    Expert consensus on enteral feeding nursing for patients with severe neurological diseases

    Objective To form an expert consensus of enteral feeding and nursing care for patients with severe neurological diseases,and to promote the standardized,safe and effective implementation of enteral feeding by neuro intensive nursing personnel,so as to provide effective nutritional support for patients. Methods The evidence in this field was searched,evaluated and summarized to extract the recommendations related to enteral feeding in patients with severe neurological diseases. The first draft of consensus was formed. Through 2 rounds of Delphi expert consultations and expert demonstration meetings,the expert suggestions were analyzed,modified and improved to form the final draft of consensus. Results The positive coefficient of inquiry experts was 100%;the coefficient of expert judgment was 0.93;the degree of familiarity was 0.88;the coefficient of authority was 0.91. The concordance coefficient of Kendall was 0.256~0.278,which was statistically significant(P<0.05). The final consensus included the evaluation of enteral feeding care for patients with several neurological illness,selection of feeding methods,nutrient solution infusion,identification and care of complications,and indicators for terminating tube feeding. Conclusion This consensus provides guidance for clinical nursing practice and quality control of enteral feeding in critically ill patients.

    Objective To understand the current situation of aspiration in ICU patients with different feeding patterns and to compare its characteristics,so as to provide a basis for targeted prevention of aspiration. Methods The study was a prospective study. The convenience sampling method was used to select patients who were admitted to the ICU of a tertiary hospital in Suzhou from January 2020 to April 2021 as the survey subjects. According to the different feeding patterns,patients were divided into an oral feeding group,a gastric tube feeding group and a post-pyloric feeding group. The occurrence and characteristics of dominant aspiration and micro-aspiration of patients with different feeding patterns were observed and compared. Results A total of 721 patients were enrolled. The incidences of dominant aspiration in the oral feeding group,the gastric tube feeding group and the post-pyloric feeding group were 8.10%,15.26%,and 11.26%. The incidences of micro-aspiration were 76.95%,77.11%,and 58.28%,respectively. The characteristics of the occurrence of dominant aspiration in 3 groups were compared. The time of first aspiration in the gastric feeding group earlier than that in the post-pyloric feeding group;the oral feeding group had the most frequent aspirations. The proportions of patients with impaired consciousness,gastric retention,intra-abdominal hypertension,dysphagia,and mechanical ventilation in the gastric feeding group and the post-pyloric feeding group were higher than that in the oral feeding group(Bonferroni method,all P<0.017). The characteristics of micro-aspiration among 3 groups were compared;the proportion of patients with vomiting in the oral feeding group was higher than that in the post-pyloric feeding group;the acute physiology and chronic health scores and ICU stay time of the patients in the post-pyloric feeding group were higher than those in the oral feeding group;the proportions of patients with impaired consciousness and mechanical ventilation in the gastric tube feeding group and the post-pyloric feeding group were higher than that in the oral feeding group. The above differences are statistically significant(Bonferroni method,all P<0.017). Conclusion The incidence of dominant aspiration and micro-aspiration in ICU patients fed by gastric tube was higher, and the characteristics of aspiration in ICU patients with different feeding patterns were significantly different. Clinically,individualized preventive measures should be taken as early as possible according to the characteristics of the patient and the way of eating.

    Analysis of potential categories and influencing factors of fatigue trajectories in post-intensive care syndrome patients

    Objective To explore the potential trajectory categories of fatigue degree in patients with post-intensive care syndrome(PICS),and to analyze the influence of patient demographic data and disease-related data on the potential categories of fatigue trajectory. Methods A convenient sampling method was used to select patients hospitalized in general ICU ward of a tertiary hospital in Guizhou Province from January 2020 to September,and the demographic and disease-related data of patients was collected. The fatigue degree of the patients was evaluated at 1 week(T1),1 month(T2) and 3 months(T3) after ICU discharge. The growth mixture model was used to identify the potential trajectory category. Logistic regression analysis was used to analyze the prediction factors of trajectory category. Results 556 research subjects were initially enrolled,and 300 of them developed PICS. 4 types of fatigue trajectories were identified in these patients,and they were sustained fatigue group(19.00%),increased fatigue group(6.00%),fatigue remission group(27.67%)and non-fatigue group(47.33%). Logistic regression analysis showed that high APACHE Ⅱ score,advanced age and history of respiratory diseases were independent predictors of fatigue trajectory categories. Conclusion The degree of fatigue in patients with PICS shows different trajectories. Medical staff should pay attention to the evaluation and intervention of fatigue in patients with high APACHE Ⅱ scores,advanced age,and history of respiratory diseases.

