    The application effect of advance care planning in hemodialysis patients

    Objective To explore the application effect of advance care planning in hemodialysis patients.Methods A total of 80 hemodialysis patients who were treated in a tertiary hospital in Zhengzhou from October 2020 to June 2021 were selected as the research subjects. According to the random number table method,they were divided into a control group and an intervention group with 40 cases each. The intervention group was implemented with a 4-week advance care planning,and the control group received the routine care. The death attitude profile-revised scale,the Chinese version of the meaning in life questionnaire and the patient dignity inventory scale were used to compare the differences in death attitude,meaning of life and dignity between the 2 groups of patients.Results After the intervention,the patients in the intervention group changed their attitude toward death;the score of meaning of life was higher than that of the control group;the score of dignity was lower than that of the control group;the differences were statistically significant(P<0.05).Conclusion Advance care planning can help hemodialysis patients change into a positive attitude toward death,increase the sense of meaning in life,and maintain the dignity of patients.

    Effect of lavender aromatic therapy combined with emotional freedom techniques on insomnia cancer breast patients with heart and spleen deficiency

    Objective To investigate the clinical nursing effect of lavender aromatherapy combined with emotional freedom techniques(EFT) on insomnia patients with breast cancer and heart-spleen deficiency.Methods A total of 140 patients with insomnia due to heart-spleen deficiency of breast cancer who met the inclusion criteria in the department of mammary gland of a tertiary hospital in Hangzhou from September 2020 to July 2021 were selected as the research subjects by convenience sampling method. Factorial design with 2 factors and 2 levels was adopted. According to the random number table method,the patients were divided into a control group(n=32),an aromatic group(n=33),an EFT group(n=32) and a combined group(n=32). The control group was given routine care of insomnia;on the basis of routine nursing for insomnia,the aromatic group was given lavender aromatherapy,and the EFT group was given EFT. The combined group was given the above 3 methods of comprehensive interventions at the same time,and the intervention period was 2 weeks. Pittsburgh sleep quality index(PSQI) was used to evaluate sleep quality within 24 hours and 2 weeks after intervention. Hospital anxiety and depression scale(HADS) was used to evaluate the improvement of negative emotions. The improvement of symptoms was evaluated by traditional Chinese Medicine(TCM) symptom score standard of deficiency of heart and spleen.Results Before and after the intervention,except the control group,the PSQI score,HASD score and TCM symptom score of the 3 groups were compared,and the differences were statistically significant(P<0.05). After intervention,the differences in PQSI score,HADS score and symptom score among the 4 groups were statistically significant(F=10.049,P<0.001;F=9.328,P<0.001;F=14.934,P<0.001;F=3.741,P<0.05),and the combined intervention of the 2 therapies was superior to any single therapy in the overall efficacy. The combined therapy has synergistic interaction effect on improving TCM symptoms of deficiency of both heart and spleen.Conclusion The combination of 2 therapies can effectively improve the sleep quality of breast cancer patients with insomnia of deficiency of both heart and spleen,and it has a magical effect in alleviating bad mood and relieving TCM symptoms.

    Objective To construct a pulmonary rehabilitation program for patients with double lung transplantation and to evaluate its application effect.Methods 46 patients admitted to our hospital for double lung transplantation from January 2020 to March 2021 were selected as the research subjects. Using random number table method,they were divided into a control group and an experimental group with 23 cases each. The experimental group received the intervention of the lung rehabilitation program for patients with double lung transplantation on the basis of the control program. The pulmonary rehabilitation team led by respiratory therapy specialist nurses guided the whole process,including preoperative pre-rehabilitation training and adaptive training,early postoperative activities,and respiratory muscles training,sequential oxygen therapy,airway clearance,active breathing and circulation techniques,etc. The control group received conventional treatment,health education and pulmonary rehabilitation guidance. The postoperative ECMO assisting time,mechanical ventilation time,hospital stay,oxygenation index,6-minute walk test,incidence of adverse events,and compliance with postoperative pulmonary rehabilitation were compared between the 2 groups.Results The length of hospital stay in the experimental group was lower than that in the control group. The oxygenation index,6-minute walk test distance,and compliance with postoperative pulmonary rehabilitation were better than those in the control group(P<0.05). There was no statistical difference in the ECMO assisting time,mechanical ventilation time and the incidence of adverse events in the 2 groups(P>0.05).Conclusion The pulmonary rehabilitation program for patients with double lung transplantation is safe and effective. It can shorten the length of ICU hospitalization,improve exercise capacity,and promote early recovery of transplanted lung function.

