类风湿关节炎患者参与共享门诊体验的质性研究

A qualitative study on the participation of patients with rheumatoid arthritis in shared medical appointments experience

摘要:

目的 挖掘类风湿关节炎患者参与共享门诊的体验,为医护人员制订针对性的干预策略提供参考。方法采用质性研究中的描述性现象学研究方法,选取2023年8月—10月在上海市某三级甲等中西医结合医院参与共享门诊的14例类风湿关节炎患者进行一对一深度访谈,采用Colaizzi 7步分析法进行资料分析、提炼主题。 结果 类风湿关节炎患者参与共享门诊的体验归纳为3个主题、11个亚主题。①参与共享门诊的驱动因素:寻求释疑和解惑;寻求支撑和希望。②感知参与共享门诊的益处:感受高效和便捷;减轻孤立感,增加安全感;减少病耻感,提高自我效能;获得启示和经验,改变生活方式。③参与共享门诊过程中存在的障碍:医学术语晦涩难懂;部分特殊群体不适用;分享的内容和形式单一;个人隐私泄露的担忧和顾虑;交通不便和时间冲突。 结论 医护人员应重视类风湿关节炎患者参与共享门诊的体验,提高医护人员的培训技能,解决共享门诊参与过程中存在的障碍,促进共享门诊的不断完善和发展。

Abstract:

Objective To explore the experience of patients with rheumatoid arthritis participating in the shared medical appointments(SMA),so as to provide references for medical staff to formulate targeted intervention strategies. Methods By the descriptive phenomenology research method in qualitative research,14 patients with rheumatoid arthritis who participated in the SMA in a tertiary A integrated traditional Chinese and western medicine hospital in Shanghai from August to October 2023 were selected for the in-depth interviews,and the data were analyzed and themes were extracted by Colaizzi 7-step analysis method. Results The experience of patients with rheumatoid arthritis participating in SMA can be summarized into 3 themes:①Driving factors of participation in SMA(seeking clarity and resolution,seeking support and hope);②Perceived benefits of participation in SMA(feelings of efficiency and convenience,reduced isolation,increased security,reduced stigma,increased self-efficacy,insights and experiences,lifestyle changes);③Barriers to participation in shared outpatient procedures(obscure medical terminology,inapplicability to some special groups,single content and form of sharing,worries and concerns about personal privacy disclosure,transportation inconvenience and time conflicts). Conclusion Medical staff should pay attention to the experience of patients with rheumatoid arthritis participating in the SMA,improve the training skills of medical staff,solve the obstacles in the process of participation in the SMA,and promote the continuous improvement and development of the SMA.