    Status and influencing factors of grip strength in elderly patients with mechanical ventilation in ICU

    Objective To investigate the current status of grip strength level in elderly patients with mechanical ventilation in ICU,and to explore its influencing factors. Methods The convenient sampling method was adopted to select 218 elderly mechanical ventilation patients in ICU of a tertiary hospital in Zhejiang Province as the research subjects from January 2020 to December,and researchers completed the cross-sectional study by using a general information questionnaire,an electronic grip strength device,and the Nutrition Risk Scaling 2002. Multivariate linear regression was used to analyse the factors influencing the grip strength level of elderly patients with mechanical ventilation. Results The average grip strength level of elderly patients with mechanical ventilation in ICU was(13.8±4.7) kg. Multivariate linear regression analysis showed that gender,age,exercise mode,time of sedative drugs,and score of Nutrition Risk Scaling 2002 were the main factors influencing their grip strength level(P<0.05). Conclusion Grip strength levels were relatively low in elderly patients with mechanical ventilation in ICU. Grip strength levels were lower in patients with mechanical ventilation who were female,elderly,previously unexercised,the time of using sedation medication for >5 d,and the score of Nutrition Risk Scaling 2002≥3. Nursing staff should strengthen the dynamic evaluation of grip strength in patients with mechanical ventilation and control the related influencing factors.

    Instruments for predicting the risk of psychological disorder of ICU discharged patients:a scoping review

    Objective A scoping review was performed to examine the risk prediction instruments of psychological disorder of ICU discharged patients,in order to provide references for clinical nursing practice and further research. Methods The PubMed,Embase,Web of Science core collection,CNKI,China Biology Medicine and WanFang Database were searched from inception to July 11,2021. 2 researchers independently screened the literature,extracted the information,and assessed the risk of bias and applicability by the prediction model risk of bias assessment tool and quality assessment of diagnostic accuracy studies-2. Results 6 studies were involved in the final review,including 2 model development studies and 4 validation studies on the effectiveness of screening instruments. Conclusion The risk prediction instruments of psychological disorder of ICU discharged patients had higher predictive performance,but there was also a high risk of bias in the included studies. It is recommended that further examination,verification and development of the existing instruments should be conducted,in order to build a localized prediction instrument with low risk of bias and high applicability.

    To summarize the nursing experience of a patient with multiple endocrine neoplasia catecholamine cardiomyopathy with sudden cardiac arrest. Key points of nursing care:setting up a multidisciplinary team to formulate diagnosis and treatment plan;reducing stimulation,avoiding inducing hypertensive crisis;recognition and emergency nursing care of catecholamine cardiomyopathy crisis;nursing care of extracorporeal membrane oxygenation and blood purification life support;target temperature management. The patient’s condition was stable on the 15th day of admission and she was successfully transferred to another department for further treatment.

    Objective To understand the real experience of post-traumatic growth of family members in patients with abdominal wall stoma after radical cystectomy. Methods The descriptive phenomenological method in qualitative research was used to conduct in-depth semi-structured interviews with family members of 11 patients from the Department of Urology of a tertiary A hospital in Hangzhou. Nvivo11 software was used to conduct data analysis in classification,coding,topic distillation. Data was analyzed by Colaizzi’s seven-step analysis methods. Results 4 themes are extracted. First,the enhancement of personal strength:the enhancement of self-efficacy,the enhancement of acceptance,the enhancement of sense of responsibility. Second,changes in relationships with others:more harmonious family relationships,“altruistic for self” behavior,feel help from others. Third,new possibilities:realizing new values,looking forward to future life. Fourth,changes in life philosophy:changing the priorities of life,appreciation of life. Conclusion Family members in patients with abdominal wall stoma after radical cystectomy can experience post-traumatic growth in the process of care and present positive self-change. The clinical nurses should pay attention to the positive experience of family members,fully explore the potential and strength of the family members,and improve the quality of their care. Post-trauma growth theory should be combined with psychological nursing to promote the formation of growth paradigm.

