    Expert consensus on parenteral nutrition safety infusion

    Objective To form an expert consensus on parenteral nutrition safety infusion, and standardize the operation process of clinical parenteral nutrition safety infusion,so as to reduce the parenteral nutrition complications. Methods On the basis of the evidence-based method,we searched the high level evidence about the parenteral nutrition safety infusion in databases such as UpToDate,BMJ Best Practice,National Institute for Health and Care Excellence,Guidelines International Network,National Guideline Clearinghouse,Registered Nurses' Association of Ontario,Scottish Intercollegiate Guidelines Network,European Society for Clinical Nutrition and Metabolism,American Society for Parenteral and Enteral Nutrition,Chinese Society for Parenteral and Enteral Nutrition,China Guide Network,Joanna Briggs Institute,China Biomedical Literature Network,VIP,Wanfang,CNKI,PubMed,Web of Science,Scopus,Embase. The retrieval period is from the establishment of the databases to January 2021. Studies were evaluated and collected; relevant recommendations and research results were selected to form the draft of the expert consensus. Experts were invited to complete 2 rounds of Delphi expert consultations and 2 rounds of expert meetings. Each item was revised and improved through 2 rounds of Delphi expert consultations and expert meetings,and the final draft of the consensus was formed. Results The positive coefficient of experts in the 2 rounds of correspondence consultations were 100%; the expert authority coefficients were 81.00% and 89.50% respectively,and the coordination coefficients of expert opinions were 0.13 and 0.15 respectively. Finally,the consensus summarizes the latest evidence on 4 aspects of multidisciplinary team formation,evaluation,operation norms,and complication management. Conclusion The establishment of the consensus can provide a reference for the standardization of safe infusion of parenteral nutrition in clinical practice to ensure patient safety and improve service quality.

    Nursing guidelines for intravenous thrombolysis in acute ischemic stroke

    Objective To develop an intravenous thrombolysis nursing guideline for acute ischemic stroke as a guiding tool for the implementation of intravenous thrombolysis for acute ischemic stroke,and to provide a reference for the practice and management of nursing in the standardization of thrombolytic therapy. Methods According to the method of evidence-based nursing practice guidelines,the draft of intravenous thrombolysis nursing guidelines for acute ischemic stroke was constructed. The draft guidelines were reviewed by experts using the catalogue of Clinical Guidelines Research and Evaluation System Ⅱ. According to the results and suggestions of expert review,the guidelines were modified to form the formal guidelines. Results The recommendations included 5 aspects,namely hospital organization management,disease observation,observation and treatment of complications,posture and bed-leaving activities,and quality management. Conclusion The guidelines for intravenous thrombolysis nursing in acute ischemic stroke are evidence-based guidelines based on the best evidence,clinical practice and the judgment of professionals,and can be used as a practical basis for clinical medical staff to make scientific decisions on intravenous thrombolysis therapy in patients with acute ischemic stroke.

    Expert consensus on risk assessment and prevention of venous thromboembolism in general surgical patients

    Objective To explore the expert consensus on risk assessment and preventive nursing of venous thromboembolism(VTE) in general surgical patients. Methods This consensus was drafted through a systemic literature review on VTE prevention and the interviews of nursing experts. The draft was thereafter reviewed by a total of experts in related fields,and it was finalized after 2 rounds of expert consultation. Results 4 domains with 26 components were established through a modified Delphi method. The effective recovery of the ques-tionnaire was 100%;the expert judgment coefficient was 0.960;the familiarity was 0.912;the authority coefficient was 0.936. The Kendall coefficients of the first and second level topics in the 2 rounds were 0.149 and 0.261,and 0.122 and 0.203,respectively,with statistically significant differences(P<0.05). The consensus includes risk factors for VTE in general surgical patients,risk assessment,risk response,education and quality control,and it is applicable to perioperative patients of general surgery. Conclusion The consensus was based on the recommendations of 25 clinical nursing experts. The results are scientific,rigorous and authoritative,and can provide guidance for clinical nursing staff in the prevention and control of VTE in general surgical patients.

    Clinical practice guideline for nursing management of iodinated contrast media extravasation

    Objective To develop “clinical practice guideline(CPG) for nursing management of iodinated contrast media extravasation(ICME)” and to provide a reference for reducing the incidence and standardizing the management of ICME. Methods The CPG for nursing management of ICME strictly followed the method of developing evidence-based nursing practice guideline. The draft of this CPG was appraised according to Appraisal of Guidelines Research and Evaluation Ⅱ(AGREE Ⅱ),and then it was finalized based on the results and suggestions of expert reviews. Results Recommendations for the constructed CPG included 4 aspects of risk factors,prevention methods,management principles,and quality improvement in ICME. Conclusion The “CPG for nursing management of ICME” was an evidence-based guideline based on the best evidence,Chinese clinical reality,and professional judg-ment,and it can provide practice bases for scientific clinical decision making by radiology and clinical medical staff.

    Expert consensus on safety management of transfusion connection devices for vascular access

    Objective The aim is to establish an expert consensus(hereinafter referred to as "Consensus")on safety management of transfusion connection and port protection devices for vascular access,to prevent catheter related bloodstream infection and provide patients with safe,effective and standardized intravenous therapy. Methods Using evidence-based methods,according to the level of evidence,the evidence in this field was searched,evaluated and summarized,and relevant recommendations and research conclusions were extracted,and then the first draft of consensus was formed. Through a round of Delphi expert consultation and 2 rounds of expert meetings,opinions from domestic experts in many fields and disciplines were sought and integrated;the content was adjusted and the consensus was reached. Results The positive coefficient is 100%;the authoritative coefficient is 0.92;the average value of each index is more than 4;the coefficient of variation is less than 0.22. The Kendall’s harmony coefficient of the expert’s opinion is 0.35( χ2=144.44,P<0.001). A consensus recommendation on the classification and maintenance of infusion connection devices was reached. Conclusion The Consensus is scientific,which can provide a reference for the safety management of transfusion connection and port protection devices in China,and provide a basis for clinical decision-making to prevent catheter related bloodstream infection.

    Expert consensus on the implementation and removal of protective restraints in psychiatry

    Objective To form an expert consensus on the implementation and removal of psychiatric protective restraints,and to standardize the related contents of the implementation and removal of psychiatric protective restraints. Methods According to the level of evidence,the evidence-based method was used to retrieve,evaluate and summarize the evidence in the field. The recommendations and research conclusions for the implementation and removal of the psychiatric medical protective restraints were extracted,and the rough consensus was formed. Through 2 rounds of enquiry and 2 experts discussion meetings,the final version of the consensus was formed by combining with the expert opinions,adjusting,modifying and perfecting the first draft. Results The positive coefficients of experts in the 2 rounds of correspondence consultation were 100%;the degree of authority of experts was 0.90;the mean value of importance of each index was>3.5,and the coefficient of variation was less than 0.25,and the expert Kendall harmony coefficient was 0.146 and 0.166,respectively (all P<0.01). Finally,a consensus was reached on the recommendation of 10 parts,including the application group of psychiatric restraints,premise and principle,objective,key points of assessment,basic requirements,record sheet,flow chart,intervention strategies in the implementation process,ethical issues and risks,and suggestions. Conclusion The consensus provides guidance for the practice of psychiatric protective restraints and makes psychiatric restraints more standardized.