    Objective To develop a quality evaluation indicator system for perioperative nursing in pediatric liver transplantation,and to provide standard and professional quantitative bases for monitoring and management of perioperative nursing quality.Methods This study was conducted from January to June 2021 based on the frame work of the three-dimensional“structure-process-outcome” quality model,using literature review,Delphi method and analytic hierarchy process(AHP) to determine the content of the system,and the weight of each index.Results The effective recovery of 2 rounds of expert consultations were 95.2% and 100%. The authority coefficients were 0.88 and 0.89. The variation coefficients of each item ranged from 0.05 to 0.19 and from 0.06 to 0.23. The Kendall harmony coefficients were 0.128 and 0.184. The final quality evaluation indicator system for perioperative nursing in pediatric liver transplantation was established,including 3 first-level indicators,14 second-lever indicators and 61 third-level indicators.Conclusion The quality evaluation indicator system for perioperative nursing in pediatric liver transplantation was scientific,reliable,comprehensive and specialized,which can provide references for the evaluation of perioperative nursing quality in pediatric liver transplantation.

    Objective To systematically summarize the psychological experience of lung transplantation recipients to comprehensively understand their needs and to provide better evidence for clinical treatment.Methods We searched databases including Cochrane Library,PubMed,Embase,Web of Science,Medline,PsycINFO,ProQuest,Scopus,CNKI,Wanfang Database,VIP and CBM from the start to January 2021,to collect the literature on qualitative studies of the psychological experience and needs of lung transplant recipients.Results A total of 12 studies were included,and 69 findings were extracted. Similar results were combined into 10 new categories,and then 4 integrated results were integrated,including the decision-making experience of lung transplantation,the joy and gratitude after lung transplantation,the distress caused by lung transplantation,and the adjustment after lung transplantation.Conclusion In the decision-making process of lung transplantation,medical personnel should provide lung transplant recipients and their families with comprehensive postoperative information to help them make a prudent choice after comprehensive consideration. Meanwhile,it is suggested that a follow-up management system should be established after lung transplantation.

    Objective To identify the specific intervention content,methods and effects of exercises,so as to provide the guidance for future research.Methods A literature search was performed in the CNKI,Wanfang,CBM,the Cochrane Library,PubMed and Embase to identify relevant studies. The included studies were summarized and analyzed.Results Totally 25 studies were included,of which 9 were randomized controlled trials and 16 were quasi-experimental studies. The results showed that exercise methods can be divided into 3 categories,namely aerobic exercise,resistance exercise and flexibility exercise. The intensity of exercise was mostly low to moderate. The frequency of exercise was mostly 3~5 times per week,lasting for 30-60 minutes per session. The results showed that exercises had positive effects on respiratory function,exercise ability,quality of life and mental health of lung transplant patients.Conclusion Exercise intervention is holistic,dynamic and comprehensive,with a positive effect on lung transplant patients. Future research should focus on exploring the best exercise intervention strategies,formulating standardized and unified evaluation standards,and formulating scientific,applicable and standardized exercise training programs for lung transplant patients based on the condition and physical function of lung transplant patients.

    Objective To test the reliability and validity of the Chinese version of the Lung Transplant Quality of Life survey(LT-QOL) in patients after lung transplantation.Methods We sinicized the LT-QOL following the forward translation,back translation,cultural adaptation and pre-survey. Evaluation of content effectiveness was conducted through 2 rounds of expert consultations. From November 2019 to March 2021,308 post-lung transplant patients who were admitted to a lung transplant clinic of a tertiary A hospital in Wuxi were selected to investigate for the test of the reliability and validity of the scale.Results The Chinese version of LT-QOL includes 3 dimensions and 11 aspects with a total of 40 items. The content validity index of the total scale is 0.912,and the content validity index of each item is between 0.781 and 1. Exploratory factor analysis resulted in an 11-factor structure,explaining 71.424% of the total variance. The Cronbach’s α coefficient for the total LT-QOL score was 0.947,and 0.710~0.929 for every common factor. The retest reliability for the total LT-QOL score was 0.870,and 0.726~0.894 for every common factor.Conclusion The Chinese version of LT-QOL appears to possess adequate validity,retest reliability and internal consistency. The newly translated Chinese version of LT-QOL can be used to assess the quality of life of lung transplant recipients in China.

