高强度间歇运动对2型糖尿病患者糖脂代谢指标影响的研究

The effects of high-intensity interval training on glucose and lipid metabolism index in patients with type 2 diabetes

摘要:

目的 探讨高强度间歇运动和中强度持续运动在2型糖尿病患者中的应用效果。方法 选取2018年6月—11月在某三级甲等医院就诊的70例2型糖尿病患者作为研究对象,随机分为高强度间歇运动组和中强度持续运动组,每组35例。高强度间歇运动组采用1 min高强度运动(70%~90%最大心率)与1 min低强度运动交替的方式;中强度持续运动组采用中强度持续运动(50%~70%最大心率)。干预12周后,比较2组患者的指标变化,包括空腹血糖、餐后2 h血糖、糖化血红蛋白、高密度脂蛋白、低密度脂蛋白、BMI、运动自我效能及依从性。结果 2组患者干预后糖脂代谢指标均优于干预前,差异有统计学意义(P<0.05);高强度间歇运动组的餐后2 h血糖、糖化血红蛋白、BMI、运动自我效能及依从性优于中强度持续运动组,差异有统计学意义(P<0.05)。结论 高强度间歇运动和中强度持续运动均能有效改善2型糖尿病患者的血糖、血脂水平,但高强度间歇运动在改善餐后2 h血糖、糖化血红蛋白、BMI、运动自我效能及依从性方面优于中强度持续运动。

Abstract:

Objective To evaluate the effects of high-intensity interval training on patients with type 2 diabetes in comparison of moderate-intensity continuous training.Methods Seventy patients with type 2 diabetes in a tertiary hospital from June to November 2018 were selected,and were randomly divided into the high-intensity interval training group and the moderate-intensity continuous training group with 35 cases in each group. The high-intensity interval training group used the method of alternating 1-min high-intensity training(70%~90% maximum heart rate) and 1-min low-intensity training,the time were increased step by step within the range of 10~30 min/d.The other group used moderate-intensity continuous training(50%~70% maximum heart rate),each time continued to 20~30 min,accumulated 50 min/d. Each group performed 3 days per week and insisted on 12 weeks. After 12 weeks,the index changes of 2 groups of patients before and after intervention were compared,including fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,body mass index,exercise self-efficacy and compliance.Results After the intervention,the index of glucolipid metabolism was better than that before intervention in both group,and the difference was statistically significant(P<0.05). The 2 h postprandial blood glucose,glycosylated hemoglobin,body mass index,exercise self-efficacy and compliance of the high-intensity interval training group were better than those of the moderate-intensity continuous training group,and the differences were statistically significant(P<0.05).Conclusion High-intensity interval training and moderate-intensity continuous training can effectively change the level of blood glucose and lipid in patients with type 2 diabetes,but high-intensity interval training is better to improve the level of 2 h postprandial blood glucose,glycosylated hemoglobin,body mass index,exercise self-efficacy and compliance of patients than moderate-intensity continuous training.