    Objective To form an expert consensus of enteral feeding and nursing care for patients with severe neurological diseases,and to promote the standardized,safe and effective implementation of enteral feeding by neuro intensive nursing personnel,so as to provide effective nutritional support for patients. Methods The evidence in this field was searched,evaluated and summarized to extract the recommendations related to enteral feeding in patients with severe neurological diseases. The first draft of consensus was formed. Through 2 rounds of Delphi expert consultations and expert demonstration meetings,the expert suggestions were analyzed,modified and improved to form the final draft of consensus. Results The positive coefficient of inquiry experts was 100%;the coefficient of expert judgment was 0.93;the degree of familiarity was 0.88;the coefficient of authority was 0.91. The concordance coefficient of Kendall was 0.256~0.278,which was statistically significant(P<0.05). The final consensus included the evaluation of enteral feeding care for patients with several neurological illness,selection of feeding methods,nutrient solution infusion,identification and care of complications,and indicators for terminating tube feeding. Conclusion This consensus provides guidance for clinical nursing practice and quality control of enteral feeding in critically ill patients.

    Expert consensus of suicide care interventions for psychiatric inpatients with depression

    Objective To form an expert consensus on suicide nursing interventions for inpatients with depression in psychiatric departments,standardize the contents of suicide nursing interventions for depression patients in psychiatric departments. Methods Evidence-based methods and literature analysis were used to extract recommendations and research conclusions of suicide nursing interventions for inpatients with depression,and the consensus draft was formed. After 2 rounds of expert letter consultation and 2 expert discussions,the draft was adjusted,modified and improved,and the final draft of the consensus was formed. Results The positive coefficients of the experts in 2 rounds of letter consultation were 100%;the authority degree of the experts was 0.924;the mean values of importance of each index were >3.5 points;the coefficient of variation was<0.25;the Kendall harmony coefficients of the experts were 0.182 and 0.260,respectively(P<0.01). Finally,the operational definition,suicide risk assessment,intervention form and time,theoretical basis of intervention,general principles of intervention,suicide impression intervention,suicidal behavior intervention,key points of intervention in special populations,evaluation of intervention effect and post-discharge evaluation of suicide care in hospitalized patients with depression were analyzed. A total of 11 components of the health education program were agreed upon. Conclusion The consensus provides the guidance for the suicide nursing interventions of psychiatric inpatients with depression,so that the suicide nursing intervention of psychiatric inpatients with depression is more standardized.

    Objective The aim of this work is to establish the expert consensus(hereinafter referred to as“consensus”) on clinical practice of injection safety in hemodialysis,to standardize the clinical practice of injection safety in hemodialysis and ensure patient safety during hemodialysis treatments. Methods Reviewing relevant literature and evidence-based nursing,summarizing the clinical work experience of care,we seeked for the best clinical evidence of injection safety. The consensus was performed by adjusting,modifying and perfecting each entry through three-round Delphi expert consultations and discussions. Results The consensus summarizes the latest evidence in 9 aspects,including environmental,personnel,syringes,safe administration of drugs,safe use of disinfectants,nursing practices on vascular access,specimen collection,medical waste and occupational safety,which covered all the elements and links in the hemodialysis therapy. In the three-round Delphi expert consultation and discussion,the positive coefficients were all 100% and the expert authority coefficients were all 0.928. The importance assignments were 4.38,4.45 and 4.68,respectively. The experts reached a fair degree of agreement. There were significant differences in experts’ coordination coefficients in the 3 rounds(P<0.05). Conclusion The consensus shows certain scientific natrue,and it may provide evidence for injection safety in hemodialysis therapy in China.

    Objective To form the Chinese expert consensus on cardiac rehabilitation nursing care,so as to standardize the work related to cardiac rehabilitation nursing. Methods The first draft was initially formed,based on a comprehensive review of domestic and foreign cardiac rehabilitation guidelines,expert consensuses,systematic reviews,original research and other literature,as well as related disease diagnosis and treatment plans,management consensuses and related literature. Results The draft was thereafter reviewed by a total of 37 experts in related fields. This consensus was finalized after 3 rounds of discussions and modifications. The expert consensus covers the following 4 topics,namely the work goals of cardiac rehabilitation nursing care,the qualifications,training,core competencies and responsibilities of cardiac rehabilitation nursing care,the components of cardiac rehabilitation nursing care,and quality control of cardiac rehabilitation nursing care,and the safety management of cardiac rehabilitation patients. Conclusion The consensus was formed on the basis of comprehensive expert opinions and relevant guidelines,consensuses and clinical research on cardiac rehabilitation,which can provide guidance for medical institutions to carry out cardiac rehabilitation nursing practice.

    Expert consensus on packaging techniques of reusable medical instruments

    Objective To standardize the packaging techniques of reusable medical instruments in central sterile supply departments. Methods From October 2021 to August 2022,the Professional Committee of the Central Sterile Supply Department of Chinese Nursing Association addresses the key problems of reusable medical instruments packaging. On the basis of clinical practice,literature analysis and the operation experience of the national central sterile supply department experts,we made the initial version of Expert Consensus on Packing Techniques of Reusable Medical Instruments(hereinafter referred to as the Consensus). Experts were invited to complete 2 rounds of Delphi expert consultations and 3 rounds of expert meetings. Each item was adjusted,revised and improved,and the final version of the Consensus was formulated. Results The positive coefficients of experts in 2 rounds of Delphi expert consultations were all 100%,and the degrees of expert authority were 0.957 and 0.973,respectively. The average importance value of each item was higher than 4.00;the variation coefficient of each item was less than 0.25. The Kendall’W were 0.181 and 0.338,respectively(all P<0.001). Finally,an agreement was reached on the terms and definitions,basic principles,personnel requirements,environmental requirements,packaging equipment requirements,packaging materials and sterilization monitoring material requirements,packaging auxiliary tools requirements,pre-packaging evaluation,packaging operation and packaging quality confirmation of the reusable medical devices packaging techniques. Conclusion The Consensus covers all aspects of the reusable medical instruments packaging techniques,which is highly practical and scientific,and it also helps to guide the sterile supply professionals to standardize the implementation of packaging techniques,improve the quality of aseptic supplies,and ensure the quality of medical treatment and the life safety of patients.

