evidence were summarized,including multidisciplinary team,management goals,assessment,management practice,psychotherapy and health education. Conclusion This study summarizes the best evidence for the management of eating behaviors in patients with AN. In the follow-up process of evidence transformation,specific clinical situations in China should be considered to select evidence in a targeted manner,in order to standardize the eating behaviors of patients and improve their clinical outcomes.

    The support and cooperation of the multidisciplinary team during the ICU patient transferring is critical to ensure the safety of the transition. This paper introduces the development of the ICU transitional care model,secure,encourage,collaborate(SEC) transitional care model,and mainly describes the content of SEC transitional care model and its application in the field of ICU nursing. It is proposed that the content of SEC transitional nursing model is not specific enough,and its connotation can be enriched based on the actual clinical use in the future. A suitable transitional nursing scheme can be developed based on this model and the characteristics of Chinese medical institutions.

    Challenging behavior is the most common abnormal behavioral symptom in patients with Alzheimer’s disease. It is also a manifestation of poor health of patients,which brings great burden to patients and their caregivers. This article introduces the main content,reliability and validity,characteristics,application status and applicability of common assessment tools for challenging behavior in patients with Alzheimer’s disease,in order to provide references for the development of challenging behavior assessment tools for patients with Alzheimer’s disease in China and provide references for medical staff to scientifically select appropriate assessment tools.

    Clown care is a non-drug intervention based on therapeutic humor,which plays a very important role in alleviating the pressure on pediatric patients,improving their bad behaviors and treatment compliance. This article reviewed the development process,implementation methods,impact on pediatric patients,difficulties and challenges in the implementation of clown care,and put forward practical suggestions to provide a reference for further application of clown care in pediatric patients in China.

    Objective To verify the reliability and validity of the Chinese version of Transition Readiness Assessment Questionnaire(EpiTRAQ) for adolescents with epilepsy(AWE). Methods The Chinese version of EpiTRAQ was translated strictly following the modified version of Brislin model,including translation,cross-cultural adaptation and pilot test. A total of 433 AWE from 8 specialist and general hospitals were recruited by convenience sampling. Results The Chinese version of EpiTRAQ consisted of 2 parts. The first part included 6 dimensions,including managing medication,making appointments,tracking health issues,talking with medical service providers,managing daily activities and female patients with 35 items. The second part consisted of 2 items that used to assess adolescents’ perception of self-health management. For the overall EpiTRAQ,the Cronbach’s α coefficient was 0.919 and each dimensions ranged from 0.724 to 0.984,and the test-retest reliability was 0.923. The questionnaire-level average content validity index was 0.910 and the I-CVI ranged from 0.863-1.000. The Chinese version of EpiTRAQ and Epilepsy Self-Management Scale were highly correlated(r=0.449,P<0.001). A total of 6 common factors were extracted from exploratory factors analysis,and the cumulative variance contribution rate reached 55.276%. Conclusion The Chinese version of EpiTRAQ has been proved to be reliable and valid,and it can be used to provide assessment and intervention of transitional services for AWE in China.
