    Construction and application of a telemedicine-based home care system of palliative care for end-of-life cancer patients

    Objective To construct a telemedicine-based home care system of palliative care for end-of-life cancer patients,and to evaluate its application effect. Methods The construction of the system was based on the literature research,interview results of the patients and their caregivers,and expert group meeting discussions. From May to August 2023,a total of 88 patients,who were about to be discharged from the palliative care ward of a cancer hospital in Hunan Province,were selected as the study subjects using a convenient sampling method,and then they were divided into an experimental group and a control group according to the order of admission. The experimental group was given home care services based on the system,while the control group was given routine discharge guidance and follow-up visits. The duration of the intervention was 8 weeks. The Chinese version of the Edmonton Symptom Assessment Scale and the Chinese version of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 15 palliative were used to evaluate the symptom burden and quality of life between 2 groups before and after intervention;the Telemedicine Satisfaction Questionnaire was used to evaluate the care satisfaction of the experimental group. Results The system of patient side includes 8 main features,namely personal health record establishment,doctor-patient communication,palliative care knowledge library,medication assistance,smart reporting and monitoring,intelligent Q&A,intelligent follow-up,and intelligent forms. The healthcare professional side includes 7 main functions,namely workbench,case management,follow-up management,warning reminder,popularization push,statistical analysis and user management. A total of 67 patients completed the intervention,with 33 in the experimental group and 34 in the control group. The symptom burden of the experimental group was lower than that of the control group(P<0.05);the quality of life in the experimental group was better than that in the control group(P<0.05);the score of care satisfaction was(59.00±6.66),which indicated a high level of satisfaction. Conclusion The system can reduce the symptom burden and improve the quality of life of end-of-life cancer patients with good practicality

    Objective To explore the effect of the nursing intervention for Internet + whole course breastfeeding support based on lactation physiology on exclusive breastfeeding of cesarean section women. Methods Using the convenience sampling method,198 women who were from prenatal examination to delivery in the Obstetrical Department of a tertiary hospital in Nanning city from July 2022 to February 2023 were selected,with 99 women in each group. The experimental group adopted the nursing intervention for Internet + whole course breastfeeding support based on lactation physiology,while the control group adopted routine nursing care. The rates of exclusive breastfeeding within 6 months,breastfeeding or milking frequency within 3 days postpartum,incidence of nipple pain,breastfeeding knowledge scores,and breastfeeding self-efficacy scores were compared between the 2 groups. Results No shedding occurred in both groups. The exclusive breastfeeding rates in the experimental group at postpartum 3 days,14 days,42 days,4 months,and 6 months were higher than that in the control group. The comparison of the rates of exclusive breastfeeding between 2 groups at different time points showed that the intergroup effect,time effect and interaction effect were statistically significant(P<0.05). The 2 groups were stratified by maternal and child separation,and the rates of exclusive breastfeeding of the experimental group at postpartum 42 days,4 months and 6 months were higher than that of the control group(P<0.05). The frequency of breastfeeding or milking within 3 days postpartum,incidence of nipple pain,breastfeeding knowledge scores,and breastfeeding self-efficacy scores were higher in the experimental group than those in the control group(P<0.05). Conclusion The nursing intervention for Internet + whole course breastfeeding support based on lactation physiology can improve the compliance of lactation behavior in cesarean section women and promote the establishment of lactation,which can significantly improve the rate of exclusive breastfeeding within 6 months.

    Objective To construct and apply a multidrug-resistant bacteria information management system,and evaluate its effectiveness and accuracy in the management of patients with multidrug-resistant bacteria infection. Methods A system construction team was established to develop a multidrug-resistant bacteria information management system,which includes 4 modules:early warning,execution,monitoring,and statistical decision-making. Patients with positive detection of multidrug-resistant bacteria admitted to a tertiary A obstetrics and gynecology hospital in Zhejiang Province from January to December 2022,members of the system construction team,and clinical medical staff were selected as the research subjects. The execution efficiency of multidrug-resistant bacteria management,as well as the system’s accuracy,usability,and satisfaction were compared before(January to June 2022) and after(July to December 2022) the application of the system. Results After the implementation of the system,the immediate feedback rate of multidrug-resistant bacteria early warning information increased from 62.87% to 89.78%;the rate of issuing isolation medical orders rose from 61.07% to 93.33%;the accuracy of the implementation of isolation measures for patients increased from 66.67% to 98.01%;all differences are statistically significant(P<0.001). The accuracy rate of the system in making decisions is 88.44%;the usability score given by medical staff for the system is 44.04 points,and the satisfaction score of the system construction team members and medical staff to the system is 121.25 points,both at a high level. Conclusion The multidrug-resistant information management system is equipped with features such as immediate alerts,multi-channel notifications,infection control department supervision,and auxiliary decision-making,which can provide medical staff with accurate decision reports. Preliminary application results show that the system has a high level of accuracy and good usability.