    Evidence summary for subcutaneous injection of low molecular weight heparin

    Objective To retrieve,evaluate and integrate evidence related to subcutaneous injection technique of low molecular weight heparin. Methods The clinical decisions,recommended practices,evidence summaries,technical reports,guidelines,expert consensuses,and systematic evaluations on the subcutaneous injection of low molecular weight heparin in domestic and foreign databases and related professional websites were searched by computer. The evidence was extracted and summarized according to the subject after the independent literature quality evaluation by 2 researchers. The literature retrieval time limit was from the establishment of the database to November 2021. Results According to the inclusion criteria,9 pieces of literature were screened,including 2 evidence summaries,2 expert consensuses,and 5 systematic reviews. Through literature reading,evidence extraction and classification,a total of 12 pieces of evidence were summarized,including preparation before injection,skills during injection,and post-injection disposal. Conclusion This study summarized the best evidence of subcutaneous injection techniques of low molecular weight heparin,so as to provide references for nurses to carry out clinical practice. Nurses should carefully select and apply evidence in combination with clinical situations and patients’ wishes,so as to ensure injection safety and reduce the incidence of related complications.

    Study on the development and application effect of an intelligent negative emotion improvement program for patients with coronary heart disease

    Objective To evaluate the efficacy of an intelligent negative emotion improvement program for patients with coronary heart disease(CHD). Methods Patients with CHD complicated with negative emotion admitted in the Department of Cardiology of a tertiary hospital in Shanxi Province from February to September 2020 were selected as subjects of the study. 38 patients admitted from February to May 2020 were included in the control group and treated with routine psychological nursing. 39 patients in the experimental group were included from June to September 2020 and treated with the intelligent negative emotion improvement program. The scores of self-rating anxiety scale,self-rating depression scale,Pittsburgh sleep scale and nurses’ satisfaction were compared between the 2 groups before and after the intervention. Results After the intervention,the scores of Self-rating Anxiety Scale,Self-rating Depression Scale and Pittsburgh Sleep Scale in the experimental group were lower than those in the control group,and the difference was statistically significant(P<0.05). Conclusion The implementation of the intelligent negative emotion improvement program for patients with CHD can improve patients’ anxiety and depression,improve their sleep quality.

    The status and influencing factors of kinesiophobia in elderly patients with chronic heart failure

    Objective To investigate the status and influencing factors of kinesiophobia in elderly patients with chronic heart failure (CHF) . Methods A total of 260 elderly CHF inpatients were recruited from the Department of Cardiology,Affiliated Hospital of Yangzhou University from October 2020 to July 2021. General data questionnaire,Chinese version of the Tampa Scale for Kinesiophobia Heart,Symptom Status Questionnaire-Heart Failure,Self-Efficacy for Exercise,Social Support Rating Scale and Medical Coping Modes Questionnaire were used for investigation. Univariate analysis and binary Logistic regression were performed to analyze the influencing factors. Results In 260 elderly patients with CHF,the incidence of kinesiophobia was 65%.Logistic regression analysis showed that education level of junior high school or below (OR=3.369,95%CI:1.249-9.091) ,course of disease <5 years (OR=2.908,95%CI:1.303~6.487) ,heart failure symptoms (OR=1.083,95%CI:1.036~1.132) ,exercise self-efficacy (OR=0.425,95%CI:0.234~0.772) ,coping mode (confronting) (OR=0.878,95%CI:0.790~0.975) ,coping mode (avoidance) (OR=1.222,95%CI:1.062-1.406) and coping mode (resignation) (OR=1.327,95%CI:1.095-1.608) were the influencing factors of kinesiophobia in elderly CHF patients. Conclusion Elderly patients with CHF have a high incidence of kinesiophobia,and patients’ education background,course of disease,heart failure symptoms,exercise self-efficacy and coping mode are influencing factors,among which educational background has the greatest impact.Clinical nursing staff can focus on elderly CHF patients with low educational level and a long course of disease,and can also develop intervention measures to promote them to participate in physical activities and exercise to improve their exercise endurance.

    Objective To establish a consensus on perioperative urospesis nursing of patients with intracavitary surgery for urinary tract calculi. Methods A consensus developing group was established,and the first edition of the "Expert consensus on perioperative urosepsis nursing of patients with intracavitary surgery for urinary tract calculi"(hereinafter referred to as the "consensus") was formed after a large amount of literature review and collation,online expert consultation and group discussion. Domestic medical experts from urology,minimally invasive operating rooms and intensive care units were invited to discuss and revise the "consensus" for 2 rounds,and then peer review was conducted,and finally the "consensus" was completed. Results The final version of the "consensus" comprises 3 major sections,namely the preoperative,intraoperative and postoperative,which covers high-risk factors,prevention,identification,condition monitoring and nursing intervention,with a total of 21 items. The positive coefficients for the 2 rounds of consultations were 100%. The coefficients of judgment,familiarity and authority in the 2 rounds of consultations were 0.975,0.960 and 0.968,respectively. In the 2 rounds of expert consultations,the importance assignments of each items were(4.847±0.304) and(4.888±0.212),and the overall Kendall’s coefficients were 0.232 and 0.287,respectively. Conclusion The "consensus" integrates the suggestions of multidisciplinary experts with a certain degree of practicality,representativeness and scientificity. It can provide guidance for clinical nursing practice of patients with perioperative urosepsis of intracavitary surgery for urinary tract calculi.

    Construction and application of a traditional Chinese medicine continuing care program for patients with acute ischemic stroke

    Objective To construct a traditional Chinese medicine(TCM) continuous care program and to evaluate its application effect in discharged patients with acute ischemic stroke. Methods A total of 102 patients with acute ischemic stroke hospitalized in the Department of Encephalopathy of a tertiary hospital in Guangxi from June 2019 to June 2020 were selected as the research subjects by convenience sampling method. They were randomly allocated into an experimental group(n=51) and a control group(n=51).The experimental group was carried out with the TCM continuous care program on the basis of routine nursing,while the control group received the routine nursing. At the time of discharge and 1 month,2 months and 3 months after discharge,the 2 groups of patients were investigated and evaluated by a simple Fugl-Meyer Assessment Scale(FMA) and Stroke Specific Quality of Life Scale(SS-QOL). Results There was no significant difference in FMA score and SS-QOL score between the experimental group and the control group at the time of discharge(P>0.05). A month after discharge,there was no significant difference in the FMA score between the experimental group and the control group(P>0.05),but the SS-QOL score in the experimental group was higher than that in the control group(P<0.05).The scores of FMA and SS-QOL in experimental group were significantly higher than those in the control group at 2 and 3 months after discharge(P<0.05). 3 months after discharge,the clinical efficacy of the experimental group was significantly better than that of the control group(P<0.05). The analysis of variance of repeated measurement data showed that the interaction between groups,time,and inter-groups and time had statistically significant effects on FMA score and SS-QOL score(P<0.05). Conclusion The constructed TCM continuous care program is scientific and feasible,which can effectively improve the limb motor function of patients with acute ischemic stroke and improve their quality of life at home.

