Objective To investigate the characteristics of cardiovascular health score trajectories in patients after percutaneous coronary intervention(PCI). Methods The convenience sampling method was used to recruit 339 acute myocardial infarction patients who underwent PCI for the first time in the cardiology department of a first-class tertiary hospital in Harbin from May to October 2019 with regular follow-up after PCI. We used the group-based trajectory model to identify the heterogeneous trajectory groups of cardiovascular health score based on the baseline,6 months and 12 months follow-up. The Cox proportional risk regression model was used to analyze the effect of trajectory groups and major adverse cardiac events(MACE). Results 5 cardiovascular health score trajectories were identified as low-increasing group(19.8%),medium-declining and then increasing group(24.8%),medium-rapid increasing group(14.4%),medium-stable group(31.9%),high-stable group(9.1%). Cox proportional risk regression model showed that, after adjustment for potential confounding factors(age,sex,high-density lipoprotein cholesterol and alcohol consumption),the risk of MACE in medium-declining first and then increasing group,medium-rapid increasing group,medium-stable group and high-stable group were 0.498(P=0.033),0.446(P=0.049),0.403(P=0.006) and 0.259(P=0.030) times of low-increasing group, respectively. Conclusion Acute myocardial infarction patients after PCI for a year showed various cardiovascular health score trajectories;the improvement and maintainance of a high level of cardiovascular health score were negatively correlated with the risk of MACE. Medical staff can monitor the characteristics of cardiovascular health score changes dynamically, strengthen risk factor management and adopt early intervention methods to improve prognosis.

Objective To understand the clinical characteristics of fasting glucose in patients with acute-on-chronic liver failure(ACLF),and to analyze the risk factors of fasting hypoglycemia,to provide reference values that can help with the early clinical recognition and prevention of hypoglycemia. Methods The clinical data of 931 patients with ACLF hospitalized from May 2013 to December 2019 in the department of infectious diseases of a tertiary hospital in Guangzhou city, were retrospectively analyzed. ACLF patients were divided into hypoglycemic and non-hypoglycemic groups according to the fasting blood glucose levels. The demographics, clinical characteristics, comorbidities and laboratory indicators of patients in the 2 groups were compared, and the risk factors of hypoglycemia were obtained by Logistic regression analysis. Results There were 399 cases(42.86%) of hypoglycemia, including 207 cases(51.88%) of mild hypoglycemia. Binary Logistic regression analysis showed that the length of hospital stay(OR=1.008),prothrombin time(OR=1.022) and cirrhosis(OR=1.745) were independent risk factors for hypoglycemia in patients with ACLF. Conclusion ACLF patients have a high incidence of mild hypoglycemia, longer hospital stays, longer prothrombin time, and cirrhosis which increase the risk of hypoglycemia. During clinical practice, more emphasis should be placed on screening high-risk patients via early assessment of their hypoglycemic risk, and hence provide timely interventions to reduce the occurrence of hypoglycemia.

Objective To search, evaluate and summarize the best evidence of self volume-management for patients with chronic heart failure(CHF) based on the situation-specific theory of heart failure self-care. Methods The computer decision support systems, guideline websites, databases were systematically searched to collect the literature related to self volume-management in CHF patients. The retrieval period is from the establishment of databases to May 4th,2021. 2 researchers assessed the quality of the included literature independently and extracted the evidence. Results A total of 9 articles were included, including 7 clinical practice guidelines and 2 expert consensuses. A total of 13 pieces of evidence have been obtained from 3 aspects including self volume-management maintenance, symptom perception of volume overload, and volume overload management. Conclusion The best evidence should be used to help CHF patients maintain a balance of volume status, recognize changes in volume status as soon as possible, and learn some skills to respond appropriately to volume overload.

Fear of cancer recurrence(FCR) commonly exists among family caregivers of cancer patients, while severe fear will affect their quality of life. However,FCR of caregivers is easily ignored by clinical staff. This paper reviews the related concept, current situation, assessment tools, influencing factors and intervention measures of family caregivers’ fear of cancer recurrence, aiming to arouse the attention of clinical staff on FCR of this population, and to provide references for further study.

Objective To translate the English version of Perioperative Thirst Discomfort Scale (PTDS) into Chinese, and to test the reliability and validity of the Chinese version of PTDS. Methods The English version of PTDS was translated into Chinese and back translated and modified for cultural adaptation according to the guidelines. A convenience sample of 316 perioperative patients at 4 tertiary hospitals in Fujian was investigated by the Chinese version of PTDS,in order to test its reliability and validity. Results The Cronbach’s α of the Chinese PTDS was 0.813; both the scale-level content validity index and the item-level content validity index were 1.Factor analysis showed 2 factors explaining 64.312% of the total variance. The score of PTDS was significantly associated with the thirst intensity score(r=0.681,P<0.001). Conclusion The Chinese version of PTDS has been proved to be reliable and valid. It can be used for measuring thirst discomfort in perioperative patients.