    Study on the effects of different modes of intermittent pneumatic compression during operation in preven-tion of venous thrombosis of the lower extremities after rectal cancer surgery

    Objective To investigate the effects of different modes of intermittent pneumatic compression(IPC) in preventing deep vein thrombosis(DVT) in patients undergoing laparoscopic rectal cancer surgery.Methods From April 2020 to April 2021,130 patients for laparoscopic radical rectal cancer surgery(Dixon) were sequentially selected in a tertiary hospital,and they were divided into the control group,group Ⅰ,group Ⅱ,and groupⅢ by random number table method. The control group were routinely treated to prevent thrombosis during perioperative period,and other groups were treated with corresponding IPC mode during the operation on the basis of the treatment in the control group. Specifically,group Ⅰ adopted the IPC bipedal mode,and the plantar airbag was applied to 130 mmHg for 6 s,and then relaxed for 12 s. Group Ⅱ adopted the IPC double calf model,and the airbag was applied to the calf sequentially by 80 mmHg for 12 s and relaxed for 24 s. Group Ⅲ adopted IPC double calf + thigh mode,and airbags sequentially pressed 80 mmHg,70 mmHg and 60 mmHg from the ankle,calf and thigh respectively for 24 s and relaxed for 24 s. The venous hemodynamic changes of the lower extremities and the incidence of DVT in the 4 groups were compared before and after operation.Results Compared with the control group,group Ⅰ and group Ⅱ have statistically significant differences in the velocity rates of bilateral femoral common,superficial and popliteal veins before and after surgery(P<0.05),while there is no difference in the change of blood vessel caliber. The incidences of DVT on the first day were improved significantly(P=0.039,P=0.042),respectively. Compared with those in the control group,there were no significant difference in the velocity rates of bilateral total femoral veins,as well as the change of blood vessel caliber of bilateral common femoral vein,superficial femoral vein and popliteal vein in the group Ⅲ(P>0.05). The incidence of DVT on the first day after surgery was not improved(P=0.820). There were no statistically significant differences in the incidences of DVT between the 4 groups on the 7th day after surgery(P=0.125).Conclusion Both the IPC foot mode and the IPC leg mode can reduce the incidence of DVT on the first day after laparoscopic rectal cancer surgery with similar efficacy.

    Objective The study aimed to construct a multidisciplinary,scientific,and standard comprehensive nursing scheme of intestinal blind intubation for patients with ischemic stroke.Methods Literature review,semi-structured interview,observational survey,and Delphi technique were used to construct the scheme. Semi-structured interview and observational survey were used from August to September 2020. 15 experts from clinical nursing,clinical medicine,and intensive medicine were consulted in 2 rounds of Delphi experts consultation from September to December 2020.Results After interviewing 20 experts and observing 7 cases,more than 100,000 words of textual materials and 325 minutes of videos were formed. After the analysis of materials,5 core categories and 18 categories were sorted out. 2 rounds of Delphi experts consultation were carried out,and the effective recovery rate of the questionnaire was 100%. The Delphi expert authority coefficients were 0.787 and 0.795,and the coordination coefficients were 0.112 and 0.367(P<0.01). The coefficient of variation of each item in the second round was 0~0.12,and the full score frequency was 20%~100%. Finally,the comprehensive nursing scheme was constructed,which included 5 first-level indicators (roles and functions,standard service,core operation specification,emergency plan,and mode implementation path),15 second-level indicators,and 43 third-level indicators.Conclusion The comprehensive nursing scheme for ischemic stroke patients was scientific,specific,and practical,which could provide references for the promotion and application of the technique of intestinal blind intubation.