    Objective To form an expert consensus on family meeting in palliative care and provide references for the staff engaged in palliative care to implement family meeting. Methods A consensus drafting group was set up to systematically search,evaluate and extract evidence related to family meeting in palliative care to form a draft consensus. After 2 rounds of Delphi expert consultation and expert demonstration meetings,expert opinions were sorted out and analyzed,the rough draft was revised,and then the final consensus draft was formed. Results The positive coefficient of the first round of expert consultation was 92.11%,and the second round was 100%. The coefficient of expert judgment was 0.95;the degree of familiarity was 0.89;the degree of authority coefficient was 0.92. The Kendall’s coefficients of the 2 rounds of consultation were 0.157 and 0.253,which were statistically significant(P<0.001). The final consensus consisted of 8 aspects,including the scope,terms and definitions,basic requirements,timing of family meetings,preparation before meetings,implementation of meetings,records and follow-up,and precaution. Conclusion The consensus is practical,and it can provide guidance for the standardized implementation of family meeting in palliative care.

    Effect of resistance exercise program on muscle strength and cardiopulmonary function in elderly bedridden patients

    Objective The aim of this study is to establish resistance exercise in the bedridden elderly in China and to study its effect on muscle strength and cardiopulmonary function. Methods This RCT consecutively enrolled 64 patients aged 80 years and above with stable diseases at a tertiary hospital during March to April 2020. The patients were randomly divided into a control group(received routine treatment and nursing care) and a training group(undergoing 6 months of low-load resistance training with routine treatment and nursing care). The differences in muscle strength and cardiopulmonary function between the patients of 2 groups were analyzed. Results 64 patients were included in the study,while 59 patients(with 29 in the training group and 30 in the control group) completed the study. In terms of muscle strength,the patients of the training group had better grip strength compared with that of the control group at 3 and 6 months(P<0.05),and better supine leg lifts and 30 s sit-to-stand actions than those of the control group at 6 months(P<0.05). In terms of cardiopulmonary function,the patients in the training group had better lung capacity compared with that in the control group at 6 months(P<0.05). There were no significant differences in blood pressure,heart rate and left ventricular ejection fraction between the 2 groups(P>0.05). Conclusion The low-load and low-intensity resistance training using elastic band and ball can effectively improve the muscle strength of the bedridden elderly,as well as the lung function to a certain extent. However,the effect on improving the cardiac function is not obvious.

    Objective To compile an expert consensus on prone position nursing for conscious adult patients with acute respiratory distress syndrom(here in after referred to as the “consensus”),aiming to promote and standardize the implementation of awake prone position in hospitals at different levels. Methods The first draft of the consensus was formed by reviewing existing literature,conducting clinical stakeholder interviews and expert meetings,followed by 2 rounds of expert correspondence,and recommendations were revised based on expert opinions,and the evidence and recommendation levels were scientifically graded. Results After 2 rounds of consultations with 20 experts,the recommendations were finally formed. The effective return rate of the questionnaire was 100%,and the expert authority coefficient was 0.90. In the first round of consultation,the frequency of full marks for necessity is 95%~100%,and the coefficient of variation of recommendation approval degree is 0~0.227. In the second round,the average value of the degree of approval rating has increased,ranging from 4.65 to 5.00,and the frequency of full marks for necessity is 100%,and the coefficient of variation is 0~0.201. The Kendall W coefficients of the 2 rounds of inquiry were 0.136 and 0.168,and the difference was statistically significant(P<0.05). The consensus includes the formation of multidisciplinary teams,nursing evaluation,implementation key points,monitoring and management,health education,prevention of adverse events,management of special populations on the obesity people,and nursing quality indicators. Conclusion This consensus combines the existing evidence,clinical needs and the recommendations of medical and nursing experts,and it can provide guidance for patients to carry out conscious prone position as early as possible.

    Objective To understand the motivation of blood glucose management behavior changes in pregnant women with gestational diabetes mellitus(GDM),in order to provide suggestions and evidence for promoting good blood glucose management in GDM patients. Methods Purposive sampling method was used to select 28 pregnant women with gestational diabetes mellitus from September to December 2020 in 4 hospitals in Shanghai for semi-structured in-depth interviews. Content analysis was used to analyze and refine the theme. Results The motivation of blood glucose management behavior change of GDM pregnant women includes perceiving the severity of the disease,perceiving the risk of bad blood glucose management behaviors,and perceiving the benefits of healthy blood glucose management behavior. Conclusion Clinical nursing staffs can improve the awareness of GDM pregnant women on the severity of disease and the risk of bad blood glucose management behaviors,and improve the awareness of GDM pregnant women on healthy blood glucose management behaviors,so as to improve their motivation to change their poor blood glucose management behavior and unhealthy lifestyles.

    Evidence-based practice in prevention of fecal incontinence associated dermatitis in ICU patients

    Objective To summarize the evidence for the prevention of fecal incontinence associated dermatitis in ICU patients and to evaluate its application effect. Methods To obtain the best evidence,we used the method of evidence-based nursing to search related guidelines,expert consensuses and systematic reviews from domestic and foreign databases from January 2010 to September 2019. The best evidence was evaluated and summarized;review indicators were developed;the baseline review was conducted,and facilitation and obstacles based on the review results were analyzed. From May to September in 2020,the evidence was applied to clinical practice,and the incidence of incontinence associated dermatitis,nurses’ cognitive level of incontinence associated dermatitis knowledge and the implementation rate of review items were compared before and after the application of evidence-based practice. Results Finally,8 articles were included,including 3 clinical practice guidelines,2 expert consensuses and 3 systematic reviews. 11 pieces of best evidence were summarized in terms of risk assessment and prevention strategies. After evidence-based practice,the incidence of incontinence associated dermatitis was 19.10%,which was lower than that before evidence-based practice (40.48%) with statistically significant differences(P<0.05);the score of nurses on incontinence associated dermatitis knowledge questionnaire was(42.35±5.77),which was higher than that before evidence-based practice (35.65±5.07),and the difference was statistically significant (P<0.001). The implementation rates of items 2,4,5,6 increased from 0,0,66.67% and 22.62% to 100%,84.27%,97.75% and 76.40%,and the differences were statistically significant(P<0.001). Conclusion The application of evidence-based practice in the prevention of fecal incontinence related dermatitis in ICU patients can reduce the incidence of incontinence associated dermatitis,improve nurses’ cognitive level and the implementation compliance.