    Objective To investigate the effects of hammock positioning on the growth and development indexes of premature infants,such as weight,body length and head circumference. Methods From January to August 2020,106 premature infants admitted to a tertiary children’s hospital in Hangzhou were selected as the research subjects. They were divided into an experimental group(n=53) and a control group(n=53) by random number table method. The control group was treated with conventional heating box. The experimental group was treated with hammock positioning in addition to conventional warm box care. The growth and development of infants in the 2 groups were compared. Results The weight gain of premature infants in the experimental group at the 2nd to 4th weeks,the 24 - hour sleep duration and the discharge weight were higher than those of the control group;the number of days required to restore birth weight and the length of hospital stay are lower than those of the control group,and the difference was statistically significant(P<0.05). After using hammock therapy,the heart rate and respiratory rate of the experimental group decreased,and the oxygen saturation increased,and the difference was statistically significant(P<0.05). Conclusion Hammock positioning plays a positive role in promoting the average weight gain per week,reducing the number of days to regain birth weight,increasing the duration of 24 hours sleep,stabilizing vital signs of premature infants and reducing the length of hospital stay,which can effectively promote the growth and development of premature infants.

    Objectives To explore the level of resilience among adolescents with Type 1 diabetes and its association with glycemic control,diabetes distress,and quality of life. Methods A total of 360 adolescents with Type 1 diabetes were recruited from the outpatient and inpatient in Endocrinology departments of 2 public hospitals in Nanjing by convenience sampling method. The data were collected by the Diabetes Strengths and Resilience Measure for Adolescents,the Problem Areas in Diabetes Scale-5,the Short Form of the Chinese version Diabetes Quality of Life for Youth Scale,and the Self-Designed General Information Questionnaire. Results The resilience of adolescents with Type 1 diabetes was 40.18±9.27,which was above the medium level. Resilience was negatively correlated with glycosylated hemoglobin value(r=-0.373,P<0.001) and diabetes distress(r=-0.336,P<0.001),while it was positively correlated with quality of life(r=0.431,P<0.001). Hierarchical regression analysis showed that resilience independently explained 10.0% of the variation in glycemic control,7.8% of diabetes distress,and 17.7% of the quality of life,after controlling for demographic and disease variables. Conclusions Interventions that target patients’ resilience may improve their glycemic control,mental health,and quality of life,which should arouse the attention of medical workers.

    Objective To explore the current situation of Traditional Chinese Medicine(TCM) nurse-led clinics in Zhejiang Province, and to provide a basis for the management and formulation of related policies. Methods From July 2020 to November 2020,our hospital used the questionnaire of TCM nurse-led clinic survey to investigate the infrastructure,operation mode and personnel qualification of the public TCM institutions in Zhejiang Province. Results 94 hospitals participated,and we got 94 valid questionnaires. The survey shows that 74.47% of hospitals have independent TCM nurse-led clinics,and 80.00% of them were opened in recent 5 years,and only 32.86% have full-time staff. 57.14% of TCM nurse-led clinics carry out over 10 suitable TCM technologies; the average daily outpatient volume was up to 200,with a wide range of diseases,including pain,sleepless,disease treatment,etc.;34.29% of TCM nurse-led clinics carry out Internet+ nursing services. Conclusion TCM nurse-led clinics in Zhejiang public TCM institutions are developing fast. It meets the health needs of patients,and promotes innovative development of TCM nursing. In the future,we should further strengthen the connotation of TCM nursing,promote relevant policies,systems and service standards,pay attention to infrastructure construction and vigorously develop TCM Internet+ nursing services.

