Development of secure，encourage and collaboration transitional nursing model and its application in ICU nursing field

Abstract

Abstract:

The support and cooperation of the multidisciplinary team during the ICU patient transferring is critical to ensure the safety of the transition. This paper introduces the development of the ICU transitional care model，secure，encourage，collaborate（SEC） transitional care model，and mainly describes the content of SEC transitional care model and its application in the field of ICU nursing. It is proposed that the content of SEC transitional nursing model is not specific enough，and its connotation can be enriched based on the actual clinical use in the future. A suitable transitional nursing scheme can be developed based on this model and the characteristics of Chinese medical institutions.

摘要：

ICU患者转出过程中多学科团队的支持与合作是确保过渡期安全的关键。该研究介绍了现有的过渡期护理模式，安全-支持-合作过渡期护理模式的发展，主要阐述了安全-支持-合作过渡期护理模式的内容以及在ICU中的应用；安全-支持-合作过渡期护理模式仍在不断完善，未来可结合临床实际使用情况丰富其内涵，可借鉴该模式，结合医疗机构特点开发适用于本土的过渡期护理方案。