    Application comparison of different tip positions of midline catheters

    Objective To compare the clinical effects of midline’s tip at different position. Methods From September 2020 to January 2021,a multi-center randomized controlled study was used to select 384 inpatients as the research subjects from 6 tertiary A general hospitals in Zhejiang province,Fujian province,Jiangsu province,and Liaoning province. They were randomly divided into 3 groups according to random numbers generated by Excel. The midline’s tips were located in the subclavian vein in the experimental group 1,in the axillary vein of the chest in the experimental group 2,and at the distal of the axillary vein in the control group. The incidences of catheter-related complications(including phlebitis,catheter-related thrombosis,catheter-related infections,catheter occlusion,catheter dislodgment,bleeding,oozing) were compared in 3 groups,as well as unplanned removal rate due to complications,catheter indwelling time,and the incidence of abnormal intima after midline removal. Results A total of 384 patients were included. The total incidence of catheter-related complications in experimental group 1,experimental group 2,and control group were 9.93%,14.63%,and 34.17%. Unplanned removal rate due to complications were 2.13%,4.07%,and 13.33% in 3 groups. Catheter indwelling time were 12.00(8.00,19.75) d,12.00(8.00,21.00) d,and 10.00(6.00,17.75) d. After the removal,the incidence of abnormal intima were 0.75%,1.69%,and 6.36%. The differences of 3 groups were statistically significant(P<0.05). Among them,the differences in the total incidence of catheter-related complications and unplanned removal rate between the experimental group 1 and the control group,the experimental group 2 and the control group were statistically significant(P<0.017);the difference between the experimental group 1 and the experimental group 2 was not statistically significant(P>0.017). In terms of the catheter indwelling time,the difference was statistically significant between the experimental group 1 and the control group(P<0.017). Conclusion When the tip of midline catheter is located in the axillary vein of the chest or subclavian vein,the incidence of catheter-related complications is lower,with longer indwelling time and better clinical outcome.

    Objective To construct the practice plan of nutrition management and nursing for young pulmonary tuberculosis(YPT) patients and to evaluate its effect. Methods The final draft of the plan was formed through the current investigation,literature research and expert meeting. YPT patients admitted to the hospital from April 1 to June 30,2019,served as the control group and received routine nutritional care;YPT patients admitted from August 1 to October 30,2019 were allocated into an intervention group,and the evidence-based and clinical needs nutrition management nursing practice program was implemented. The differences in nutritional status,immune function,and quality of life were compared between the 2 groups. Results The weight change of the intervention group in the first and third months of discharge was better than that of the control group(P<0.05).The immune function,total scores of specific dimensions and quality of life in the intervention group were better than those of the control group in 3 months after discharge from the hospital(P<0.05),and there was no statistical difference between the 2 groups in physiological,psychological and social dimensions(P>0.05). Conclusion Nutritional management and nursing practice programs for YPT patients can improve weight,immune function,quality of life,and promote recovery.

    Effect of Return-to-Family intervention on rehabilitation efficacy of breast cancer patients

    Objective To explore the effect of “Return-to-Family” intervention program on rehabilitation effects of breast cancer survivors. Methods A total of 70 breast cancer survivors were recruited by convenient sampling in Breast Surgery Department and Breast Nursing Clinic of a tertiary hospital in Beijing. They were divided into a control group and an intervention group,and each group has 35 patients. In the intervention group,the program of “Return-to-Family” intervention was carried out. In the control group,the routine care was provided. The effectiveness of the intervention program was assessed before and after intervention by the function of shoulder joint and arm at affected side,the Disabilities of the Arm,Shoulder and Hand questionnaire(Quick-DASH),the Cancer Rehabilitation Evaluation System-Short Form(CARES-SF),the Family Adaptability and Cohesion Evaluation Scales (FACES). Results After intervention,the total scores of the function of shoulder joint and arm at affected side,the FACES in the intervention group were higher than those in the control group and those before intervention(P< 0.05). The total scores of the Quick-DASH and the CARES-SF in the intervention group were lower than those in the control group and those before intervention(P<0.05). Conclusion “Return-to-Family” intervention benefits the improvement of upper limb function,quality of life,family function for breast cancer patients,and help breast cancer survivors return to family smoothly.

    Latent class analysis of symptom characteristics in patients with post-intensive care syndrome

    Objective To explore the latent classes of symptom characteristics of post-intensive care syndrome(PICS)patients and to analyze the differences in demographic and disease characteristics of patients among different classes. Methods According to the inclusion and exclusion criteria,patients in the general ICU of a tertiary hospital in Guizhou were selected as the research subjects from August 16,2019 to January 16,2020,and from June 8,2020 to October 30,2020 by using convenient sampling method. Patients were followed up by telephone after being transferred out of ICU for 1 month to assess their PICS-related symptoms. Latent class analysis(LCA)was used to explore the latent classes of the symptom characteristics of PICS patients. Single factor analysis and multiple logistic regression were used to identify the influencing factors of the latent classes of PICS. Results A total of 299 samples were obtained and PICS occurred in 165 patients(55.18%). The symptom characteristics of PICS could be divided into 3 latent classes,which were named as “fatigue-sleep disorder group”(44.24%),“anxiety group”(16.97%),and “high symptom group”(38.79%). Compared “high symptom group” with “fatigue-sleep disorder group”,the higher acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score was,the less likely it was to be classified as “fatigue-sleep disorder group”(OR=0.882,P=0.001),and the more likely it was to be classified as “fatigue-sleep disorder group” without respiratory disease history or tracheotomy(OR=5.443,P=0.048;OR=4.015,P=0.006). Compared “high symptom group” with “anxiety group”,the higher APACHE Ⅱ score was Less likely to be classified to be “anxiety group”(OR=0.903,P=0.027),and patients younger than 50 years old were more likely to be classified as “anxiety group”(OR=3.392,P=0.025). Compared with “fatigue-sleep disorder group”,patients with age younger than 50 years were more likely to be classified as “anxiety group”(OR=4.422,P=0.005). Conclusion The symptomatic characteristics of PICS are heterogeneous and can be divided into 3 latent classes. Patients with a high APACHE II score,a history of respiratory disease,and a tracheotomy were more likely to be classified as “high symptom group”,while those younger than 50 were more likely to be classified as “anxiety group”. It is suggested that the clinical staff should pay attention to the heterogeneity of PICS individual symptom characteristics and give targeted intervention measures to different classes of patients.