    Objective To understand the status quo and influencing factors of safe behavior of new nurses,in order to provide a theoretical basis for managers to do a good job in the management and training of new nurses,improve their level of safe behavior,and ensure medical safety. Methods In September 2020,the convenience sampling method was used to select nurses enrolled in 4 tertiary A hospitals in Shandong Province from September 2018 to September 2019 as the research subjects. A general data questionnaire,Nurse Safety Behavior Questionnaire,Patient Safety Culture Assessment Scale,Perceived Organizational Support Questionnaire,Patient Safety Competency Self-rating Scale of Nurses were used to investigate. Results The safety behavior score of 454 new nurses was(56.56±5.19);the safety behavior of new nurses was positively correlated with patients’ safety culture perception(except pressure dimension),organizational support,total score of patients’ safety competence and all dimensions,and negatively correlated with the pressure dimension of patients’ safety culture perception;AMOS analysis showed that patients’ safety culture perception and patient safety competence had a direct positive impact on the safety behavior of new nurses(β=0.246,β=0.048,both P<0.001). Bootstrap analysis showed that organizational support and patient safety competence had significant mediating effects on patients’ safety culture perception and nurses’ safety behavior(95%CI 0.031-0.091). The total effect of new nurses’ patients safety culture perception on nurses’ safety behavior was 0.242(P<0.001,95%CI:0.207-0.277),and the direct effect was 0.094(P<0.001,95%CI:0.049-0.140). The indirect effect was 0.148(P<0.001,95%CI:0.085-0.215),and the mediating effect accounted for 61.16% of the total effect value. Conclusion Organizational support and patient safety competency play a mediating role between patient safety culture perception and safety behavior of new nurses. The hospital should strengthen cultural awareness training for new nurses security,strengthen occupational stress management. The safety cognitive level and safety competency of the new nurses can be improved by the construction of a positive safety culture atmosphere,so as to raise the safety behavior level of new nurses.

    Objective To develop a New Nurse Leaders’ Job Readiness Scale based on the competency quality model,and to test its reliability and validity. Methods Based on the conceptual framework of competency quality model,we employed the literature review method,brainstorming method,on-site observation method,semi-structured in-depth interview method,and expert consultation method to construct the first draft of the scale. 189 new nurse leaders in Wuhan tertiary hospitals were surveyed from May 2019 to August 2021 to test the reliability and validity of the scale. Results The scale contained 4 primary indicators,6 secondary indicators and 29 entries. 6 common factors were extracted by exploratory factor analysis,which explained 86.90% of the total variables. The content validity index of the scale was 0.974,and the content validity index of each entry ranged from 0.712 to 1.000. The Cronbach’s α coefficient of the scale was 0.965,and the Cronbach’s α coefficient of the 6 factors ranged from 0.808 to 0.971,with a split-half reliability of 0.827 and a retest reliability of 0.749. Conclusion The reliability and validity of the New Nurse Leaders’ Job Readiness Scale are good,and it can provide the guidance for new nurse leaders to prepare for their job.

    Objective The position competence level and influencing factors of sterilization personnel in Central Sterile Supply Department(CSSD) were investigated and analyzed to provide references for the standardized construction of sterilization personnel team in CSSD. Methods In June 2021,the convenience sampling was adopted to investigate 487 sterilization personnel in 238 hospitals in Henan province by a self-designed questionnaires on the competency of sterilization personnel in the sterilization supply center platform of Henan Hospital Central Sterile Supply Quality Control Center. Results The total score of the post competence of sterilization personnel in CSSD was(60.58±19.74); regression analysis showed that the hospital level,sterilization working years,to the history of receiving training,the type of training received and the frequency of training were the main influencing factors for the post competency of sterilization personnel in CSSD. Conclusion The post competence of sterilization personnel in CSSD of Henan hospital is at a medium level,and those hospitals with lower level and no pressure steam sterilizer training have lower post competencies. The manager of CSSD should formulate targeted professional personnel training plans according to the weak competency factors of sterilization personnel,so as to improve the competency of sterilization personnel in CSSD,to ensure the quality of sterilization and the safety of patients.