    Virtual reality applications in visual training for amblyopic children:a scoping review

    Objective To provide a scoping review of relevant studies on the application of virtual reality technology in visual training for children with amblyopia,in order to provide a reference for standardising its application. Methods In accordance with the guidelines for scoping reviews,a systematic search of relevant studies was conducted in Embase,Web of Science,CINAHL,Cochrane Library,Scopus,PubMed,CBM,Wanfang Database,VIP Database,and CNKI from the establishment of the database to 1 November 2023. Following this search,the included literature was screened,summarized,and analyzed. Results A total of 24 papers were included. Among them,dichoptic training,visual perception training and visual push-pull model training are the main modalities of visual training based on virtual reality technology,usually with interventions in the form of games,animations,and video images. Evaluation indicators include visual resolution,best-corrected visual acuity,stereopsis,stereopsis acuity,contrast sensitivity,visual evoked potentials,tertiary visual function,as well as safety,training compliance,acceptance and satisfaction. Conclusion The application of visual training based on virtual reality technology in amblyopic children is safe and feasible,which can improve the children’s visual acuity and training compliance. In the future,an intelligent visual training system can be developed based on virtual reality technology,and a more scientific and effective structured visual training programme can be constructed according to the age,type and degree of amblyopia of the children.

    Objective To develop “a guideline for pressure injury risk assessment and prevention of neonates in NICU”,and to provide guidance and references for clinical staff in the implementation of pressure injury assessment and prevention for neonates in NICU. Methods Based on the World Health Organization Guideline Development Manual and the results of systematic search for identified clinical problems,the GRADE method was used to evaluate the evidence and grade the recommendations,and the RIGHT report specifications were referred to for writing,and the guideline was developed and revised according to the results and recommendations of the expert review,so as to form the official guideline. Results The guideline included 2 aspects of pressure injury risk assessment and prevention,resulting in 12 clinical questions and 19 recommendations. Conclusion The guideline for risk assessment and prevention of pressure injury of neonates in NICU is an evidence-based guideline based on the best available evidence,clinical practice,and professional judgment,and it can provide a practical basis for scientific decision-making by clinical staff and managers.

    Objective To establish a risk prediction model for the occurrence of venous thromboembolism(VTE) in hospitalized children and to validate the application of the model. Methods Retrospective analysis was conducted on 62 children who developed VTE during hospitalization in our hospital from January 2022 to July 2023 and were selected in a VTE group. In the ratio of 1:4,248 children from the same department and time who did not develop VTE were selected in a non-VTE group. Logistic regression analysis was used to develop the risk prediction model. Hosmer-Lemeshow test and receiver operator characteristic(ROC) curves were used to test the goodness of fit and predictive effectiveness of the model. Bootstrap self-help method was used for the internal validation of the model by re-sampling the sample 1,000 times. Results In our study,the incidence of VTE was 0.43‰. Logistic regression analysis showed that the length of central venous access device(PICC) stay ≥3 d,stay with fully implantable intravenous port,chemotherapy,ICU length of stay ≥4 d,and bracing duration >72 h were the independent risk factors for the development of VTE in hospitalized children(P<0.05). Hosmer-Lemeshow test showed that χ2=5.291,P=0.152. Sensitivity was 0.857,specificity was 0.218,and area under the ROC curve was 0.823 by Bootstrap self-help method resampling method. Conclusion The risk predictive model of VTE risk in hospitalized children constructed in this study has good predictive performance,which can be used for clinical medical staff to identify children at high risk of VTE at an early stage and take preventive measures.