    Discussion on the starting time of shoulder and neck rehabilitation exercise in patients after endoscopic thyroid cancer surgery through gasless transaxillary endoscopic thyroidectomy

    Objective To investigate the optimal starting time,safety and effectiveness of shoulder and neck functional rehabilitation exercise for patients undergoing gassless transaxillary endoscopic thyroidectomy(GTET). Methods 175 patients who underwent GTET were selected by convenient sampling,with 121 patients in 2 test groups(61 patients in the test group 1 and 60 patients in the test group 2) and 54 patients in a control group. The starting time of postoperative shoulder and neck rehabilitation training in the control group,test group 1 and test group 2 were postoperative 7 days,postoperative 48 hours and 2 hours after general anesthesia wakening,respectively. The analysis among 3 groups included the amount of drainage(ml),the retention time of drainage tube (h),the postoperative hospital stay(d),the cases of bleeding,the pain score of incision(neck and armpit) at postoperative day 1 and day 3,the range of shoulder and neck movement and the quality of life. Results There was no statistically significant difference in term of drainage amount,retention time of drainage tube,bleeding cases,hospital stay and pain in neck and incisions at postoperative day 1 and day 3 among the 3 groups(P>0.05). The motion range of cervical extension,lateral cervical flexion,cervical rotation,shoulder abduction,shoulder adduction,shoulder flexion and shoulder extension among different groups were statistically significant 1 month after surgery(P<0.001). In term of quality of life(QOL),there were statistically significant differences in aspects of stiffness,pain,numbness of the neck or shoulder,and the impact on leisure and entertainment(P<0.05). Conclusion Starting shoulder neck rehabilitation exercise 2 hours after awake under anesthesia in patients undergoing GTET would not cause bleeding and affect the comfort,and could safely and effectively improve the early QOL of patients in the early postoperative period.

    Objective To explore the application effect of advance care planning in hemodialysis patients.Methods A total of 80 hemodialysis patients who were treated in a tertiary hospital in Zhengzhou from October 2020 to June 2021 were selected as the research subjects. According to the random number table method,they were divided into a control group and an intervention group with 40 cases each. The intervention group was implemented with a 4-week advance care planning,and the control group received the routine care. The death attitude profile-revised scale,the Chinese version of the meaning in life questionnaire and the patient dignity inventory scale were used to compare the differences in death attitude,meaning of life and dignity between the 2 groups of patients.Results After the intervention,the patients in the intervention group changed their attitude toward death;the score of meaning of life was higher than that of the control group;the score of dignity was lower than that of the control group;the differences were statistically significant(P<0.05).Conclusion Advance care planning can help hemodialysis patients change into a positive attitude toward death,increase the sense of meaning in life,and maintain the dignity of patients.

    The exploration of the influencing factors of occupation fatigue of nurses

    Objective This research analyzed the latent profile of occupation fatigue of nurses and discussed the influencing factors of occupation fatigue among different types. Methods Based on the Effort-Reward Imbalance Model,we used the convenience sampling method to select 551 nurses from 2 tertiary hospitals in Shanghai in December 2020. The demographic characteristics,Occupation Fatigue Exhaustion Recovery Scale and Effort-Reward Imbalance Questionnaire were used to make a cross-section survey of nurses’ occupation fatigue,and the latent profile analysis was carried out with the results of 3 explicit indexes of Occupational Fatigue Exhaustion Recovery Scale. At the same time,the influencing factors of occupational fatigue of nurses were explored by univariate analysis and ordered multi-classification logistic regression. Results The 55l nurses could be divided into 3 latent subgroups,including low fatigue and high recovery(14.5%),medium fatigue and medium recovery(54.3%) and high fatigue and low recovery(31.2%). The influencing factors of nurses’ occupational fatigue included total weekly working hours,work-family conflict,external effort,over commitment and reward(P<0.05). Conclusion The occupational fatigue of nurses can be divided into 3 subgroups. The nursing managers can make individualized psychological interventions according to the influencing factors to improve the self-regulation ability of nurses,so as to reduce the degree of fatigue.

    Objective To comprehensively search the relevant research of Ottawa Decision Support Framework (ODSF) and to clarify the specific connotation of its constructs,understand the areas covered by its clinical application,and to analyze its specific role in the research.Methods Using the method of Arksey and O’Malley scoping review,we systematically searched CNKI,SINOMED,Wanfang,VIP,PubMed,Cochrane Library,CINAHL,Embase,Web of science,and screened the literature according to the inclusion and exclusion criteria. 2 researchers independently extracted the key information of the ODSF. Descriptive analysis and topic analysis were used to classify the extraction results.Results A total of 143 documents are included. There are 3 types of conceptual descriptions of ODSF in previous studies,which correspond to the 3 versions of ODSF. The framework covers a wide range of research areas,mainly including cancer,orthopedic diseases,cardiovascular diseases,etc. The guiding role of the framework in the research mainly has 7 categories,and there are obvious differences between domestic and foreign research.Conclusion This study summarizes the connotation and clinical application of ODSF in detail,which can provide guidance for future research.

    Design and application of an infusion drip counter

    Objective An infusion drip counter is designed,and its application effect is evaluated. Methods An infusion drip counter was self-developed,and it consists of infusion set buckle,display,start/mode switch button,battery,buzzer,infrared sensor area,etc. In the laboratory,the infusion drip rate measured by the drip counter was compared with the infusion drip rate set by the infusion pump,in order to evaluate the accuracy of the counter. Convenience sampling method was used to recruit 200 emergency patients with infusion admitted from March to May 2020 in a tertiary hospital,and they were divided into an experimental group (n=103) and a control group (n=97) according to the seat number during the infusion. The experimental group used the drip counter for counting,and the control group used the manual counting for counting. The difference between the predicted infusion completion time and the actual infusion completion time was compared,as well as the incidence of the intravenous infusion complications such as phlebitis,extravasation,tube blockage,blood return,and the patient satisfaction rate in the 2 groups. The satisfaction rate of nurses using the drip counter was evaluated. Results In the laboratory test,the accuracy of the drip rate counting of this counter was 99.6%,and the drip rate displayed by the counter was compared with the drip rate set by the infusion pump,and the difference was not statistically significant (P=0.829). In the process of clinical application,the difference between the predicted infusion completion time and the actual infusion completion time of the experimental group was smaller than that of the control group;the incidence of blood return was lower than that of the control group;the patient satisfaction rate was higher than that of the control group;the differences were statistically significant (P<0.05). The satisfaction rate of nurses was 96.1%. Conclusion The infusion drip counter has high counting accuracy. The use of this drip counter is beneficial to control the infusion drip rate,reduce the incidence of blood return,and improve the patient’s satisfaction rate.