    Development of an Assessment Scale of Aspiration Risk for Nasal Feeding Patients and its test of validity and reliability

    Objective To compile an Assessment Scale of Aspiration Risk for Nasal Feeding Patients,and to test its reliability and validity,aiming to provide a scientific and objective aspiration risk assessment tool for doctors and nurses. Methods The draft of the Assessment Scale of Aspiration Risk for Nasal Feeding Patients was constructed based on the literature analysis and 2 rounds of Delphi consultations. Using convenience sampling,the data of 122 patients with nasal feeding from November 2020 to March 2021 in tertiary A hospital of Shenzhen was collected,and item screening and the test of reliability and validity were carried out. Results The Assessment Scale of Aspiration Risk for Nasal Feeding Patients includes 3 dimensions,10 first-level items and 22 second-level items. The exploratory factor analysis extracts 3 common factors,and the cumulative variance contribution rate is 61.699%. The load value of common factors of each item is between 0.473 to 0.931. The correlation coefficient of the effective standard(whether aspiration occurs) is 0.739;the area under the ROC curve for predicting the risk of aspiration in nasal feeding patients is 0.961,and the confidence interval of 95% is between 0.929 to 0.993(P<0.001);the content validity is 0.953;the Cronbach’s α coefficient of the scale is 0.739. Conclusion The Assessment Scale of Aspiration Risk for Nasal Feeding Patients has good reliability and validity,and it can be used as a tool in clinical practice to assist in the identification and prediction of aspiration.

    Application of cognitive interview in cultural adjustment of Attitudes Related to Trauma-Informed Care Scale

    Objective Cognitive interviews were used to explore the Chinese target population’s cognition and understanding of Attitudes Related to Trauma-Informed Care Scale(ARTICS) in the Chinese context. The purpose was to verify the consistency of the language expression of the revised ARTICS and the original scale. Methods Using the purpose sampling method,26 psychiatric nurses who met the inclusion criteria were selected as interviewees from a secondary mental health centre in a district of Shanghai. 2 rounds of cognitive interviews were conducted to explore the respondents’ understanding of the items of the Chinese version of ARTICS. The items with ambiguities were revised according to the interview results to form the Chinese version of ARTICS. Results ARTICS consists of 35 items across 5 dimensions. The results of the first round of interviews showed that some respondents had ambiguities in the semantic expression and understanding of the 6 items in the scale,and these items were modified after discussion. The results of the second round of interviews showed that the content of the revised items was semantically consistent with the original scale. Conclusion The use of cognitive interview can help the scale adapt to different cultural background,improve the target population’s understanding and acceptance of scale items,reduce measurement errors caused by misunderstanding of certain items,and improve the response rate of the scale.

    Incidence of peripherally inserted central catheter-related thrombosis in Chinese adult cancer patients:a meta-analysis

    Objective To systematically review the incidence of peripherally inserted central catheter(PICC)-related thrombosis in Chinese adult cancer patients. Methods PubMed,Embase,the Cochrane Library,Web of Science,CBM,CNKI,WanFang Data and VIP databases were electronically searched to collect studies on the incidence of PICC-related thrombosis in Chinese adult cancer patients from inception to May,2021. 2 reviewers independently screened the literature,extracted the data and assessed the risk of bias of included studies,and then the meta-analysis was performed by Stata 14.0 software. Results A total of 23 pieces of the literature involving 15 066 cancer patients were included. Meta-analysis results showed that the total incidence of PICC-related thrombosis in Chinese adult cancer patients was 8.82%[95%CI(5.93%~12.18%)]. Subgroup analysis showed that the incidence of PICC-related thrombosis was higher in the publication year from 2019 to 2021. The incidence was also higher in patients with age≥ 60 years old,BMI>25,cephalic vein puncture,traditional puncture method,the number of punctures more than 1,catheter size of 5 Fr,lung cancer,cancer stage of Ⅲ~Ⅳ,diabetes mellitus,history of PICC catheterization,history of venous thrombosis,fibrinogen content≥4 g/L before catheterization and D-dimer level≥0.5 mg/L before catheterization. Conclusions The incidence of PICC-related thrombosis in Chinese adult cancer patients is high,so health workers should pay more attention to early screening and prevention of high-risk patients.