    Objective To explore the experience of patients with rheumatoid arthritis participating in the shared medical appointments(SMA),so as to provide references for medical staff to formulate targeted intervention strategies. Methods By the descriptive phenomenology research method in qualitative research,14 patients with rheumatoid arthritis who participated in the SMA in a tertiary A integrated traditional Chinese and western medicine hospital in Shanghai from August to October 2023 were selected for the in-depth interviews,and the data were analyzed and themes were extracted by Colaizzi 7-step analysis method. Results The experience of patients with rheumatoid arthritis participating in SMA can be summarized into 3 themes:①Driving factors of participation in SMA(seeking clarity and resolution,seeking support and hope);②Perceived benefits of participation in SMA(feelings of efficiency and convenience,reduced isolation,increased security,reduced stigma,increased self-efficacy,insights and experiences,lifestyle changes);③Barriers to participation in shared outpatient procedures(obscure medical terminology,inapplicability to some special groups,single content and form of sharing,worries and concerns about personal privacy disclosure,transportation inconvenience and time conflicts). Conclusion Medical staff should pay attention to the experience of patients with rheumatoid arthritis participating in the SMA,improve the training skills of medical staff,solve the obstacles in the process of participation in the SMA,and promote the continuous improvement and development of the SMA.

    Analysis of the current state of physical activity levels and influencing factors in lung cancer patients a month post-surgery

    Objective To investigate the current postoperative physical activity levels of lung cancer patients after a month and to analyze the influencing factors. Methods From January to December 2023,a total of 1,195 lung cancer patients from 16 hospitals in eastern,central and western China were selected as the survey subjects by the convenient sampling method. A cross-sectional survey was conducted by General Information Questionnaires,Health-Related Physical Fitness Surveys,MD Anderson Symptom Inventory,Hospital Anxiety and Depression Scale,Social Support Rating Scale,Self-Rated Abilities for Health Practices Scale,and the International Physical Activity Questionnaire. Results 36 questionnaires with missing data exceeding 10% and 13 duplicate questionnaires were excluded,resulting in the analysis of 1,146 questionnaires. After a month postoperatively,lung cancer patients with moderate and low levels of physical activity were 732(63.9%) and 204(17.8%),respectively. Multivariate regression analysis showed that the influencing factors included residence,average monthly family income,regional lymph node staging,family history of lung cancer,grip strength,hemoglobin,and depression score(P<0.05). Conclusion The physical activity level of lung cancer patients a month after surgery is influenced by a combination of sociodemographic,disease-related,health-related physical fitness,and psychological factors. Clinical healthcare professionals should accurately assess the physical activity levels of lung cancer patients,identify those with insufficient physical activity,and develop and implement personalized intervention strategies based on their physical condition and potential influencing factors to enhance their physical activity levels and improve their quality of life.

    Objective To investigate the incidence and risk factors of post-transplant diabetes mellitus(PTDM) in adult lung transplant recipients. Methods Conducting a convenience sampling method,we retrospectively analyzed the clinical data of lung transplant recipients from January 2020 to December 2022 at a tertiary A hospital in Hangzhou,Zhejiang Province. According to the PTDM diagnostic criteria,lung transplant recipients are divided into a PTDM group and a non-PTDM group. The incidence rate of PTDM is calculated,and the influencing factors for PTDM occurrence are analyzed using both univariate and multivariate logistic regression methods. Results A total of 140 patients were included in this study,and 54 lung transplant recipients developed PTDM within 6 months,with an incidence of 38.57%. Univariate analysis showed that there were significant differences in age,gender,BMI,smoking history,pre-operative glycated albumin,pre-operative fasting blood glucose,early post-operative blood glucose and pre-operative creatinine between the 2 groups(P<0.05). Multivariate Logistic analysis showed that gender (OR=5.283),BMI(OR=6.122),pre-operative glycated albumin(OR=1.330),and early post-operative blood glucose(OR=1.444) were the influencing factors. Conclusion Lung transplant recipients who were male,BMI≥24.0,with high levels of glycated albumin before surgery,and high blood sugar early after surgery had a higher risk of developing PTDM. Clinical nurses can formulate relevant nursing measures according to the influencing factors to prevent the occurrence of PTDM.

    Objective To develop Pregnant Women Weight Self-Management Scale and test its reliability and validity. Methods Based on the concepts and theories of self-management,the first draft of the scale was formed through literature analysis,semi-structured interviews,2 rounds of Delphi expert correspondence and pre-surveys. The scale was used to test the reliability and validity of the scale by facilitating the selection of 300 pregnant women who gave birth in April 18—23 2023 at a maternal and child healthcare hospital in Ningxia Hui Autonomous Region. Results The Pregnant Women Weight Self-Management Scale consisted of 8 dimensions and 38 entries with a cumulative variance contribution of 68.560%. The Cronbach’s alpha coefficient for the total scale was 0.944,and each dimension ranged from 0.772 to 0.929. The split-half reliability for the total was 0.858,and each dimension ranged from 0.715 to 0.902. The content validity index at the level of the total scale was 0.921,and at the level of each entry was from 0.833 to 1.000. There was a positive correlation between the dimensions and with the total scale score(r=0.178~0.854,P<0.001). Conclusion The scale has good reliability and validity,and it can be used as a tool to evaluate the self-management ability of pregnant women.