    Objective To explore the true perception of patients with breast cancer who participated the rehabilitation exercise of limb function.Methods Using the purposive sampling method,18 postoperative breast cancer patients admitted in a tertiary hospital in Wuhan,China,from February 2021 to March 2021 were recruited for semi-structured interviews based on the theory of planned behavior. Collected data were analyzed to summarize the main themes by the Colaizzi phenomenological method(7 steps).Results The true experience of postoperative breast cancer patients on the rehabilitation behavior of limb function can be summarized as 3 themes. Behavioral attitude:negative emotions following stress,rehabilitation behaviors that are systematically underestimated,and the value of reconstructing the image of women. Subjective normative measurement:influence of social roles,peer effect,and medical staff support. Perceived behavior control:the confinement of the comfort zone,and the needs of the rehabilitation environment.Conclusion The behavior of limb function rehabilitation in postoperative breast cancer patients is affected by the behavioral attitude,subjective norms measurement and perceived behavioral control. From the perspective of patients,medical staff should correct cognitive bias in rehabilitation behavior of breast cancer patients. Negative cognition needs to be avoided through positive belief factors. Meanwhile,multiple sources of support should be highlighted to stimulate patients’ sense of self-management,create a rich rehabilitation environment,and promote the improvement of breast function rehabilitation behavior of postoperative breast cancer patients.

    Objective To understand the burden experience of the caregivers of children with short bowel syndrome(SBS),and to provide references for making individual effective nursing interventions.Methods Using qualitative research,questions were set based on the Social Ecological Theory before the interview,then a semi-structured interview was conducted on 16 caregivers of children with SBS. Data were transcribed and analyzed using Colaizzi’s method.Results The burden experience of caregivers of children with SBS can be summarized into 3 themes,including micro system-caregiver distress and growth coexist,mezzo system-changes of lifestyle,macro system-weak support system.Conclusion The caregivers of children with SBS reported poor social ecological system,and there is a heavy burden of care. Nursing staff should implement START intervention,provide professional guidance through “Internet +Nursing Service”,construct family-centered collaborative nursing model in order to improve the health level of children and their caregivers.

    Efficacy of group narrative career construction guidance on male nurses

    Objective To provide a new way of career construction guidance to the administrators of hospital to improve professional career development of male nurses,through the way of group narrative career construction guidance.Methods From February to March,2021,we chose 42 male nurses in a tertiary hospital for a 3-week intensive career planning narrative group counseling. After the intensive counseling,post-supervision was provided by the head nurse and the nursing department. The nursing career planning questionnaire and nursing career management questionnaire were filled in the week before the training and in the post-supervision of the nursing department to evaluate our training effect.Results After the intervention,career construction,career goals and strategies,willingness to continue education,self-presentation,career management and training,senior career information gathering and willingness to dedicate into organizational development were significantly improved,and the difference was statistically significant(P<0.05). The scores of career relationships and the fairness of promotion were not significantly improved(P>0.05).Conclusion Group narrative career construction guidance can improve the ability of career construct and management of male nurses,and further promote their career development. However,it has no obvious effect on the improvement of career relationship and promotion of fairness perception.

    Objective To investigate the influence of transition shock and adversity quotient on the feedback seeking behavior of newly graduated nurses,and to clarify the model of the relationship among them.Methods The general information questionnaire,adversity response profile,the transition shock of new graduated nurse scale and feedback seeking behavior of newly graduated nurse scale were used to investigate 216 newly graduated nurses from 2 hospitals in Jiangsu Province.Results Transition shock of new graduated nurses had a negative impact on feedback seeking behavior(r=-0.707;P<0.01),and the transition shock could affect the feedback seeking behavior through the mediating influence of newly graduated nurses’ adversity quotient,with a mediating effect of 56.31%.Conclusion Adversity quotient is a mediating variable between the transition shock and feedback seeking behavior of newly graduated nurses. Nursing managers and educators should help newly graduated nurses realize their self-potential and enhance feedback-seeking behaviors.

    The perioperative nursing care of a child with type Ⅲ gaucher disease with severe kyphosis is summarized. Nursing points include preoperative multidisciplinary collaborative diagnosis and treatment,adequate preoperative preparation,prevention of neurological damage,implementation of individualized rapid plans,postoperative care for paralytic intestinal obstruction,infection prevention,safety management,and psychological health of children and their families. After active treatment and meticulous care by the medical team,the child was discharged 18 days later. After 1 year of following up,he is in good condition and has returned to the normal school and family life.