    Quality appraisal of family resilience assessment tools for cancer patients

    Objective To evaluate the quality of family resilience assessment tools for cancer patients, so as to provide references for the selection of high-quality research tools. Methods We searched databases including PubMed,the Cochrane Library,Web of Science,Embase, PsycINFO, CINAHL, CBM, CNKI, Wanfang, VIP to collect all studies about assessment tools that measure cancer patients’ family resilience. Based on the consensus-based standards for the selection of health measurement instruments(COSMIN) guideline,the methodological and measure-ment properties quality of the included assessment tools were evaluated by the COSMIN risk of bias checklist and quality criteria,and the modified grading of recommendations assessment, development of evaluation(GRADE) was used to classify the grade of evidence and recommendations of the tools. Results None of the 10 assessment tools reported criterion validity, measurement error or responsiveness. The GRADE of Family Resilience Assessment(FRA) and Family Resilience Questionnaire(FaREQ) were high. The recommendation level of Family Resilience Assessment Scale(FRAS), FRA and FaREQ were A, and the others were B. Conclusion FRAS and Chinese version of Family Resilience Assessment Scale(C-FRAS) translated by Fan are the most widely used family resilience assessment tools for cancer patients at home and abroad. FRAS, FRA and FaREQ have good methodological quality, measurement properties quality and evidence level, so they are recommended for use in cancer patients.

    Breast cancer is the most common malignant tumor in the world,which brings the heavy physical and mental afflictions to patients. Internet-based cognitive behavioral therapy is a new type of internet medical method that employs the internet platform to give patients cognitive behavioral treatment,in order to reduce the physical and psychological burden of patients. This review introduced the advantages of internet-based cognitive behavioral therapy,application of this method in breast cancer patients in other countries,and proposed the future exploration focus of internet-based cognitive behavioral therapy,so as to provide a reference for our country to carry out related studies.

    Objective To analyze the development of transitional care and its application,attributes cases,antecedents and consequences through literature review,so as to clarify the concept of transitional care in China from the perspective of nursing practice. Methods CNKI,Wanfang,PubMed,CINAHL and PsycINFO databases were searched systematically,and Rorgers evolutionary concept analysis was used as the theoretical framework for analysis and discussion. Results Transitional care had the following 5 attributes,namely patient-centered,transferring,continuity,coordination and educational support. The antecedents of transitional care included factors related to patients and their families,hospital-related factors and social factors. Quality of life,self-efficacy and length of stay in hospital were the consequences observed.The measurement indicators are mostly patients’ transition preparation,patient knowledge improvement,self-efficacy and treatment compliance. Conclusion Transitional care is a dynamic concept. With the development of the times,it is constantly updated and improved. In the future,the connotation of transitional care needs to be further expanded and analyzed in depth,which is conducive to the further development of transitional care.

    Objective To sinicize the Technophobia scale and evaluate its validity and reliability in older adults. Methods The expressions of the Chinese version of the Technophobia scale were determined through translation,back translation and cross-cultural adaptation. Totally 431 elderly people were recruited for questionnaire survey. Results The Chinese scale includes 13 items. Findings from the exploratory factor analysis revealed that 3 factors including techno-anxiety,techno-paranoia and privacy concerns were extracted and explained 67.24% of the cumulative variance. In validated factor analysis,the three-factor model fitted the data well. Cronbach’s alpha coefficient for the overall scale was 0.911;the Cronbach’s α coefficient of the 3 factors was 0.759~0.885;the half-reliability coefficient was 0.851. Conclusion The Technophobia scale has been proved to be reliable and valid,which can be used to assess the level of technophobia,and it can further provide a reference for future interventions for technophobia and individualized digital health services among the elderly population.