    Objective To analyze the influencing factors of postoperative subsyndromal delirium in elderly patients undergoing abdominal surgery,and to develop a nomogram risk prediction model. Methods A convenience sampling method was used to select 497 elderly patients who underwent abdominal surgery in a tertiary hospital in Sichuan Province from February to October 2022. The risk prediction model and nomograms model were constructed using Logistic regression analysis and R software. The area under the subject’s working characteristic curve(AUC) and Hosmer-Lemshow test were used to evaluate the discrimination and calibration of the model. Results The results of Logistic regression showed that age(OR=1.066),functional activities(OR=1.143),patient controlled intravenous analgesia(OR=5.811),transanal drainage tube(OR=2.276) and postoperative blood transfusion (OR=4.322) were independent influences on the occurrence of postoperative subsyndromal delirium. The p-values of the prediction model in both the training and validation sets were greater than 0.05;the area under the ROC curves were 0.734 and 0.691;the model was presented in the form of nomogram. Conclusion The prediction model developed in this study has good discrimination and accuracy. It can be used to assist clinical staff in identifying patients at high risk of developing postoperative subsyndromal delirium and provide the reference for developing preventive and intervention measures.

    To summarize the perioperative nursing care in 32 cases of pancreatic cancer undergoing radical surgery combined with intestinal autotransplantation. Key points:to set up a multidisciplinary surgical team and create surgical emergency plans;to well prepare equipment,supplies,and environment;to utilize node control concepts to control operation safety;to carry out the clustered surgical infection management and phased implementation of isolation technology;to prevent and control the inadvertent perioperative hypothermia;organ irrigation,repair and preservation;to prevent the complication. After above precise surgical cooperation and meticulous care,all operations were implemented successfully,and patients were transferred to the intensive care unit for further treatment safely.

    Objective To analyze the latent profiles of active aging among community older adults and its influencing factors,and explore relationship of different categories and nursing care needs. Methods Multistage stratified random sampling was used to select 341 community older people in Qingdao from June 2023 to August 2023 as the survey population. The study instruments included the General Information Questionnaire,the Active Ageing Scale,the Care Needs of the Elderly Scale. Latent profile analysis was used to explore the latent profiles of active aging on the community older adults. The influencing factors of latent profiles were identified by multivariate Logistic regression. Results The active aging of the community older people was identified as a model with 3 latent categories,defined as low active aging type(38.1%),medium active aging-high spiritual intelligence type type(51.6%),and high active aging-comprehensive type(10.3%). The influencing factors include age,education,monthly income level,nursing needs(all P<0.05). Community older adults with low self-development and contribution needs are more likely to be low active aging type. Community older adults with high needs for life care and living environment(OR=3.268,P=0.011),physical and psychological support(OR=1.972,P=0.025),and interpersonal communication and health knowledge(OR=3.433,P<0.001) were more likely to be medium active aging-high spiritual intelligence type. Those with low protection and security needs(OR=0.446,P=0.012) were more likely to be medium active aging-high spiritual intelligence type type. Community older adults with low health monitoring needs(OR=0.297,P=0.029) and high specialty care needs (OR=3.019,P=0.033) were more likely to be high active aging-comprehensive type. Conclusion The level of active aging of older adults in the community is medium,which is characterized by 3 categories. Community nursing staffs should focus on the elderly with low active aging type and medium active aging-high spiritual intelligence type,and targeted intervention should be adopted according to different category characteristics,so as to accurately meet their nursing needs,finally improve the level of active aging of community older adults.