    The best evidence summary for the prevention of catheter related skin injuries in cancer patients with PICC

    Objective To retrieve,evaluate,integrate the evidence of catheter related skin injuries prevention practices in cancer patients with PICC from home and abroad,and then summarize the relevant best evidence to provide evidence-based practice for clinical standardized management of catheter related skin injuries. Methods Evidence on prevention of catheter related skin injuries in cancer patients with PICC was systemically retrieved in the websites and databases,namely the BMJ Best Practice,UpToDate,Guidelines International Network,National Guideline Clearinghouse,National Institute for Health and Care Excellence,Scottish Intercollegiate Guidelines Network,Registered Nurses Association of Ontario,Intravenous Nurses Society,National Comprehensive Cancer Network,Oncology Nursing Society,medlive,Cochrane Library,Joanna Briggs Institute,PubMed,Embase,CINAHL,Taiwan academic literature database,CNKI,WanFang,VIP,SinoMed,including clinical decision-making,guidelines,evidence summaries,best practice,recommended practice,systematic reviews,expert consensuses,and government documents. The literature retrieval period was from the database construction to September 12,2021. The guidelines were individually evaluated by 4 experts,and the remaining literature was independently evaluated by 2 researchers. The literature that met the criteria was extracted and graded. Finally,the expert group integrated the evidence and summarized the evidence topics. Results A total of 12 articles were involved,including 1 government document,3 guidelines,4 evidence summaries,3 expert consensuses,and 1 systematic review. Finally,7 evidence topics and 26 pieces of best evidence were formed,including assessment,skin cleaning and antisepsis,dressing selection,operation and securement techniques,skin protection at PICC insertion site,patient education,supervision and training. Conclusion The best evidence of catheter related skin injuries in cancer patients with PICC can provide resources preparation for clinical transformation and offer nurses clinical decision-making for scientific and effective nursing practice. Specialist nurses should reasonably choose the best evidence for localized application,and constantly update the best evidence,standardize nursing practice,and ensure the safety of patients.

    To summarize the nursing experience of a patient with synchronous multiple primary malignant tumors and situs inversus totalis. Key points of nursing:establishing a case nursing group;predicting the surgical risk based on physiology and surgical severity,using individualized multimodal pre-rehabilitation management strategies,doing a good job in risk prevention and control nursing;sorting out the relevant operations on the left and right sides,cultivating the reverse thinking ability of total situs inversus;adequate bowel preparation,reducing gastrointestinal tension;doing a good job in the prevention and nursing of gastrointestinal fistula after surgery;continuing to do a good job in extended nursing after discharge. The patient was discharged 36 days after surgery and followed up for 3 months with good appetite and weight gain.

    Current knowledge level of oxygen therapy among clinical nurses

    Objective To learn about the current status of nurses’ knowledge concerning oxygen therapy,so as to provide the references for implementing training programs to promote safer oxygen therapies. Methods A self-designed questionnaire was carried out from May 4th to June 30th,2019 to investigate the mastery of knowledge concerning oxygen therapy among nurses who are the members of internal medicine nursing committee,association,critical care nursing committee,geriatric nursing committee and community nursing committee in certain nurse associations. The questionnaire is consisted of 5 parts,namely the content of the oxygen therapy from doctors’ prescription,demographic characteristics of the subjects,the indication of oxygen therapy,the assessment of oxygenation status and implementing oxygen therapy(practicing,monitoring and evaluation). Results 2 789 nurses participated the survey,among which there were 2 719 effective and valid questionnaire,with the response rate of 97.49%. 87.75% of nurses are aware that the oxygen therapy should be undertaken based on the prescription. Only 18.50% of nurses insist that the target saturation should be included in the prescription. 82.42% of nurses mistakenly believe that the oxygen therapy could treat the underlying diseases which lead to the hypoxaemia. Over 95.29% of nurses believe that breathlessness could be relieved by oxygen therapy,while this is a common mistake. 3.49% of nurses could indicate the correct saturation range of the needed continuous monitoring. Except for chronic obstructive lung disease,the correct answer rates of nurses to several diseases risking of hypercapnia are less than 70.00%. 22.25% of nurses could indicate the correct target saturation for the patients with the risk of hypercapnia. Only 16.77% of nurses could indicate the correct time period of monitoring saturation once the oxygen therapy is paused. Conclusion Based on the results from the survey,the knowledge mastery of nurses on oxygen therapy needs to be improved. In order to correct the misunderstandings in oxygen therapy cognition and to provide more accurate and safer oxygen practice for the patients,the systematic training for the nurses should be developed and implemented.

    Construction and application of a rehabilitation management programme for urinary incontinence after prostate cancer surgery

    Objective This study aims to construct a rehabilitation management programme for urinary incontinence after prostate cancer surgery,and to apply the programme to clinical practice,so as to provide references for future work. Methods Through evidence-based literature search and qualitative research,a grief rehabilitation management programme for urinary incontinence after prostate cancer surgery was formed. From January to March 2022,15 experts were consulted using Delphi method,and a rehabilitation management programme for urinary incontinence after prostate cancer surgery was formed. From May to July 2022,the convenience sampling was used to select patients with urinary incontinence after prostate cancer surgery in the urology clinic of a hospital in Shanghai. The experimental group was given the urinary incontinence rehabilitation management program,while the control group was given the routine nursing. The application effect of the programme was evaluated by the International Consultation on Incontinence Questionnaire-Short Form and 24-hour urinary pad test. Results The final programme included 6 first-level items,15 second-level items,and 43 third-level items. The expert authority coefficients were 0.88 and 0.98,and the Kendall harmony coefficients were 0.17 and 0.12(P<0.001). The ICIQ-SF scores and 24-hour pad weight at 1 month and 3 months after surgery in the intervention group were lower than the control group(P<0.05). Conclusion The application of the rehabilitation management programme improved the recovery of postoperative incontinence and reduced the 24-hour pad weight of patients at 1 month and 3 months after surgery. The programme can be popularized and applied after further verification.

    Expert consensus on management of palliative care wards in Hunan province

    Objective To form an expert consensus on management of palliative care wards (hereinafter referred to as the expert consensus) and to standardize the development,construction,management and other processes of palliative care wards in Hunan Province. Methods According to evidence-based methods,the literature related to the construction and management of hospice care wards was systematically searched in domestic and foreign guide websites or databases,with a search period from the establishment of the databases to January 2021. The relevant recommendation opinions were evaluated,extracted,and summarized to form a consensus draft. Through a round of expert consultations and 2 rounds of expert argumentation meetings,expert opinions were combined to modify and improve the content of each item to form an expert consensus final draft. Results The expert consensus includes 7 aspects,including environmental management,facilities and equipment management,personnel management,process management,infection prevention and control,safety management,quality evaluation and improvement. Conclusion The expert consensus can provide guidance for the standardization and institutionalization of the management of palliative care wards,and plays an active role in promoting the development of palliative care wards in Hunan Province.