    Objective To retrieve,evaluate and summarize the evidence of self-management of patients with venous leg ulcer(VLU),and to provide evidence-based references for health education and self-management of patients.Methods We systematically searched all relevant literature on the self-management of patients with VLU,including clinical practice guidelines,best practices,expert consensuses,evidence summaries,systematic reviews and original studies,from the guideline websites,databases and professional association websites,with the search period from the database establishment to August 2021. 2 researchers independently evaluated the quality of the included literature,and extracted and integrated evidence based on the judgment of professionals.Results A total of 14 articles were included,including 9 guidelines,2 best practices,2 systematic reviews and 1 expert consensus. Finally,21 pieces of best evidence were summarized,including 5 aspects of patient self-assessment,stress management,diet and nutrition management,exercise management,and skin management around the wound.Conclusion This study summarizes the best evidence of self-management of patients with VLU,and clinical medical staff can use the best evidence to guide the self-behavior management of patients with VLU in a more targeted and scientific way.

    Objective To systematically evaluate the experience of elderly patients on coping with multiple chronic conditions(MCCs).Methods Pubmed,the Cochrane Library,Web of Science,EmBase,CNKI,Wanfang Database and VIP database were searched for qualitative research studies on experience of elderly patients with MCCs. The retrieval period was from inception to August 2021. The Joanna Briggs Institute Critical Appraisal Tool for qualitative research(2016) in Australia was used to evaluate quality of the studies. A meta-synthesis method was used to integrate the results.Results A total of 15 studies were included;60 findings were extracted;10 cate-gories were summarized;3 integrated results were formed,namely changes caused by MCCs and aging,reflection and response to MCCs,adjusting to a normal life.Conclusion Aging with MCCs brings many challenges to the life of elderly patients. Health professionals should pay attention to the different coping styles of patients with MCCs,and develop feasible and support programs to help them better cope with MCCs which derived from their wisdom.

    Objective To comprehensively search the relevant research of Ottawa Decision Support Framework (ODSF) and to clarify the specific connotation of its constructs,understand the areas covered by its clinical application,and to analyze its specific role in the research.Methods Using the method of Arksey and O’Malley scoping review,we systematically searched CNKI,SINOMED,Wanfang,VIP,PubMed,Cochrane Library,CINAHL,Embase,Web of science,and screened the literature according to the inclusion and exclusion criteria. 2 researchers independently extracted the key information of the ODSF. Descriptive analysis and topic analysis were used to classify the extraction results.Results A total of 143 documents are included. There are 3 types of conceptual descriptions of ODSF in previous studies,which correspond to the 3 versions of ODSF. The framework covers a wide range of research areas,mainly including cancer,orthopedic diseases,cardiovascular diseases,etc. The guiding role of the framework in the research mainly has 7 categories,and there are obvious differences between domestic and foreign research.Conclusion This study summarizes the connotation and clinical application of ODSF in detail,which can provide guidance for future research.

    Objective To translate and adapt the Recommending Cardiac Rehabilitation Scale(ReCaRe) into Chinese,and to analyze its reliability and validity.Methods ReCaRe was translated into Chinese version based on Brislin’s translation model,and then cultural adjustment and pretest were carried out to determine the Chinese version of ReCaRe (C-ReCaRe). From July to August 2021,324 healthcare workers in the department of cardiology or cardiac surgery in Hunan Province were selected to conduct an online survey to evaluate the reliability and validity of C-ReCaRe.Results The C-ReCaRe has 16 items with good content validity. The item-level content validity index was 0.833~1.000;scale-level content validity index/universal agreement was 0.875;scale-level content validity index/average was 0.979. Exploratory factor analysis extracted 3 common factors (Eigenvalue>1),namely perceived benefit and barriers,perceived severity and susceptibility,perceived service accessibility,and the cumulative contribution rate was 67.858%. The Cronbach’s alpha coefficient for the 16-item scale was 0.867 with 0.881~0.908 for each dimension. The split-half reliability was 0.759 with 0.837~0.864 for each dimension.Conclusion The C-ReCaRe showed good reliability and validity,and it can act as a reliable tool to evaluate the recommending cardiac rehabilitation for Chinese healthcare workers.