    Objective To explore the current status and potential subtypes of frailty among family caregivers of patients with dementia,and to analyze the related influencing factors of different subtypes. Methods Dementia patients and their family caregivers in 8 community health service centers in Shanghai from June to October 2023 were recruited by convenience sampling. General information questionnaire,Tilburg Frailty Indicator(TFI),Pittsburgh Sleep Quality Index(PSQI),Self-Rating Depression Scale(SDS),Zarit Caregiver Burden Interview(ZBI),and Connor‐Davidson Resilience Scale(CD-RISC) were conducted for investigation. Latent profile analysis was used to explore the potential subtypes of frailty among family caregivers of patients with dementia. The influencing factors associated with the potential subtypes were identified by univariate analysis and multivariate Logistic regression analysis. Results A total of 470 family caregivers of patients with dementia were surveyed,and 46.17% of them suffered from frailty. Frailty among family caregivers of patients with dementia can be classified into 3 potential subtypes:comprehensive-low frailty subtype(70.64%),psychosocial-medium frailty subtype(19.57%),and physical-high frailty subtype(9.79%). Family caregivers of patients with dementia who had poor sleep quality and suffered from 2 or more chronic diseases were more likely to be classified into the physical-high frailty subtype(P<0.05). Family caregivers of patients with dementia who had higher levels of depression,lower mastery levels of caregiving knowledge and skills and spousal caregivers were more likely to be classified into the psychosocial-medium frailty subtype(P<0.05). Family caregivers of patients with dementia who had higher levels of resilience were more likely to be classified into the comprehensive-low frailty subtype(P<0.05). Conclusion The incidence of frailty among family caregivers of patients with dementia is at a high level with significant heterogeneity. It is suggested that medical staff should pay attention to the frailty of family caregivers,with a focus on family caregivers in the psychosocial-medium frailty subtype or physical-high frailty subtype,and take timely and targeted interventions according to the characteristics and influencing factors of different subtypes,so as to prevent or delay the occurrence and development of frailty.

    Objective To systematically evaluate the real-life experience of stroke patients in the hospital-home transition period,and to provide a reference for better clinical development of transitional nursing practice. Methods We searched PubMed,Web of Science,Embase,CINAHL,Cochrane library,CNKI,Wanfang Data,VIP database,and China Biomedical Literature Database for qualitative studies on the real experience of stroke patients in the hospital-to-home transition period from the establishment of the database to October 2023. The quality of the literature was evaluated using the Joanna Brigg Institute(JBI) Australian Centre for Evidence-Based Health Care Quality Assessment Criteria for Qualitative Research(2016),and the results were integrated using meta-integration methods. Results A total of 14 studies were included,and 48 research results were extracted and 9 categories were summarized. The final synthesis included 3 integrated results:the discharge preparation period was in contradiction;after being discharged from the hospital,life changed dramatically and there was a variety of transition barriers;active response to illness and the experience of self-growth. Conclusion The hospital-home transition period for stroke patients is a critical period for patients’ rehabilitation,and medical staff should strengthen the implementation of the transitional discharge plan,strengthen the connection between hospitals and families and communities,pay attention to the psychological experience of patients,so as to help stroke patients achieve a smooth transition from hospital to home.

    Objective To analyze and clarify the concept of intrinsic capacity of the elderly. Methods We searched studies on intrinsic capacity of the elderly from websites and databases,including PubMed,Cochrane Library,Web of Science,Embase,CINAHL,China Biomedical Literature Service System databases,CNKI,WanFang databases and VIP. We selected relevant papers from the inception of databases to July 2023 according to inclusion criteria. Rodgers evolutionary method of concept analysis was used. Results A total of 30 articles were retrieved. 4 attributes were identified on intrinsic capacity,including:guided by the goal of achieving healthy aging,inherent physiological reserve capacity of individuals,rich and interactive dimensions,and a dynamic and reversible development trajectory. Its prerequisites include demographic factors,socio-economic factors,health-related charac-teristics,external environment,and other factors;post effects include early identification of declining intrinsic abilities in the elderly and timely adoption of targeted intervention measures,which are of great significance for improving the quality of life of the elderly and promoting healthy aging. The decline in intrinsic capabilities is closely related to various adverse health outcomes,posing a serious threat to the health status of the elderly. Conclusion The concept attributes of intrinsic capacity were identified by concept analysis method. In the future,research and clinical practice should be carried out based on the concept of intrinsic capacity.

    The update of multi-omics technology is a key means to promote the rapid development of accurate health model in the whole life cycle. It can formulate dynamic and accurate nursing measures and provide massive data information from the perspective of nursing biology of health and disease. At present,clinical nursing research faces many challenges such as insufficient application and transformation ability of multi-omics technology. This paper introduces the multi-omics technology,reviews the application status of multi-omics technology in cancer nursing,maternal and child nursing,chronic metabolic disease nursing and symptom management,and puts forward the cross integration and prospect of multi-omics technology and nursing research,so as to strengthen the information mining ability of nurses at different levels of health and disease,and provide an important basis for accelerating the clinical transformation of precision nursing.