    Development of an Assessment Scale of Aspiration Risk for Nasal Feeding Patients and its test of validity and reliability

    Objective To compile an Assessment Scale of Aspiration Risk for Nasal Feeding Patients,and to test its reliability and validity,aiming to provide a scientific and objective aspiration risk assessment tool for doctors and nurses. Methods The draft of the Assessment Scale of Aspiration Risk for Nasal Feeding Patients was constructed based on the literature analysis and 2 rounds of Delphi consultations. Using convenience sampling,the data of 122 patients with nasal feeding from November 2020 to March 2021 in tertiary A hospital of Shenzhen was collected,and item screening and the test of reliability and validity were carried out. Results The Assessment Scale of Aspiration Risk for Nasal Feeding Patients includes 3 dimensions,10 first-level items and 22 second-level items. The exploratory factor analysis extracts 3 common factors,and the cumulative variance contribution rate is 61.699%. The load value of common factors of each item is between 0.473 to 0.931. The correlation coefficient of the effective standard(whether aspiration occurs) is 0.739;the area under the ROC curve for predicting the risk of aspiration in nasal feeding patients is 0.961,and the confidence interval of 95% is between 0.929 to 0.993(P<0.001);the content validity is 0.953;the Cronbach’s α coefficient of the scale is 0.739. Conclusion The Assessment Scale of Aspiration Risk for Nasal Feeding Patients has good reliability and validity,and it can be used as a tool in clinical practice to assist in the identification and prediction of aspiration.

    Construction and application of a matching exercise training program for patients after lung transplantation

    Objective To construct a postoperative matching exercise training program for lung transplant patients and to evaluate the clinical application effect. Methods A non-concurrent controlled study was used. By convenience sampling method,patients undergoing lung transplantation in a tertiary A hospital in Hangzhou from December 2021 to March 2022 were included in a trial group with 35 cases,while the patients from April to November 2021 were recruited in a control group with 34 cases. Baseline data was consistent between the 2 groups. On the basis of the intervention program in the control group,the trial group received the matching exercise training program which was constructed based on the evidence and Delphi expert consultation method. It was divided into 4 stages according to the BODE index,including primary exercise,advanced exercise,intensive exercise and stable exercise,covering exercise mode,time,and intensity,etc. The control group received a routine pulmonary rehabilitation training program. The 6MWT distance standard time,deep inhalation volume standard time,hospitalization time,FEV1%,BMI,MBI and the incidence of adverse events during exercise training of 2 groups were compared. Results The standard time of the 6MWT distance and the deep inhalation volume in the trial group were lower than it in the control group(P<0.05). The FEV1% and MBI at discharge in the trial group were lower than those in the control group(P<0.05). There was no significant difference in length of stay,BMI and incidence of adverse events between the 2 groups(P>0.05). Conclusion The matching exercise training program is accurate,safe and feasible,and it can promote the rapid postoperative rehabilitation of lung transplant patients.

    Objective This study aims to construct a comprehensive exercise rehabilitation program for elderly patients with cirrhosis and frailty,and to explore its application effect. Methods Through literature research and qualitative research,a comprehensive exercise rehabilitation program was drafted. From November to December 2021,16 experts were consulted,and a comprehensive exercise rehabilitation program for elderly patients with cirrhosis and frailty was formed. From January to June 2022,the convenience sampling was used to select the elderly patients with cirrhosis and frailty in the infection department of a tertiary A hospital in Zhengzhou City as the research subjects. The experimental group was given the comprehensive exercise rehabilitation program on the basis of routine care,while the control group was only given routine care. Liver weakness index,quality of life and adverse events were compared between the 2 groups at 1 and 3 months after the intervention. Results The effective questionnaire recovery rates of the 2 rounds of expert consultations were 85.00% and 94.12%;the expert authority coefficients were 0.823 and 0.833;Kendall harmony coefficients were 0.175 and 0.196(P<0.001). Finally,the comprehensive exercise rehabilitation program included 2 first-level items and 8 second-level items and 24 third-level items. During the application of this program,10 cases dropped,and 58 cases were finally included in the experimental group and the control group respectively. At 1 month after the intervention,the Liver Frailty Index between the 2 groups were statistically significant(P=0.036). At 3 months after intervention,there were significant differences in the Liver Frailty Index and Quality of Life between the 2 groups(P<0.05). No adverse events such as falls and fractures occurred in the 2 groups. Conclusion The comprehensive exercise rehabilitation program constructed in this study is safe and effective for elderly patients with frailty cirrhosis,and it can improve the frailty status and quality of life of elderly patients with cirrhosis.

    Effect of lavender aromatic therapy combined with emotional freedom techniques on insomnia cancer breast patients with heart and spleen deficiency

    Objective To investigate the clinical nursing effect of lavender aromatherapy combined with emotional freedom techniques(EFT) on insomnia patients with breast cancer and heart-spleen deficiency.Methods A total of 140 patients with insomnia due to heart-spleen deficiency of breast cancer who met the inclusion criteria in the department of mammary gland of a tertiary hospital in Hangzhou from September 2020 to July 2021 were selected as the research subjects by convenience sampling method. Factorial design with 2 factors and 2 levels was adopted. According to the random number table method,the patients were divided into a control group(n=32),an aromatic group(n=33),an EFT group(n=32) and a combined group(n=32). The control group was given routine care of insomnia;on the basis of routine nursing for insomnia,the aromatic group was given lavender aromatherapy,and the EFT group was given EFT. The combined group was given the above 3 methods of comprehensive interventions at the same time,and the intervention period was 2 weeks. Pittsburgh sleep quality index(PSQI) was used to evaluate sleep quality within 24 hours and 2 weeks after intervention. Hospital anxiety and depression scale(HADS) was used to evaluate the improvement of negative emotions. The improvement of symptoms was evaluated by traditional Chinese Medicine(TCM) symptom score standard of deficiency of heart and spleen.Results Before and after the intervention,except the control group,the PSQI score,HASD score and TCM symptom score of the 3 groups were compared,and the differences were statistically significant(P<0.05). After intervention,the differences in PQSI score,HADS score and symptom score among the 4 groups were statistically significant(F=10.049,P<0.001;F=9.328,P<0.001;F=14.934,P<0.001;F=3.741,P<0.05),and the combined intervention of the 2 therapies was superior to any single therapy in the overall efficacy. The combined therapy has synergistic interaction effect on improving TCM symptoms of deficiency of both heart and spleen.Conclusion The combination of 2 therapies can effectively improve the sleep quality of breast cancer patients with insomnia of deficiency of both heart and spleen,and it has a magical effect in alleviating bad mood and relieving TCM symptoms.
