Abstract:

Objective To develop and assess the usability of a digital management platform for risk assessment and stratified intervention of disability in older adults，and conduct usability evaluation，so as to providing a tool for early precise identification and continuous management of disability. Methods The stratified intervention program for disability in older adults was developed based on literature analysis and expert consultation. Guided by the Iron Triangle framework，the functions of a management platform for disability risk assessment and stratified intervention in older adults were designed. From November to December 2025，30 older adults from 2 community health service centers in Hangzhou were recruited to pilot the platform. Platform usability was evaluated using the System Usability Scale（SUS） and the completion rate of intervention task check-ins. Results The effective response rates of the 2 rounds of expert consultation were both 100%. The expert authority coefficient was 0.93，and the Kendall’s coefficients of concordance were 0.351 and 0.426，respectively（both P<0.05）. The disability risk stratified intervention strategy was constrained by risk levels along with auxiliary rules. Intervention content，dosage，recommended frequency，and implementation requirements were established across 4 dimensions，including physical activity，dietary nutrition，rest and sleep，cognitive psychology and social support. A total of 28 valid questionnaires were included in the usability evaluation. The score of SUS was（81.43±12.41） points，and the completion rate of intervention task check-ins was 78.9%. Conclusion The management platform for disability risk assessment and stratified intervention in older adults demonstrated good usability and provides a practical tool for continuous management in the prevention and control of disability among older adults.

目的 开发老年人失能风险评估与分层干预数字化管理平台并进行可用性评价，为失能的早期精准识别与连续管理提供工具。 方法 通过文献分析、专家函询等形成老年人失能分级干预方案，基于铁三角框架设计老年人失能风险评估与分层干预管理平台功能。2025年11—12月招募杭州市2所社区卫生服务中心30名老年人进行试用，并采用系统可用性量表、干预任务打卡完成率进行评价。 结果 2轮专家函询的有效问卷回收率均为100%，专家的权威系数为0.930，肯德尔和谐系数分别为0.351和0.426（均P<0.05）。老年人失能风险分级干预策略以风险级别及辅助规则为约束条件，围绕身体活动、膳食营养、休息与睡眠、认知心理与社会支持4个维度设置干预内容、剂量、推荐频次等。可用性评价最终纳入28份有效问卷，系统可用性量表得分为（81.43±12.41）分，干预任务打卡完成率为78.9%。 结论 老年人失能风险评估与分层干预管理平台易用性良好，可为老年失能防控提供连续管理的实践工具。
